La simulazione medica nel futuro: un tentativo di visioning The health simulation in the future: an attempt of visioning
La pratica clinica Ăš un componente indispensabile dellâapprendimento e del mantenimento delle competenze in molte delle discipline sanitarie e la simulazione offre lâopportunitĂ di praticare in maniera sicura ed efficace senza rischi per il paziente.
La simulazione nella didattica medica si Ăš giĂ ampiamente evoluta rispetto alle sue fasi iniziali. Lâintroduzione dei simulatori di pazienti umani, intorno alla fine del XX secolo, rappresenta una tappa fondamentale nella scienza dellâeducazione in sanitĂ . Fino ad oggi lâattenzione degli sviluppatori si Ăš concentrata da un lato sulla capacitĂ dei simulatori di dare feedback verosimili e coerenti con il comportamento del paziente e con la fisiologia umana e dallâaltro sullâinterazione fra simulatore e utilizzatore.
Le emergenti tecnologie nel campo della percezione sensoriale apporteranno miglioramenti significativi sia nella formazione in simulazione che nella gestione della simulazione stessa. Basandosi sullâanalisi delle tecnologie attualmente disponibili e in via di sviluppo, questo scritto vuole essere uno sforzo di visioning sul futuro delle tecniche di simulazione. Nano-sensori e nano-attuatori, usati nella realtĂ aumentata/virtuale/ mista, permetteranno sia la concettualizzazione di sistemi capaci di aumentare il livello di realismo e di immersione sensoriale ma anche di mappare i movimenti e le azioni al fine di generare protocolli univoci di intervento. Proiettandoci in un ipotetico 2025, la simulazione medica sarĂ unâesperienza democratica, largamente accessibile, dinamica e immersiva a 360°, grazie alle tecniche di implementazione neuro-percettive.Practice is a key component of skills learning and maintenance in many health disciplines and simulation offers the opportunity to practice in a safe and effective manner improving patient safety.
Simulation for healthcare training has already evolved through its initial historical phases. The introduction of human patient simulator toward the end of the 20th century was a milestone in health sciences education.
To date the attention of developers has been paid to the capability of the simulator to give feedback, consistent with patient behavior and physiologic response, and the interaction between simulator and users.
The emerging technologies in the field of sensorial perception will bring improvements both in education and in management of the simulation. Grounding on the analysis of current and under-development technology, the manuscript is an attempt of visioning simulation-based training in health education in a coming future. Nano-sensors and nano-actuators, used in augmented/virtual/mixed reality, will allow the conceptualization of systems increasing the level of realism and the sensorial immersion, and the mapping of movements and action in order to generate univocal protocols of intervention. Projecting ourselves in 2025, medical simulation will be a democratic, largely accessible, dynamic and 360° sensorial experience, thanks to techniques of neuro-perceptive implementation
Transition product design. ï»żA framework proposal for a holistic approach to systemic design
The Transition Design (TD) discipline is based on a design framework and methodology that prioritises social and environmental well-being and the goals of local resilience and radical changes in socio-technical contexts. Terry Irwin (2019) sees two aspects of TD as fundamental: the idea that entire societies will face a transition to sustainable futures and the realisation that this will entail changes at the systemic level. As part of the ongoing disciplinary debate, this contribution investigates the potential of new design drivers, such as territorial value and local resilience, linked to specific case studies that can be seen as best practices. The result is a theoretical framework of eight guidelines that aims to answer some of the TD processâs problems and model the requirements that can then be replicated
Evidence for perinatal and child health care guidelines in crisis settings: can Cochrane help?
<p>Abstract</p> <p>Background</p> <p>It is important that healthcare provided in crisis settings is based on the best available research evidence. We reviewed guidelines for child and perinatal health care in crisis situations to determine whether they were based on research evidence, whether Cochrane systematic reviews were available in the clinical areas addressed by these guidelines and whether summaries of these reviews were provided in Evidence Aid.</p> <p>Methods</p> <p>Broad internet searches were undertaken to identify relevant guidelines. Guidelines were appraised using AGREE and the clinical areas that were relevant to perinatal or child health were extracted. We searched The Cochrane Database of Systematic Reviews to identify potentially relevant reviews. For each review we determined how many trials were included, and how many were conducted in resource-limited settings.</p> <p>Results</p> <p>Six guidelines met selection criteria. None of the included guidelines were clearly based on research evidence. 198 Cochrane reviews were potentially relevant to the guidelines. These reviews predominantly addressed nutrient supplementation, breastfeeding, malaria, maternal hypertension, premature labour and prevention of HIV transmission. Most reviews included studies from developing settings. However for large portions of the guidelines, particularly health services delivery, there were no relevant reviews. Only 18 (9.1%) reviews have summaries in Evidence Aid.</p> <p>Conclusions</p> <p>We did not identify any evidence-based guidelines for perinatal and child health care in disaster settings. We found many Cochrane reviews that could contribute to the evidence-base supporting future guidelines. However there are important issues to be addressed in terms of the relevance of the available reviews and increasing the number of reviews addressing health care delivery.</p
Genome-wide association analyses identify new Brugada syndrome risk loci and highlight a new mechanism of sodium channel regulation in disease susceptibility.
Brugada syndrome (BrS) is a cardiac arrhythmia disorder associated with sudden death in young adults. With the exception of SCN5A, encoding the cardiac sodium channel Na1.5, susceptibility genes remain largely unknown. Here we performed a genome-wide association meta-analysis comprising 2,820 unrelated cases with BrS and 10,001 controls, and identified 21 association signals at 12 loci (10 new). Single nucleotide polymorphism (SNP)-heritability estimates indicate a strong polygenic influence. Polygenic risk score analyses based on the 21 susceptibility variants demonstrate varying cumulative contribution of common risk alleles among different patient subgroups, as well as genetic associations with cardiac electrical traits and disorders in the general population. The predominance of cardiac transcription factor loci indicates that transcriptional regulation is a key feature of BrS pathogenesis. Furthermore, functional studies conducted on MAPRE2, encoding the microtubule plus-end binding protein EB2, point to microtubule-related trafficking effects on Na1.5 expression as a new underlying molecular mechanism. Taken together, these findings broaden our understanding of the genetic architecture of BrS and provide new insights into its molecular underpinnings

premesse per una ricerca sul concetto di modello per la progettazione
La volontĂ di compiere unâanalisi sul concetto di âmodelloâ in relazione alle sue applicazioni in vari campi operativi del design, e alla sua reificazione in modelli pratici, nasce nellâambito dellâesperienza didattica svolta nei corsi di industrial design.
Grazie alla suddetta esperienza si Ăš potuto constatare:
- lâenorme importanza del ruolo dei modelli nel campo della progettazione;
- la insostituibilitĂ dei modelli quali strumenti essenziali di supporto alla ricerca, alla progettazione e alla comunicazione;
- la necessitĂ di una piĂč rigorosa cultura della progettualitĂ basata sulla conoscenza delle filosofie, delle metodologie e degli strumenti;
- la carenza di studi sulla metodologia di progettazione dal punto di vista dello strumento âmodelloâ;
- la conseguente disattenzione nella cultura per una trattazione sistematica tesa a razionalizzare un âapproccio per modelliâ al problema progettuale;
- lâabbandono, a metodi e operatori tradizionali o al genio e alla sensibilitĂ del singolo, del problema âmodelloâ.
Lâobiettivo di questo lavoro Ăš quindi preparare il campo per un successivo sviluppo che porti ad una vera e propria âteoria dei modelliâ nellâambito della progettazione, proponendo una trattazione sistematica del concetto e della funzione del modello, ottenuta raccogliendo e analizzando vari contributi evinti dalla letteratura e dagli ambiti operativi. FinalitĂ di questo lavoro Ăš intessere una trama di relazioni fra questi contributi per ricercare nuovi imput che producano nuove idee e nuove soluzioni per una âprogettualitĂ â moderna e pregnante.
Un obiettivo ipotizzabile Ăš provare ad indagare e âstudiare come nasca un linguaggioâ dei modelli âin un determinato campo operativoâ della progettazione âmediante tutti gli strumenti e i mezzi che gli appartengono, ora della geometria e ora della matematica, ora delle scienze naturali e ora della teoria e della critica, ora della tecnologia e ora della letteratura artistica o âscienza delle fontiâ, ora della psicologia della percezione e ora della semantica delle arti
