    Neonatal near miss: the need for a standard definition and appropriate criteria and the rationale for a prospective surveillance system

    In Latin American, there is currently a regional action with the main purposes of putting the concept of severe neonatal morbidity in practice and formulating proposals for interventions. A general overview of neonatal health conditions, including morbidity and mortality, is provided to update regional knowledge on the topic. An example of the development and implementation of the concept of maternal near miss is also provided, followed by results from a systematic review covering all previously published studies on Neonatal Near Miss. Finally, some proposals for building a common concept on the topic and for launching a prospective surveillance study are presented. A Neonatal Near Miss is a neonate who had a severe morbidity (organ dysfunction or failure) but who survived this condition within the first 27 days of life. The pragmatic criteria recommended to be used are as follows: birth weight below 1700 g, Apgar score below 7 at 5 minutes of life and gestational age below 33 weeks. As a proxy for organ dysfunction, the following management criteria are also confirmed: parenteral therapeutic antibiotics; nasal continuous positive airway pressure; any intubation during the first 27 days of life; phototherapy within the first 24 h of life; cardiopulmonary resuscitation; the use of vasoactive drugs, anticonvulsants, surfactants, blood products and steroids for refractory hypoglycemia and any surgical procedure. Although this study starts from a regional perspective, this topic is clearly globally relevant. All nations, especially low and middle-income countries, could benefit from the proposed standardization701282082

    Crack-cocaine users have less family cohesion than alcohol users

    Objective: Many studies correlate characteristics of family functioning and the development of drug addiction. This study sought to evaluate and compare the family environment styles of two groups of psychoactive substance users: 1) alcohol-only users and 2) crack-cocaine users. Methods: Three hundred and sixty-four users of alcohol, crack-cocaine, and other drugs, recruited from research centers in four Brazilian capitals participated in this study. Subjects were evaluated through the Family Environment Scale and the Addiction Severity Index, 6th version (ASI-6). ASI-6 t-scores were compared by analysis of variance (ANOVA) and post-hoc tests. A final model was obtained using a logistic regression analysis. All analyses were adjusted for partner, age, and psychiatric t-score. Results: We found a significant difference between groups in the cohesion subscale (p = 0.044). The post-hoc test revealed a difference of 1.06 points (95% CI 0.11-2.01) between groups 1 (6.45 +/- 0.28) and 2 (5.38 +/- 0.20). No significant between-group differences were observed in the other subscales. However, categorical analyses of variables regarding family dynamic showed that crack users more often reported that sometimes people in their family hit each other (30.4% vs. 13.2%, p = 0.007) and that people in their family frequently compared each other regarding work and/or school achievement (57.2% vs. 42.6%, p = 0.041). Conclusion: These results suggest that families of crack-cocaine users are less cohesive than families of alcohol users.     IMUNOEXPRESSÃO DE MASPIN E KI-67 COMO MARCADORES PROGNÓSTICOS DA CARACTERIZAÇÃO BENIGNA DA HIPERPLASIA FIBROEPITELIAL MAMÁRIA CANINA

    A hiperplasia mamária é caracterizada por um aumento de toda a glândula mamária, considerada benigna e de bom prognóstico.  Uma cadela de dois anos apresentou aumento de volume de toda a cadeia mamária após estímulo hormonal. Para caracterizar o prognóstico do processo foi utilizada a técnica de imunohistoquímica com avaliação da marcação do Maspin, considerado um marcador de bom prognóstico e o Ki-67, marcador de proliferação celular e que, portanto, mostraria alta expressão na malignidade. Como resultado obteve-se uma baixa expressão do Ki-67 e uma alta expressão do Maspin, sugerindo um excelente prognóstico para o animal.     COVID-19 in long-term care facilities in Brazil: serological survey in a post-outbreak setting

    This cross-sectional seroepidemiological survey presents the seroprevalence of SARS-CoV-2 in a population living in 15 Long-Term Care Facilities (LTCFs), after two intra-institutional outbreaks of COVID-19 in the city of Botucatu, Sao Paulo State, Brazil. Residents were invited to participate in the serological survey performed in June and July 2020. Sociodemographic and clinical characterization of the participants as well as the LTCF profile were recorded. Blood samples were collected, processed and serum samples were tested using the rapid One Step COVID-19 immunochromatography test to detect IgM and IgG anti-SARS-CoV-2. Among 209 residents, the median of age was 81 years old, 135 (64.6%) were female and 171 (81.8%) self-referred as being white. An overall seroprevalence of 11.5% (95% CI: 7.5% – 16.6%) was found. The highest seroprevalences of 100% and 76.9% were observed in LTCFs that had experienced COVID-19 outbreaks. Most residents with positive immunochromatography tests (70.8%) referred previous contact with a confirmed COVID-19 case. Although there was a relatively low seroprevalence of COVID-19 in the total number of elderly people, this population is highly vulnerable and LTCFs are environments at higher risk for COVID-19 dissemination. A well-established test for COVID-19 policies, the adequate characterization of the level of interaction between residents and the healthcare provider team and the level of complexity of care are crucial to monitor and control the transmission of SARS-CoV-2 in these institutions

    Atividade antioxidante de macrófagos alveolares em ratos endotoxêmicos

    Avaliou-se o efeito da endotoxemia sobre a atividade antioxidante de macrófagos alveolares em ratos da linhagem Wistar. Foram utilizados 24 ratos machos, com idade entre 90 e 120 dias, os quais foram divididos em dois grupos: controle e endotoxêmico. O grupo endotoxêmico foi submetido à injeção intraperitonial de lipopolissacarídio na dose de 1mg/kg de peso corporal. Após 24 h, coletou-se sangue para contagem total e diferencial de leucócitos; lavado broncoalveolar para contagem total e diferencial dos leucócitos e, a partir de macrófagos isolados deste lavado, foram realizadas as dosagens de superóxido e superóxido dismutase. A endotoxemia aumentou a contagem total de leucócitos e o número de neutrófilos no sangue periférico, no lavado broncoalveolar, e aumentou a produção de superóxido sem modificar a produção da superóxido dismutase. Esses resultados sugerem que a endotoxemia induz a uma resposta inflamatória no pulmão. Contudo, não altera a atividade antioxidante em ratos adultos. Tal fato potencializa a resposta contra agentes infecciosos pelo hospedeiro, mas também pode contribuir na patogênese de injúria pulmonar.The effects of endotoxemia on the antioxidant activity in alveolar macrophages of Wistar rats were evaluated. Twenty-four male rats, 90-120 days of age, were separated into 2 groups: control and endotoxemic. To the endotoxemic animals was administered, intraperitoneally, a lipopolyssaccaride at dosage of 1mg/kg body weight. Twenty-four hours after this procedure, blood was collected for total and differential leukocytes counts. In addition, bronchoalveolar lavage was collected for total and differential leukocyte counting. From this lavage macrophages were isolated for the dosage of superoxide and superoxide dismutase. The endotoxemia increased the total leukocyte counts and the number of neutrophils in the peripheral blood and bronchoalveolar lavage of the rats. There was an increased superoxide production without changing the superoxide dismutase. Our findings indicate that endotoxemia induces lung inflammatory response. However, it does not alter the antioxidant activity in adult rats. This fact not only enhances host response against infectious agents, but might also contribute to the pathogenesis of pulmonary injury

    The effect of low-level laser irradiation (In-Ga-Al-AsP - 660 nm) on melanoma in vitro and in vivo

    <p>Abstract</p> <p>Background</p> <p>It has been speculated that the biostimulatory effect of Low Level Laser Therapy could cause undesirable enhancement of tumor growth in neoplastic diseases. The aim of the present study is to analyze the behavior of melanoma cells (B16F10) <it>in vitro </it>and the <it>in vivo </it>development of melanoma in mice after laser irradiation.</p> <p>Methods</p> <p>We performed a controlled <it>in vitro </it>study on B16F10 melanoma cells to investigate cell viability and cell cycle changes by the Tripan Blue, MTT and cell quest histogram tests at 24, 48 and 72 h post irradiation. The <it>in vivo </it>mouse model (male Balb C, n = 21) of melanoma was used to analyze tumor volume and histological characteristics. Laser irradiation was performed three times (once a day for three consecutive days) with a 660 nm 50 mW CW laser, beam spot size 2 mm<sup>2</sup>, irradiance 2.5 W/cm<sup>2 </sup>and irradiation times of 60s (dose 150 J/cm<sup>2</sup>) and 420s (dose 1050 J/cm<sup>2</sup>) respectively.</p> <p>Results</p> <p>There were no statistically significant differences between the <it>in vitro </it>groups, except for an increase in the hypodiploid melanoma cells (8.48 ± 1.40% and 4.26 ± 0.60%) at 72 h post-irradiation. This cancer-protective effect was not reproduced in the <it>in vivo </it>experiment where outcome measures for the 150 J/cm<sup>2 </sup>dose group were not significantly different from controls. For the 1050 J/cm<sup>2 </sup>dose group, there were significant increases in tumor volume, blood vessels and cell abnormalities compared to the other groups.</p> <p>Conclusion</p> <p>LLLT Irradiation should be avoided over melanomas as the combination of high irradiance (2.5 W/cm<sup>2</sup>) and high dose (1050 J/cm<sup>2</sup>) significantly increases melanoma tumor growth <it>in vivo</it>.</p

    Atmosfera modificada e refrigeração para conservação pós-colheita de uva 'Niagara Rosada'

    O objetivo deste trabalho foi avaliar os efeitos da atmosfera modificada na conservação pós-colheita da uva 'Niagara Rosada' armazenada sob refrigeração, em dois experimentos. No primeiro experimento avaliou-se o acondicionamento de cachos nas seguintes embalagens: papelão ondulado (testemunha); tereftalato de polietileno (PET); cloreto de polivinila (PVC) 17 &#956;m; polietileno linear de baixa densidade (PELBD) 25 &#956;m; e PELBD 50 &#956;m. Em outro experimento, avaliaram-se os sistemas de acondicionamento: sacolas de plástico abertas (testemunha); polietileno de baixa densidade (PEBD) 25 &#956;m; PEBD 25 &#956;m, com injeção de mistura gasosa (21% O2/5% CO2); PEBD 25 &#956;m (21% O2/10% CO2); PEBD 25 &#956;m (21% O2/20% CO2). Os cachos foram armazenados a 1±1°C e 90±5% de umidade relativa (UR) por 28 dias, seguido de armazenamento em condições do ambiente (25±2°C e 80±5% UR). Os cachos foram avaliados quanto à perda de massa de matéria fresca, firmeza, cor das bagas, esbagoamento, sólidos solúveis totais (SST), acidez titulável (AT), relação SST/AT e incidência de podridões. O filme PELBD 50 &#956;m, a partir do 14º dia a 1°C, seguido por mais três dias a 25°C, causou a fermentação dos cachos. As embalagens PELBD 25 &#956;m, com ou sem injeção de mistura gasosa, e PVC 17 &#956;m reduzem a perda de massa de matéria fresca dos cachos, mas não reduzem o esbagoamento e a incidência de podridões
