    Is cancer prevention influenced by the built environment? A multidisciplinary scoping review

Globally, the growing prevalence of preventable cancers suggests a need to understand how features of the built environment shape exposure to cancer development and distribution within a population. Methods: We undertook a scoping review of how researchers across disparate fields understand and discuss the built environment in primary and secondary cancer prevention. We focused exclusively on peer-reviewed sources published from research conducted in Australia, Canada, Ireland, New Zealand, the United Kingdom, and the United States from 1990 to 2017. Results: The review captured 9958 potential results in the academic literature. We scoped this body of results to 268 relevant peer-reviewed journal articles indexed across 14 subject databases. Spatial proximity, transportation, land use and housing are well understood features of the built environment that shape cancer risk. Conclusions: Built environment features predominantly influence air quality, substance use, diet, physical activity and screening adherence, with impacts on breast, lung, colorectal, and overall cancer risk. The majority of evidence fails to provide direct recommendations for advancing cancer prevention policy and program objectives for municipalities.     Environmental, maternal, and reproductive risk factors for childhood acute lymphoblastic leukemia in Egypt : a case-control study

    BACKGROUND\ud Acute lymphocytic leukemia (ALL) is the most common pediatric cancer. The exact cause is not known in most cases, but past epidemiological research has suggested a number of potential risk factors. This study evaluated associations between environmental and parental factors and the risk for ALL in Egyptian children to gain insight into risk factors in this developing country.\ud METHODS\ud We conducted a case-control design from May 2009 to February 2012. Cases were recruited from Children's Cancer Hospital, Egypt (CCHE). Healthy controls were randomly selected from the general population to frequency-match the cumulative group of cases by sex, age groups (<1; 1 - 5; >5 - 10; >10Ā years) and region of residence (Cairo metropolitan region, Nile Delta region (North), and Upper Egypt (South)). Mothers provided answers to an administered questionnaire about their environmental exposures and health history including those of the father. Odds ratios (ORs) and 95Ā % confidence intervals (CI) were calculated using logistic regression with adjustment for covariates.\ud RESULTS\ud Two hundred ninety-nine ALL cases and 351 population-based controls frequency-matched for age group, gender and location were recruited. The risk of ALL was increased with the mother's use of medications for ovulation induction (ORadjā€‰=ā€‰2.5, 95Ā % CI =1.2 -5.1) and to a lesser extend with her age (ORadjā€‰=ā€‰1.8, 95Ā % CIā€‰=ā€‰1.1 - 2.8, for mothers ā‰„ 30Ā years old). Delivering the child by Cesarean section, was also associated with increased risk (ORadjā€‰=ā€‰2.01, 95Ā % CI =1.24-2.81).\ud CONCLUSIONS\ud In Egypt, the risk for childhood ALL appears to be associated with older maternal age, and certain maternal reproductive factors

    Population mixing, socioeconomic status and incidence of childhood acute lymphoblastic leukaemia in England and Wales: analysis by census ward

    In this population-based study of acute lymphoblastic leukaemia (ALL) diagnosed among children aged under 15 years in England and Wales during 1986ā€“1995, we analysed incidence at census ward level in relation to a range of variables from the 1991 census, which could be relevant to theories of infectious aetiology. ā€˜Population-mixing' measures, used as surrogates for quantity and diversity of infections entering the community, were calculated from census data on the origins and destinations of migrants in the year before the census. Incidence at ages 1ā€“4 years tended independently to be higher in rural wards, to increase with the diversity of origin wards from which in-migrants had moved during the year before the census, and to be lower in the most deprived areas as categorised by the Carstairs index. This last association was much weaker when urban/rural status and in-migrants' diversity were allowed for. There was no evidence of association with population mixing or deprivation for ALL diagnosed at ages 0 or 5ā€“14 years. The apparent specificity to the young childhood age group suggests that these associations are particularly marked for precursor B-cell ALL, with the disease more likely to occur when delayed exposure to infection leads to increased immunological stress, as predicted by Greaves. The association with diversity of incomers, especially in rural areas, is also consistent with the higher incidence of leukaemia predicted by Kinlen, where population mixing results in below average herd immunity to an infectious agent

    Association of total energy intake and macronutrient consumption with colorectal cancer risk: results from a large population-based case-control study in Newfoundland and Labrador and Ontario, Canada

    <p>Abstract</p> <p>Background</p> <p>Diet is regarded as one of the most important environmental factors associated with colorectal cancer (CRC) risk. A recent report comprehensively concluded that total energy intake does not have a simple relationship with CRC risk, and that the data were inconsistent for carbohydrate, cholesterol and protein. The objective of this study was to identify the associations of CRC risk with dietary intakes of total energy, protein, fat, carbohydrate, fiber, and alcohol using data from a large case-control study conducted in Newfoundland and Labrador (NL) and Ontario (ON), Canada.</p> <p>Methods</p> <p>Incident colorectal cancer cases (n = 1760) were identified from population-based cancer registries in the provinces of ON (1997-2000) and NL (1999-2003). Controls (n = 2481) were a random sample of residents in each province, aged 20-74 years. Family history questionnaire (FHQ), personal history questionnaire (PHQ), and food frequency questionnaire (FFQ) were used to collect study data. Logistic regression was used to evaluate the association of intakes of total energy, macronutrients and alcohol with CRC risk.</p> <p>Results</p> <p>Total energy intake was associated with higher risk of CRC (OR: 1.56; 95% CI: 1.21-2.01, <it>p</it>-trend = 0.02, 5<sup>th </sup>versus 1<sup>st </sup>quintile), whereas inverse associations emerged for intakes of protein (OR: 0.85, 95%CI: 0.69-1.00, <it>p</it>-trend = 0.06, 5<sup>th </sup>versus 1<sup>st </sup>quintile), carbohydrate (OR: 0.81, 95%CI: 0.63-1.00, <it>p</it>-trend = 0.05, 5<sup>th </sup>versus 1<sup>st </sup>quintile) and total dietary fiber (OR: 0.84, 95% CI:0.67-0.99, <it>p</it>-trend = 0.04, 5<sup>th </sup>versus 1<sup>st </sup>quintile). Total fat, alcohol, saturated fatty acids, monounsaturated fatty acids, polyunsaturated fatty acids, and cholesterol were not associated with CRC risk.</p> <p>Conclusion</p> <p>This study provides further evidence that high energy intake may increase risk of incident CRC, whereas diets high in protein, fiber, and carbohydrate may reduce the risk of the disease.</p
