    Conversion of Vernonia galamensis oil to pyridinyl-vernolamides and their antimicrobial activities

    Vernonia oil obtained by Soxhlet extraction from the seeds of Vernonia galamensis ssp. nairobensis was reacted with 2-aminopyridine, 2-(aminomethyl)pyridine and 2-(aminoethyl)pyridine using a 1:3 mole ratio of vernonia oil to amine at varied temperatures to give the corresponding vernolamides. The expected vernolamide from the reaction of 2-aminopyridine and the oil was not formed. N-(2-pyridinylmethyl)vernolamide and N-(2-pyridinylethyl)vernolamide exhibited antibacterial activity that was shown to be higher against gram-positive (Bacillus subtilis) than gram-negative (Escherichia coli) bacteria.     Subsurface profiling of granite pluton using microtremor method: southern Aravalli, Gujarat, India

    We report, using the microtremor method, a subsurface granitic pluton underneath the Narukot Dome and in its western extension along a WNW profile, in proximity of eastern fringe of Cambay Rift, India. The dome and its extension is a part of the Champaner Group of rocks belonging to the Mesoproterozoic Aravalli Supergroup. The present finding elucidates development of an asymmetric double plunge along Narukot Dome. Microtremor measurements at 32 sites were carried out along the axial trace (N95°) of the dome. Fourier amplitude spectral studies were applied to obtain the ratio between the horizontal and vertical components of persisting Rayleigh waves as local ambient noise. Fundamental resonant frequencies with amplitude ≥1-sigma for each site are considered to distinguish rheological boundary. Two distinct rheological boundaries are identified based on frequency ranges determined in the terrain: (1) 0.2219–10.364 Hz recorded at 31 stations identified as the Champaner metasediment and granite boundary, and (2) 10.902–27.1119 Hz recorded at 22 stations identified as the phyllite and quartzite boundary. The proposed equation describing frequency–depth relationship between granite and overlaying regolith matches with those already published in the literature. The morphology of granite pluton highlights the rootless character of Champaner Group showing sharp discordance with granitic pluton. The findings of manifestation of pluton at a shallower depth imply a steep easterly plunge within the Champaner metasediments, whereas signature of pluton at a deeper level implies a gentle westerly plunge. The present method enables to assess how granite emplacement influences the surface structure

    A two-dimensional Heisenberg model with random antiferromagnetic nearest-neighbor exchange is studied using quantum Monte Carlo techniques. As the strength of the randomness is increased, the system undergoes a transition from an antiferromagnetically ordered ground state to a gapless disordered state.     Re-examining the widespread policy of stopping sodium-glucose cotransporter-2 inhibitors during acute illness: A perspective based on the updated evidence

    Sodium-glucose cotransporter-2 (SGLT2) inhibitors are now seen as an integral part of therapy in type 2 diabetes to control not only blood glucose but to improve cardiovascular and kidney outcomes. Diabetic ketoacidosis (DKA) is an uncommon but serious complication of type 2 diabetes, which has a high case fatality rate. The absolute risk of DKA in large, prospective randomized clinical trials in people with type 2 diabetes using SGLT2 inhibitors has been low, although the relative risk is higher in those assigned to SGLT2 inhibitors compared with placebo. In those without diabetes but prescribed SGLT2 inhibitors for heart failure or chronic kidney disease, the risk of DKA is similar to placebo. Over the course of the COVID-19 pandemic, cases of DKA have also been reported in cases of COVID-19 hospitalizations. Consensus guidelines have recommended that SGLT2 inhibitors should be avoided in cases of serious illness and suggest they are not recommended for routine in-hospital use. However, recent data suggest potential beneficial effects of SGLT2 inhibitors in the setting of acute illness with COVID-19 with no increase in adverse events and low rates of DKA, which were non-severe. Given the low rates of DKA in cardiovascular outcome trials and in hospitalized patients with type 2 diabetes, the potential for SGLT2 inhibitors not being re-initiated following discharge and their cardiovascular and kidney benefits, we believe the practice of routine ‘sick day’ guidance should be re-examined based on current evidence with a call for further research in this area. Furthermore, high-quality trials of initiation of SGLT2 inhibitors in people admitted to hospital with cardiovascular disease or kidney disease, and trials of continuation of SGLT2 inhibitors in people, with careful monitoring of DKA should be conducted. These should be further supplemented with large observational studies

    Quantum Griffiths effects and smeared phase transitions in metals: theory and experiment

    In this paper, we review theoretical and experimental research on rare region effects at quantum phase transitions in disordered itinerant electron systems. After summarizing a few basic concepts about phase transitions in the presence of quenched randomness, we introduce the idea of rare regions and discuss their importance. We then analyze in detail the different phenomena that can arise at magnetic quantum phase transitions in disordered metals, including quantum Griffiths singularities, smeared phase transitions, and cluster-glass formation. For each scenario, we discuss the resulting phase diagram and summarize the behavior of various observables. We then review several recent experiments that provide examples of these rare region phenomena.     Reduction of cardiovascular risk in patients with metabolic syndrome in a community health center after a pharmaceutical care program of pharmacotherapy follow-up

    The objective of this study was to determine the impact of a pharmaceutical care (PC) program in a sample of public outpatients with metabolic syndrome (MS) who were being treated in Brazil's health system; the patients were randomized into PC or standard care. The pharmacotherapy follow-up (PF) was performed in a total of 120 patients with type 2 diabetes for 6 months. Adherence to treatment (measured with the Morisky test), negative outcomes associated with medication (NOM) and anthropometric and biochemical parameters were measured before and after PF. The Framingham scoring method was used to estimate changes in 10-year coronary heart disease risk scores in all patients. Ninety-six of 120 patients had characteristics of MS and were randomized into two groups (G): the control group (CG: 36) and the intervention group (IG: 38). Among the MS patients, 100% were taking a glucose-lowering drug; many were also taking anti-hypertensive drugs (CG: 72%; IG: 73%), and some patients were also taking hypolipemic drugs (CG: 12.0%; IG: 14.7%). Only 20.7% of the IG patients were considered adherent to their prescribed drugs. In the CG, an increase of coronary heart disease (CHD) risk (22±2 to 26±3; pO objetivo deste estudo foi o de determinar o impacto de um Programa de atenção Farmacêutica (AF) em uma amostra de pacientes ambulatoriais de Sistema Público de Saúde do Brasil portadores de Síndrome Metabólica, randomizados em AF ou atenção à saúde usual. Realizou-se o seguimento farmacoterapêutico com 120 pacientes com diabetes tipo 2 durante seis meses. Avaliou-se o nível de aderência ao tratamento (teste Morisky), resultados clínicos negativos associados a medicamentos (RNM), parâmetros bioquímicos e antropométricos, antes e após o seguimento. O método de Framingham foi usado para calcular as variações no risco de doenças coronarianas em 10 anos em todos os pacientes. Dos 120 pacientes, 96 tiveram características de SM e foram então randomizados em dois grupos (G): Controle (GC: 36) e Intervenção (GI: 38). Entre os pacientes com SM, 100% faziam uso de medicamentos para diminuir a glicose, anti-hipertensivos (GC: 72%; GI: 73%) e hipoglicemiantes (GC: 12.0%; GI: 14.7%). Apenas 20,7% do GI foram considerados aderentes aos fármacos prescritos. No GC foi observado aumento do risco de Doença Arterial Coronariana (DAC) (22±2 para 26±3; p<0,05), enquanto no GI foi observado redução (22±2 para 14±2%; p<0,01). O Programa de AF para pacientes com SM monitorados na atenção primária do Sistema de Saúde Pública brasileiro melhora o funcionamento do serviço resultando na melhoria clínica dos pacientes com redução do risco de doença cardiovascular em um período de dez anos
