Financial and relational impact of having a boy with posterior urethral valves
IntroductionChildhood chronic diseases affect family functioning and well-being. The aim of this study was to measure the impact of caring for a child with PUV, and the factors that most impact the burden of care.Patients and methodWe gave a questionnaire on the familial impact of having a child with posterior urethral valves to all parents of a child included in the CIRCUP trial from 2015 onwards. The questionnaire included questions about the parents' demographics, health, professional, financial and marital status and how these evolved since the child's birth as well as the “impact on family scale” (IOFS), which gives a total score ranging from 15 (no impact) to 60 (maximum impact). We then analyzed both the results of the specific demographic questions as well as the factors which influenced the IOFS score.ResultsWe retrieved answers for 38/51 families (74.5% response rate). The average IOFS score was 23.7 (15–51). We observed that the child's creatinine level had an effect on the IOFS score (p = 0.02), as did the parent's gender (p = 0.008), health status (p = 0.015), being limited in activity since the birth of the child (p = 0.020), being penalized in one's job (p = 0.009), being supported in one's job (p = 0.002), and decreased income (p = 0.004). Out of 38 mother/father binomials, 8/33 (24.2%) declared that they were no longer in the same relationship afterwards.ConclusionIn conclusion, having a boy with PUV significantly impacts families. The risk of parental separation and decrease in revenue is significant. Strategies aiming to decrease these factors should be put in place as soon as possible
Incisives centrales maxillaires et symétrie : un enjeu orthodontique majeur
Les incisives centrales maxillaires jouent un rôle déterminant dans le rendu esthétique
du sourire. La correction de leur malposition et notamment de leur asymétrie est
d’ailleurs l’un des principaux motifs de consultation des patients qui souhaitent
bénéficier d’un traitement orthodontique. L’étiologie de cette asymétrie qui peut être
verticale (une incisive centrale vis-à -vis de son homologue contro-latérale) ou
horizontale (milieu inter-incisif par rapport au plan sagittal médian) sera objectivée au
moyen de l’examen clinique et des examens complémentaires, en particulier radiologiques,
afin de choisir le plan de traitement le plus adapté. En fonction de l’étiopathogénie
(traumatisme alvéolo-dentaire, asymétrie squelettique, asymétrie d’arcade...), le
traitement orthodontique interviendra ensuite Ă trois niveaux : localement au niveau des
dents elles-mêmes (ingression, égression), globalement au niveau de l’arcade (translation,
recul en masse) et régionalement au niveau squelettique (chirurgie maxillo-faciale). La
complexité de certains traitements liée à la coordination des différents niveaux d’action
thérapeutique est parfois accrue par la nécessité de faire intervenir d’autres spécialités
(chirurgien-dentiste, parodontiste, chirurgien maxillo-facial). La planification initiale
dĂ©taillĂ©e revĂŞt ici toute son importance, de mĂŞme que le choix des praticiens associĂ©s Ă
la thérapeutique. Ces traitements, une fois terminés, sont cependant une source de grande
satisfaction pour le praticien et son patient en raison de l’amélioration esthétique très
probante obtenue
Robust Day-Ahead Forecasting of Household Electricity Demand and Operational Challenges
In the recent years, several short-term forecasting models of household electricity demand have been proposed in the literature. This is partly due to emerging smart-grid applications, which require these kinds of forecasts to manage systems such as smart homes, prosumer aggregations, etc., and partly thanks to the availability of data from smart meters, which enable the development of such models. Since most models are academically developed, they often do not address challenges related to their implementation in a real-world environment. In the latter case, several issues arise, related to data quality and availability, which affect the operational performance and robustness of a forecasting system. In this paper, we design a hierarchical forecasting framework based on a total of 5 probabilistic models of varying complexity, after analyzing the respective performance and advantages of the models with an offline dataset. This multi-layered framework is necessary to address the various problematic situations occurring in practice and abide by the requirements for a real-world deployment. The forecasting system is deployed in a real-world case and evaluated here on data from 20 households. Field data, comprising forecasts and measurements, are analyzed for each household. A detailed comparison is drawn between the online and offline performances. Since a notable degradation is observed in the operational environment, we discuss at length the reasons for such an effect. We determine that the exact settings of the training and test periods are marginally responsible, but that the main cause is the intrinsic evolution of the demand time series, which hinders the forecasting performance. This evolution is due to unknown household characteristics that need to be monitored to provide more adaptable models
La déglutition dysfonctionnelle a-t-elle une influence sur la posture ?
La déglutition est une des premières fonctions à se mettre en
place chez le fœtus pour des raisons vitales. Elle connaît ensuite
une maturation physiologique et psychique pour passer d'un Ă©tat
dysfonctionnel Ă un Ă©tat fonctionnel. Chez certains individus, la
maturation est incomplète, et la déglutition reste dysfonctionnelle.
L'incidence d'un changement d'occlusion dentaire, d'une dysfonction linguale
sur la posture est montrée dans la littérature clinique. Ce travail
se propose de découvrir si la déglutition dysfonctionnelle, par
l'absence de contact dentaire et par la dysfonction linguale qui la
caractérisent, a une incidence sur la posture. L' étude porte sur une
population de 20 adultes jeunes, scindée en deux groupes : un groupe de
sujets présentant une déglutition fonctionnelle, et un groupe de
sujets présentant une déglutition dysfonctionnelle. Le protocole
expérimental met en jeu quatre conditions : repos mandibulaire,
tâche cognitive d'articulation (le sujet articule une suite de nombres
qui l'oblige à réfléchir à ce qu'il énonce),
déglutition fonctionnelle, déglutition dysfonctionnelle. Leur effet
sur la posture est évalué au moyen d'une plate-forme
stabilométrique normalisée, et complété par une étude
électromyographique d'un muscle manducateur (le muscle masséter) et
d'un muscle de la posture céphalique (le muscle
sterno-cléïdo-mastoïdien). Les résultats montrent que la
déglutition aurait les mêmes effets posturaux que la tâche
cognitive en augmentant les oscillations posturales et l'Ă©nergie
dépensée par le système postural. Et cette déglutition
aurait des effets accrus lorsqu'elle correspond à une déglutition
forcée non naturelle pour le sujet
Détection et thérapeutique des troubles temporo-mandibulaires en orthodontie
L'orthodontiste est confronté chaque jour à des troubles de
l'occlusion, dont la prise en charge lui est habituelle. En revanche, la
gestion de troubles temporo-mandibulaires est moins Ă©vidente, que ce
soit au niveau détection, diagnostic ou prise en charge
thérapeutique.
Aussi, une connaissance approfondie de l'anatomo-physiologie de
l'articulation temporo-mandibulaire, de ses mécanismes d'action est
indispensable pour permettre au praticien de détecter et diagnostiquer
les différentes pathologies résultant de phénomènes
étiologiques variés.
Le diagnostic rigoureux, au moyen d'un examen clinique approfondi et
d'examens complémentaires axiographiques et radiologiques, du
dysfonctionnement temporo-mandibulaire et de ses Ă©tiologies (notamment
dento-alvéolaires et occlusales) permettra de mettre en place une
thérapeutique appropriée, qu'elle soit orthodontique pure, ou en
collaboration avec des confrères d'autres disciplines (occlusodontiste,
chirurgien, ostéopathe...)
ELMAS: a one-year dataset of hourly electrical load profiles from 424 French industrial and tertiary sectors
International audienceAbstract The combination of ongoing urban expansion and electrification of uses challenges the power grid. In such a context, information regarding customers’ consumption is vital to assess the expected load at strategic nodes over time, and to guide power system planning strategies. Comprehensive household consumption databases are widely available today thanks to the roll-out of smart meters, while the consumption of tertiary premises is seldom shared mainly due to privacy concerns. To fill this gap, the French main distribution system operator, Enedis, commissioned Mines Paris to derive load profiles of industrial and tertiary sectors for its prospective tools. The ELMAS dataset is an open dataset of 18 electricity load profiles derived from hourly consumption time series collected continuously over one year from a total of 55,730 customers. These customers are divided into 424 fields of activity, and three levels of capacity subscription. A clustering approach is employed to gather activities sharing similar temporal patterns, before averaging the associated time series to ensure anonymity
Assessment of probabilistic PV production forecasts performance in an operational context
International audienceNowadays, solar power (PV) capacity is undergoing a fast growth. The development of network management systems facilitating its penetration in the distribution network may rely on individual forecasts for each PV plant connected to the grid. This paper describes a probabilistic model for short-term forecasting of PV production which has been developed and tested under operational conditions in the frame of the Nice Grid demonstrator project in France. Detailed results on the performance of the forecasting tool are presented both in terms of deterministic and probabilistic forecasts for a portfolio of 35 PV installations. The results show that in general, even at the household producer level, the forecasts yield good performance

L’ankylose temporo-mandibulaire touche préférentiellement l’enfant et l’adulte jeune.
Malgré sa rareté, c’est une pathologie extrêmement invalidante dont les conséquences au
niveau de la croissance, du développement morphologique et du développement personnel sont
majeures. La connaissance de ses Ă©tiologies, de ses formes anatomiques et de ses signes
cliniques est indispensable à sa prise en charge. De ces connaissances va découler
également le choix de la thérapeutique la plus adaptée, qui pourra présenter malgré tout
des échecs et des récidives, nécessitant une réhabilitation prothétique
