    Impact of organic hydroperoxides on rat testicular tissue and epididymal sperm

    Organic hydroperoxides such as t-butyl hydroperoxide and cumene hydroperoxide have been implicated to cause oxidative stress leading to damage in membrane lipids, proteins, carbohydrates and DNA. This study was aimed to develop an in vivo animal model. The effects of  hydroperoxides on testicular tissue and epididymal sperm were investigated. Male Wistar rats aged 10 - 12 weeks were randomly placed in groups and received standard rat chow and water ad libitum. Animals were injected intraperitoneally with saline (0.5 ml), t-butyl hydroperoxide (5, 10, 20 and 40 ìM; 0.5 ml) or cumene hydroperoxide cHP (2.5, 5, 10 and 20 ìM; 0.5 ml) over a 60 day period. It was found that cumenehydroperoxide cHP (10 and 20 ìM) and t-butyl hydroperoxide tbHP (20 and 40 ìM) led to significantly lower epididymal sperm concentrations and motility. Superoxide dismutase and glutathione activities were also higher with an accompanying increase in lipid peroxidation in both testicular tissue and epididymal sperm. It can be concluded that in vivo intraperitoneal administration of organic hydroperoxides negatively affect the male reproductive system. We have therefore successfully createdan animal model to test the adverse effects of oxidative stress on male reproductive parameters, thereby, enabling us to study possible in vivo treatments

    Deep-water channel-lobe transition zone dynamics: Processes and depositional architecture, an example from the Karoo Basin, South Africa

    Submarine channel-lobe transition zones separate well-defined channels from well-defined lobes and form morphologically complicated areas, commonly located at breaks in slope. These areas play a vital role in the transfer of sediment through deep-water systems. Extensive outcrop exposures in the Karoo Basin, South Africa, permit investigation of the depositional architecture and evolution of entirely exhumed dip transects of a channel-lobe transition zone for the first time. Furthermore, the excellent paleogeographic constraint allows correlation to genetically related updip channel-levee systems and downdip lobe deposits over 40 km, with strike control over 20 km. Unlike the single time slice afforded by modern systems, the Karoo example uniquely allows study of the temporal shifting of the channel-lobe transition zone and transfer into the stratigraphic record. Key lateral changes along the base of slope include the variation from an interfingering levee-lobe transition zone to a bypass-dominated channel-lobe transition zone over a width of 14 km. Key recognition criteria for channel-lobe transition zones in the ancient record include combinations of scours and megaflutes, composite erosional surfaces, mudstone clast/coarse-grained sediment lags, and remnants of depositional bed forms, such as sediment waves. Documented here in a single channel-lobe transition zone, these features are arranged in a zone of juxtaposed remnant erosional and depositional features. The zone reaches 6 km in length, formed by at least four stages of expansion/contraction or migration. Strike variations and changes in the dimensions of the channel-lobe transition zone through time are interpreted to be the result of physiographic changes and variations in flow dynamics across the base of slope. The dynamic nature of channel-lobe transition zones results in complicated and composite stratigraphy, with preservation potential generally low but increasing distally and laterally away from the mouth of the feeder channel system. Here, we present the first generic model to account for dynamic channel-lobe transition zone development, encompassing distinctive recognition criteria, fluctuations in the morphology and position of the zone, and the complex transfer into the sedimentary record

    Lung metastasis 21 years after initial diagnosis of osteosarcoma: a case report

    <p>Abstract</p> <p>Introduction</p> <p>To the best of our knowledge, this case report describes the longest disease-free interval between primary diagnosis and metastatic recurrence of an osteosarcoma.</p> <p>Case presentation</p> <p>A 35-year-old Caucasian American man presented with asymptomatic lung metastases 21 years after being diagnosed and treated for lower extremity osteosarcoma. He underwent curative lung resection, but 2 years thereafter developed metastatic disease in the scapula and tibia and, after resection and chemotherapy, is in remission 1 year later.</p> <p>Conclusion</p> <p>This case highlights the importance of long follow-up periods and continued surveillance of osteosarcoma patients after initial curative treatment.</p

    Association between the c.*229C>T polymorphism of the topoisomerase IIb binding protein 1 (TopBP1) gene and breast cancer

    Topoisomerase IIb binding protein 1 (TopBP1) is involved in cell survival, DNA replication, DNA damage repair and cell cycle checkpoint control. The biological function of TopBP1 and its close relation with BRCA1 prompted us to investigate whether alterations in the TopBP1 gene can influence the risk of breast cancer. The aim of this study was to examine the association between five polymorphisms (rs185903567, rs116645643, rs115160714, rs116195487, and rs112843513) located in the 30UTR region of the TopBP1 gene and breast cancer risk as well as allele-specific gene expression. Five hundred thirty-four breast cancer patients and 556 population controls were genotyped for these SNPs. Allele-specific Top- BP1 mRNA and protein expressions were determined by using real time PCR and western blotting methods, respectively. Only one SNP (rs115160714) showed an association with breast cancer. Compared to homozygous common allele carriers, heterozygous and homozygous for the T variant had significantly increased risk of breast cancer (adjusted odds ratio = 3.81, 95 % confidence interval: 1.63–8.34, p = 0.001). Mean TopBP1 mRNA and protein expression were higher in the individuals with the CT or TT genotype. There was a significant association between the rs115160714 and tumor grade and stage. Most carriers of minor allele had a high grade (G3) tumors classified as T2-T4N1M0. Our study raises a possibility that a genetic variation of TopBP1 may be implicated in the etiology of breast cancer

    Health-related quality of life as measured with EQ-5D among populations with and without specific chronic conditions: A population-based survey in Shaanxi province, China

This is an open-access article distributed under the terms of the Creative Commons Attribution License, which permits unrestricted use, distribution, and reproduction in any medium, provided the original author and source are credited.Introduction: The aim of this study was to examine health-related quality of life (HRQoL) as measured by EQ-5D and to investigate the influence of chronic conditions and other risk factors on HRQoL based on a distributed sample located in Shaanxi Province, China. Methods: A multi-stage stratified cluster sampling method was performed to select subjects. EQ-5D was employed to measure the HRQoL. The likelihood that individuals with selected chronic diseases would report any problem in the EQ-5D dimensions was calculated and tested relative to that of each of the two reference groups. Multivariable linear regression models were used to investigate factors associated with EQ VAS. Results: The most frequently reported problems involved pain/discomfort (8.8%) and anxiety/depression (7.6%). Nearly half of the respondents who reported problems in any of the five dimensions were chronic patients. Higher EQ VAS scores were associated with the male gender, higher level of education, employment, younger age, an urban area of residence, access to free medical service and higher levels of physical activity. Except for anemia, all the selected chronic diseases were indicative of a negative EQ VAS score. The three leading risk factors were cerebrovascular disease, cancer and mental disease. Increases in age, number of chronic conditions and frequency of physical activity were found to have a gradient effect. Conclusion: The results of the present work add to the volume of knowledge regarding population health status in this area, apart from the known health status using mortality and morbidity data. Medical, policy, social and individual attention should be given to the management of chronic diseases and improvement of HRQoL. Longitudinal studies must be performed to monitor changes in HRQoL and to permit evaluation of the outcomes of chronic disease intervention programs. © 2013 Tan et al.National Nature Science Foundation (No. 8107239

    Has education lost sight of children?

    The reflections presented in this chapter are informed by clinical and personal experiences of school education in the UK. There are many challenges for children and young people in the modern education system and for the professionals who support them. In the UK, there are significant gaps between the highly selective education provided to those who pay privately for it and to the majority of those educated in the state-funded system. Though literacy rates have improved around the world, many children, particularly boys, do not finish their education for reasons such as boredom, behavioural difficulties or because education does not ‘pay’. Violence, bullying, and sexual harassment are issues faced by many children in schools and there are disturbing trends of excluding children who present with behavioural problems at school whose origins are not explored. Excluded children are then educated with other children who may also have multiple problems which often just make the situation worse. The experience of clinicians suggests that school-related mental health problems are increasing in severity. Are mental health services dealing with the consequences of an education system that is not meeting children’s needs? An education system that is testing- and performance-based may not be serving many children well if it is driving important decisions about them at increasingly younger ages. Labelling of children and setting them on educational career paths can occur well before they reach secondary schools, limiting potential very early on in their developmental trajectory. Furthermore, the emphasis at school on testing may come at the expense of creativity and other forms of intelligence, which are also valuable and important. Meanwhile the employment marketplace requires people with widely different skills, with an emphasis on innovation, creativity, and problem solving.     Study of Peri-Articular Anaesthetic for Replacement of the Knee (SPAARK): study protocol for a patient-blinded, randomised controlled superiority trial of liposomal bupivacaine

    Background: Optimising the management of peri-operative pain and recovery following knee replacement has been identified as a patient priority. Current pain relief strategies use opiate-based analgesia; however, up to 50% of patients experience significant side effects. Local anaesthetic incisional infiltration is one alternative. The length of the duration of action is a major limiting factor of current local anaesthetic techniques. Liposomal bupivacaine has been reported to be effective for up to 72 h. This randomised controlled trial will evaluate the clinical and cost effectiveness of liposomal bupivacaine. Methods: SPAARK is a patient-blinded, multi-centre, active comparator, superiority, two-arm, parallel-group randomised controlled trial. Five hundred patients undergoing knee replacement will be recruited and randomised to liposomal bupivacaine plus bupivacaine hydrochloride or bupivacaine hydrochloride alone. The co-primary outcomes are the Quality of Recovery 40 measured at 72 h post-surgery and also cumulative pain measured daily using a 0–10 visual analogue scale for the first 3 days following surgery. Secondary outcomes include cumulative opioid consumption, fitness for discharge, functional outcomes assessed using the Oxford Knee Score and American Knee Society Score, the EuroQol five dimensions instrument and complications. A cost utility analysis is also planned. Discussion: The clinical effectiveness and cost effectiveness of liposomal bupivacaine have yet to be evaluated in the National Health Service, making this trial appropriate and timely.     The impact of tuberculosis on the well-being of adolescents and young adults

    The health needs of adolescents and young adults (AYAs) have been neglected in tuberculosis (TB) care, control, and research. AYAs, who are distinct from younger children and older adults, undergo dynamic physical, psychological, emotional, cognitive, and social development. Five domains of adolescent well-being are crucial to a successful transition between childhood and adulthood: (1) Good health; (2) connectedness and contribution to society; (3) safety and a supportive environment; (4) learning, competence, education, skills, and employability; and (5) agency and resilience. This review summarizes the evidence of the impact of TB disease and treatment on these five domains of AYA well-being
