    The Application of Geospatial Techniques in Flood Risk and Vulnerability Mapping for Disaster Management at Lokoja, Kogi State, Nigeria

    A flood risk and vulnerability assessment of Lokoja town in Nigeria was investigated. The objective is to identify the risk lives and vulnerability level of the settlement to flood disasters.  Geospatial techniques using remote sensing data and GIS was utilised in the analysis. Results indicate that the substantial part of the settlement falls within the high flood risk zone. The mapping identified three zones with respect to vulnerability level.  Buildings within 30 meters buffer distance from the river banks and flood plains are classified as highly vulnerable, 50 meters buffer distance is moderately vulnerable and 70 meters above as safe zones. Building challenges, rainfall impact and water releases from hydropower dam were identified as major causes of floods in the study area. It is recommended that continuous inventorying of hydroclimatic variables at dam reservoirs be intensified and adherence to site selection for building should continuously be enforced to reduce the risk levels and safeguard the settlement from flood disasters.     The low fertility status of most tropical soils has hindered maize production as maize has a strong exhausting effect on the soil. It has been generally observed that maize fails to produce good grain in plots without adequate nutrients (Adediran and Banjoko 2003). Legumes generally are able to fix nitrogen to the soil which the companion crop could benefit from, but usually the nitrogen fixed occurs in insufficient quantities for marginally nitrogen sensitive crops such as maize and hence the required use of fertilizers. Inorganic fertilizers exert strong influences on plant growth, development and yield (Stefano et al. 2004). The use of legumes in mixed cropping systems is one of the traditional soil-fertility maintenance strategies. Legume-cereal intercropping often increases the quantity and quality of residues, which could improve livestock production considerably in addition to benefits in soil fertility but may decrease the yield of the companion cereal crop (Nandi and Haque 1986). Although the beneficial effects of intercropping legumes have been demonstrated by intercropping cereals with a few legumes such as cowpea, lablab, soybean etc, the same cannot be said for all legumes.     Emergence of Clinical Clostridioides Difficile Isolates With Decreased Susceptibility to Vancomycin

    BACKGROUND: Clostridioides difficile infection (CDI) is a leading cause of hospital-associated antibiotic-related diarrhea and deaths worldwide. Vancomycin is one of the few antibiotics recommended for both nonsevere and severe CDI cases. We sought to determine whether vancomycin nonsusceptible C. difficile strains are circulating in the patient population. METHODS: Stool samples from patients with CDI were collected from 438 and 98 patients at a large university hospital in Houston, Texas, and Nairobi, Kenya, respectively. The stools were examined for the presence of vancomycin and metronidazole nonsusceptible C. difficile using broth dilution culture, Etest (BioMĂ©rieux, France), polymerase chain reaction (PCR), whole-genome sequencing, and in vivo testing in a CDI mouse model. RESULTS: Of the Houston stool samples, 114/438 (26%) had vancomycin nonsusceptible C. difficile isolates and 128/438 (29%) were metronidazole nonsusceptible. Similarly, 66 out of 98 (67%) and 83/98 (85%) of the Nairobi patients harbored vancomycin and metronidazole nonsusceptible isolates, respectively. Vancomycin treatment of a CDI mouse model infected with a vancomycin nonsusceptible isolate failed to eradicate the infection. Whole-genome sequencing analyses did not identify vanA genes, suggesting a different mechanism of resistance. CONCLUSIONS: C. difficile strains exhibiting reduced susceptibility to vancomycin are currently circulating in patient populations. The spread of strains resistance to vancomycin, a first-line antibiotic for CDI, poses a serious therapeutic challenge. Routine susceptibility testing may be necessary

    Evidence-based assessment of male-only infertility: prevalence and associated risk factors in Port Harcourt metropolis, Rivers State, Nigeria

    Background: Male infertility is the condition in which a male is unable to establish pregnancy in a fertile woman over 12 months of unprotected sexual intercourse. In this study, the prevalence of male-factor infertility and some associated risk factors in Port Harcourt, Rivers State was carried out. Methods: The study design was a case-controlled randomized one, in which semen specimens were collected from case and control groups randomly amongst males visiting urology/fertility clinics by masturbation after 3 days of abstinence. A total of 276 males indicated interest to participate in study of which 193 male subjects were recruited. Results: The result showed that 20.8% were azoospermic, 27.4% were oligospermic, 23.7% were asthenozoospermic, 27.9% oligoasthenozoospermic, 15.1% teratozoospermic, 19.4% asthenoteratozoospermic, and 12.9% oligoasthenoteratospermic. Furthermore, the microbial quality of the semen assessed indicated the prevalence of scanty, moderate, and heavy growth as 12.5%, 9.3%, and 7.3% respectively. Likewise, organisms isolated and identified were Staphylococcus aureus, Escherichia coli, Klebsiella pneumoniae, and mixed growth of staphylococcus aureus, and Escherichia coli with a prevalence of 18.2%, 5.6%, 2.0%, 1.04%, and 2.6% respectively. Civil servants had the highest prevalence of 20.8% followed by artisans with 11.9%. The prevalence of primary and secondary infertility was observed to be 30.1% and 18.1% respectively. Conclusions: Male-only factor infertility is on the increase and occupations that are accompanied by prolonged sitting, sedentary work style, or working in or close to high-temperature sources as seen in civil servants and welders (artisans) were observed to be more prone to male-only factor infertility

    Socioeconomic dynamism and the growth of baby factories in Nigeria

    Abstract: Illegally breeding babies for marketing purposes otherwise known as “infant commodification” is increasing in Nigeria. This menace is a vice that threatens the lives and wellbeing of babies, young girls, and women. This article investigates through an in-depth review of scholarly publication and media coverages the factors that have contributed to the emergence and growth of the illicit industry in Nigeria. It examines the role of cultural beliefs, social attitudes, and norms as well as the harsh economic conditions of the nation as factors playing pivotal roles in the continual growth of baby farming in Nigeria. Some of these sociocultural factors are social stigmatization of pregnancy outside wedlock, stigmatization of adopted children, and the importance attached to fertility and the demonization of childlessness. The article draws on Emile Durkheim anomie theory and Chambers dimensions of poverty to explain how breakdown within the nation’s socioeconomic structure has a concomitant impact in breading social vices. It concludes by recommending that there is a need to jettison stigmatizing sociocultural beliefs within the nation’s social fabrics. Responsive attitude toward family reproductive issues should be encouraged and finally the activities of health care providers, operators of nongovernmental organizations taking custody of babies and young girls be properly monitored. As well, stringent punishment be meted out to apprehended operators of these baby farms to serve as deterrent to others

    Clinical features, predictive factors and outcome of hyperglycaemic emergencies in a developing country

    <p>Abstract</p> <p>Background</p> <p>Hyperglycaemic emergencies are common acute complications of diabetes mellitus (DM) but unfortunately, there is a dearth of published data on this entity from Nigeria. This study attempts to describe the clinical and laboratory scenario associated with this complication of DM.</p> <p>Methods</p> <p>This study was carried out in DM patients who presented to an urban hospital in Nigeria with hyperglycaemic emergencies (HEs). The information extracted included biodata, laboratory data and hospitalization outcome. Outcome measures included mortality rates, case fatality rates and predictive factors for HEs mortality. Statistical tests used are <it>χ</it><sup>2</sup>, Student's t test and logistic regression.</p> <p>Results</p> <p>A total of 111 subjects with HEs were recruited for the study. Diabetes ketoacidosis (DKA) and hyperosomolar hyperglycaemic state (HHS) accounted for 94 (85%) and 17 (15%) respectively of the HEs. The mean age (SD) of the subjects was 53.9 (14.4) years and their ages ranged from 22 to 86 years. DKA occurred in all subjects with type 1 DM and 73 (81%) of subjects with type 2 DM. The presence of HSS was noted in 17 (19%) of the subjects with type 2 DM.</p> <p>Hypokalaemia (HK) was documented in 41 (37%) of the study subjects. Elevated urea levels and hyponatraemia were noted more in subjects with DKA than in those subjects with HHS (57.5%,19% vs 53%,18%). The mortality rate for HEs in this report is 20% and the case fatality rates for DKA and HHS are 18% and 35% respectively.</p> <p>The predictive factors for HEs mortality include, sepsis, foot ulceration, previously undetected DM, hypokalaemia and being elderly.</p> <p>Conclusion</p> <p>HHS carry a higher case fatality rate than DKA and the predictive factors for hyperglycaemic emergencies' mortality in the Nigerian with DM include foot ulcers, hypokalaemia and being elderly.</p

    Malariometric indices among Nigerian children in a rural setting

    Malaria contributes to high childhood morbidity and mortality in Nigeria. To determine its endemicity in a rural farming community in the south-south of Nigeria, the following malariometric indices, namely, malaria parasitaemia, spleen rates, and anaemia were evaluated in children aged 2-10 years. This was a descriptive cross-sectional survey among school-age children residing in a rubber plantation settlement. The children were selected from six primary schools using a multistaged stratified cluster sampling technique. They were all examined for pallor, enlarged spleen, or liver among other clinical parameters and had blood films for malaria parasites. Of the 461 children recruited, 329 (71.4%) had malaria parasites. The prevalence of malaria parasitaemia was slightly higher in the under fives than that of those ≥5 years, 76.2% and 70.3%, respectively. Splenic enlargement was present in 133 children (28.9%). The overall prevalence of anaemia was 35.7%. Anaemia was more common in the under-fives (48.8%) than in those ≥5 years (32.8%). The odds of anaemia in the under fives were significantly higher than the odds of those ≥5 years (OR = 1.95 [1.19-3.18]). Malaria is highly endemic in this farming community and calls for intensification of control interventions in the area with special attention to school-age children

    Antiviral CD8(+) T Cells Restricted by Human Leukocyte Antigen Class II Exist during Natural HIV Infection and Exhibit Clonal Expansion.

    CD8(+) T cell recognition of virus-infected cells is characteristically restricted by major histocompatibility complex (MHC) class I, although rare examples of MHC class II restriction have been reported in Cd4-deficient mice and a macaque SIV vaccine trial using a recombinant cytomegalovirus vector. Here, we demonstrate the presence of human leukocyte antigen (HLA) class II-restricted CD8(+) T cell responses with antiviral properties in a small subset of HIV-infected individuals. In these individuals, T cell receptor β (TCRβ) analysis revealed that class II-restricted CD8(+) T cells underwent clonal expansion and mediated killing of HIV-infected cells. In one case, these cells comprised 12% of circulating CD8(+) T cells, and TCRα analysis revealed two distinct co-expressed TCRα chains, with only one contributing to binding of the class II HLA-peptide complex. These data indicate that class II-restricted CD8(+) T cell responses can exist in a chronic human viral infection, and may contribute to immune control
