    A 62-year-old male came to emergency department (ED) with a complaint of ā€œseparation of footā€ during sleep at night. He noticed he lost his foot while getting up from bed to go to toilet at midnight. Upon arrival in ED, his blood pressure was 218/96 mmHg and random blood sugar 556mg/dl. The patient had large ketone bodies in urine. He was a known case of diabetes and hypertension for many years. He was on homeopathic medications for his diabetes, hypertension and diabetic foot. He refused any form of invasive treatment and hospitalization. The patient was adamant on not getting admitted and wanted to continue his homeopathic medications as before. He only wanted his leg stump covered with a dressing after which he was discharged

    Introduction: Bull injuries are common in India. An injury by a bull is more common in rural and agriculture dependent regions of the country. The incident causing injury by a bull can be sports related or work related. Work related injuries are almost mostly seen in farmers. Case presentation: We would like to present six cases of bull injury over a period of three years who reported to our EM. All six patients were admitted. Five out of six patients survived the hospital stay. Conclusion: Trauma sustained due to being hit by a large animal should be treated akin to a high velocity trauma and such a patient definitely warrants a period of observation even if the injuries are not life threatening. A high index of suspicion is needed for suspecting occult injuries. Fluid resuscitation and age of the patient are important considerations

    Seeking information about assistive technology: Exploring current practices, challenges, and the need for smarter systems

    Ninety percent of the 1.2 billion people who need assistive technology (AT) do not have access. Information seeking practices directly impact the ability of AT producers, procurers, and providers (AT professionals) to match a user's needs with appropriate AT, yet the AT marketplace is interdisciplinary and fragmented, complicating information seeking. We explored common limitations experienced by AT professionals when searching information to develop solutions for a diversity of users with multi-faceted needs. Through Template Analysis of 22 expert interviews, we find current search engines do not yield the necessary information, or appropriately tailor search results, impacting individualsā€™ awareness of products and subsequently their availability and the overall effectiveness of AT provision. We present value-based design implications to improve functionality of future AT-information seeking platforms, through incorporating smarter systems to support decision-making and need-matching whilst ensuring ethical standards for disability fairness remain

    STEP-UP: Enabling low-cost IMU sensors to predict the type of dementia during everyday stair climbing

    Posterior Cortical Atrophy is a rare but significant form of dementia which affects peoplesā€™ visual ability before their memory. This is often misdiagnosed as an eyesight rather than brain sight problem. This paper aims to address the frequent, initial misdiagnosis of this disease as a vision problem through the use of an intelligent, cost-effective, wearable system, alongside diagnosis of the more typical Alzheimerā€™s Disease. We propose low-level features constructed from the IMU data gathered from 35 participants, while they performed a stair climbing and descending task in a real-world simulated environment. We demonstrate that with these features the machine learning models predict dementia with 87.02% accuracy. Furthermore, we investigate how system parameters, such as number of sensors, affect the prediction accuracy. This lays the groundwork for a simple clinical test to enable detection of dementia which can be carried out in the wild

    Estimating need and coverage for five priority assistive products: a systematic review of global population-based research.

    INTRODUCTION: To improve access to assistive products (APs) globally, data must be available to inform evidence-based decision-making, policy development and evaluation, and market-shaping interventions. METHODS: This systematic review was undertaken to identify studies presenting population-based estimates of need and coverage for five APs (hearing aids, limb prostheses, wheelchairs, glasses and personal digital assistants) grouped by four functional domains (hearing, mobility, vision and cognition). RESULTS: Data including 656 AP access indicators were extracted from 207 studies, most of which (n=199, 96%) were cross-sectional, either collecting primary (n=167) or using secondary (n=32) data. There was considerable heterogeneity in assessment approaches used and how AP indicators were reported; over half (n=110) used a combination of clinical and self-reported assessment data. Of 35 studies reporting AP use out of all people with functional difficulty in the corresponding functional domains, the proportions ranged from 4.5% to 47.0% for hearing aids, from 0.9% to 17.6% for mobility devices, and from 0.1% to 86.6% for near and distance glasses. Studies reporting AP need indicators demonstrated >60% unmet need for each of the five APs in most settings. CONCLUSION: Variation in definitions of indicators of AP access have likely led to overestimates/underestimates of need and coverage, particularly, where the relationship between functioning difficulty and the need for an AP is complex. This review demonstrates high unmet need for APs globally, due in part to disparate data across this sector, and emphasises the need to standardise AP data collection and reporting strategies to provide a comparable evidence base to improve access to APs

    Antidiabetic activity of leaves of Anthocephalus indicus A. Rich. in alloxan induced diabetic rats

    The present study aims to examine the antidiabetic potential of leaves of Anthocephalus indicus A. Rich. The aqueous extract of leaves was screened for serum glucose lowering activity. Diabetes was induced in Sprague Dawley adult male rats by intra peritoneal injection of alloxan monohydrate at 80mg/kg bw. to the rats. Aqueous leaves extract of Anthocephalus indicus A. Rich. at 400mg/kg bw was given orally to control and diabetic rats for 21 days. Blood samples taken from retro orbital plexus of rats were analysed for serum glucose level, total cholesterol, high density lipoprotein (HDL- cholesterol) and low density lipoprotein (LDL- cholesterol) as per standard kit method. The rats feed with aqueous leaves extract showed significant reduction in blood glucose, total cholesterol, triglycerides, HDL and LDL as compared to diabetic rats. Aqueous leaf extract results for antidiabetic activity were compared with standard drug glibenclamide at 10mg/kg bw. and the antidiabetic acitivity was found to be significant. Histopathological study of liver and pancreas of rats showed that alloxan caused damage in the liver cells and degeneration of pancreatic islet cells. Administration of aqueous leaves extract caused an improvement in damaged liver cells and degenerated pancreatic islet cells. Thus, Anthocephalus indicus can be considered as a good natural antidiabetic drug

    India is the second major producer of fruits and vegetables in the world. Fruits form the rich source of bioactive molecules. Phytochemical compounds can provide different health benefits and give protection against various diseases as per the literature. As a result of this, the search for new bioactive molecules with multifunctional properties have been rapidly intensified. Pomegranate known as the ā€˜Natureā€™s power fruit' and ā€˜Fruit of dead' is one of the oldest fruit to be cultivated. Because of high nutritional value, these fruits are consumed at a greater extent. As a result, large amount of peel waste is generated. Disposal of these waste is of major concern. Thus, in the present study, an attempt was made to bring utilization of peels by finding the bioactive compounds present in the peels and further its application in food and cosmetics.
