    The Skin Ivory Spot. A Possible Indicator for Skinfield Photo-Carcinogenesis in Recreational Sunbed Addicts

    Introduction: For a decade or so, artificial sources of restricted light wavelengths, particularly sunbeds, have progressively gained popularity among adolescents and young adults. Warnings were raised focusing on the risk of accelerated photoaging and photocarcinogenesis. The ULEV (ultraviolet light-enhanced visualization) method is a convenient noninvasive way identifying subtle pigmentary changes presenting as a mottled subclinical melanoderma (MSM). Of note, rare spotty amelanotic macules presenting as skin ivory spots (SIS) was reported on any part of the body. Subjects and method: This work is the first attempt at evaluating the changes in the MSM and SIS spots developed on the skin of 33 phototype III young women designated as avid users involved in frequent exposures to sunshine and sunbeds for lifestyle purposes for a duration of at least 120 months. Results: MSM was markedly heterogeneous and was distinctly obvious in the majority of adepts of frequent natural and artificial photoexposures. SIS was particularly developed in subjects presenting with severe MSM patterns. Discussion: MSM and SIS are more severe in subjects frequently exposed to sunbeds and sun exposures. These signs possibly represent a risk marker for field photocarcinogenesis

    Identification of six new susceptibility loci for invasive epithelial ovarian cancer.

    Genome-wide association studies (GWAS) have identified 12 epithelial ovarian cancer (EOC) susceptibility alleles. The pattern of association at these loci is consistent in BRCA1 and BRCA2 mutation carriers who are at high risk of EOC. After imputation to 1000 Genomes Project data, we assessed associations of 11 million genetic variants with EOC risk from 15,437 cases unselected for family history and 30,845 controls and from 15,252 BRCA1 mutation carriers and 8,211 BRCA2 mutation carriers (3,096 with ovarian cancer), and we combined the results in a meta-analysis. This new study design yielded increased statistical power, leading to the discovery of six new EOC susceptibility loci. Variants at 1p36 (nearest gene, WNT4), 4q26 (SYNPO2), 9q34.2 (ABO) and 17q11.2 (ATAD5) were associated with EOC risk, and at 1p34.3 (RSPO1) and 6p22.1 (GPX6) variants were specifically associated with the serous EOC subtype, all with P < 5 × 10(-8). They may also represent complications supervening in an already treated diabetic patient. Some of these dermatoses (acanthosis nigricans, purpuric and pigmented capillaritis) are markers of macrovascular complications. The same disorders and some others (xerosis, Dupuytren's disease) are more frequently associated with microangiopathy in Type II diabetes. Other skin diseases (alopecia areata, vitiligo) are markers of auto-immunity in Type I diabetes

    Les indications thérapeutiques émergentes de l’IRM (« immune response modifier ») imiquimod 5% en crème (Aldara®) sont des carcinomes basocellulaires peu agressifs et les kératoses actiniques. La maladie de Bowen est un autre candidat potentiel pour une telle option thérapeutique innovatrice. Cet IRM peut s’avérer particulièrement précieux, sous supervision dermatologique, tant en milieu hospitalier qu’en dehors, et ce, particulièrement chez des patients à mobilité réduite et chez ceux refusant toute intervention chirurgicale, de même que dans le cadre de lésions cutanées multiples et/ou difficilement accessibles à d’autres thérapeutiques. L’imiquimod s’affirme à la fois comme un traitement non-invasif et comme une alternative à d’autres techniques utilisées en cancérologie cutanée (cryothérapie, curetage, exérèse chirurgicale, radiothérapie, 5-fluorouracile topique, photothérapie dynamique, …)
