Louis Chuâs Eat a Bowl of Tea is the most recent addition (and a most welcome one) to the series of Asian-American classics that the University of Washington Press is reprinting in paperback format. Other titles in this series that come to mind are the uncompromisingly naturalistic memoirs of the Filipino-American Carlos Bulosan, America is in the Heart, and the gripping novel about a Japanese-American who resists the World War II draft, John Okadaâs No-No Boy. Louis Chuâs novel, which had first appeared in 1961 to unappreciative and uncomprehending notices, is every bit as essential to the canon of Asian America as the works of Okada or Bulosan
A two-stage estimation of hospital performance using mortality outcome measures: an application using Victorian hospital data
This paper proposes a method of deriving a hospital quality performance indicator using mortality outcome measures. The method aggregates any number of mortality outcome measures observed over several years into a single indicator. The paper begins with the supposition that there exists an abstract quality index which drives all observed mortality outcomes in each hospital. This abstract index is not directly observable but manifested via the observed mortality outcomes, which the authors make use of to provide an estimate of the abstract index. The method is applied to a sample of heart disease episodes extracted from hospital administrative data from the state of Victoria, Australia. Using the quality estimates, the authors show that teaching hospitals and large regional hospitals perform better than other hospitals and this superior performance is related to hospital size
Sampling rare fluctuations of height in the Oslo ricepile model
We have studied large deviations of the height of the pile from its mean
value in the Oslo ricepile model. We sampled these very rare events with
probabilities of order by Monte Carlo simulations using importance
sampling. These simulations check our qualitative arguement [Phys. Rev. E, {\bf
73}, 021303, 2006] that in steady state of the Oslo ricepile model, the
probability of large negative height fluctuations about
the mean varies as as with

Approach to Rectal Cancer Surgery
Rectal cancer is a distinct subset of colorectal cancer where specialized disease-specific management of the primary tumor is required. There have been significant developments in rectal cancer surgery at all stages of disease in particular the introduction of local excision strategies for preinvasive and early cancers, standardized total mesorectal excision for resectable cancers incorporating preoperative short- or long-course chemoradiation to the multimodality sequencing of treatment. Laparoscopic surgery is also increasingly being adopted as the standard rectal cancer surgery approach following expertise of colorectal surgeons in minimally invasive surgery gained from laparoscopic colon resections. In locally advanced and metastatic disease, combining chemoradiation with radical surgery may achieve total eradication of disease and disease control in the pelvis. Evidence for resection of metastases to the liver and lung have been extensively reported in the literature. The role of cytoreductive surgery and hyperthermic intraperitoneal chemotherapy for peritoneal metastases is showing promise in achieving locoregional control of peritoneal dissemination. Vision screening in children:a retrospective study of social and demographic factors with regard to visual outcomes
BACKGROUND: Amblyopia and its risk factors have been demonstrated to be more common among children from low socioeconomic backgrounds. We sought to investigate this association in a region with orthoptic-delivered screening and whole population coverage, and to also examine the association of the Health Plan Indicator (HPI) with screening outcome. METHODS: Screening examination outcomes, postcodes and HPIs were extracted from the community child health database for every child who underwent preschool vision screening between March 2010 and February 2011 Tayside. We obtained the Scottish Index of Multiple Deprivation score for every child as a measure of area-based deprivation. We assessed the vulnerability/needs of the individual family through the HPIââCoreâ (children and families receiving universal health visiting service), âAdditionalâ (receiving additional health/social support) and âIntensiveâ (receiving high levels of support). The outcomes from follow-up examinations for those who failed screening were extracted from the orthoptic department database. RESULTS: 4365 children were screened during the year 2010â2011 of whom 523 (11.9%) failed. The odds of children from the least deprived socioeconomic group passing the visual screening test was 1.4 times higher than those from the most deprived socioeconomic group (OR 1.4, 95% CI 1.07 to 1.89, p=0.01). The odds of a child from a family assigned as âIntensiveâ failing the preschool visual screening test was three times greater than the odds of a child from a family assigned as âCoreâ (OR 3.59, 95% CI 1.6 to 7.8, p=0.001). CONCLUSIONS: We found that children from the most deprived backgrounds and those from unstable homes were more likely to fail preschool vision screening
Mechanistic insights into the pathogenesis of microtubuleâtargeting agentâinduced peripheral neuropathy from pharmacogenetic and functional studies
Chemotherapy-induced peripheral neuropathy (CIPN) is a common dose-limiting toxicity that affects 30%-40% of patients undergoing cancer treatment. Although multiple mechanisms of chemotherapy-induced neurotoxicity have been described in preclinical models, these have not been translated into widely effective strategies for the prevention or treatment of CIPN. Predictive biomarkers to inform therapeutic approaches are also lacking. Recent studies have examined genetic risk factors associated with CIPN susceptibility. This review provides an overview of the clinical and pathologic features of CIPN and summarizes efforts to identify target pathways through genetic and functional studies. Structurally and mechanistically diverse chemotherapeutics are associated with CIPN; however, the current review is focused on microtubule-targeting agents since these are the focus of most pharmacogenetic association and functional studies of CIPN. Genome-wide pharmacogenetic association studies are useful tools to identify not only causative genes and genetic variants but also genetic networks implicated in drug response or toxicity and have been increasingly applied to investigations of CIPN. Induced pluripotent stem cell-derived models of human sensory neurons are especially useful to understand the mechanistic significance of genomic findings. Combined genetic and functional genomic efforts to understand CIPN hold great promise for developing therapeutic approaches for its prevention and treatment.Fil: Chua, Katherina C.. Evidence for Factorization in Three-body B --> D(*) K- K0 Decays
Motivated by recent experimental results, we use a factorization approach to
study the three-body B --> D(*) K- K0 decay modes. Two mechanisms are proposed
for kaon pair production: current-produced (from vacuum) and transition (from B
meson). The Bbar0 --> D(*)+ K- K0 decay is governed solely by the
current-produced mechanism. As the kaon pair can be produced only by the vector
current, the matrix element can be extracted from e+ e- --> K Kbar processes
via isospin relations. The decay rates obtained this way are in good agreement
with experiment. Both current-produced and transition processes contribute to
B- --> D(*)0 K- K0 decays. By using QCD counting rules and the measured B- -->
D(*)0 K- K0 decay rates, the measured decay spectra can be understood.Comment: 17 pages, 6 figure
Strong duality in conic linear programming: facial reduction and extended duals
The facial reduction algorithm of Borwein and Wolkowicz and the extended dual
of Ramana provide a strong dual for the conic linear program in the absence of any constraint qualification. The facial
reduction algorithm solves a sequence of auxiliary optimization problems to
obtain such a dual. Ramana's dual is applicable when (P) is a semidefinite
program (SDP) and is an explicit SDP itself. Ramana, Tuncel, and Wolkowicz
showed that these approaches are closely related; in particular, they proved
the correctness of Ramana's dual using certificates from a facial reduction
algorithm.
Here we give a clear and self-contained exposition of facial reduction, of
extended duals, and generalize Ramana's dual:
-- we state a simple facial reduction algorithm and prove its correctness;
and
-- building on this algorithm we construct a family of extended duals when
is a {\em nice} cone. This class of cones includes the semidefinite cone

Phenomenological Consequences of Right-handed Down Squark Mixings
The mixings of quarks, hidden from view in Standard Model (SM), are
naturally the largest if one has an Abelian flavor symmetry. With supersymmetry
(SUSY) their effects can surface via squark loops. Squark and
gluino masses are at TeV scale, but they can still induce effects comparable to
SM in (or ) mixings, while mixing could be close to recent
hints from data. In general, CP phases would be different from SM, as may be
indicated by recent B Factory data. Presence of non-standard soft SUSY
breakings with large could enhance (or )
