Incidencia y factores de riesgo para el desarrollo de eventos cardíacos en pacientes con leucemia mieloblástica aguda
La incidencia de morbi-mortalidad de origen cardíaco no está bien establecida en la leucemia mieloblástica aguda (LMA). Los pacientes con LMA reciben múltiples fármacos con potencial cardiotóxico como las antraciclinas, así como fármacos que pueden prolongar el intervalo QT en el electrocardiograma. Por otro lado, los eventos cardíacos también pueden surgir en pacientes con antecedentes cardiológicos previos, o debido a complicaciones de la propia leucemia. El objetivo de la presente tesis doctoral es evaluar la incidencia de eventos cardíacos en pacientes con LMA e identificar los factores de riesgo para su aparición. Se realizó un estudio retrospectivo de todos los pacientes con LMA diagnosticados y/o tratados en el Hospital La Fe con una revisión exhaustiva de las historias clínicas. Las conclusiones principales de la tesis fueron:
1. La incidencia de eventos cardíacos en pacientes adultos sometidos a terapia de LMA es elevada, con una morbi-mortalidad significativa debido a la cardiotoxicidad. Más de la mitad de los pacientes presentaron eventos cardíacos y un 3,6% falleció debido a complicaciones cardiacas.
2. Los eventos cardíacos se presentaron de forma consistente en las distintas líneas de tratamiento que recibieron los pacientes a lo largo de la evolución de la LMA.
3. La principal fase de tratamiento durante la que se produjeron eventos cardíacos fue el ciclo de inducción, pero también los ciclos de posremisión y el periodo posterior al trasplante estuvieron acompañados de un número significativo de eventos.
4. En los pacientes tratados con quimioterapia intensiva, el desarrollo de eventos cardíacos de grado 3-4 tuvo un impacto negativo en la supervivencia global y supervivencia libre de evento.
5. Respecto a los factores pronósticos, la presencia de antecedentes cardíacos relevantes se asoció a un mayor riesgo de eventos cardíacos mortales; y la edad avanzada, los antecedentes cardíacos relevantes y el uso de esquemas de tratamiento no intensivos se asociaron al desarrollo de eventos cardíacos no mortales.
6. La incidencia de insuficiencia cardíaca fue del 18,4%, la mayoría fue de grado 3-4, con una mortalidad de 2,1%. El ECOG superior a 1 se relacionó con desarrollo de insuficiencia cardíaca grado 3-4.
7. La incidencia de arritmia cardíaca fue del 12,2%, la mayoría de eventos fue de grado 3-4, con una mortalidad del 1,1%. La edad mayor de 65 años y el ECOG superior a 1 se relacionaron con mayor incidencia de arritmia grado 3-4.
8. La incidencia de prolongación del intervalo QTc fue del 13,9%, la mayoría de eventos fue grado 1-2, y no hubo mortalidad asociada. La edad mayor de 65 años, los antecedentes cardiológicos relevantes, la inclusión en ensayo clínico y el uso de inhibidores de FLT3 se relacionaron con desarrollo de prolongación del intervalo QTc grado 1-2.
9. La incidencia de isquemia fue del 3,8%, y no hubo mortalidad asociada. Ningún factor se relacionó con desarrollo de isquemia miocárdica.
10. La identificación de los pacientes con alto riesgo de complicaciones cardíacas puede ser útil para implementar monitorización intensiva en los mismos.The incidence of cardiac morbimortality in acute myeloid leukemia (AML) is not well known. We aim to estimate the cumulative incidence (CI) of cardiac events in AML patients and to identify risk factors for their occurrence. Among 571 newly diagnosed AML patients, 26 (4.6%) developed fatal cardiac events, and among 525 treated patients, 19 (3.6%) experienced fatal cardiac events(CI: 2% at 6 months; 6.7% at 9 years). Prior heart disease was associated with the development of fatal cardiac events (hazard ratio (HR) = 6.9 ). The CI of non-fatal cardiac events was 43.7% at 6 months and 56.9% at 9 years. Age ≥ 65 (HR = 2.2), relevant cardiac antecedents (HR = 1.4), and non-intensive chemotherapy (HR = 1.8) were associated with non-fatal cardiac events. The 9-year CI of grade 1–2 QTcF prolongations was 11.2%, grade 3 was 2.7%, and no patient had grade 4–5 events. The 9-year CI of grade 1–2 cardiac failures was 1.3%, grade 3–4 was 15%, and grade 5 was 2.1%; of grade 1–2, arrhythmia was 1.9%, grade 3–4 was 9.1%, and grade 5 was 1%. Among 285 intensive therapy patients, median overall survival decreased in those experiencing grade 3–4 cardiac events (p < 0.001). Impact of FLT3–ITD mutation status and its ratio in a cohort of 2901 patients undergoing upfront intensive chemotherapy: a PETHEMA registry study
FLT3–ITD results in a poor prognosis in terms of overall survival (OS) and relapse-free
survival (RFS) in acute myeloid leukemia (AML). However, the prognostic usefulness of the allelic
ratio (AR) to select post-remission therapy remains controversial. Our study focuses on the prognostic
impact of FLT3–ITD and its ratio in a series of 2901 adult patients treated intensively in the pre-FLT3
inhibitor era and reported in the PETHEMA registry. A total of 579 of these patients (20%) harbored FLT3–ITD mutations. In multivariate analyses, patients with an FLT3–ITD allele ratio (AR) of >0.5
showed a lower complete remission (CR rate) and OS (HR 1.47, p = 0.009), while AR > 0.8 was
associated with poorer RFS (HR 2.1; p < 0.001). Among NPM1/FLT3–ITD-mutated patients, median
OS gradually decreased according to FLT3–ITD status and ratio (34.3 months FLT3–ITD-negative,
25.3 months up to 0.25, 14.5 months up to 0.5, and 10 months ≥ 0.5, p < 0.001). Post-remission
allogeneic transplant (allo-HSCT) resulted in better OS and RFS as compared to auto-HSCT in
NPM1/FLT3–ITD-mutated AML regardless of pre-established AR cutoff (≤0.5 vs. >0.5). Using the
maximally selected log-rank statistics, we established an optimal cutoff of FLT3–ITD AR of 0.44
for OS, and 0.8 for RFS. We analyzed the OS and RFS according to FLT3–ITD status in all patients,
and we found that the group of FLT3–ITD-positive patients with AR < 0.44 had similar 5-year OS
after allo-HSCT or auto-HSCT (52% and 41%, respectively, p = 0.86), but worse RFS after auto-HSCT
(p = 0.01). Among patients with FLT3–ITD AR > 0.44, allo-HSCT was superior to auto-HSCT in terms
of OS and RFS. This study provides more evidence for a better characterization of patients with AML
harboring FLT3–ITD mutations
Impact of BCR-ABL1 Transcript Type on Response, Treatment-Free Remission Rate and Survival in Chronic Myeloid Leukemia Patients Treated with Imatinib
The most frequent BCR-ABL1-p210 transcripts in chronic myeloid leukemia (CML) are e14a2 and e13a2. Imatinib (IM) is the most common first-line tyrosine-kinase inhibitor (TKI) used to treat CML. Some studies suggest that BCR-ABL1 transcript types confer different responses to IM. The objective of this study was to correlate the expression of e14a2 or e13a2 to clinical characteristics, cumulative cytogenetic and molecular responses to IM, acquisition of deep molecular response (DMR) and its duration (sDMR), progression rate (CIP), overall survival (OS), and treatment-free remission (TFR) rate. We studied 202 CML patients, 76 expressing the e13a2 and 126 the e14a2, and correlated the differential transcript expression with the above-mentioned parameters. There were no differences in the cumulative incidence of cytogenetic responses nor in the acquisition of DMR and sDMR between the two groups, but the e14a2 transcript had a positive impact on molecular response during the first 6 months, whereas the e13a2 was associated with improved long-term OS. E14a2 Transcript Favors Treatment-Free Remission in Chronic Myeloid Leukemia When Associated with Longer Treatment with Tyrosine Kinase Inhibitors and Sustained Deep Molecular Response
e13a2 and e14a2 are the most frequent transcript types of the BCR::ABL1 fusion gene in chronic myeloid leukemia (CML). The current goal with tyrosine kinase inhibitors (TKI) is to achieve sustained deep molecular response (DMR) in order to discontinue TKI treatment and remain in the so-called treatment-free remission (TFR) phase, but biological factors associated with these goals are not well established. This study aimed to determine the effect of transcript type on TFR in patients receiving frontline treatment with imatinib (IM) or second-generation TKI (2G-TKI). Patients treated at least 119 months with IM presented less post-discontinuation relapse than those that discontinued IM before 119 months (p = 0.005). In addition, cases with the e14a2 transcript type treated at least 119 months with IM presented a better TFR (p = 0.024). On the other hand, the type of transcript did not affect the cytogenetic or molecular response in 2G-TKI treated patients; however, the use of 2G-TKI may be associated with higher and earlier DMR in patients with the e14a2 transcript
Predicting Survival after Allogeneic Hematopoietic Cell Transplantation in Myelofibrosis : Performance of the Myelofibrosis Transplant Scoring System (MTSS) and Development of a New Prognostic Model
Accurate prognostic tools are crucial to assess the risk/benefit ratio of allogeneic hematopoietic cell transplantation (allo-HCT) in patients with myelofibrosis (MF). We aimed to evaluate the performance of the Myelofibrosis Transplant Scoring System (MTSS) and identify risk factors for survival in a multicenter series of 197 patients with MF undergoing allo-HCT. After a median follow-up of 3.1 years, 47% of patients had died, and the estimated 5-year survival rate was 51%. Projected 5-year risk of nonrelapse mortality and relapse incidence was 30% and 20%, respectively. Factors independently associated with increased mortality were a hematopoietic cell transplantation-specific comorbidity index (HCT-CI) ≥3 and receiving a graft from an HLA-mismatched unrelated donor or cord blood, whereas post-transplant cyclophosphamide (PT-Cy) was associated with improved survival. Donor type was the only parameter included in the MTSS model with independent prognostic value for survival. According to the MTSS, 3-year survival was 62%, 66%, 37%, and 17% for low-, intermediate-, high-, and very high-risk groups, respectively. By pooling together the low- and intermediate-risk groups, as well as the high- and very high-risk groups, we pinpointed 2 categories: standard risk and high risk (25% of the series). Three-year survival was 62% in standard-risk and 25% in high-risk categories (P <.001). We derived a risk score based on the 3 independent risk factors for survival in our series (donor type, HCT-CI, and PT-Cy). The corresponding 5-year survival for the low-, intermediate-, and high-risk categories was 79%, 55%, and 32%, respectively (P <.001). In conclusion, the MTSS model failed to clearly delineate 4 prognostic groups in our series but may still be useful to identify a subset of patients with poor outcome. We provide a simple prognostic scoring system for risk/benefit considerations before transplantation in patients with MF

Acute leukemia arising from myeloproliferative or myelodysplastic/myeloproliferative neoplasms: A series of 372 patients from the PETHEMA AML registry
PETHEMA group.Treatment of acute myeloid leukemia (AML) evolving from myeloproliferative (MPN) or myelodysplastic/myeloproliferative neoplasms (MDS/MPN) is challenging. We evaluated disease characteristics, treatment patterns and outcomes in 372 patients diagnosed with AML after MPN or MDS/MPN over a 27-year period. Frontline treatment was intensive chemotherapy (38%), hypomethylating agents [HMAs] (17%), non-intensive chemotherapy (14%), and supportive care (31%). Median overall survival was 4.8 months, with a 5-year survival rate of 4%. Median survival was 2.8, 3.9 and 8.3 months for the 1992-2010, 2011-2015 and 2016-2019 periods, respectively (test for trend p < 0.001). Complete response (CR) rate was higher with intensive chemotherapy (43%) than with non-intensive chemotherapy (12%) or HMAs (8.5%) [p < 0.001], but responses were short-lived without allogeneic hematopoietic cell transplantation. Patients treated with intensive chemotherapy or HMAs had superior survival than those receiving non-intensive chemotherapy (median: 8.5 vs. 8.6 vs. 4.2 months, respectively). No differences in treatment response or survival were observed according to prior disease subtypes. Patients undergoing transplantation in CR had better survival than those transplanted in other response categories (3-year survival rate of 64% vs. 22%, p = 0.002). Our results support the use of intensive chemotherapy followed by transplant whenever possible, and the preferential use of HMAs over attenuated chemotherapy regimens in unfit patients. In spite of the survival improvement in recent years, this subset of AML constitutes an unmet medical need and deserves systematic incorporation in clinical trials.Peer reviewe
Characteristics and outcomes of adult patients in the PETHEMA registry with relapsed or refractory FLT3-ITD mutation-positive acute myeloid leukemia
This retrospective study investigated outcomes of 404 patients with relapsed/refractory (R/R) FMS-like tyrosine kinase 3 (FLT3)-internal tandem duplication (ITD) acute myeloid leukemia (AML) enrolled in the PETHEMA registry, pre-approval of tyrosine kinase inhibitors. Most patients (63%) had received first-line intensive therapy with 3 + 7. Subsequently, patients received salvage with intensive therapy (n = 261), non-intensive therapy (n = 63) or supportive care only (n = 80). Active salvage therapy (i.e., intensive or non-intensive therapy) resulted in a complete remission (CR) or CR without hematological recovery (CRi) rate of 42%. More patients achieved a CR/CRi with intensive (48%) compared with non-intensive (19%) salvage therapy (p < 0.001). In the overall population, median overall survival (OS) was 5.5 months; 1- and 5-year OS rates were 25% and 7%. OS was significantly (p < 0.001) prolonged with intensive or non-intensive salvage therapy compared with supportive therapy, and in those achieving CR/CRi versus no responders. Of 280 evaluable patients, 61 (22%) had an allogeneic stem-cell transplant after they had achieved CR/CRi. In conclusion, in this large cohort study, salvage treatment approaches for patients with FLT3-ITD mutated R/R AML were heterogeneous. Median OS was poor with both non-intensive and intensive salvage therapy, with best long-term outcomes obtained in patients who achieved CR/CRi and subsequently underwent allogeneic stem-cell transplant.This study was supported by Centro de Investigación Biomédica en Red Cáncer (CIBERONC), Valencia, Spain [CB16/12/00284]
