    Are the attitudes of exercise instructors who work with older adults influenced by training and personal characteristics?

    Little is known about the relationship between attitudes and characteristics of instructors and uptake and adherence of older people to exercise classes. This article explores these issues.Methods:The authors surveyed 731 UK exercise instructors with specialist older adult exercise qualifications. A questionnaire investigated instructors’ characteristics and attitudes toward older adults’ participation in exercise.Results:For mostly seated classes, EXTEND qualification (B = 0.36, p = .005) had a positive effect on instructors’ attitudes. Later Life Training qualification (B = −2.80, p = .003), clinical background (B = −3.99, p = .005), and delivering classes in National Health Services (B = −3.12, p &lt; .001), leisure centers (B = −2.75, p = .002), or nursing homes (B = −2.29, p = .005) had a negative effect on attitudes. For mostly standing classes, experience (B = 0.20, p = .003) and delivering in leisure centers (B = 0.46, p = .032) had a positive and clinical background (B = −1.78, p = .018) had a negative effect on instructors’ attitudes.Conclusions:Most instructors have positive attitudes, but training and work context can influence attitudes toward older people’s participation in exercise classes both positively and negatively.</jats:sec

    Randomised controlled trials of complex interventions and large-scale transformation of services

    Complex interventions and large-scale transformations of services are necessary to meet the health-care challenges of the 21st century. However, the evaluation of these types of interventions is challenging and requires methodological development. Innovations such as cluster randomised controlled trials, stepped-wedge designs, and non-randomised evaluations provide options to meet the needs of decision-makers. Adoption of theory and logic models can help clarify causal assumptions, and process evaluation can assist in understanding delivery in context. Issues of implementation must also be considered throughout intervention design and evaluation to ensure that results can be scaled for population benefit. Relevance requires evaluations conducted under real-world conditions, which in turn requires a pragmatic attitude to design. The increasing complexity of interventions and evaluations threatens the ability of researchers to meet the needs of decision-makers for rapid results. Improvements in efficiency are thus crucial, with electronic health records offering significant potential

    A Study of the Relationship of the Ectopic Maxillary Canine Which Has Been Displaced Palatally to the Morphology of the Adjacent Lateral Incisor

    The hypothesis that palatally displaced canines are associated with smaller than average lateral incisors with reduction of the root or tooth length, or with congenital absence of the adjacent lateral incisor was tested on a West of Scotland population. A retrospective study of the records of orthodontic patients attending Glasgow Dental Hospital was carried out. The palatal position of the canine was confirmed radiographically, or if erupted, on study casts. The tooth length of lateral and central incisors was measured on radiographs; the crown widths of lateral incisors was measured on study casts. An association was sought between the size of the lateral incisor or its absence, the position of the adjacent maxillary canine, and between crown size and tooth length of the lateral incisors. A separate study of extracted lateral incisors was undertaken further to test the hypothesis that narrow crowned lateral incisors have shorter roots than those with normally sized crowns. The conclusions supported the hypothesis that palatally displaced maxillary canines are associated with lateral incisors of smaller than average crown width. There was weak support for the association between palatal canines and absence of the adjacent lateral incisor, or between lateral incisor crown width and tooth length

    Attention control comparisons with SLT for people with aphasia following stroke: methodological concerns raised following a systematic review

    Objective: Attention control comparisons in trials of stroke rehabilitation require care to minimize the risk of comparison choice bias. We compared the similarities and differences in SLT and social support control interventions for people with aphasia. Data sources: Trial data from the 2016 Cochrane systematic review of SLT for aphasia after stroke. Methods: Direct and indirect comparisons between SLT, social support and no therapy controls. We double-data extracted intervention details using the template for intervention description and replication. Standardized mean differences and risk ratios (95% confidence intervals (CIs)) were calculated. Results: Seven trials compared SLT with social support (n  =  447). Interventions were matched in format, frequency, intensity, duration and dose. Procedures and materials were often shared across interventions. Social support providers received specialist training and support. Targeted language rehabilitation was only described in therapy interventions. Higher drop-out (P  =  0.005, odds ratio (OR) 0.51, 95% CI 0.32–0.81) and non-adherence to social support interventions (P  <  0.00001, OR 0.18, 95% CI 0.09–0.37) indicated an imbalance in completion rates increasing the risk of control comparison bias. Conclusion: Distinctions between social support and therapy interventions were eroded. Theoretically based language rehabilitation was the remaining difference in therapy interventions. Social support is an important adjunct to formal language rehabilitation. Therapists should continue to enable those close to the person with aphasia to provide tailored communication support, functional language stimulation and opportunities to apply rehabilitation gains.     Entrepreneurial orientation among arts managers in Western Australia

    As a region, Western Australia is the largest and most isolated state in Australia, and supports a community of vibrant Arts Organisations. The Arts is widely recognised for its creativity and innovation, but what about the managers of these organisations, are they equally innovative, or entrepreneurial? Rusak (2016) explored this question and found that their Entrepreneurial Orientation (EO) matched the three core dimensions of Innovativeness, Pro-activeness, and Risk-taking, but the study did not include the other two EO dimensions of Autonomy and Competitive Aggressiveness. It did however observe that arts companies don’t generally try to take offensive postures or aggressive responses to competitive threats and rather work collaboratively, as this sample shows . This assertion was not the focus of the article, nor was it explored in any depth in that paper. There are at least two possibilities here: it could be a passive aversion to competitive aggression, or a more deliberate counter-behaviour of collaboration. Either of these would appear to contradict the EO construct, in particular the expectation that all EO dimensions covary, which makes it interesting from a theory perspective. This paper explores this challenge to the EO theory in some detail, using software-aided analysis to tease out the finer nuances in this dimension of Competitive Aggressiveness. While the sample size and its geographical confines limit the generalisations that can be made, there is solid evidence that in this sample of Arts Managers, the Arts acts as a powerful contextual modifier to the expectations of EO theory. The dimension of Competitive Aggressiveness has not simply been altered or toned-down by this context, it has been replaced by a polar opposite

    Teaching and learning in virtual worlds: is it worth the effort?

    Educators have been quick to spot the enormous potential afforded by virtual worlds for situated and authentic learning, practising tasks with potentially serious consequences in the real world and for bringing geographically dispersed faculty and students together in the same space (Gee, 2007; Johnson and Levine, 2008). Though this potential has largely been realised, it generally isn’t without cost in terms of lack of institutional buy-in, steep learning curves for all participants, and lack of a sound theoretical framework to support learning activities (Campbell, 2009; Cheal, 2007; Kluge & Riley, 2008). This symposium will explore the affordances and issues associated with teaching and learning in virtual worlds, all the time considering the question: is it worth the effort
