    Method for the Integration of Applications Based on Enterprise Service Bus Technologies

    Companies have developed and acquired multiple business software applications to respond to new organizational requirements. In many cases, the applications have logical dependencies or are used in isolation. This practice increases the possibility of errors, produces inconsistencies and duplications of data, and hinders management of the organization's processes. To take into account the complexities of an integration initiative in terms of integration technologies, staff capabilities and the business applications themselves, organizations that are new to Enterprise Application Integration should follow guidelines and best practices to implement their integration solutions in an integrated manner. The present work conceptualizes a method for developing an integration solution based on Enterprise Service Bus technologies. To achieve this objective, a case study methodology is used for the conception and evaluation of the proposal in terms of systematizing the lessons learned in the development of projects to integrate applications in multiple Cuban organizational sectors in the 2013-2016 period. The proposed method is independent of technology and is modelled as a process in terms of stages, activities and artefacts. The results obtained offer a mechanism that guides and favours the development of a service-based flexible integration solution using integration technologies such as the Enterprise Service Bus

    Conceptualising IT Consulting Services: An Approach from IT-Business Alignment Models and Design Sciences

    The constant integration of business and manufacturing processes is a difficult task that can be facilitated through IT consulting services. However, if these services do not adequately address the problems of alignment between IT and business, efficiency in integration can be seriously compromised. This article presents a methodology that systematises IT consulting services for the acquisition, incorporation, and integration of IT elements in an organisation in such a way that is aligned with the business and by contemplating its contribution to the value chain. The proposal is based on a set of rules, methods, guidelines, patterns, and artefacts that define a flow of action and implement a strategy that provides a consulting solution as a final result. Likewise, a method is proposed to evaluate the applied methodology and the solution obtained. For validation of the method, a set of business processes based on a case study applied in several Cuban companies related to the food sector has been defined to help adjust the parameters and corroborate the generalisation of the proposal. This research allows to ensure the alignment of business and IT to avoid the failure in the incorporation of IT to companies. It also analyzes and establishes the ideal artefacts for IT consulting and generates an IT consulting methodology that makes the analysis of consultants more robust in order to guarantee the success of incorporating IT into companies.This work has been supported by grant TIN2016-78103-C2-2-R and University of Alicante [GRE14-02 and SmartUniversity2017]

    Efficacy of the creation of a patient school program in the detection of needs in patients with autosomal dominant polycystic kidney disease

    Introducción: La Poliquistosis Renal Autosómica Dominante es una enfermedad renal crónica responsable del 10% de los casos de insuficiencia renal terminal. La participación y los grupos de apoyo entre iguales son herramientas que mejoran el bienestar, evitando complicaciones y retrasando el avance de la enfermedad. Objetivos: Detectar necesidades informativas, así como recursos de apoyo, en este grupo de pacientes mediante la puesta en marcha de una Escuela de Pacientes con poliquistosis renal autosómica dominante. Material y Método: Se utilizó un diseño mixto (cuantitativo y cualitativo). El estudio se desarrolló mediante cuatro fases: 1) Grupo focal: pacientes con poliquistosis renal y sus cuidadores; 2) Selección de los pacientes expertos; 3) Elaboración de los contenidos del programa de la Escuela de pacientes con poliquitstosis renal autosómica dominante; 4) Pilotaje del programa. Resultados: Se detectaron necesidades de información referentes al tratamiento oral y al afrontamiento de la poliquistosis renal que no están cubiertas por los equipos de nefrología. Conclusiones: La Escuela de Pacientes ha demostrado ser una herramienta útil para detectar necesidades y recursos en pacientes con poliquistosis renal autosómica dominante que han de enfrentarse a una enfermedad crónica donde se requiere la participación del paciente para garantizar la adhesión al tratamiento.Introduction: Autosomal Dominant Polycystic Kidney Disease is a chronic kidney disease responsible for 10% of cases of end-stage renal failure. Participation and peer support groups are tools that improve well-being, avoiding complications and delaying disease progression. Objectives: To detect information needs, as well as support resources, in patients with autosomal dominant polycystic kidney disease trough a Patient School. Material and Method: A mixed design (quantitative and qualitative) was used. The study was developed through four phases: 1) Focus group: patients with autosomal dominant polycystic kidney disease and their caregivers; 2) Selection of expert patients; 3) Preparation of the contents of the program of the Patient School with autosomal dominant polycystic kidney disease; 4) Piloting the program. Results: Information needs regarding oral treatment and coping with autosomal dominant polycystic kidney disease were detected, which are not covered by nephrology teams. Conclusions: Patients School has proven to be a useful tool to detect needs and resources in patients with autosomal dominant polycystic kidney disease who have to face a chronic disease where patient participation is required to ensure adherence to treatmen

Asemade, dáse conta dos resultados das intervencións en dous xacementos, un de carácter gandeiro que presenta un palimpsesto estrutural e cronolóxico (Río Barbanza), e outro relacionado con sistemas de control territorial (Outeiro da Torre).We review the survey works carried through in the district of Barbanza, now extended to the parishes of Macenda and Bealo in search of livestock / pastoral settlements. In the course of these explorations a Copper Age site was discovered and, nearby, a previously unknown hillfort. An account is given of the results of the digging in two sites; one related to husbandry tasks, that presents a structural and chronological palimpsest (Rio Barbanza); and another more, related to territorial control systems (Outeiro da Torre)

    Real-world effectiveness of caplacizumab vs the standard of care in immune thrombotic thrombocytopenic purpura

    Immune thrombotic thrombocytopenic purpura (iTTP) is a thrombotic microangiopathy caused by anti-ADAMTS13 antibodies. Caplacizumab is approved for adults with an acute episode of iTTP in conjunction with plasma exchange (PEX) and immunosuppression. The objective of this study was to analyze and compare the safety and efficacy of caplacizumab vs the standard of care and assess the effect of the concomitant use of rituximab. A retrospective study from the Spanish TTP Registry of patients treated with caplacizumab vs those who did not receive it was conducted. A total of 155 patients with iTTP (77 caplacizumab, 78 no caplacizumab) were included. Patients initially treated with caplacizumab had fewer exacerbations (4.5% vs 20.5%; P <.05) and less refractoriness (4.5% vs 14.1%; P <.05) than those who were not treated. Time to clinical response was shorter when caplacizumab was used as initial treatment vs caplacizumab used after refractoriness or exacerbation. The multivariate analysis showed that its use in the first 3 days after PEX was associated with a lower number of PEX (odds ratio, 7.5; CI, 2.3-12.7; P <.05) and days of hospitalization (odds ratio, 11.2; CI, 5.6-16.9; P <.001) compared with standard therapy. There was no difference in time to clinical remission in patients treated with caplacizumab compared with the use of rituximab. No severe adverse event was described in the caplacizumab group. In summary, caplacizumab reduced exacerbations and refractoriness compared with standard of care regimens. When administered within the first 3 days after PEX, it also provided a faster clinical response, reducing hospitalization time and the need for PEX
