Removing foreign bodies in bariatric patient
The ingestion of foreign bodies or food bolus impaction occur at different natural or pathologic levels of narrowing of the esophagus. In bariatric patients, postoperative "strictures" could arise at the level of the gastric body (in sleeve gastrectomy) or the gastrojejunal anastomosis (in Roux-en-Y gastric bypass) or at the level of surgical foreign body insertion: the ring (for vertical banded gastroplasty) and band (for laparoscopic adjustable banding). In these patients, food intolerance is a common complication requiring combination of medical, endoscopic, and surgical management that we review here. We emphasize the role of endoscopic dilation of strictures and the newly described endoscopic techniques for the removal of dysfunctioning ring or band after restrictive surgery, by the use of a temporary self-expandable stent insertion and/or of a wire to initiate band/ring cutting. Novel biliary self-expanding metal stents: indications and applications.
Endoscopic insertion of a self-expanding metal stent (SEMS) through a malignant common bile duct stricture is the first line of palliation for malignant jaundice. Patency of these stents remains a major concern. SEMS dysfunction can result from tumor ingrowth, overgrowth and/or clogging. Initial SEMS modifications involved covering the central part of the stent in order to reduce ingrowth and ultimately increase patency. Fully covered stents became available shortly after reports of their use in human patients. The potential removability and radial strength of SEMS have led to evaluation of their use in new indications including benign biliary strictures, post sphincterotomy bleeding and perforation. Other aspects of development include the addition of features such as anti-reflux valves, drug elution and spontaneous biodegradability. Endoskopische Therapie der Adipositas
Morbid obesity is defined by Body Mass Index (BMI) of 40 kg/m2 or more, or by a BMI of 35 kg/m2 or more in the presence of co morbidities. The only effective therapy for it is currently surgery. Efficacy varies with the type of procedure. Restrictive forms include lap band, vertical gastroplasty and sleeve gastrectomy, a malabsorptive form is the biliopancreatic diversion, and a gastric by-pass is a combination of both. Endoluminal surgery, performed entirely through natural orifice, offers the potential for a less invasive weight loss procedure. We review the various endoluminal techniques which are currently either in routine use or in evaluation, and will also address the role of endoscopy in management of complications occurring after bariatric surgery. The two SH2-domain-containing inositol 5-phosphatases SHIP1 and SHIP2 are coexpressed in human T lymphocytes.
The activation of many hematopoietic cells via cytokine receptors, as well as B and T cell receptors, leads to the tyrosine phosphorylation of Shc and its association with both Grb2-Sos1 complexes and with a 145 kDa protein referred to as the SH2 containing inositol 5-phosphatase (SHIP1). In a search of putative 5-phosphatase isoenzymes, we have isolated a second SH2 domain containing inositol 5-phosphatase, referred to as (SHIP2). Both SHIP1 and SHIP2 are coexpressed in human T lymphocytes. This was shown at the protein level by Western blot analysis in transformed T cell lines and in peripheral blood T lymphocytes either unstimulated or after in vitro activation through TCR-CD3 complex. SHIP1 protein level was not modulated after activation of T lymphocytes, in contrast to SHIP2, which was increased after long-term stimulation. SHIP1 was tyrosine phosphorylated in resting naive T cells. This was not observed in the transformed T cell lines.
