    Parents’ experiences of neonatal care in England

    With the greater need for specialist neonatal care in England over the last decade, increased attention has been given to developing and implementing quality measures to ensure that babies and their families receive the highest quality care. Patient experience is recognised as a key measure of quality, therefore it is essential to assess parents’ experiences of neonatal services to understand how these can be improved. In this paper we detail findings from the second large scale survey of parents’ experiences of neonatal care carried out in England in 2014, focusing on results that highlight aspects of family-centred care: information sharing; communication; support; and involvement. The results reveal great variations between individual units, as well as highlight key areas in which units could improve to provide family-centred care. This includes parents being able to speak to their doctor more, as well as receiving important information to understand their baby’s condition, and about support services available. Positively, many parents felt they were able to visit their baby as much as they wanted and were involved in their day-to-day care, which are fundamental to parents forming a bond with their baby. The survey is a rich data source that provides neonatal units in England with results that allow them to focus improvement efforts on what matters most to those using their services. It also enables neonatal units to evaluate how they are performing on key standards of care, supporting them in striving for clinical best practice

    Analytical response time estimation in parallel relational database systems

    Techniques for performance estimation in parallel database systems are well established for parameters such as throughput, bottlenecks and resource utilisation. However, response time estimation is a complex activity which is difficult to predict and has attracted research for a number of years. Simulation is one option for predicting response time but this is a costly process. Analytical modelling is a less expensive option but requires approximations and assumptions about the queueing networks built up in real parallel database machines which are often questionable and few of the papers on analytical approaches are backed by results from validation against real machines. This paper describes a new analytical approach for response time estimation that is based on a detailed study of different approaches and assumptions. The approach has been validated against two commercial parallel DBMSs running on actual parallel machines and is shown to produce acceptable accuracy

    Anatomical bases of sex- and size-related acoustic variation in herring gull alarm calls

    The hypothesis that anatomical or physiological factors can constrain the production of vocalizations is supported by an increasing number of examples from a range of taxa, where acoustic variation is related to sex, body-size or condition. In this study, we combine anatomical and acoustic investigations in herring gulls Larus argentatus to 1) identify co-variation between sex, body size and the dimensions of the vocal apparatus and 2) test the possible effect of this co-variation on interindividual variation in the acoustics of alarm calls. We found that the vocal apparatus was sexually dimorphic, with males having longer trachea and bigger vibratile membranes than females. We also identified a relationship between the head–bill length – a secondary sexual trait – and the length of the trachea in males only. However, we failed to identify corresponding sex- and body-size related variation in the acoustic components of alarm calls. We suggest that this absence of a relationship between anatomical and acoustic dimensions may reflect the lack of biomechanical constraints exerted during the production of alarm calls, and that such relationships are more likely to be expressed in this species’ sexual calls, whose production is characterised by more pronounced, ritualised postures that are more likely to highlight inter-individual size variation

    Migratory Movements and Home Ranges of Geographically Distinct Wintering Populations of a Soaring Bird

    Migratory soaring birds exhibit spatiotemporal variation in their circannual movements. Nevertheless, it remains uncertain how different winter environments affect the circannual movement patterns of migratory soaring birds. Here, we investigated annual movement strategies of American white pelicans Pelecanus erythrorhynchos (hereafter, pelican) from two geographically distinct wintering grounds in the Southern and Northern Gulf of Mexico (GOM).We hypothesized that hourly movement distance and home range size of a soaring bird would differ between different geographic regions because of different thermals and wind conditions and resource availability. We calculated average and maximum hourly movement distances and seasonal home ranges of GPS-tracking pelicans. We then evaluated the effects of hour of the day, seasons, two wintering regions in the Southern and Northern GOM, human footprint index, and relative pelican abundance from Christmas Bird Count data on pelican hourly movement distances and seasonal home ranges using linear mixed models and generalized linear mixed models. American white pelicans moved at greatest hourly distance near 1200 h at breeding grounds and during spring and autumn migrations. Both wintering populations in the Northern and Southern GOM exhibited similar hourly movement distances and seasonal home ranges at the shared breeding grounds and during spring and autumn migrations. However, pelicans wintering in the Southern GOM showed shorter hourly movement distances and smaller seasonal home ranges than those in the Northern GOM. Hourly movement distances and home ranges of pelicans increased with increasing human footprint index. Winter hourly movements and home ranges of pelicans differed between the Northern and Southern GOM; however, the winter difference in pelican movements did not carry over to the shared breeding grounds during summers. Therefore, exogenous factors may be the primary drivers to shape the flying patterns of migratory soaring birds

    Worker Well-Being: What It Is, and How It Should Be Measured

    Worker well-being is a hot topic in organizations, consultancy and academia. However, too often, the buzz about worker well-being, enthusiasm for new programs to promote it and interest to research it, have not been accompanied by universal enthusiasm for scientific measurement. Aim to bridge this gap, we address three questions. To address the question ‘What is worker well-being?’, we explain that worker well-being is a multi-facetted concept and that it can be operationalized in a variety of constructs. We propose a four-dimensional taxonomy of worker well-being constructs to illustrate the concept’s complexity and classify ten constructs within this taxonomy. To answer the question ‘How can worker well-being constructs be measured?’, we present two aspects of measures: measure obtrusiveness (i.e., the extent to which obtaining a measure interferes with workers’ experiences) and measure type (i.e., closed question survey, word, behavioral and physiological). We illustrate the diversity of measures across our taxonomy and uncover some hitherto under-appreciated avenues for measuring worker well-being. Finally, we address the question ‘How should a worker well-being measure be selected?’ by discussing conceptual, methodological, practical and ethical considerations when selecting a measure. We summarize these considerations in a short checklist. It is our hope that with this study researchers – working in organizations, in academia or both – will feel more competent to find effective strategies for the measurement worker well-being and eventually make policies and choices with a better understanding of what drives worker well-being

    Performance analysis of AlGaAs/GaAs tunnel junctions for ultra-high concentration photovoltaics

    An n(++)-GaAs/p(++)-AlGaAs tunnel junction with a peak current density of 10 100Acm(-2) is developed. This device is a tunnel junction for multijunction solar cells, grown lattice-matched on standard GaAs or Ge substrates, with the highest peak current density ever reported. The voltage drop for a current density equivalent to the operation of the multijunction solar cell up to 10 000 suns is below 5 mV. Trap-assisted tunnelling is proposed to be behind this performance, which cannot be justified by simple band-to-band tunnelling. The metal-organic vapour-phase epitaxy growth conditions, which are in the limits of the transport-limited regime, and the heavy tellurium doping levels are the proposed origins of the defects enabling trap-assisted tunnelling. The hypothesis of trap-assisted tunnelling is supported by the observed annealing behaviour of the tunnel junctions, which cannot be explained in terms of dopant diffusion or passivation. For the integration of these tunnel junctions into a triple-junction solar cell, AlGaAs barrier layers are introduced to suppress the formation of parasitic junctions, but this is found to significantly degrade the performance of the tunnel junctions. However, the annealed tunnel junctions with barrier layers still exhibit a peak current density higher than 2500Acm(-2) and a voltage drop at 10 000 suns of around 20 mV, which are excellent properties for tunnel junctions and mean they can serve as low-loss interconnections in multijunction solar cells working at ultra-high concentrations

    Defective axonal transport in motor neuron disease

    Several recent studies have highlighted the role of axonal transport in the pathogenesis of motor neuron diseases. Mutations in genes that control microtubule regulation and dynamics have been shown to cause motor neuron degeneration in mice and in a form of human motor neuron disease. In addition, mutations in the molecular motors dynein and kinesins and several proteins associated with the membranes of intracellular vesicles that undergo transport cause motor neuron degeneration in humans and mice. Paradoxically, evidence from studies on the legs at odd angles (Loa) mouse and a transgenic mouse model for human motor neuron disease suggest that partial limitation of the function of dynein may in fact lead to improved axonal transport in the transgenic mouse, leading to delayed disease onset and increased life span
