Kinetics of Ordering in Fluctuation-Driven First-Order Transitions: Simulations and Dynamical Renormalization
Many systems where interactions compete with each other or with constraints
are well described by a model first introduced by Brazovskii. Such systems
include block copolymers, alloys with modulated phases, Rayleigh-Benard Cells
and type-I superconductors. The hallmark of this model is that the fluctuation
spectrum is isotropic and has a minimum at a nonzero wave vector represented by
the surface of a d-dimensional hyper-sphere. It was shown by Brazovskii that
the fluctuations change the free energy structure from a to a
form with the disordered state metastable for all quench depths.
The transition from the disordered to the periodic, lamellar structure changes
from second order to first order and suggests that the dynamics is governed by
nucleation. Using numerical simulations we have confirmed that the equilibrium
free energy function is indeed of a form. A study of the dynamics,
however, shows that, following a deep quench, the dynamics is described by
unstable growth rather than nucleation. A dynamical calculation, based on a
generalization of the Brazovskii calculations shows that the disordered state

Micronutrients: highlights and research challenges from the 1994-5 National Diet and Nutrition Survey of people aged 65 years and over
The aims of the National Diet and Nutrition Survey series are summarized, and the new National Diet and Nutrition Survey of people aged 65 years and over is explored, with particular emphasis on micronutrient intakes and status indices. Mean nutrient intakes were generally satisfactory for most micronutrients, but intakes of vitamin D, Mg, K and Cu were low. Intakes of vitamin D were far below the reference nutrient intake for people aged 65 years and over, and there was also biochemical evidence of vitamin D deficiency, for 8% of free-living and 37% of institution participants, attributed partly to limited exposure to sunlight. A substantial proportion of people living in institutions had inadequate biochemical status indices, notably for vitamin C, Fe and folate. Relationships between intake and status were close for vitamins. Mineral intakes did not correlate well with currently used status indices. Some intakes and indices, especially those of vitamin C, carotenoids, Na and K, were strongly correlated with socio-economic status and with north-south gradients in Britain. Future research challenges should address the functional and health significance of low intakes and sub-optimal biochemical indices for certain micronutrients, especially for people living in institutions; the shortcomings of mineral status indices especially as indicators of mineral intake; the social and geographical inequalities of micronutrient intakes and status, and why micronutrient status deteriorates with increasing age. The answers to these questions will help to define the characteristics of nutritional risk for older people in Britain, and to clarify future needs for education and intervention
ALTERED PHOSPHORYLATION STATUS, PHOSPHOLIPID-METABOLISM AND GLUCONEOGENESIS IN THE HOST LIVER OF RATS WITH PROSTATE-CANCER - A P-31 MAGNETIC-RESONANCE SPECTROSCOPY STUDY
31P magnetic resonance spectroscopy (MRS) in vivo and in vitro was used to study modulation of host liver (HL) metabolism in rats bearing the MAT-LyLu variant of the Dunning prostate tumour. Animals were inoculated either with 10(6) or 10(7) MAT-LyLu cells, or with saline to serve as controls. Carcass weight in tumour-bearing (TB) animals decreased despite similar food and water intake in both groups. Absence of metastatic tumour cells from HL of all TB animals was confirmed by histological examination. Twenty-one days after inoculation, 31P MRS showed a 2.5-fold increase in [Pi]/[ATP] ratios in HL in vivo (P < 0.001) which was confirmed by 31P MRS of liver extracts in vitro (P < 0.005). Phosphodiester to ATP ratios were significantly increased (P < 0.05) in HL in vivo, but absolute PDE levels were similar in both groups. Phosphomonoester to ATP ratios did not change, although absolute phosphomonoester levels in HL were reduced by -41% (not significant). In HL extracts in vitro, sharp reductions in the levels of glucose-6-phosphate (P < 0.05), fructose-6-phosphate (P = 0.05), phosphocholine (P < 0.001), glycerophosphocholine (P < 0.001), and glycerophosphoethanolamine (P < 0.001) were observed. Electron microscopy revealed increased amounts and altered distribution of rough endoplasmic reticulum in HL. These findings show that experimental prostate cancer significantly affects hepatic phosphorylation status, phospholipid metabolism, and gluconeogenesis in the host animal, and demonstrate the value of combined MRS in vivo and in vitro in monitoring HL metabolism in cancer
The frequency of inappropriate nonformulary medication alert overrides in the inpatient setting
Background Experts suggest that formulary alerts at the time of medication order entry are the most effective form of clinical decision support to automate formulary management. Objective Our objectives were to quantify the frequency of inappropriate nonformulary medication (NFM) alert overrides in the inpatient setting and provide insight on how the design of formulary alerts could be improved. Methods Alert overrides of the top 11 (nâ=â206) most-utilized and highest-costing NFMs, from January 1 to December 31, 2012, were randomly selected for appropriateness evaluation. Using an empirically developed appropriateness algorithm, appropriateness of NFM alert overrides was assessed by 2 pharmacists via chart review. Appropriateness agreement of overrides was assessed with a Cohenâs kappa. We also assessed which types of NFMs were most likely to be inappropriately overridden, the override reasons that were disproportionately provided in the inappropriate overrides, and the specific reasons the overrides were considered inappropriate. Results Approximately 17.2% (nâ=â35.4/206) of NFM alerts were inappropriately overridden. Non-oral NFM alerts were more likely to be inappropriately overridden compared to orals. Alerts overridden with âblankâ reasons were more likely to be inappropriate. The failure to first try a formulary alternative was the most common reason for alerts being overridden inappropriately. Conclusion Approximately 1 in 5 NFM alert overrides are overridden inappropriately. Future research should evaluate the impact of mandating a valid override reason and adding a list of formulary alternatives to each NFM alert; we speculate these NFM alert features may decrease the frequency of inappropriate overrides
Use of human GH in elderly patients with accidental hip fracture
OBJECTIVE: To investigate whether early intervention with recombinant
human growth hormone (hGH) after hip fracture improves functional recovery
and long-term outcome. SUBJECTS AND METHODS: Functional recovery after hip
fracture is often incomplete. The catabolic situation that develops after
the hip fracture accident, and a state of malnutrition either pre-existing
or developing after surgery, are main contributing factors for the poor
clinical outcome. hGH has been used to promote anabolism in a variety of
clinical catabolic situations. The study design was randomized,
double-blind and placebo-controlled. A total of 111 patients older than 60
years with an accidental hip fracture (mean age 78.5+/-9.1 (s.d.) years)
were randomized to receive either hGH (20 microg/kg per day) or placebo
for a period of 6 weeks, starting within 24 h after the hip fracture
accident. Thereafter patients were followed up for an additional period of
18 weeks. Efficacy was assessed by comparing the changes in the Barthel
Index score of activities of daily living and in a patient's living
situation between the hGH- and the placebo-treated subjects. RESULTS:
Eighty-five (78.5%) patients completed the first 8 weeks of the study and
76 (68.5%) the entire study period of 24 weeks. When split according to
age, a trend was found that for patients older than 75 years the changes
in Barthel Index score from baseline were less in the hGH group than in
the placebo group (-18.6+/-18 vs -28.1+/-26) at 6 weeks after surgery
(P<0.075). There was an overall trend to a higher rate of return to the
pre-fracture independent living situation in the hGH group than in the
placebo group. Analysis by age revealed a significantly higher proportion
of hGH- than placebo-treated patients returning to the pre-fracture living
situation for subjects older than 75 years (93.8 vs 75.0%, P=0.034). hGH
treatment increased IGF-I values to levels in the range of those of normal
subjects of 50-60 years of age. CONCLUSIONS: A 6 week treatment with hGH
(20 microg/kg per day) of otherwise healthy patients after an accidental
hip fracture may be of benefit if given to subjects older than 75 years of
age. The rate of return to the pre-fracture living situation in subjects
of this age treated with hGH was significantly increased when compared
with the placebo-treated group. The treatment intervention was well
tolerated and no safety issues were recorded
The nature of slow dynamics in a minimal model of frustration-limited domains
We present simulation results for the dynamics of a schematic model based on
the frustration-limited domain picture of glass-forming liquids. These results
are compared with approximate theoretical predictions analogous to those
commonly used for supercooled liquid dynamics. Although model relaxation times
increase by several orders of magnitude in a non-Arrhenius manner as a
microphase separation transition is approached, the slow relaxation is in many
ways dissimilar to that of a liquid. In particular, structural relaxation is
nearly exponential in time at each wave vector, indicating that the mode
coupling effects dominating liquid relaxation are comparatively weak within
this model. Relaxation properties of the model are instead well reproduced by
the simplest dynamical extension of a static Hartree approximation. This
approach is qualitatively accurate even for temperatures at which the mode
coupling approximation predicts loss of ergodicity. These results suggest that
the thermodynamically disordered phase of such a minimal model poorly
caricatures the slow dynamics of a liquid near its glass transition
Search for direct production of charginos and neutralinos in events with three leptons and missing transverse momentum in âs = 7 TeV pp collisions with the ATLAS detector
A search for the direct production of charginos and neutralinos in final states with three electrons or muons and missing transverse momentum is presented. The analysis is based on 4.7 fbâ1 of protonâproton collision data delivered by the Large Hadron Collider and recorded with the ATLAS detector. Observations are consistent with Standard Model expectations in three signal regions that are either depleted or enriched in Z-boson decays. Upper limits at 95% confidence level are set in R-parity conserving phenomenological minimal supersymmetric models and in simplified models, significantly extending previous results
Measurement of the polarisation of W bosons produced with large transverse momentum in pp collisions at sqrt(s) = 7 TeV with the ATLAS experiment
This paper describes an analysis of the angular distribution of W->enu and
W->munu decays, using data from pp collisions at sqrt(s) = 7 TeV recorded with
the ATLAS detector at the LHC in 2010, corresponding to an integrated
luminosity of about 35 pb^-1. Using the decay lepton transverse momentum and
the missing transverse energy, the W decay angular distribution projected onto
the transverse plane is obtained and analysed in terms of helicity fractions
f0, fL and fR over two ranges of W transverse momentum (ptw): 35 < ptw < 50 GeV
and ptw > 50 GeV. Good agreement is found with theoretical predictions. For ptw
> 50 GeV, the values of f0 and fL-fR, averaged over charge and lepton flavour,
are measured to be : f0 = 0.127 +/- 0.030 +/- 0.108 and fL-fR = 0.252 +/- 0.017
+/- 0.030, where the first uncertainties are statistical, and the second
