    Bile Duct Cancer: Preoperative Evaluation and Management

    Cholangiocarcinomas (CCAs) are malignant tumors that can develop anywhere along the biliary tree. Almost 10% of cholangiocarcinomas arise from the intra-hepatic bile ducts (iCCA); 50–60% from the bifurcation of the hepatic duct (perhilar cholangiocarcinoma, pCCA); and 20–30% from the distal bile duct (dCCA). The 7th edition of the AJCC staging system, released in 2010, divides the tumors into two major categories: perihilar (pCCA) and distal (dCCA) cholangiocarcinoma, given the differences in anatomy of the bile duct and consideration of local factors related to resectability. There are separate histological classifications for intrahepatic and extrahepatic cholangiocarcinoma. The majority of CCAs (90%) are well or moderately differentiated adenocarcinomas. Other features include invasiveness with early neural, perineural, periductal and lymphatic infiltration (more than 50% of cases at diagnosis) and longitudinal subepithelial infiltration along the wall of the bile duct up to 2 cm proximally and 1 cm distally.     Surgical and Interventional Management of Complications Caused by Pancreatitis

    Acute pancreatitis has a broad clinical spectrum: from mild, self-limited disease to fulminant illness resulting in multi-organ failure leading to a prolonged clinical course with up to 30% mortality in case of infected necrosis. Management of local complications such as pseudocysts and walled-off necrosis may vary from clinical observation to interventional treatment procedures. Gram negative bacteria infection may develop in up to one-third of patients with pancreatic necrosis leading to a clinical deterioration with the onset of the systemic inflammatory response syndrome and organ failure. When feasible, an interventional treatment is indicated. Percutaneous or endoscopic drainage approach are the first choices. A combination of minimally invasive techniques (step-up approach) is possible in patients with large or multiple collections. Open surgical treatment has been revised both in the timing and in the operating modalities in the last decades. Since 1990s, the surgical treatment of infected necrosis shifted to a more conservative approach. Disruption of the main pancreatic duct is present in up to 50% of patients with pancreatic fluid collections. According to the location along the Wirsung, treatment may vary from percutaneous drainage, endoscopic retrograde pancreatography with sphincterectomy or stenting to traditional surgical procedures. Patients may suffer from vascular complications in up to 23% of cases. Tissue disruption provoked by lipolytic and proteolytic enzymes, iatrogenic complications during operative procedures, splenic vein thrombosis, and pseudoaneurysms are the pathophysiological determinants of bleeding. Interventional radiology is the first line treatment and when it fails or is not possible, an urgent surgical approach should be adopted. Chylous ascites, biliary strictures and duodenal stenosis are complications that, although uncommon and transient, may have different treatment modalities from non-operative, endoscopic to open surgery

    Management of Acute and Chronic Pancreatitis

    Pancreatitis is a major public health issue worldwide. There is geographical variation in the burden of acute and chronic pancreatitis (CP). Globally, the age-standardized prevalence rate increased from 1990 to 2017. Acute pancreatitis (AP) is now one of the most common reasons for hospitalization with a gastrointestinal condition. The essential requirements for the management of AP are accurate diagnosis, appropriate triage, high-quality supportive care, monitoring for and treatment of complications, and prevention of relapse. Clinicians should be aware of the time course and the best management of AP, identifying which patient will have a severe course allowing earlier triage to an intensive care unit and earlier initiation of effective therapy. CP is a pathologic fibroinflammatory syndrome of the pancreas in individuals with genetic, environmental, and other risk factors who develop persistent pathologic responses to parenchymal injury or stress. Diagnosing the underlying pathologic process early in the disease course and managing the syndrome to change the natural course of disease and minimize adverse disease effects are the managing paradigm.     Liver resection in elderly: comparative study between younger and older than 70 years patients. Outcomes and implications for therapy

    Background and aim: to identify the factors that could influence the outcome of the old aged patients underwent liver resection for hepatocellular carcinoma (HCC) or colorectal liver metastases (LMCRC). Patient and methods: the Authors identified 51 patients older 70 years-old over 12-years period underwent resection for HCC (n 26) or for LMCRC (n 25). This group was compared with a cohort of 93 patients younger than 70 years who underwent resections in the same period. We have evaluated the results in terms of peroperative morbidity and mortality. Results: the mean age of 51 elderly patients was 74 years-old. Thirty-five were treated with anatomical resection. Cirrhosis was present in 26 patients while 27 had co-morbidities. Thirteen patients developed complications and the mean age of these were 76 years compared with 73 of the patients who have not (p= .01). No mortality was registered. The cirrhosis, blood transfusions, anatomical resection and diameter of the lesion did not influence the outcome. Conclusions: our results indicate the age per se should not be considered a contraindication for surgery, that proved to be safe and curative therapy, but showed that old age, using 75 years as a cut-off, in association with at least one comorbid medical condition could be considered as relevant factor of morbidit

    Hepatic cystic lesion in adult: a challenging diagnosis of undifferentiated primary embryonal sarcoma

    Cystic lesions of the liver are common. Most of these represent benign simple cysts that generally require no intervention. The diagnosis of simple hepatic cysts is straightforward in the face of typical radiographic findings; however, diagnostic uncertainty, based solely on radiographic criteria alone, is not an uncommon clinical dilemma. In this report, we present a case of a 47 old woman with a cystic liver lesion and acute epigastric pain. The patient had no previous history of oral contraceptive assumption or liver disease. A tender mass was palpable on the upper right quadrant of the abdomen. The abdominal ultrasound (US) showed a cystic lesion with a possible hemorrhagic component on right liver lobe. Abdominal computed tomography (CT) and magnetic resonance (MR) revealed a complex cystic lesion with possible intracystic hemorrhage. The patient was submitted to operation and resection; the final histologic diagnosis revealed a primary undifferentiated embryonal sarcoma of the liver. She relapsed 24 months after a new hepatic resection and chemo-embolization, she is alive at 29 months after first surgery

    Perianal lesions are common in patients with Crohn’s disease, and display aggressive behavior in some cases. An accurate diagnosis is necessary for the optimal management of perianal lesions. Treatment of perianal Crohn’s disease includes medical and/or surgical options. Recent discoveries in the pathogenesis of this disease have led to advances in medical and surgical therapy with good results. Perianal lesions in Crohn’s disease remain a challenging aspect for both gastroenterologists and surgeons and lead to a greatly impaired quality of life for all patients affected by this disease. A multidisciplinary approach is mandatory to obtain the best results

    Intrahepatic chemotherapy for unresectable cholangiocarcinoma: review of literature and personal experience

    Most patients with intrahepatic cholangiocarcinoma (IH-CCA) are unresectable and treatment options are limited. This study evaluates the efficacy of hepatic artery infusion (HAI) chemotherapy in patients whose disease is not initially treatable with resection. We selected patients with unresectable IH-CCA treated only with HAI chemotherapy at our centre between January 2008 and December 2012. We compared our outcome, using mRECIST, with published results of patients treated with systemic chemotherapy during the same period. Eleven patients underwent HAI chemotherapy with fluorouracil and oxaliplatin after placement of an HAI pump. A CT scan performed after the sixth cycle of therapy revealed that 5 of them had partial hepatic response (more than 45\ua0%), 2 stable disease and 4 showed clear signs of disease progression. The average survival of the entire group was 17.6\ua0months. Three of the patients with partial hepatic response underwent resection and 2 had more than 70\ua0% tumour necrosis, both of whom are still alive and disease-free. The median survival of patients with liver-only disease treated with systemic chemotherapy, who were not submitted for resection, was 15.3\ua0months. HAI chemotherapy enables this small group of patients to have their unresectable IH-CCA disease converted into a resectable one, thus confirming its role in treatment of this disease

    Synchronous polyps predict metachronous colorectal lesions after curative resection of colorectal cancer

    Background: The principal aim of endoscopic follow-up programs after curative resection of colorectal cancer (CRC) is to improve survival and identify local recurrence and metachronous CRC. The aim of our study was to identify the possible predictors of metachronous colorectal lesions. Methods: The records of 348 consecutive patients with CRC and who completed at least 1 year of endoscopic follow-up after surgery were analyzed. In this group, 336 patients underwent surgery for primitive CRC and 12 for metachronous cancer. Patients' characteristics, operative details, and endoscopical follow-up findings were retrieved. Multivariate survival analyses were used to identify patient categories at risk of metachronous colonic lesions. Results: 128 patients presented a metachronous lesion: 118 adenomas and 10 adenocarcinomas. At multivariate analysis, active smoke (HR = 1.84, p = 0.03), neoadjuvant therapy (HR = 0.24, p = 0.01), and presence of synchronous polyps (HR = 1.55, p = 0.04) resulted independent predictors of metachronous adenoma after CRC removal while neoadjuvant therapy (HR = 0.25, p = 0.02), active smoke (HR = 1.54, p = 0.04), and presence of synchronous polyps (HR = 1.86, p = 0.02) resulted independent predictors of metachronous lesions after CRC removal. Conclusions: This study demonstrated a high rate of metachronous lesions in the early follow-up after curative CRC resection. The negative effects of synchronous polyps should be carefully evaluated when planning patients' follow-up
