Ovarian function during hormonal contraception assessed by endocrine and sonographic markers: a systematic review
This systematic review focuses on the literature evidence for residual ovarian function during treatment with hormonal contraceptives. We reviewed all papers which assessed residual ovarian activity during hormonal contraceptive use, using endocrine markers such as serum anti-MĂŒllerian hormone (AMH) concentrations, FSH, LH, oestradiol, progesterone and sonographic markers such as antral follicle count (AFC), ovarian volume and vascular indices. We considered every type (oestroprogestin or only progestin) and dosage of hormonal contraceptive and every mode of administration (oral, vaginal ring, implant, transdermal patch). We performed an electronic database search for papers published from 1 January 1990 until 30 November 2015 using PubMed and MEDLINE. We pre-selected 113 studies and judged 48 studies suitable for the review. Most studies showed that follicular development continues during treatment with hormonal contraceptives, and that during treatment there is a reduction in serum concentrations of FSH, LH and oestradiol, and also a reduction in endometrial thickness, ovarian volume and the number and size of antral follicles. The ovarian reserve parameters, namely AFC and ovarian volume, are lower among users than among non-users of hormonal contraception; regarding the effect of hormonal contraception on AMH, there are still controversies in the literature
A rare case of omental extra-gastrointestinal stromal tumor showing two coexisting mutations on exon 14 of the PDGFRA gene
Gastrointestinal stromal tumors (GISTs) are neoplasms arising
from mesenchymal cells localized into the muscularis propria
of the gastrointestinal (GI) tract [1]; 5% of GISTs are extra-GISTs
(EGISTs), as they differently originate from adipose tissue adjacent
to the GI tract (omentum and mesentery) or from the pancreas
[2]. So far, both GISTs and EGISTs have been managed
indistinctively by combining surgery, histopathological distinctive
features, imaging, and molecular analysis. Moreover, despite
the contribution of defined genetic backgrounds whose
influence is acknowledged in this type of tumor (i.e. Carneyâs
triad or familiar form of GIST), the pathobiology of both GISTs
and EGISTs is not yet fully understood. We describe an interesting
case of an extensively diffuse EGIST involving only omentum
and mesocolon with multinodular growth and peculiar
histological features, and for which a deeper histopathological/
molecular analysis is reported.
Case presentation
A 74-year-old female with a historical diagnosis of multiple myeloma
was referred for anemia, alvus disorders (diarrhea and
constipation), weight loss (15 kg in 6 months), and palpable
mass of the right flank that had appeared 8 weeks before. On
medication for multiple myeloma since 2016 (melphalan combined
with prednisone and bortezomib9; carfilzomib/lenalidomide/
desametasone6 until complete remission), she also
had type II diabetes, treated with oral medications and open
cholecystectomy in the 1980s. Physical examination revealed
the presence of a large mobile non-painful mass in the right
flank apparently from the right colon, without signs of occlusion
or intestinal bleeding. Blood analysis showed: hemoglobin
7.9 g/dL, white blood cells 2.3103/lL, glycemia 191 mg/dL, and
a low potassium level of 2.8 mEq/L.
We first treated the glycemia by insulin infusion and, second,

The Impact of Lifestyle Interventions in High-Risk Early Breast Cancer Patients: A Modeling Approach from a Single Institution Experience
none21noA healthy lifestyle plays a strategic role in the prevention of BC. The aim of our prospective study is to evaluate the effects of a lifestyle interventions program based on special exercise and nutrition education on weight, psycho-physical well-being, blood lipid and hormonal profile among BC patients who underwent primary surgery. From January 2014 to March 2017, a multidisciplinary group of oncologists, dieticians, physiatrists and an exercise specialist evaluated 98 adult BC female patients at baseline and at different time points. The patients had at least one of the following risk factors: BMI â„ 25 Kg/m2, high testosterone levels, high serum insulin levels or diagnosis of MS. Statistically significant differences are shown in terms of BMI variation with the lifestyle interventions program, as well as in waist circumference and blood glucose, insulin and testosterone levels. Moreover, a statistically significant difference was reported in variations of total Hospital Anxiety and Depression Scale (HADS) score, in the anxiety HADS score and improvement in joint pain. Transplantation of human fetal biliary tree stem/progenitor cells into two patients with advanced liver cirrhosis.
Efforts to identify cell sources and approaches for cell therapy of liver diseases are ongoing, taking into consideration the limits recognized for adult liver tissue and for other forms of stem cells. In the present study, we described the first procedure of via hepatic artery transplantation of human fetal biliary tree stem cells in patients with advanced cirrhosis.MethodsThe cells were immune-sorted from human fetal biliary tree by protocols in accordance with current good manufacturing practice (cGMP) and extensively characterized. Two patients with advanced cirrhosis (Child-Pugh C) have been submitted to the procedure and observed through a 12Â months follow-up.ResultsThe resulting procedure was found absolutely safe. Immuno-suppressants were not required, and the patients did not display any adverse effects correlated with cell transplantation or suggestive of immunological complications. From a clinical point of view, both patients showed biochemical and clinical improvement during the 6Â month follow-up (Table1), and the second patient maintained a stable improvement for 12Â months.ConclusionThis report represents proof of the concept that the human fetal biliary tree stem cells are a suitable and large source for cell therapy of liver cirrhosis. The isolation procedure can be carried out under cGMP conditions and, finally, the infusion procedure is easy and safe for the patients. This represents the basis for forthcoming controlled clinical trials
TRY plant trait database â enhanced coverage and open access
Plant traits - the morphological, anatomical, physiological, biochemical and phenological characteristics of plants - determine how plants respond to environmental factors, affect other trophic levels, and influence ecosystem properties and their benefits and detriments to people. Plant trait data thus represent the basis for a vast area of research spanning from evolutionary biology, community and functional ecology, to biodiversity conservation, ecosystem and landscape management, restoration, biogeography and earth system modelling. Since its foundation in 2007, the TRY database of plant traits has grown continuously. It now provides unprecedented data coverage under an open access data policy and is the main plant trait database used by the research community worldwide. Increasingly, the TRY database also supports new frontiers of traitâbased plant research, including the identification of data gaps and the subsequent mobilization or measurement of new data. To support this development, in this article we evaluate the extent of the trait data compiled in TRY and analyse emerging patterns of data coverage and representativeness. Best species coverage is achieved for categorical traits - almost complete coverage for âplant growth formâ. However, most traits relevant for ecology and vegetation modelling are characterized by continuous intraspecific variation and traitâenvironmental relationships. These traits have to be measured on individual plants in their respective environment. Despite unprecedented data coverage, we observe a humbling lack of completeness and representativeness of these continuous traits in many aspects. We, therefore, conclude that reducing data gaps and biases in the TRY database remains a key challenge and requires a coordinated approach to data mobilization and trait measurements. This can only be achieved in collaboration with other initiatives

Acclimation ex vitro: How to improve the root system
Acclimation ex vitro is considered one of the most critical stages of the whole
micro-propagation cycle. The root system is still weak and root growth and
absorption need to be improved to compensate the increasing leaf transpiration and
need of nutrients. The experiments presented in this study aimed to improve the
aerial and root growth by using different substrates (type and mode of filling of the
pots) in combination with specific fertilizers and replacing part of the root absorption
with nutrient application to the leaves. The supply of mineral elements and organic
compounds to the leaves in synergy with the available reserves in the seedlings can
avoid the increasing of the cost of the absorption process for the plantlet. Results
confirm the importance of the substrate to affect root growth and establishment. The
quality of the growth of root and foliar P, K, Ca, Mg, and humic acids application
positively influenced the growth and the plant settlement. Results showed that foliar
applications in fertilized growing media are able to enhance vegetative growth, dry
matter accumulation and basal steam diameter in all the tested clones. When nonfertilized
growing media are used, foliar application is less effective in controlling
vegetative growth to obtain the required height and basal stem diameter. This study
suggests that is advisable to have a micro-porous substrate and apply moderate
fertilizations through the substrate. The supply of the nutrients to the leaves resulted
in bigger, but well hardened plants. It is possible to argue that changing
concentrations and ratio among the different elements according to the acclimation
cycle stage would balance vegetative growth and reduce hidden stresses
