    The Maine Tidal Power Initiative: Transdisciplinary sustainability science research for the responsible development of tidal power

    The Maine Tidal Power Initiative (MTPI), an interdisciplinary team of engineers, marine scientists, oceanographers, and social scientists, is using a transdisciplinary sustainability science approach to collect biophysical and social data necessary for understanding interactions between human and natural systems in the context of tidal power development in Maine. MTPI offers a unique opportunity to better understand how group structure and process influence outcomes in transdisciplinary sustainability science research. Through extensive participant observation and semi-structured interviews we: (1) describe MTPI’s organizational structure; (2) examine MTPI’s research approach and engagement with stakeholders from different sectors of society (i.e., industry, government, and the local community); and (3) identify challenges and opportunities for involving different disciplinary expertise and diverse stakeholders in transformational sustainability science research. We found that MTPI’s holistic mission, non-hierarchical structure, and iterative stakeholder engagement process led to important benefits and significant challenges. Positive outcomes include knowledge development, a transferable research framework, shared resources, personal reward, and a greater understanding of the local environment and community. Challenges identified include balancing diverse interests and priorities, maintaining engagement, managing stakeholder relationships, and limited resources. Lessons learned from the process of integrative collaborative research in Maine can offer guidance on what should be considered when carrying out similar transdisciplinary sustainability science projects in other research contexts

    The Influence of Scale Preferences on the Design of a Water Innovation: A Case in Dutch River Management

    The debate on scale use in river management focuses primarily on the (lack of) fit between the bio-geophysical and institutional systems. However, in this article we focus on the ‘subjective’ aspect of scale preferences in water governance. We apply an adapted version of the Integrated Scale Hierarchy for Rivers to determine the degree of fit between the scale preferences of the actors involved in a Dutch case study and the scale requirements of the innovative river management concept. This allows us to understand which riverine processes and characteristics are regarded as important by the different actors and to identify mismatches in scale perspectives as they manifest themselves in water management practice. We discover that inflexibility in scale use on the part of the involved actors places bounds on the design and quality of interventions and demonstrate that a more flexible use of scales in the design phase of a river management intervention has the potential to lead to more effective solutions

    Modelling to bridge many boundaries: the Colorado and Murray-Darling River basins

    Increasing pressure on shared water resources has often been a driver for the development and utilisation of water resource models (WRMs) to inform planning and management decisions. With an increasing emphasis on regional decision-making among competing actors as opposed to top-down and authoritative directives, the need for integrated knowledge and water diplomacy efforts across federal and international rivers provides a test bed for the ability of WRMs to operate within complex historical, social, environmental, institutional and political contexts. This paper draws on theories of sustainability science to examine the role of WRMs to inform transboundary water resource governance in large river basins. We survey designers and users of WRMs in the Colorado River Basin in North America and the Murray-Darling Basin in southeastern Australia. Water governance in such federal rivers challenges inter-governmental and multi-level coordination and we explore these dynamics through the application of WRMs. The development pathways of WRMs are found to influence their uptake and acceptance as decision support tools. Furthermore, we find evidence that WRMs are used as boundary objects and perform the functions of ‘boundary work’ between scientists, decision-makers and stakeholders in the midst of regional environmental changes

    Exploring the science–policy interface on climate change: The role of the IPCC in informing local decision-making in the UK

    Building on the Intergovernmental Panel on Climate Change’s (IPCC) review of how to make its Assessment Reports (ARs) more accessible in the future, the research reported here assesses the extent to which the ARs are a useful tool through which scientific advice informs local decision-making on climate change in the United Kingdom. Results from interviews with local policy representatives and three workshops with UK academics, practitioners and local decision makers are presented. Drawing on these data, we outline three key recommendations made by participants on how the IPCC ARs can be better utilized as a form of scientific advice to inform local decision-making on climate change. First, to provide more succinct summaries of the reports paying close attention to the language, content, clarity, context and length of these summaries; second, to better target and frame the reports from a local perspective to maximize engagement with local stakeholders; and third, to work with local decision makers to better understand how scientific advice on climate change is being incorporated in local decision-making. By adopting these, the IPCC would facilitate local decision-making on climate change and provide a systematic review of how its reports are being used locally. We discuss implications of these recommendations and their relevance to the wider debate within and outside the IPCC as to the most effective way the IPCC can more effectively tailor its products to user needs without endangering the robustness of its scientific findings.     Image guidance using 3D-ultrasound (3D-US) for daily positioning of lumpectomy cavity for boost irradiation

    <p>Abstract</p> <p>Purpose</p> <p>The goal of this study was to evaluate the use of 3D ultrasound (3DUS) breast IGRT for electron and photon lumpectomy site boost treatments.</p> <p>Materials and methods</p> <p>20 patients with a prescribed photon or electron boost were enrolled in this study. 3DUS images were acquired both at time of simulation, to form a coregistered CT/3DUS dataset, and at the time of daily treatment delivery. Intrafractional motion between treatment and simulation 3DUS datasets were calculated to determine IGRT shifts. Photon shifts were evaluated isocentrically, while electron shifts were evaluated in the beam's-eye-view. Volume differences between simulation and first boost fraction were calculated. Further, to control for the effect of change in seroma/cavity volume due to time lapse between the 2 sets of images, interfraction IGRT shifts using the first boost fraction as reference for all subsequent treatment fractions were also calculated.</p> <p>Results</p> <p>For photon boosts, IGRT shifts were 1.1 ± 0.5 cm and 50% of fractions required a shift >1.0 cm. Volume change between simulation and boost was 49 ± 31%. Shifts when using the first boost fraction as reference were 0.8 ± 0.4 cm and 24% required a shift >1.0 cm. For electron boosts, shifts were 1.0 ± 0.5 cm and 52% fell outside the dosimetric penumbra. Interfraction analysis relative to the first fraction noted the shifts to be 0.8 ± 0.4 cm and 36% fell outside the penumbra.</p> <p>Conclusion</p> <p>The lumpectomy cavity can shift significantly during fractionated radiation therapy. 3DUS can be used to image the cavity and correct for interfractional motion. Further studies to better define the protocol for clinical application of IGRT in breast cancer is needed.</p

    The social and scientific values that shape national climate scenarios: a comparison of the Netherlands, Switzerland and the UK

    This paper seeks to understand why climate information is produced differently from country to country. To do this, we critically examined and compared the social and scientific values that shaped the production of three national climate scenarios in the Netherlands, Switzerland and the UK. A comparative analysis of documentary materials and expert interviews linked to the climate scenarios was performed. Our findings reveal a new typology of use-inspired research in climate science for decision-making: (i) innovators, where the advancement of science is the main objective; (ii) consolidators, where knowledge exchanges and networks are prioritised; and (iii) collaborators, where the needs of users are put first and foremost. These different values over what constitutes ‘good’ science for decision-making are mirrored in the way users were involved in the production process: (i) elicitation, where scientists have privileged decision-making power; (ii) representation, where multiple organisations mediate on behalf of individual users; and (iii) participation, where a multitude of users interact with scientists in an equal partnership. These differences help explain why climate knowledge gains its credibility and legitimacy differently even when the information itself might not be judged as salient and usable. If the push to deliberately co-produce climate knowledge is not sensitive to the national civic epistemology at play in each country, scientist–user interactions may fail to deliver more ‘usable’ climate information

    Cable Pili and the Associated 22 Kda Adhesin Contribute to Burkholderia Cenocepacia Persistence In Vivo

    Infection by Burkholderia cenocepacia in cystic fibrosis (CF) patients is associated with poor clinical prognosis. Previously, we demonstrated that one of the highly transmissible strains, BC7, expresses cable pili and the associated 22 kDa adhesin, both of which contribute to BC7 binding to airway epithelial cells. However, the contribution of these factors to induce inflammation and bacterial persistence in vivo is not known.Wild-type BC7 stimulated higher IL-8 responses than the BC7 cbl and BC7 adhA mutants in both CF and normal bronchial epithelial cells. To determine the role of cable pili and the associated adhesin, we characterized a mouse model of B. cenocepacia, where BC7 are suspended in Pseudomonas aeruginosa alginate. C57BL/6 mice were infected intratracheally with wild-type BC7 suspended in either alginate or PBS and were monitored for lung bacterial load and inflammation. Mice infected with BC7 suspended in PBS completely cleared the bacteria by 3 days and resolved the inflammation. In contrast, mice infected with BC7 suspended in alginate showed persistence of bacteria and moderate lung inflammation up to 5 days post-infection. Using this model, mice infected with the BC7 cbl and BC7 adhA mutants showed lower bacterial loads and mild inflammation compared to mice infected with wild-type BC7. Complementation of the BC7 cblS mutation in trans restored the capacity of this strain to persist in vivo. Immunolocalization of bacteria revealed wild-type BC7 in both airway lumen and alveoli, while the BC7 cbl and BC7 adhA mutants were found mainly in airway lumen and peribronchiolar region.B. cenocepacia suspended in alginate can be used to determine the capacity of bacteria to persist and cause lung inflammation in normal mice. Both cable pili and adhesin contribute to BC7-stimulated IL-8 response in vitro, and BC7 persistence and resultant inflammation in vivo

    Development of a context model to prioritize drug safety alerts in CPOE systems

    Background: Computerized physician order entry systems (CPOE) can reduce the number of medication errors and adverse drug events (ADEs) in healthcare institutions. Unfortunately, they tend to produce a large number of partly irrelevant alerts, in turn leading to alert overload and causing alert fatigue. The objective of this work is to identify factors that can be used to prioritize and present alerts depending on the 'context' of a clinical situation. Methods: We used a combination of literature searches and expert interviews to identify and validate the possible context factors. The internal validation of the context factors was performed by calculating the inter-rater agreement of two researcher's classification of 33 relevant articles. Results: We developed a context model containing 20 factors. We grouped these context factors into three categories: characteristics of the patient or case (e. g. clinical status of the patient); characteristics of the organizational unit or user (e. g. professional experience of the user); and alert characteristics (e. g. severity of the effect). The internal validation resulted in nearly perfect agreement (Cohen's Kappa value of 0.97). Conclusion: To our knowledge, this is the first structured attempt to develop a comprehensive context model for prioritizing drug safety alerts in CPOE systems. The outcome of this work can be used to develop future tailored drug safety alerting in CPOE systems

    The Relationship Between Homework Compliance and Therapy Outcomes: An Updated Meta-Analysis

    The current study was an updated meta-analysis of manuscripts since the year 2000 examining the effects of homework compliance on treatment outcome. A total of 23 studies encompassing 2,183 subjects were included. Results indicated a significant relationship between homework compliance and treatment outcome suggesting a small to medium effect (r = .26; 95% CI = .19–.33). Moderator analyses were conducted to determine the differential effect size of homework on treatment outcome by target symptoms (e.g., depression; anxiety), source of homework rating (e.g., client; therapist), timing of homework rating (e.g., retroactive vs. contemporaneous), and type of homework rating (e.g., Likert; total homeworks completed). Results indicated that effect sizes were robust across target symptoms, but differed by source of homework rating, timing of homework rating, and type of homework rating. Specifically, studies utilizing combined client and therapist ratings of compliance had significantly higher mean effect size relative to those using therapist only assessments and those using objective assessments. Further, studies that rated the percentage of homeworks completed had a significantly lower mean effect size compared to studies using Likert ratings, and retroactive assessments had higher effect size than contemporaneous assessments
