Sedation practice: is it time to wake up and embrace change?

    Get PDF
Recommendations for sedation regimes in the intensive care unit (ICU) have evolved over the last decade based on findings that relate the clinical approach to improved patient outcomes. Martin and co-workers conducted two surveys into German sedation practice covering the time period during which these changes occurred and as such provide an insight into how these recommendations are being incorporated into everyday clinical practice

Haemodynamic optimisation: are we dynamic enough?

    Get PDF
Perioperative haemodynamic optimisation of high-risk surgical patients has long been documented to improve both short-term and long-term outcomes, as well as to reduce the rate of postoperative complications. Based on the evidence, cardiac output monitoring and fluid resuscitation, combined with the use of inotropes, would seem to be the gold standard of care for these difficult surgical cases. However, clinicians do not universally apply these techniques and principles in their everyday practice. By exploring the reasons why this is so, perhaps we could move forward in the standardisation of care and the application of evidence-based practice

Early Exposure/Long-Term Gains: Encouraging Underrepresented Middle School Students to Pursue STEM Careers

    Get PDF
Preparing middle school students in STEM for their transition to high school and ultimately for post-secondary endeavors requires early exposure to STEM subjects. This early exposure not only builds a foundation for the ever-changing skills that are needed for the workforce, but also boosts student achievement and test scores (STEM Education Works, 2021). The MUREP project focuses on increasing the number of underrepresented minority students interested in STEM fields. The purpose of this study is to examine underrepresented minority middle school students’ attitudes toward STEM and their expressed interest in STEM post-secondary subjects and careers after participation in the MUREP project. The study employed a quantitative survey research design. The sample of the study was comprised of 418 middle school 6th, 7th, and 8th grade students. The data of the current study were collected using the Middle and High School (6-12th) survey. The results of the study indicate over 50% of the students expressed a positive attitude and interest in the STEM subjects. Many of the students also showed interest in continuing their STEM education and pursuing post-secondary STEM careers. Fifty-eight percent (58%) of the students expressed interest in specifically identified STEM post-secondary careers and subjects

Biomimetic direction of arrival estimation for resolving front-back confusions in hearing aids

    Get PDF
Sound sources at the same angle in front or behind a two-microphone array (e.g., bilateral hearing aids) produce the same time delay and two estimates for the direction of arrival: A front-back confusion. The auditory system can resolve this issue using head movements. To resolve front-back confusion for hearing-aid algorithms, head movement was measured using an inertial sensor. Successive time-delay estimates between the microphones are shifted clockwise and counterclockwise by the head movement between estimates and aggregated in two histograms. The histogram with the largest peak after multiple estimates predicted the correct hemifield for the source, eliminating the front-back confusions

Quality of life and persisting symptoms in intensive care unit survivors: implications for care after discharge

Abstract

Background

We assessed the quality of life of ICU survivors using SF-36 at 4 months after ICU discharge and investigated any correlation of PCS and MCS with age, illness severity and hospital or ICU length of stay. We examined the relationship between these variables, persisting physical and psychological symptoms and the perceived benefit of individual patients of follow-up.

Findings

For one year, adult patients admitted for multiple organ or advanced respiratory support for greater than 48 hours to a 16-bedded teaching hospital general intensive care unit were identified. Those surviving to discharge were sent a questionnaire at 4 months following ICU discharge assessing quality of life and persisting symptoms. Demographic, length of stay and illness severity data were recorded. Higher or lower scores were divided at the median value. A two-tailed Students t-test assuming equal variances was used for normally-distributed data and Mann-Whitney tests for non-parametric data.

87 of 175 questionnaires were returned (50%), but only 65 had sufficient data giving a final response rate of 37%. Elderly patients had increased MCS as compared with younger patients. The PCS was inversely related to hospital LOS. There was a significant correlation between the presence of psychological and physical symptoms and desire for follow-up.

Conclusion

Younger age and prolonged hospital stay are associated with lower mental or physical quality of life and may be targets for rehabilitation. Patients with persisting symptoms at 4 months view follow-up as beneficial and a simple screening questionnaire may identify those likely to attend outpatient services.</p

Analysing Slavery through Satellite Technology: How Remote Sensing Could Revolutionise Data Collection to Help End Modern Slavery

    Get PDF
An estimated 40.3 million people are enslaved globally across a range of industries. Whilst these industries are known, their scale can hinder the fight against slavery. Some industries using slave labour are visible in satellite imagery, including mining, brick kilns, fishing and shrimp farming. Satellite data can provide supplementary details for large scales which cannot be easily gathered on the ground. This paper reviews previous uses of remote sensing in the humanitarian and human rights sectors and demonstrates how Earth Observation as a methodology can be applied to help achieve the United Nations Sustainable Development Goal target 8.7
