Erlotinib therapy in advanced non small-cell lung cancer
Erlotynib jest jednym z inhibitorów kinazy tyrozynowej receptora naskórkowego czynnika wzrostu. Terapia tymi cząsteczkami może przynieść korzyść taką jak wydłużenie czasu przeżycia oraz poprawy jakości życia w wyselekcjonowanej grupie chorych z niedrobnokomórkowym rakiem płuca (NDRP). Chorzy odnoszący największą korzyść z tej terapii to pacjenci z potwierdzoną obecnością mutacji aktywującej w obrębie genu EGFR, w dobrym stanie ogólnym. Leki te są dobrze tolerowane, podanie ich nie wymaga hospitalizacji co jest bardzo ważne dla chorych oraz ich rodzin. W pracy przedstawiono przypadek mężczyzny leczonego erlotynibem.Erlotinib is one of epithelial growth factor receptor tyrosine kinase inhibitors. Treatment of selected patient groups may result in prolonged survival time and improved quality of life during therapy. Patients who benefit most from this therapy are those with confirmed activating mutations within the EGFR gene, in good general condition. These drugs are well tolerated and do not require hospitalization, which is very important for the patients and their families We present the case of a male with advanced non small cell lung cancer who receives erlotinib
Prevalence of risk factors in children and youth with essential hypertension
Wstęp Nadciśnienie tętnicze jest jednostką chorobową
coraz częściej rozpoznawaną wśród dzieci
i młodzieży. Bardzo często współistnieje z nadwagą/otyłością i zaburzeniami gospodarki lipidowej. Ze
względu na implikacje kliniczne współistnienie powyższych
jednostek chorobowych wymaga działań
w zakresie profilaktyki pierwotnej.
Celem badania była ocena częstości występowania czynników
ryzyka, czyli wywiadu rodzinnego w kierunku
nadciśnienia tętniczego, nadmiernej masy ciała i parametrów
zaburzeń gospodarki lipidowej u dzieci i młodzieży
z pierwotnym nadciśnieniem tętniczym.
Materiał i metody Badaną populację stanowiło 86
osób w wieku 14-17 lat (50 chłopców i 36 dziewcząt)
z pierwotnym nadciśnieniem tętniczym. Oceniano: wywiad rodzinny, wartości ciśnienia tętniczego,
wzrost i masę ciała (obliczono BMI), parametry gospodarki
lipidowej, glikemię na czczo. Populację
kontrolną stanowiła odpowiednia pod względem
wieku i płci grupa 93 zdrowych dzieci.
Wyniki U 39,5% ojców i 27% matek dzieci i młodzieży
z populacji badanej stwierdzono nadciśnienie
tętnicze. Także otyłość częściej dotyczyła rodziców
osób z nadciśnieniem tętniczym. W większości grup
wiekowych badanej populacji średnie wartości BMI
były wyższe niż w odpowiednich grupach kontrolnych.
Jedynie w grupie chłopców 14-letnich (23,71 ± 4,23 kg/m2 vs. 20,71 ± 2,78 kg/m2) i 17-letnich
(24,22 ± 4,23 kg/m2 vs. 22,32 ± 2,84 kg/m2) różnice
nie były istotne statystycznie. Średnie wartości skurczowego
ciśnienia tętniczego u dziewcząt i chłopców
były najwyższe w grupach 17-latków (143,3 ± 8,2 mm Hg u dziewcząt i 145,0 ± 7,8 mm Hg
u chłopców), a średnie wartości rozkurczowego ciśnienia
tętniczego w grupach 17-letnich dziewcząt
(88,3 ± 9,8 mm Hg) oraz 15- i 17-letnich chłopców
(odpowiednio 85,0 ± 9,1 mm Hg i 85,1 ± 8,0 mm Hg).
U 39,5% badanych osób stwierdzono współistnienie
nadciśnienia tętniczego i nadwagi/otyłości.
Wnioski 1. Nadwaga lub otyłość jest najczęściej
stwierdzanym czynnikiem ryzyka u młodych osób
z pierwotnym nadciśnieniem tętniczym. 2. U większości
badanych osób z nadciśnieniem tętniczym
współistnieją dodatkowe czynniki ryzyka, takie jak
zaburzenia lipidowe czy dodatni wywiad rodzinny.Background The percentage of hypertensive patients
among children and youth grows every year. Very often
hypertension coexists with obesity (or overweight) and
lipids disorders. The aim of this work was to assess how
often we meet risk factors like positive family history, obesity
and dyslipidemia in groups of children and youth with
primary hypertension.
Material and methods There were 86 patients under examination,
aged 14-17 (50 boys and 36 girls) with primary
hypertension. The given parameters were estimated: family
history, blood pressure, height, weight, BMI, lipid profile.
The control group consists of 93 healthy subjects
matched by sex and age.
Results In the analysed group 39.53% of fathers and 27% of
mothers had hypertension. There were also high percentage
of obesity among parents - 24.42% of fathers and 29.07% of
mothers were obese. In examined population BMI was significant
higher than in controls. Only in groups of 14 year old boys
and 17 year old girls, the differences failed to reach the statistically
significance. The average systolic blood pressure was the
highest in the group of 17 year old boys (145 ± 7,8 mm Hg)
and girls (143,3 ± 8,2 mm Hg). The average diastolic blood
pressure was the highest in group of 17 years old girls (88,3 ± 9,8 mm Hg) and 15- and 17 year old boys (85 ± 9,1 mm Hg
and 85,1 ± 8,0 mm Hg). In analysed group 39.5% of patients
had both hypertension and obesity.
Conclusion 1. Overweight or obesity is the most popular
risk factor in young hypertensives. 2. In those population
hypertension was very often associated with dyslipidemia
and positive family history
Effectiveness of osimertinib in patients with lung adenocarcinoma in clinical practice — the Expanded Drug Access Program in Poland
Introduction: Osimertinib is a third-generation, irreversible epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor that has demonstrated efficacy in the treatment of EGFR-mutant non-small-cell lung cancer (NSCLC) in prospective clinical trials. Material and methods: This retrospective analysis evaluated the outcomes of 32 pretreated patients with EGFR T790M mutation who received osimertinib in clinical practice at seven centers in Poland within the Expanded Drug Access Program. Osimertinib was used in the second line in 59% of patients and in later lines in 41%. Results: Objective response was attained in 16 patients (50%), and 12 subjects (38%) had stable disease. Median progression -free survival was 11.3 months in the overall population, 12.6 months in patients with EGFR exon 19 mutation and 7.5 months in patients with EGFR exon 21 mutation (p = 0.045). Median overall survival (OS) was 18.3 months. Overall, 58.4% and 45.6% of patients remained in follow-up after 12 and 24 months, respectively. Median OS appeared longer for patients without cerebral metastases than for those with cerebral metastases (27.4 vs 9.4 months, respectively; p = 0.078), and for patients with the Eastern Cooperative Oncology Group performance status (ECOG PS) 0–1 than those with ECOG PS 2 (27.4 vs 11.8 months, respectively; p = 0.189), although neither result reached statistical significance. Median OS of patients with partial response, stable disease and progressive disease was 27.4, 12.7 and 4.5 months, respectively (p < 0.001). Age, comorbidities, line of treatment with osimertinib, and type of activating EGFR mutation did not impact on OS. Adverse events of any grade or grade 3/4 were reported in 38% and 9% of patients, respectively. One person discontinued due to interstitial pneumonia.Conclusion: These results confirm the value of osimertinib in patients with previously treated EGFR T790M-mutant NSCLC. Clinical benefit was evident in patients with cerebral metastases and moderate performance status
Optimization of diagnostic and therapeutic management in patients with stage III non-small cell lung cancer — experience of the centers in Poznań
Lung cancer is one of the most frequently diagnosed malignancies, with one of the worst prognoses. Non-small cell lung cancer (NSCLC) is the dominant histological type, accounting for 85% of cases. In Poland, in more than one-third of patients, NSCLC is diagnosed at stage III. One of the most effective methods of radical treatment in such cases is concurrent radiochemotherapy. However, in Poland the percentage of patients eligible for this type of therapy is quite low, due to delayed diagnosis, lack of reference centers, long qualification process for treatment, and ineffective treatment organization. This article discusses the optimization of therapeutic management in patients with stage III NSCLC based on the experience of centers in Poznań (the Greater Poland Cancer Center and Greater Poland Center for Pulmonology and Thoracic Surgery). Some modifications include introduction of a surgery qualification form, urgent early evaluation using combined positron emission tomography (PET)/computed tomography (CT) and invasive mediastinum evaluation, and initial qualification for radiochemotherapy (with the setting of dates) already during diagnostics.
