    Factors of local conversion of iodothyronines correlate with indicators of hormonal, biochemical, and hematological profiles in patients with spinal canal and dural sac stenosis of the lumbar spine

    In order to find out the mechanisms of pathogenesis of degenerative-dystrophic diseases of the spine, it is of particular interest to search for body parameters which are directly or indirectly interrelated with the key factors of peripheral conversion of nidus iodothyronines and constitute a system of network interactions, affecting metabolic indicators at the local and systemic level.The aim. To search for correlations of local key factors of peripheral conversion of Ligamentum flavum iodothyronines with indicators of biochemical, hematological and hormonal blood profiles of patients with stenosing processes of the spinal canal and dural sac in the lumbar spine.Materials and methods. 33 patients (15 males, 18 females) with stenosing processes of the spinal canal and dural sac in the lumbar spine were examined (mean age – 45.73 ± 1.95 years). The expression of deiodinase genes and other candidate genes was determined in Ligamentum flavum biopsies collected during surgical treatment. Biochemical, hematological and hormonal parameters were determined in peripheral blood. The resulting data array was processed in order to find correlations between the parameters of systemic and local metabolism.Results. The relationships of deiodinases with the expression of GDF5, MMP1, MMP3 and TIMP1 in Ligamentum flavum (p < 0.05) were found. Of the hormonal profile of the blood serum, the most significant indicators were thyreotropin, free triiodothyronine and thyroperoxidase antibodies. In the biochemical profile, levels of direct bilirubin, total cholesterol, HDL and LDL cholesterol and triglycerides changed along with the expression of deiodinases. Correlative relationships with the expression of deiodinases were found for the following hematological analytes of whole peripheral blood: hemoglobin, mean corpuscular hemoglobin concentration, numbers of granulocytes, lymphocytes, eosinophils, monocytes, band neutrophils, red cell distribution width and platelet crit. The data obtained indicate the involvement of peripheral conversion factors in the pathogenetic process and provide information to form a new view on the pathogenesis of degenerative-dystrophic processes in the Ligamentum flavum of patients with stenosing processes of the spinal canal and the dural sac in the lumbar spine

    The principles of constru.ction and implementation of semi-active radar systems, using the illumination from third-party sources are presented. The results of theoretical and experimental researches of the Saint-Petersburg state electrotechnical university "LETI" n. a. V. I. Ulyanov (Lenin) and JSC «SRI "Vector"» in this direction are given. The advantages of semi-active radar systems, the possibility of coherent processing with a large accumulation time in com.bination with the digital formation of the directional diagrams of the receiving antenna arrays, as well as prospects of application systems semi-active radar for the protection of important sites and monitoring areas, including the detection and orbital support of ground, surface and aerial objects. The stru.cture of the developed passive coherent radar com.plex is described; its basic technical characteristics are given.Представлены принципы построения и вопросы практической реализации систем полуактивной радиолокации, использующих сигналы подсвета сторонних источников. Приведены результаты теоретических и экспериментальных исследований, выполненных в Санкт-Петербургском государственном электротехническом университете "ЛЭТИ" им. В. И. Ульянова (Ленина) и АО «НИИ "Вектор"» в этом направлении. Рассмотрены преимущества систем полуактивной радиолокации, возможности когерентной обработки с большим временем накопления, а также перспективы применения систем полуактивной радиолокации для охраны важных объектов и мониторинга территорий, включая обнаружение и траекторное сопровождение наземных, надводных и воздушных объектов.     ФАТАЛЬНАЯ ПОСТТРАНСФУЗИОННАЯ РЕАКЦИЯ ТРАНСПЛАНТАТ ПРОТИВ ХОЗЯИНА У ПАЦИЕНТА ПОСЛЕ АОРТОКОРОНАРНОГО ШУНТИРОВАНИЯ

    Goal: to demonstrate the rare case of transfusion-associated graft-versus-host disease (TA-GvHD) after transfusion of blood components. The cause of TA-GvHD is implantation of donor lymphocytes, contained in blood components, which is related to immunological HLA-homology between the donor and recipient, that does not eliminate the donor cells. Mortality makes 90-100%. 68 year old patient had planned surgery - aortocoronary bypass. In the post-operative period under pressure anemia three doses of unirradiated packed red blood cells were transfused. On the 17th day after transfusion the patient developed febrile fever and manifested erythematous cutaneous rush. On the 24th day after transfusion neutropenia developed. Evaluation results of allele discrimination of high-polymorphic markers confirmed the diagnosis of TA-GvHD. HLA-typing of the donor's and recipient's blood was done retrospectively. The donor was homozygous in five loci in low resolution and in loci of HLA-А*, HLA-В*, HLA-DRB1* he had the gene common with the patient. On the 34th day after the transfusion the patient died of infectious complications. X-ray or gamma-irradiation of blood components in order to inactivate the donor's viable T-lymphocytes is considered to be the only way to guarantee the prevention of TA-GvHD. Since there is no effective therapy for TA-GvHD, prevention of risk to develop the disease, strict indications to blood transfusions and capacity for early diagnosis become crucial. Цель: продемонстрировать редкий случай проявления посттрансфузионной реакции трансплантат против хозяина (ПТ-РТПХ) после трансфузии компонентов крови. Причина возникновения ПТ-РТПХ – приживление донорских лимфоцитов, содержащихся в компоненте крови, связанное с иммунологической HLA-гомологией между донором и реципиентом, и дефектом иммунной системы реципиента, которая не элиминирует донорские клетки. Летальность – 90–100%. Больной 68 лет перенес плановую операцию – аортокоронарное шунтирование. В послеоперационном периоде на фоне анемии выполнены трансфузии трех доз необлученной эритроцитной массы. На 17-е сут после трансфузии у пациента развилась фебрильная температура, появилась сливная эритематозная сыпь на кожных покровах. На 24-е сут после трансфузии развилась нейтропения. По результатам оценки аллельной дискриминации высокополиморфных маркеров подтвержден диагноз ПТ-РТПХ. Ретроспективно выполнено HLA-типирование доноров крови и реципиента. Донор оказался гомозиготен в пяти локусах на низком разрешении и в локусах HLA-А*, HLA-В*, HLA-DRB1* имел общий ген с пациентом. На 34-й день после гемотрансфузии пациент умер от инфекционных осложнений. Единственным методом гарантированного предотвращения ПТ-РТПХ считается рентгеновское или гамма-облучение компонентов крови для инактивации жизнеспособных Т-лимфоцитов донора. Поскольку эффективная терапия ПТ-РТПХ отсутствует, предотвращение риска развития реакции, строгие показания к гемотрансфузиям и возможность ранней диагностики имеют решающее значение.
