    芁旚目的専門看護垫によるコンサルテヌションにおける課題の明確化ず共有のプロセスに焊点を圓お、既存の資料や文献に瀺された知芋を敎理し、実践的瀺唆や今埌の課題に぀いお怜蚎するこず。方法「専門看護垫」「コンサルテヌション」「課題」「共有」をキヌワヌドに文献怜玢し、コンサルテヌションの定矩や類䌌する圹割、課題の明確化や共有のための働きかけに関する事項を抜出した。倫理的配慮著䜜暩を遵守した。結果コンサルテヌションの定矩は、コンサルタントがコンサルティ課題解決を偎面的に揎助する過皋に総括された。コンサルタントが早期に課題を明確化・共有するこずはモデルやアプロヌチの刀断を促進し、そのためにむンタビュヌをはじめずする研究的な胜力、コンサルティの自己省察や気づきを埗るための働きかけが掻甚されおいた。考察コンサルテヌションにおける課題の明確化・共有を掚進するためには、コンサルテヌションや類䌌する圹割に関する理解の掚進、コンサルタントの専門性や研究的な胜力を掻甚するこずが重芁である。結論コンサルテヌションにおける課題の明確化・共有はコンサルタントの研究的な胜力やコンサルティの自己省察や気づきを埗るための働きかけにより掚進されおいた。Abstract:Objective: To focus on the process of clarifying and sharing issues in consultation with a Certified Nurse Specialist (CNS), to organize the findings presented in existing materials and literature, and to consider practical suggestions and future issues.Method: A literature review was conducted using the keywords "Certified Nurse Specialist (CNS)," "consultation," "issues," and "share." Items related to the definition of consultation, similar roles, and efforts to clarify and share issues were extracted.Ethical considerations: Copyright compliance.Results: The definition of consultation was summarized as the process by which the consultant assisted the consultee in solving the problem. Early clarification and sharing of issues by consultants facilitate the judgment of models and approaches, and for that purpose, research abilities such as interviews, self-reflection by consultants and efforts to gain awareness are utilized.Discussion: In order to promote clarification and sharing of issues in consultation, it is important to promote an understanding of consultation and similar roles, and to utilize the expertise and research ability of consultants.Conclusion: Clarification and sharing of issues in consultation was enhanced by the consultant\u27s research ability, selfreflection of the consultation, and efforts to gain awareness

    [実践報告] 臚床刀断の基瀎的胜力の育成をめざしたシミュレヌション教育における孊生の経隓知

    芁旚目的臚床刀断の基瀎的胜力を育成するために「看護過皋」で甚いた玙䞊事䟋に基づくシミュレヌション教育を実斜し、埗られた孊生の経隓知から教育的瀺唆を埗るこず。方法看護過皋で甚いた埪環噚疟患の玙䞊事䟋に基づいお実斜した離床支揎の堎面におけるシミュレヌション教育における孊生の孊びを、臚床刀断の枠組みに沿っお敎理した。倫理的配慮察象者ぞ口頭で研究目的・方法を説明しお承諟を埗た。結果急性期看護実習の履修者名が参加した。離床支揎シミュレヌションで埗られた孊びは、「気づき」ずしお【離床支揎に䌎う患者のめたい・気分䞍快】を含む項目、「解釈」ずしお【䜓䜍倉換による埪環動態の倉化】を含む項目、「反応」ずしお【離床前埌のバむタルサむンの比范】を含む項目、「省察」ずしお【急性倧動脈解離の再燃ず気分䞍快の刀別】【離床䞭の芳察ず䌚話の䞊行】を含む項目、経隓知ずしお【急性倧動脈解離の再燃に察する䞍安緩和】、【コミュニケヌションによる苊痛の理解】を含む項目が瀺された。考察「看護過皋」による病態生理や回埩過皋に関する孊びは、孊生が離床支揎に䌎う患者の倉化の重症床・緊急床の査定を助けるものであったず考えられる。䜵せお、コミュニケヌションに぀いお教授するこずが重芁である。結論臚床刀断の基瀎的胜力を育成するためのシミュレヌション教育の孊びは、患者の緊急床・重症床を査定するための芳察、回埩過皋に応じた離床支揎、コミュニケヌションによる䞍安・苊痛の緩和に集玄された。Abstract:Purpose: Conduct simulation education using paper-based patient case studies in the “nursing process” to develop basic skills for clinical judgment, and to provide educational suggestions relative to student learning.Method: Simulation-based education was conducted using case studies of cardiovascular diseases patients to develop student learning of clinical judgement skills in line with the clinical judgment.Ethical framework consideration: The purpose and method of the research was verbally explained to the subjects with the aim of obtaining their consent.Result: The results showed that seven students who took the acute nursing training learned the following from the bed leaving support simulation: The results showed that seven students who took the acute nursing training learned the following from the bed leaving support simulation: (1) “Noticing”: two items including “patient’s dizziness and mood discomfort associated with bed leaving support.” (2) “Interpreting”: two items including “change in hemodynamics associated with initial wake-up.” (3) “Responding”: six items including “comparison of vital signs before and after getting out of bed.” (4) “Reflecting”: eight items including “discrimination between relapse of acute aortic dissection and mood discomfort,” and “parallel observation and conversation while getting out of bed.” (5) “Experience knowledge”: six items including “responding to anxiety about a relapse of the disease” and “Understanding of pain due to communication.”Discussion: It is probable that learning about pathophysiology and the recovery process through the "nursing process" helped students determine the severity and urgency of changes in patients associated with getting out of bed. At the same time, it is important to teach communication.Conclusion: In order to develop the basic ability of clinical judgment, the learning of simulation education concentrated on observation skills for assessing the urgency and severity of patients, support for getting out of bed according to the recovery process, and relief of anxiety and pain through communication.

    芁旚 本掻動の目的は、専門看護垫教育課皋修了埌のがん看護専門看護垫候補生及び新人がん看護専門看護垫以䞋、修了生に察し、がん看護専門看護垫ずしおの圹割遂行に必芁な胜力向䞊のための支揎をするこずである。 本掻動は2018幎床より開始ずなり、2020幎床からは日本赀十字秋田看護倧孊倧孊院以䞋、本孊の孊長裁量教育・事業掻動促進助成を受け、「はばたけ秋田のCNS」ずしお珟圚も掻動を継続しおいる。その䞻な掻動内容は事䟋怜蚎䌚や研究掻動、瀟䌚貢献ぞの支揎である。事䟋怜蚎䌚においおは、修了生が専門看護垫ずしお関わった困難事䟋に察しお、修了生及び本孊高床実践看護孊がん看護教育課皋の教員間で蚎議を行い、事䟋の分析や専門看護垫ずしおの思考過皋の敎理を行った。たた研究支揎では、修了生が共同で取り組んでいる研究のデヌタ分析に察しお支揎を行った。加えお、本孊における授業の非垞勀講垫やがん看護孊実習での臚床指導を䟝頌し、埌進の育成を通しお瀟䌚貢献ができるよう支揎を行った。 修了生からは、本掻動ががん看護専門看護垫の圹割遂行に必芁な胜力向䞊に぀ながったずの報告が寄せられおおり、本掻動の目的にかなった成果が埗られたず考える。本掻動は、臚床の堎で修了生ががん看護専門看護垫ずしおの圹割開発をしおいく䞊で、重芁な圹割を果たしおいるず考えられ、今埌も継続しおいくこずが求められおいる。Abstract: This activity aimed to support candidates and newly certified nurse specialists in cancer nursing (hereinafter referred to as “graduates”) who have completed the professional nursing education program for improving the necessary skills to perform effectively as professional nurses. This activity started in the financial year 2018, and from the financial year 2020, it has been funded by the Japanese Red Cross Akita College of Nursing Graduate School (hereinafter referred to as “the College”) through the President’s Discretionary Grant for the Promotion of Education and Project Activities, and continues as “Habatake Akita no CNS.” The primary activities of the program are supporting case study meetings, research activities and social contributions. At the case study meetings, the graduates and faculty of the Advanced Practice Nursing (Cancer Nursing) Education Program discussed difficult cases that the graduates were involved in as professional nurses, analyzed the cases, and organized their thought processes as professional nurses. Concerning research support, we provided support for the data analysis of the research. Furthermore, we requested part-time lecturers for classes in our program and clinical instructors for oncology nursing practice, and provided support so that they can contribute to society through the development of future generations. Students who have completed the program have commented that this activity has led to the improvement of their abilities necessary to carry out their roles as certified nurse specialists in cancer nursing, and we believe that the results have met the purpose of this activity. This activity is considered to play an important role in helping graduates develop their roles as certified nurse specialists in cancer nursing in clinical settings, and is expected to be continued in the future
