The development of a risk score for unplanned removal of peripherally inserted central catheter in newborns
;;OBJETIVO:;; elaborar un puntaje de riesgo para remoción no electiva del catéter central de inserción periférica en neonatos.;;;;MÉTODO:;; estudio de cohorte prospectivo realizado en una unidad de terapia intensiva neonatal con recién nacidos sometidos a instalación de 524 catéteres centrales de inserción periférica. Las características clínicas del neonato, la técnica de inserción del catéter y la terapia intravenosa fueron verificadas como factores de riesgo para remoción no electiva del catéter en un análisis bivariado. El puntaje de riesgo fue elaborado a partir de regresión logística, fue validado internamente y su precisión evaluada por medio de la área bajo la curva receiver operating characteristic.;;;;RESULTADOS:;; el puntaje de riesgo fue compuesto por los factores de riesgo: diagnóstico de trastorno transitorio del metabolismo, inserción previa del catéter, uso de catéter doble lumen de poliuretano, infusión de múltiples soluciones intravenosas a través de catéter mono lumen, y posición no central de la punta del catéter. O escore de risco foi elaborado a partir da regressão logística, validado internamente e sua acurácia avaliada por meio da área sob a curva receiver operating characteristic.;;;;RESULTADOS:;; o escore de risco foi composto pelos fatores de risco: diagnóstico de transtorno transitório do metabolismo, inserção prévia do cateter, uso de cateter duplo lúmen de poliuretano, infusão de múltiplas soluções endovenosas através de cateter mono lúmen e posição não central da ponta do cateter. Extinction risk of Mesoamerican crop wild relatives
Ensuring food security is one of the world's most critical issues as agricultural systems are already being impacted by global change. Crop wild relatives (CWR)—wild plants related to crops—possess genetic variability that can help adapt agriculture to a changing environment and sustainably increase crop yields to meet the food security challenge.
Here we report the results of an extinction risk assessment of 224 wild relatives of some of the world's most important crops (i.e. chilli pepper, maize, common bean, avocado, cotton, potato, squash, vanilla and husk tomato) in Mesoamerica—an area of global significance as a centre of crop origin, domestication and of high CWR diversity.
We show that 35% of the selected CWR taxa are threatened with extinction according to The International Union for Conservation of Nature (IUCN) Red List demonstrates that these valuable genetic resources are under high anthropogenic threat. The dominant threat processes are land use change for agriculture and farming, invasive and other problematic species (e.g. pests, genetically modified organisms) and use of biological resources, including overcollection and logging. The most significant drivers of extinction relate to smallholder agriculture—given its high incidence and ongoing shifts from traditional agriculture to modern practices (e.g. use of herbicides)—smallholder ranching and housing and urban development and introduced genetic material.
There is an urgent need to increase knowledge and research around different aspects of CWR. Policies that support in situ and ex situ conservation of CWR and promote sustainable agriculture are pivotal to secure these resources for the benefit of current and future generations
Insertion, maintenance and removal procedures of peripherally inserted central catheters in neonates
O cateter central de inserção periférica (PICC) é um dispositivo cada vez mais utilizado nas Unidades de Terapia Intensiva Neonatal (UTIN). O objetivo do estudo foi caracterizar os neonatos (RN) submetidos ao procedimento de inserção do cateter PICC e descrever suas práticas de inserção, manutenção e remoção em RN. Estudo observacional com delineamento longitudinal realizado no Berçário Anexo à Maternidade do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. Os dados foram obtidos pela observação dos procedimentos e informações registradas nos prontuários dos neonatos submetidos ao procedimento. No estudo, incluíram-se todos os procedimentos de inserção, manutenção e remoção de cateter PICC ocorridos entre de março e setembro de 2006, em recém-nascidos internados na unidade neonatal citada. Antes do início da coleta dos dados, o projeto de pesquisa foi apreciado e aprovado pelo Comitê de Ética em Pesquisa da Instituição, campo do estudo. No período estudado, ocorreram 37 procedimentos de inserção do cateter PICC. A média da idade gestacional e o peso dos RN eram de 32,2 semanas e 1.289,2 gramas, respectivamente. A maioria, 22 (59,4%) RN, era do sexo masculino, 18 (48,7%) RN tinham menos de três dias de vida no dia do procedimento e 35 (94,6%) RN foram submetidos à inserção do cateter para infundir nutrição parenteral total. A maioria, 21 (56,8%) RN, apresentou diagnóstico de síndrome do desconforto respiratório. A administração de fármaco sedativo previamente ao procedimento ocorreu em 4 (10,8%), nenhum RN recebeu analgesia. O tipo de cateter mais utilizado foi o de poliuretano, 35 (94,6%). A média da freqüência de punções venosas foi 3,4 e em 8 (21,6%) RN foram obtidos sucesso na introdução do cateter na primeira punção venosa. As veias mais puncionadas foram as localizadas em membros superiores, a basílica foi puncionada em 29,9% dos RN e a cefálica, em 22,8% dos RN. Fragilidade vascular, transfixação venosa e obstrução do cateter foram os principais motivos de insucesso na inserção do PICC. Alteração da viscosidade sangüínea foi uma intercorrência identificada pelas enfermeiras na inserção do PICC. A prevalência de sucesso do procedimento foi de 64,9% (24 RN). Obteve-se posicionamento central da ponta do cateter em 20 (83,3%) RN e periférico em 4 (16,7%) RN. A média da extensão do cateter introduzido em MSD foi de 11,4 cm, em MSE, 13,5cm e em região cervical, 7,1 cm. Solução de clorexidina a 0,5% foi o anti-séptico mais utilizado nos curativos e a NPT foi a solução mais infundida pelo cateter. O tempo médio de permanência do cateter foi 8,9 dias, 11 (27,5%) foram removidos em decorrência de infecção do cateter, 7 (17,5%) pelo término da terapia intravenosa e 7 (17,5%) por obstrução. Dos 24 cateteres removidos, 14 (58,3%) foram enviados para cultura, dos quais, 10 (71,4%) tiveram resultado negativo. Das quatro pontas com resultado positivo, em duas (14,3%) foram identificados Estafilococos coagulase negativaThe peripherally inserted central catheter (PICC) are increasingly employed in the neonatal intensive care units. The aims of the study were to characterize the neonates who underwent PICC catheter insertion and to describe insert, upkeep and removal PICC catheter practices in neonates. Prospective cohort study carried out at Neonatal Intensive Care Unit of University of São Paulo School Medical Hospital. Data were obtained by nurse procedures performance observation and from the medical chart records of the neonates who underwent PICC catheter insertion. Data were collected from March to September, 2006 and all the neonates underwent PICC catheter insertion during this period were included in the study. The study protocol was approved by the Research Ethics Board of the Hospital where the study was carried out. Written informed consents were obtained from parents or legal guardian of all infant newborns underwent PICC catheter insertion and from certified registered nurses responsible for catheter insertion, maintenance of the line and for removal it. It was obtained data from 32 infants newborn underwent procedure of PICC catheter insertion. The gestacional age and infant weight mean were, 32.2 weeks and 1289.2 g, respectively; 22 (59.4%) neonates were male, the PICC catheter was inserted in 18 (48.7%) neonates in the first three days of life and 35 (94.6%) babies were underwent PICC insertion to provide total parenteral nutrition; 21 (56.8%) neonates had diagnosis of respiratory distress syndrome, any of them received analgesics and 4 (10,8%) newborns received sedation. The polyurethane catheter was more used, 35 (94,6%) than the silicone catheter, 2 (5,4%). The mean of venous puncture frequency was of 3.4 and the rate success obtained in the first attempt was 21,6% (8 newborn). The most accessed veins for insertion PICC lines were basilic, 29,9% and cephalic, 22,8%. Vascular fragility, venous transfixation and obstruction were the majorities causes of failure to insert the PICC line. The frequency of success on PICC line insertion was 64,9% (24 neonates). The position of tip catheter in 83,3% (20 neonates) was central (superior vena cava), others 16,7% (4 neonates) tips was peripherally. The length of catheter line mean to thread in right arm veins was 11,4 cm, in the left arm,13,5cm and in the jugular vein was 7,1 cm. It was used chlorhexidine 0.5% antiseptic solution in the majority of catheter insertion site dressing. Total parenteral nutrion was the soluction infused by the catheter line. The mean time of remaining the catheter was 8.9 days, 11(27,5%) catheters were withdrawn due to suspect of catheter infection, 7(17,5%) catheters withdrawn after end the intravenous therapy and 7 (17,5%) due to obstruction. From 24 catheters withdrawn, 14 (58,3%) tip catheters were underwent to microbyological analysis and the results were negative for 10 (71,4%) analysis, from others four tips catheter which results were positive, in two (14,3%) identified coagulase-negative staphylococc

Analgesia e sedacao durante a instalacao do cateter central de insercao periferica em neonatos
This study aimed to characterize the analgesia and sedation strategies in neonates having a peripherally inserted central catheter (PICC) placed, and to relate it to the number of venipunctures, duration of procedure and catheter tip position. This was a cross-sectional study with prospective data collection, conducted in a neonatal intensive care unit of a private hospital in the city of São Paulo, during the period from August 31, 2010 to July 1, 2011, which evaluated 254 PICC insertions. The adoption of analgesic or sedative strategies occurred in 88 (34.6%) catheter placements and was not related to the number of venipunctures, duration of procedure or catheter tip position. Intravenous administration of midazolam, in 47 (18.5%), and fentanyl, in 19 (7.3%), catheter insertions were the most frequent strategies. Wider adoption of analgesic strategies is recommended before, during and after the procedure.El objetivo fue caracterizar las estrategias de la analgesia y sedación en neonatos sometidos a la instalación del catéter venoso central de inserción periférica (CCIP) y relacionarlas con el número de punciones venosas, duración del procedimiento y la posición de la punta del catéter. Estudio transversal con recolección prospectiva de datos conducido en unidad de cuidados intensivos neonatal de un hospital privado en la ciudad de Sao Paulo; realizado en el periodo del 31 de agosto de 2010 al 01 de julio de 2011 donde fueron evaluadas 254 instalaciones del CCIP. La adopción de estrategias analgésicas o sedación ocurrió en 88 (34,6%) instalaciones de catéter y no estuvo relacionada con el número de punciones venosas, duración del procedimiento o la posición de la punta del catéter. Las estrategias más utilizadas fueron la administración intravenosa de Midazolam en 47 (18,5%) y Fentanil en 19 (7,3%) respectivamente. Breast Milk and Glucose for Pain Relief in Preterm Infants: A Noninferiority Randomized Controlled Trial
OBJECTIVE: The study goal was to compare the efficacy of expressed breast milk (EBM) versus 25% glucose on pain responses of late preterm infants during heel lancing. METHODS: In a noninferiority randomized controlled trial, a total of 113 newborns were randomized to receive EBM (experimental group [EG]) or 25% glucose (control group [CG]) before undergoing heel lancing. The primary outcome was pain intensity (Premature Infant Pain Profile [PIPP]) and a 10% noninferiority margin was established. Secondary outcomes were incidence of cry and percentage of time spent crying and adverse events. Intention-to-treat (ITT) analysis was used. RESULTS: Groups were similar regarding demographics and clinical characteristics, except for birth weight and weight at data collection day. There were lower pain scores in the CG over 3 minutes after lancing (P<.001). A higher number of infants in the CG had PIPP scores indicative of minimal pain or absence of pain (P = .002 and P = .003 on ITT analysis) at 30 seconds after lancing, and the mean difference in PIPP scores was 3 (95% confidence interval: 1.507-4.483). Lower incidence of cry (P = .001) and shorter duration of crying (P = .014) were observed for CG. Adverse events were benign and self-limited, and there was no significant difference between groups (P = .736 and P = .637 on ITT analysis). CONCLUSIONS: Results based on PIPP scores and crying time indicate poorer effects of EBM compared with 25% glucose during heel lancing. Additional studies exploring the vol and administration of EBM and its combination with other strategies such as skin-to-skin contact and sucking are necessary. Pediatrics 2012;129:664-67
Neotropical freshwater fisheries : A dataset of occurrence and abundance of freshwater fishes in the Neotropics
The Neotropical region hosts 4225 freshwater fish species, ranking first among the world's most diverse regions for freshwater fishes. Our NEOTROPICAL FRESHWATER FISHES data set is the first to produce a large-scale Neotropical freshwater fish inventory, covering the entire Neotropical region from Mexico and the Caribbean in the north to the southern limits in Argentina, Paraguay, Chile, and Uruguay. We compiled 185,787 distribution records, with unique georeferenced coordinates, for the 4225 species, represented by occurrence and abundance data. The number of species for the most numerous orders are as follows: Characiformes (1289), Siluriformes (1384), Cichliformes (354), Cyprinodontiformes (245), and Gymnotiformes (135). The most recorded species was the characid Astyanax fasciatus (4696 records). We registered 116,802 distribution records for native species, compared to 1802 distribution records for nonnative species. The main aim of the NEOTROPICAL FRESHWATER FISHES data set was to make these occurrence and abundance data accessible for international researchers to develop ecological and macroecological studies, from local to regional scales, with focal fish species, families, or orders. We anticipate that the NEOTROPICAL FRESHWATER FISHES data set will be valuable for studies on a wide range of ecological processes, such as trophic cascades, fishery pressure, the effects of habitat loss and fragmentation, and the impacts of species invasion and climate change. Colorectal cancer incidences in Lynch syndrome: a comparison of results from the prospective lynch syndrome database and the international mismatch repair consortium
Abstract Objective To compare colorectal cancer (CRC) incidences in carriers of pathogenic variants of the MMR genes in the PLSD and IMRC cohorts, of which only the former included mandatory colonoscopy surveillance for all participants. Methods CRC incidences were calculated in an intervention group comprising a cohort of confirmed carriers of pathogenic or likely pathogenic variants in mismatch repair genes (path_MMR) followed prospectively by the Prospective Lynch Syndrome Database (PLSD). All had colonoscopy surveillance, with polypectomy when polyps were identified. Comparison was made with a retrospective cohort reported by the International Mismatch Repair Consortium (IMRC). This comprised confirmed and inferred path_MMR carriers who were first- or second-degree relatives of Lynch syndrome probands. Results In the PLSD, 8,153 subjects had follow-up colonoscopy surveillance for a total of 67,604 years and 578 carriers had CRC diagnosed. Average cumulative incidences of CRC in path_MLH1 carriers at 70 years of age were 52% in males and 41% in females; for path_MSH2 50% and 39%; for path_MSH6 13% and 17% and for path_PMS2 11% and 8%. In contrast, in the IMRC cohort, corresponding cumulative incidences were 40% and 27%; 34% and 23%; 16% and 8% and 7% and 6%. Comparing just the European carriers in the two series gave similar findings. Numbers in the PLSD series did not allow comparisons of carriers from other continents separately. Cumulative incidences at 25 years were < 1% in all retrospective groups. Conclusions Prospectively observed CRC incidences (PLSD) in path_MLH1 and path_MSH2 carriers undergoing colonoscopy surveillance and polypectomy were higher than in the retrospective (IMRC) series, and were not reduced in path_MSH6 carriers. These findings were the opposite to those expected. CRC point incidence before 50 years of age was reduced in path_PMS2 carriers subjected to colonoscopy, but not significantly so
