    Reflections on the Occasion of the 100th Anniversary of the Monthly Labor Review

    It is an honor to comment on directions for the Monthly Labor Review MLR over its next 25 years. The MLR is the federal government\u27s oldest continuous publication—first printed in 1915 and now published online by the Bureau of Labor Statistics (BLS), one of the nation\u27s oldest statistical agencies, established in 1884. BLS embodies the standards articulated by the Committee on National Statistics (CNSTAT) in the fifth edition of its quadrennial volume Principles and Practices for a Federal Statistical Agency (National Research Council, 2013). P&P lays down four principles: that a statistical agency produce data relevant to policy issues, earn credibility with data users, earn the trust of data providers (e.g., households, businesses), and maintain independence from political and other undue external influence

    Health Reform Reconstruction

    This Article connects the failed, inequitable U.S. coronavirus pandemic response to conceptual and structural constraints that have held back U.S health reform for decades and calls for reconstruction. For more than a half-century, a cramped “iron triangle” ethos has constrained health reform conceptually. Reforms aimed to balance individual interests in cost, quality, and access to health care, while marginalizing equity, solidarity, and public health. In the iron triangle era, reforms unquestioningly accommodated four legally and logistically entrenched fixtures — individualism, fiscal fragmentation, privatization, and federalism — that distort and diffuse any reach toward social justice. The profound racial disparities and public health failures of the U.S. pandemic response have agonizingly manifested the limitations of pre-2020 health reform and demand a reconstruction. Health reform reconstruction begins with a new conceptual framework that aims to realize health justice. Health justice requires commitments to anti-racism, equitable distribution of the burdens and benefits of public investments in health care and public health (for which health care access, quality, and cost are useful, but not exhaustive, metrics), and community empowerment. These commitments put health justice on a collision course with the fixtures of individualism, fiscal fragmentation, privatization, and federalism. Thus, incremental reforms must be measured by the extent to which they confront these fixtures. This Article describes how health reform reconstruction can chart the path for legal change and proposes “confrontational incrementalism” as a method for recognizing the necessity of reconstructive reform, along with its near impossibility

    VEGF-A induces tumor and sentinel lymph node lymphangiogenesis and promotes lymphatic metastasis

    The mechanisms of tumor metastasis to the sentinel lymph nodes are poorly understood. Vascular endothelial growth factor (VEGF)-A plays a principle role in tumor progression and angiogenesis; however, its role in tumor-associated lymphangiogenesis and lymphatic metastasis has remained unclear. We created transgenic mice that overexpress VEGF-A and green fluorescent protein specifically in the skin, and subjected them to a standard chemically-induced skin carcinogenesis regimen. We found that VEGF-A not only strongly promotes multistep skin carcinogenesis, but also induces active proliferation of VEGF receptor-2–expressing tumor-associated lymphatic vessels as well as tumor metastasis to the sentinel and distant lymph nodes. The lymphangiogenic activity of VEGF-A–expressing tumor cells was maintained within metastasis-containing lymph nodes. The most surprising finding of our study was that even before metastasizing, VEGF-A–overexpressing primary tumors induced sentinel lymph node lymphangiogenesis. This suggests that primary tumors might begin preparing their future metastatic site by producing lymphangiogenic factors that mediate their efficient transport to sentinel lymph nodes. This newly identified mechanism of inducing lymph node lymphangiogenesis likely contributes to tumor metastasis, and therefore, represents a new therapeutic target for advanced cancer and/or for the prevention of metastasis

    Reducing orbital eccentricity in binary black hole simulations

    Binary black hole simulations starting from quasi-circular (i.e., zero radial velocity) initial data have orbits with small but non-zero orbital eccentricities. In this paper the quasi-equilibrium initial-data method is extended to allow non-zero radial velocities to be specified in binary black hole initial data. New low-eccentricity initial data are obtained by adjusting the orbital frequency and radial velocities to minimize the orbital eccentricity, and the resulting (5\sim 5 orbit) evolutions are compared with those of quasi-circular initial data. Evolutions of the quasi-circular data clearly show eccentric orbits, with eccentricity that decays over time. The precise decay rate depends on the definition of eccentricity; if defined in terms of variations in the orbital frequency, the decay rate agrees well with the prediction of Peters (1964). The gravitational waveforms, which contain 8\sim 8 cycles in the dominant l=m=2 mode, are largely unaffected by the eccentricity of the quasi-circular initial data. The overlap between the dominant mode in the quasi-circular evolution and the same mode in the low-eccentricity evolution is about 0.99.Comment: 27 pages, 9 figures; various minor clarifications; accepted to the "New Frontiers" special issue of CQ

    On the algorithmic construction of classifying spaces and the isomorphism problem for biautomatic groups

    We show that the isomorphism problem is solvable in the class of central extensions of word-hyperbolic groups, and that the isomorphism problem for biautomatic groups reduces to that for biautomatic groups with finite centre. We describe an algorithm that, given an arbitrary finite presentation of an automatic group Γ\Gamma, will construct explicit finite models for the skeleta of K(Γ,1)K(\Gamma,1) and hence compute the integral homology and cohomology of Γ\Gamma.Comment: 21 pages, 4 figure

    CD27 Expression Promotes Long-Term Survival of Functional Effector–Memory CD8+Cytotoxic T Lymphocytes in HIV-infected Patients

    Human immunodeficiency virus (HIV)-specific CD8+ T cells persist in high frequencies in HIV-infected patients despite impaired CD4+ T helper response to the virus, but, unlike other differentiated effector cytotoxic T lymphocytes, most continue to express the tumor necrosis factor receptor family member CD27. Because the ligand for CD27 (CD70) is also overexpressed in HIV-infected hosts, we examined the nature of expression and potential functional consequences of CD27 expression on HIV-specific CD8+ T cells. Analysis of CD27+ and CD27− T cells derived from the same HIV-specific clone revealed that retention of CD27 did not interfere with acquisition of effector functions, and that after T cell receptor stimulation, CD27+ cells that concurrently were triggered via CD27 exhibited more resistance to apoptosis, interleukin 2 production, and proliferation than CD27− T cells. After transfer back into an HIV-infected patient, autologous HIV-specific CD27− T cells rapidly disappeared, but CD27+ T cells derived from the same clone persisted at high frequency. Our findings suggest that the CD27–CD70 interaction in HIV infection may provide CD27+ CD8+ T cells with a survival advantage and compensate for limiting or absent CD4+ T help to maintain the CD8 response

    The effects of extraverted temperament on agoraphobia in panic disorder

    Although situational avoidance is viewed as the most disabling aspect of panic disorder, few studies have evaluated how dimensions of neurotic (i.e., neuroticism, behavioral inhibition) and extraverted (i.e., extraversion, behavioral activation) temperament may influence the presence and severity of agoraphobia. Using logistic regression and structural equation modeling, we examined the unique effects of extraverted temperament on situational avoidance in a sample of 274 outpatients with a diagnosis of panic disorder with and without agoraphobia. Results showed low extraverted temperament (i.e., introversion) to be associated with both the presence and the severity of situational avoidance. Findings are discussed in regard to conceptualizations of conditioned avoidance, activity levels, sociability, and positive emotions within the context of panic disorder with agoraphobia
