    One year survival with poorly differentiated metastatic pancreatic carcinoma following chemoembolization with gemcitabine and cisplatin.

    While hepatic arterial chemoembolization is efficacious for a number of malignancies, there is scant data regarding treatment of pancreatic adenocarcinoma. We report a complete radiographic response at one year from diagnosis of metastatic pancreatic carcinoma. Gemcitabine/cisplatin based chemoembolization may be of potential benefit for patients with liver-dominant metastases from pancreatic carcinoma. Given the typical survival of 6 months or less in this patient group with standard therapies, further research is warranted

    Industrial constructions of publics and public knowledge: a qualitative investigation of practice in the UK chemicals industry

Using the chemicals sector as an example, this paper explores how companies conceive of publics and "public knowledge," and how this relates to modes of engagement/communication with them. Drawing on qualitative empirical research in four companies, we demonstrate that the public for industry are primarily conceived as "consumers" and "neighbours," having concerns that should be allayed rather than as groups with knowledge meriting engagement. We conclude by highlighting the dissonance between current advocacy of engagement and the discourses and practices prevalent within industry, and highlight the need for more realistic strategies for industry/public engagement.Funding was received from the ESRC Science in Society Programme

    Local time and the pricing of time-dependent barrier options

    A time-dependent double-barrier option is a derivative security that delivers the terminal value ϕ(ST)\phi(S_T) at expiry TT if neither of the continuous time-dependent barriers b_\pm:[0,T]\to \RR_+ have been hit during the time interval [0,T][0,T]. Using a probabilistic approach we obtain a decomposition of the barrier option price into the corresponding European option price minus the barrier premium for a wide class of payoff functions ϕ\phi, barrier functions b±b_\pm and linear diffusions (St)t[0,T](S_t)_{t\in[0,T]}. We show that the barrier premium can be expressed as a sum of integrals along the barriers b±b_\pm of the option's deltas \Delta_\pm:[0,T]\to\RR at the barriers and that the pair of functions (Δ+,Δ)(\Delta_+,\Delta_-) solves a system of Volterra integral equations of the first kind. We find a semi-analytic solution for this system in the case of constant double barriers and briefly discus a numerical algorithm for the time-dependent case.Comment: 32 pages, to appear in Finance and Stochastic

    Demographics, distribution and experiences of UK clinical academic trainees using GMC NTS Survey data

    Involvement in research plays an integral role in the delivery of high-quality patient care, benefitting doctors, patients and employers. It is important that access to clinical academic training opportunities are inclusive and equitable. To better understand the academic trainee population, distribution of academic posts and their reported experience of clinical training, we analysed 53 477 anonymous responses from General Medical Council databases and the 2019 National Training Survey. Academic trainees are more likely to be men, and the gender divide begins prior to graduation. There are very low numbers of international medical graduates and less than full-time academic trainees. A small number of UK universities produce a greater prevalence of doctors successfully appointed to academic posts; subsequent academic training also clusters around these institutions. At more senior levels, academic trainees are significantly more likely to be of white ethnicity, although among UK graduates, no ethnicity differences were seen. Foundation academic trainees report a poorer experience of some aspects of their clinical training placements, with high workloads reported by all academic trainees. Our work highlights important disparities in the demographics of the UK clinical academic trainee population and raises concerns that certain groups of doctors face barriers accessing and progressing in UK academic training pathways

    New result for the neutron β-asymmetry parameter A_0 from UCNA

    Background: The neutron β-decay asymmetry parameter A_0 defines the angular correlation between the spin of the neutron and the momentum of the emitted electron. Values for A_0 permit an extraction of the ratio of the weak axial-vector to vector coupling constants, λ≡gA/gV, which under assumption of the conserved vector current hypothesis (gV=1) determines gA. Precise values for gA are important as a benchmark for lattice QCD calculations and as a test of the standard model. Purpose: The UCNA experiment, carried out at the Ultracold Neutron (UCN) source at the Los Alamos Neutron Science Center, was the first measurement of any neutron β-decay angular correlation performed with UCN. This article reports the most precise result for A_0 obtained to date from the UCNA experiment, as a result of higher statistics and reduced key systematic uncertainties, including from the neutron polarization and the characterization of the electron detector response. Methods: UCN produced via the downscattering of moderated spallation neutrons in a solid deuterium crystal were polarized via transport through a 7 T polarizing magnet and a spin flipper, which permitted selection of either spin state. The UCN were then contained within a 3-m long cylindrical decay volume, situated along the central axis of a superconducting 1 T solenoidal spectrometer. With the neutron spins then oriented parallel or anti-parallel to the solenoidal field, an asymmetry in the numbers of emitted decay electrons detected in two electron detector packages located on both ends of the spectrometer permitted an extraction of A_0. Results: The UCNA experiment reports a new 0.67% precision result for A_0 of A_0=−0.12054(44)_(stat)(68)_(syst), which yields λ=gA/gV=−1.2783(22). Combination with the previous UCNA result and accounting for correlated systematic uncertainties produces A0=−0.12015(34)stat(63)syst and λ=gA/gV=−1.2772(20). Conclusions: This new result for A0 and gA/gV from the UCNA experiment has provided confirmation of the shift in values for gA/gV that has emerged in the published results from more recent experiments, which are in striking disagreement with the results from older experiments. Individual systematic corrections to the asymmetries in older experiments (published prior to 2002) were >10%, whereas those in the more recent ones (published after 2002) have been of the scale of <2%. The impact of these older results on the global average will be minimized should future measurements of A0 reach the 0.1% level of precision with central values near the most recent results

    Dangerous work: The gendered nature of bullying in the context of higher education

    This paper discusses results from a research project which set out to investigate gender differences in the nature and experience of bullying within the higher education sector. Gender differences emerged in the form and perception of bullying as well as in target response. Results also indicate that, irrespective of gender, bullies can capture and subvert organizational structures and procedures (official hierarchies, mentoring systems, probationary reviews) to further their abuse of the target and to conceal aggressive intent. These outcomes are discussed in relation to gendered assumptions behind management practices and in relation to the masculinist ethic that underpins many higher education management initiatives. Overall, results indicate that bullying cannot be divorced from gender and that such behaviour needs to be seen in a gendered context

    First direct constraints on Fierz interference in free neutron β\beta decay

    Precision measurements of free neutron β\beta-decay have been used to precisely constrain our understanding of the weak interaction. However the neutron Fierz interference term bnb_n, which is particularly sensitive to Beyond-Standard-Model tensor currents at the TeV scale, has thus far eluded measurement. Here we report the first direct constraints on this term, finding bn=0.067±0.005stat0.061+0.090sysb_n = 0.067 \pm 0.005_{\text{stat}} {}^{+0.090}_{- 0.061}{}_{\text{sys}}, consistent with the Standard Model. The uncertainty is dominated by absolute energy reconstruction and the linearity of the beta spectrometer energy response

    Immune responses in pigs vaccinated with adjuvanted and non-adjuvanted A(H1N1)pdm/09 influenza vaccines used in human immunization programmes.

    Following the emergence and global spread of a novel H1N1 influenza virus in 2009, two A(H1N1)pdm/09 influenza vaccines produced from the A/California/07/09 H1N1 strain were selected and used for the national immunisation programme in the United Kingdom: an adjuvanted split virion vaccine and a non-adjuvanted whole virion vaccine. In this study, we assessed the immune responses generated in inbred large white pigs (Babraham line) following vaccination with these vaccines and after challenge with A(H1N1)pdm/09 virus three months post-vaccination. Both vaccines elicited strong antibody responses, which included high levels of influenza-specific IgG1 and haemagglutination inhibition titres to H1 virus. Immunisation with the adjuvanted split vaccine induced significantly higher interferon gamma production, increased frequency of interferon gamma-producing cells and proliferation of CD4(-)CD8(+) (cytotoxic) and CD4(+)CD8(+) (helper) T cells, after in vitro re-stimulation. Despite significant differences in the magnitude and breadth of immune responses in the two vaccinated and mock treated groups, similar quantities of viral RNA were detected from the nasal cavity in all pigs after live virus challenge. The present study provides support for the use of the pig as a valid experimental model for influenza infections in humans, including the assessment of protective efficacy of therapeutic interventions
