Local Temperature and Universal Heat Conduction in FPU chains
It is shown numerically that for Fermi Pasta Ulam (FPU) chains with
alternating masses and heat baths at slightly different temperatures at the
ends, the local temperature (LT) on small scales behaves paradoxically in
steady state. This expands the long established problem of equilibration of FPU
chains. A well-behaved LT appears to be achieved for equal mass chains; the
thermal conductivity is shown to diverge with chain length N as N^(1/3),
relevant for the much debated question of the universality of one dimensional
heat conduction. The reason why earlier simulations have obtained
systematically higher exponents is explained.Comment: 4 pages, 3 figures, revised published versio
Energy localization on q-tori, long term stability and the interpretation of FPU recurrences
We focus on two approaches that have been proposed in recent years for the
explanation of the so-called FPU paradox, i.e. the persistence of energy
localization in the `low-q' Fourier modes of Fermi-Pasta-Ulam nonlinear
lattices, preventing equipartition among all modes at low energies. In the
first approach, a low-frequency fraction of the spectrum is initially excited
leading to the formation of `natural packets' exhibiting exponential stability,
while in the second, emphasis is placed on the existence of `q-breathers', i.e
periodic continuations of the linear modes of the lattice, which are
exponentially localized in Fourier space. Following ideas of the latter, we
introduce in this paper the concept of `q-tori' representing exponentially
localized solutions on low-dimensional tori and use their stability properties
to reconcile these two approaches and provide a more complete explanation of
the FPU paradox.Comment: 38 pages, 7 figure
A Bayesian Sample Size Estimation Procedure Based on a B-Splines Semiparametric Elicitation Method
Sample size estimation is a fundamental element of a clinical trial, and a binomial experiment is the most common situation faced in clinical trial design. A Bayesian method to determine sample size is an alternative solution to a frequentist design, especially for studies conducted on small sample sizes. The Bayesian approach uses the available knowledge, which is translated into a prior distribution, instead of a point estimate, to perform the final inference. This procedure takes the uncertainty in data prediction entirely into account. When objective data, historical information, and literature data are not available, it may be indispensable to use expert opinion to derive the prior distribution by performing an elicitation process. Expert elicitation is the process of translating expert opinion into a prior probability distribution. We investigated the estimation of a binomial sample size providing a generalized version of the average length, coverage criteria, and worst outcome criterion. The original method was proposed by Joseph and is defined in a parametric framework based on a Beta-Binomial model. We propose a more flexible approach for binary data sample size estimation in this theoretical setting by considering parametric approaches (Beta priors) and semiparametric priors based on B-splines
The Severe Heart Failure Questionnaire: Italian translation and linguistic validation
Introduction. The quality of life (QoL) is an important outcome indicator for heart failure management. As the use of a validate questionnaire in a different cultural context can affect data interpretation our main objective is the Italian translation and linguistic validation of the Severe Heart Failure Questionnaire (SHF) and its comparison with the MLHF (Minnesota Living with Heart Failure) Questionnaire.
Methods. The SHF and ?The Minnesota Living with Heart Failure Questionnaire? were translated. A consensus involving parallel back-translations was established among a group of cardiologists, psychologists and biostatisticians. SHF and MLHF were both administrated to a sample of 50 patients
Results. The patients? median age was 63 years. Ace inhibitors therapy was administered in 88% of cases and betablockers in 56% of cases. Finally the Italian version of SHF correlates well with MLHF for all domains, except life satisfaction SHF domain.
Discussion: The Italian version of the SHF correlates well with MLHF for almost all domains and it represents a valid alterna- tive for quality of life assessment in heart failure patients
Smoking habits among Italian adolescents: what has changed in the last decade?
Tobacco use, alcohol abuse, overweight and obesity are risk factors for numerous diseases in Italy as elsewhere. However, children and adolescents are not usually included in official national surveys although it is at this stage of life when unhealthy habits are often established. Italian participation in HBSC and GYTS surveys allows our country to implement standardized surveillance systems providing reliable information on tobacco-related behaviors of this population. Data from three HBSC surveys (2002–2010) show that following the drop in the first half of the decade, prevalence of tobacco use stabilized in the second half. The decline was significant for younger age groups, while prevalence of regular tobacco use remained stable among 15-year-olds. Many adolescents reported being exposed to secondhand smoke, to have at least one parent who smokes, and having seen teachers and students smoking at school. Although the sale of tobacco products to minors is prohibited, the vast majority had no trouble in buying cigarettes. Data from GYTS and HBSC surveys provide a wealth of information about attitudes and behaviors of Italian adolescents with respect to smoking. Despite some progress, sizeable gaps remain in meeting standard recommendations for discouraging smoking initiation and motivating adolescent smokers to quit the habit
Should all patients with HR-positive HER2-negative metastatic breast cancer receive CDK 4/6 inhibitor as first-line based therapy? A network meta-analysis of data from the PALOMA 2, MONALEESA 2, MONALEESA 7, MONARCH 3, FALCON, SWOG and FACT trials
Background: We aim to understand whether all patients with hormonal receptor (HR)-positive (+)/human epidermal growth factor receptor-2 (HER2)-negative (−) metastatic breast cancer (MBC) should receive cyclin D-dependent kinase (CDK) 4/6 inhibitor-based therapy as a first-line approach. Methods: A network meta-analysis (NMA) using the Bayesian hierarchical arm-based model, which provides the estimates for various effect sizes, were computed. Results: First-line treatment options in HR+/HER2− MBC, including CDK 4/6 inhibitors combined with aromatase inhibitors (AIs) or fulvestrant (F), showed a significantly longer progression-free survival (PFS) in comparison with AI monotherapy, with a total of 26% progression risk reduction. In the indirect comparison across the three classes of CDK 4/6 inhibitors and F endocrine-based therapies, the first strategy resulted in longer PFS, regardless of specific CDK 4/6 inhibitor (HR: 0.68; 95% CrI: 0.53–0.87 for palbociclib + AI, HR: 0.65; 95% CrI: 0.53–0.79 for ribociclib + AI, HR: 0.63; 95% CrI: 0.47–0.86 for abemaciclib + AI) and patient’s characteristics. Longer PFS was also found in patients with bone-only and soft tissues limited disease treated with CDK 4/6 inhibitors. Conclusions: CDK 4/6 inhibitors have similar efficacy when associated with an AI in the first-line treatment of HR+ MBC, and are superior to either F or AI monotherapy, regardless of any other patients or tumor characteristics
