    Electronic Appraisal Methodology for Right-of-Way Acquisition in Highway Projects

    When right-of-way is acquired for highway projects, state departments of transportation (DOTs) must ensure property owners receive a fair market value for their land by delivering high-quality appraisals. Despite recent technological advances, the highly complicated appraisal process often results in similar properties being assessed differently. Several DOTs sponsored a study to develop an electronic appraisal method to improve the appraisal process and to reduce the likelihood of inconsistent appraisal values by capturing, transmitting, storing, managing, and analyzing the appraisal data. The proposed method’s framework is discussed and a prototype of the system has been developed to demonstrate its features

    An investigation of the factors effecting high-risk individuals' decision-making about prophylactic total gastrectomy and surveillance for hereditary diffuse gastric cancer (HDGC).

    Hereditary diffuse gastric cancer has an early onset and poor prognosis, therefore, individuals who carry a pathogenic (CDH1) mutation in the E-cadherin gene (CDH1) are offered endoscopic surveillance and advised to undergo prophylactic total gastrectomy (PTG) in their early to mid-twenties. Patients not ready or fit to undergo gastrectomy, or in whom the genetic testing result is unknown or ambiguous, are offered surveillance. Little is known about the factors that influence decisions to undergo or decline PTG, making it difficult to provide optimal support for those facing these decisions. Qualitative interviews were carried out with 35 high-risk individuals from the Familial Gastric Cancer Study in the UK. Twenty-seven had previously undergone PTG and eight had been identified as carrying a pathogenic CDH1 mutation but had declined surgery at the time of interview. The interviews explored the experience of decision-making and factors influencing risk-management decisions. The data suggest that decisions to proceed with PTG are influenced by a number of potentially competing factors: objective risk confirmation by genetic testing and/or receiving a positive biopsy; perceived familial cancer burden and associated risk perceptions; perceptions of post-surgical life; an increasing inability to tolerate endoscopic procedures; a concern that surveillance could miss a cancer developing and individual's life stage.     Development of Best Practices for Right-of-Way Valuations and Negotiations in Transportation Projects

    The valuations of properties and the negotiations with property owners are two major tasks in the right-of-way acquisition process for transportation projects. If improved, those tasks can increase the overall project delivery efficiency. The Texas Department of Transportation (TxDOT) funded a research project that aimed to recommend some best practices for successful valuations and negotiations in Texas. The authors reviewed the different strategies and procedures followed in TxDOT by conducting interviews and surveys with right-of-way personnel. Guidelines supported by recommended practices were identified for both valuation and negotiation and then reviewed and validated by experts. Lastly, implementation guides were developed

    A metadata-aware application for remote scoring and exchange of tissue microarray images.

    BACKGROUND: The use of tissue microarrays (TMA) and advances in digital scanning microscopy has enabled the collection of thousands of tissue images. There is a need for software tools to annotate, query and share this data amongst researchers in different physical locations. RESULTS: We have developed an open source web-based application for remote scoring of TMA images, which exploits the value of Microsoft Silverlight Deep Zoom to provide a intuitive interface for zooming and panning around digital images. We use and extend existing XML-based standards to ensure that the data collected can be archived and that our system is interoperable with other standards-compliant systems. CONCLUSION: The application has been used for multi-centre scoring of TMA slides composed of tissues from several Phase III breast cancer trials and ten different studies participating in the International Breast Cancer Association Consortium (BCAC). The system has enabled researchers to simultaneously score large collections of TMA and export the standardised data to integrate with pathological and clinical outcome data, thereby facilitating biomarker discovery.RIGHTS : This article is licensed under the BioMed Central licence at http://www.biomedcentral.com/about/license which is similar to the 'Creative Commons Attribution Licence'.     Drenagem subterrânea no perimetro irrigado de Maniçoba: critérios de dimensionamento e avaliação de performance

    This study was carried out on a parallel subsurface pipe drainage system (1.5 ha) installed for this purpose in a Class 3 soil plot of Irrigation Project Maniçoba, BA, Brazil, in order to determine the applicability of drain spacing equations, check some soil hydrological parameters and adjust design criteria. Drain spacing and combination of envelope materials were the variables considered. The drains spacing (L.) was calculated based on parameters determined in situ and using equations of Hooghoudt and Glover-Dumm. Twice the calculated spacing was considered for variation of this parameter. Depth of water table and rain discharges as response to recharge (surface irrigation or rainfall) were registered for drains spaced L = 15 m and L = 30 m also monitoring soil moisture. Eventually, chemical composition and sediments content of drainage water was determined in the laboratory. Results of this study show that the L = 30 m spacing kept better soil moisture regime in the profile. The peak instantaneous drain discharge was 8 to 10 mm/day falling to 4 mm/day or less three days after recharge took place. The mean water table depth during the study was 0.6 m to 1.0 m below ground surface for L = 30 m and 1.2 m to 1.5 m for L = 15 m. Contribution of soil layers below drain level to underground flow was found negligible. Auger hole method proved a very precise one for the determination of hydraulic conductivity in situ. Hooghoudt equation overestimated the spacing in 24% and Glover-Dumm equation underestimated it in 30%. Drainable porosity values found were 7.8% and 24% and drainage intensity factor determined varied from 0.15 to 0.21. Os trabalhos foram conduzidos num sistema de drenagem subterrânea entubado (1,5 ha) instalado nos solos classe 3 do Perímetro Irrigado de Maniçoba para observar a aplicabilidade das equações de espaçamento de drenos usadas, para aferir alguns parâmetros hidrológicos dos solos e para ajustar parâmetros de dimensionamento. Foram consideradas variáveis, o espaçamento entre drenos e uma combinação de materiais de envoltura. O espaçamento entre drenos (L.) foi calculado com base em parâmetros determinados in loco e usando as equações de Hooghoudt e de Glover e Dumm. O duplo do espaçamento calculado foi considerado como variação deste parâmetro. As variações do lençol freático e as vazões dos drenos como resposta às recargas de irrigação superficial (sulcos) e chuva, foram levantadas para drenos paralelos espaçados de 15 m a 30 m, acompanhado os regimes de umidade no perfil do solo. Ademais, foram determinadas em análise de laboratório, as características químicas e a quantidade de sedimentos das águas de drenagem. Resultados do estudo mostraram que o espaçamento L = 30 m manteve melhor regime de umidade no perfil do solo. A descarga máxima instantânea dos drenos é de 8 a 10 mm/dia, caindo a 4 mm/dia ou menos no terceiro dia após a recarga. O lençol freático manteve-se entre 0,6 m e 1,0 m sob a superfície na área com L = 30 e entre 1,2 m e 1,5 m sob a área com L = 15 m. Demonstrou-se a contribuição desprezível da camada abaixo dos drenos ao fluxo subterrâneo, e a precisão do método do furo de trado (auger hole) na determinação da condutividade hidráulica. A fórmula de Hooghoudt sobreestima o espaçamento em 24% e a de Glover & Dumm os subestima em 30%. Os valores de porosidade drenável obtidos foram 7,8% e 24% e do fator de intensidade de drenagem 0,15 e 0,21.

    Combined quantitative measures of ER, PR, HER2, and KI67 provide more prognostic information than categorical combinations in luminal breast cancer.

    Although most women with luminal breast cancer do well on endocrine therapy alone, some will develop fatal recurrence thereby necessitating the need to prospectively determine those for whom additional cytotoxic therapy will be beneficial. Categorical combinations of immunohistochemical measures of ER, PR, HER2, and KI67 are traditionally used to classify patients into luminal A-like and B-like subtypes for chemotherapeutic reasons, but this may lead to the loss of prognostically relevant information. Here, we compared the prognostic value of quantitative measures of these markers, combined in the IHC4-score, to categorical combinations in subtypes. Using image analysis-based scores for all four markers, we computed the IHC4-score for 2498 patients with luminal breast cancer from two European study populations. We defined subtypes (A-like (ER + and PR + : and HER2- and low KI67) and B-like (ER + and/or PR + : and HER2 + or high KI67)) by combining binary categories of these markers. Hazard ratios and 95% confidence intervals for associations with 10-year breast cancer-specific survival were estimated in Cox proportional-hazard models. We accounted for clinical prognostic factors, including grade, tumor size, lymph-nodal involvement, and age, by using the PREDICT-score. Overall, Subtypes [hazard ratio (95% confidence interval) B-like vs. A-like = 1.64 (1.25-2.14); P-value < 0.001] and IHC4-score [hazard ratio (95% confidence interval)/1 standard deviation = 1.32 (1.20-1.44); P-value < 0.001] were prognostic in univariable models. However, IHC4-score [hazard ratio (95% confidence interval)/1 standard deviation = 1.24 (1.11-1.37); P-value < 0.001; likelihood ratio chi-square (LRχ2) = 12.5] provided more prognostic information than Subtype [hazard ratio (95% confidence interval) B-like vs. A-like = 1.38 (1.02-1.88); P-value = 0.04; LRχ2 = 4.3] in multivariable models. Further, higher values of the IHC4-score were associated with worse prognosis, regardless of subtype (P-heterogeneity = 0.97). These findings enhance the value of the IHC4-score as an adjunct to clinical prognostication tools for aiding chemotherapy decision-making in luminal breast cancer patients, irrespective of subtype

    Usefulness of manufactured tomato extracts in the diagnosis of tomato sensitization: Comparison with the prick-prick method

    <p>Abstract</p> <p>Background</p> <p>Commercial available skin prick test with fruits can be negative in sensitized or allergic patients due to a reduction in biological activity during the manufacturing process. Prick-prick tests with fresh foods are often preferred, but they are a non-standardized procedure. The usefulness of freeze-dried extracts of Canary Islands tomatoes, comparing the wheal sizes induced by prick test with the prick-prick method in the diagnosis of tomato sensitization has been analyzed.</p> <p>The objective of the study was to assess the potential diagnostic of freeze-dried extracts of Canary Islands tomatoes, comparing the wheal sizes induced by prick test with the prick-prick method.</p> <p>Methods</p> <p>Two groups of patients were analyzed: Group I: 26 individuals reporting clinical symptoms induced by tomato contact or ingestion. Group II: 71 control individuals with no symptoms induced by tomato: 12 of them were previously skin prick test positive to a tomato extract, 39 were atopic and 20 were non-atopic. All individuals underwent prick-prick with fresh ripe peel Canary tomatoes and skin prick tested with freeze-dried peel and pulp extracts obtained from peel and pulp of Canary tomatoes at 10 mg/ml. Wheal sizes and prick test positivity (≥ 7 mm<sup>2</sup>) were compared between groups.</p> <p>Results</p> <p>In group I, 21 (81%) out of 26 patients were prick-prick positive. Twenty patients (77%) had positive skin prick test to peel extracts and 12 (46%) to pulp extracts. Prick-prick induced a mean wheal size of 43.81 ± 40.19 mm<sup>2 </sup>compared with 44.25 ± 36.68 mm<sup>2 </sup>induced by the peel extract (Not significant), and 17.79 ± 9.39 mm<sup>2 </sup>induced by the pulp extract (p < 0.01).</p> <p>In group II, 13 (18%) out of 71 control patients were prick-prick positive. Twelve patients (all of them previously positive to peel extract) had positive skin prick test to peel and 3 to pulp. Prick-prick induced a mean wheal size of 28.88 ± 13.12 mm<sup>2 </sup>compared with 33.17 ± 17.55 mm<sup>2 </sup>induced by peel extract (Not significant), and 13.33 ± 4.80 mm<sup>2 </sup>induced by pulp extract (p < 0.05 with peel extract and prick-prick).</p> <p>Conclusion</p> <p>Canary peel tomato extract seems to be as efficient as prick-prick tests with ripe tomatoes to diagnose patients sensitized to tomato. The wheal sizes induced by prick-prick and peel extracts were very similar and showed a high correlation coefficient.</p

    Genome-driven integrated classification of breast cancer validated in over 7,500 samples

    Abstract Background: IntClust is a classification of breast cancer comprising 10 subtypes based on molecular drivers identified through the integration of genomic and transcriptomic data from 1,000 breast tumors and validated in a further 1,000. We present a reliable method for subtyping breast tumors into the IntClust subtypes based on gene expression and demonstrate the clinical and biological validity of the IntClust classification. Results: We developed a gene expression-based approach for classifying breast tumors into the ten IntClust subtypes by using the ensemble profile of the index discovery dataset. We evaluate this approach in 983 independent samples for which the combined copy-number and gene expression IntClust classification was available. Only 24 samples are discordantly classified. Next, we compile a consolidated external dataset composed of a further 7,544 breast tumors. We use our approach to classify all samples into the IntClust subtypes. All ten subtypes are observable in most studies at comparable frequencies. The IntClust subtypes are significantly associated with relapse-free survival and recapitulate patterns of survival observed previously. In studies of neo-adjuvant chemotherapy, IntClust reveals distinct patterns of chemosensitivity. Finally, patterns of expression of genomic drivers reported by TCGA (The Cancer Genome Atlas) are better explained by IntClust as compared to the PAM50 classifier
