    Context. The use of the Electronic health record (EHR) could facilitate the support of migrants but its implementation in France is laborious.Objective. Explore the representations on the use of the EHR among migrants, with migrants and general practitioners. Method. Exploratory qualitative study by semi-structured individual interviews and focus group, carried out between April and July 2020 with sampling in maximum variation. The two populations were recruited and analyzed independently until data saturation. An inductive analysis was performed with double blind coding. Results. Twelve participants were recruited from each population. The use of the EHR among migrants was considered by both populations as beneficial but difficult according to the specificities of migrant life. The EHR appears to be a practical solution to access migrant health data but the access should be simplified. Thus, the facilitating elements were the involvement of socialworkers and the interest in accessing the EHR upon the arrival of the migrants on the French territory. Opinions differed on the impact that the EHR would have on the doctor-patient relationship, which is specificamong migrants. Conclusion. The implementation of the EHR among migrants will not take place without the main actors. Their opinion is essential to avoid multiplying tools that are not adapted to the patients who need them the most. Involving healthcare users and primary care professionals is the only way to develop a partnership approach.L’utilisation du Dossier médical partagé (DMP) pourrait faciliter l’accompagnement des migrants mais son implémentation en France est laborieuse. L'objet de cet article est d'explorer les représentations sur l’utilisation du DMP pour les migrants auprès des médecins généralistes et des migrants. Une étude qualitative exploratoire par entretiens semi-dirigés individuels et focus group a été réalisée entre avril et juillet 2020 avec échantillonnage en variation maximale. Les deux populations étaient recrutées et analysées indépendamment jusqu'à saturation des données. Une analyse inductive a été réalisée avec un double codage en aveugle. Douze participants ont été recrutés dans chaque population. L’utilisation du DMP chez les migrants était considérée par les deux populations comme bénéfique mais difficile compte tenu des spécificités de la vie de migrant. Le DMP apparaît comme une solution pratique pour accéder aux données de santé des migrants et son accès mériterait d’être simplifié. Les éléments facilitants étaient l’implication des travailleurs sociaux, et l’intérêt de l’accès au DMP dès l’arrivée sur le territoire français. Les avis divergeaient sur l’impact qu’aurait le DMP sur la relation médecin-patient, spécifique aux migrants

    Sunshine on KOLs: assessment of the nature, extent and evolution of financial ties between the leaders of professional medical associations and the pharmaceutical industry in France from 2014 to 2019: a retrospective study

    International audienceObjective To investigate the financial relationships between key opinion leader (KOL) or non-KOL physicians and pharmaceutical and device companies in France.Design Retrospective and descriptive study. Setting All doctors practising in France, with a focus on 548 KOLs (board members of the professional medical associations that published guidelines in 2018-2019, identified on the associations' websites between 2018 and 2020). Ties were collected from the 'Transparency in Healthcare' database. Main outcome measures The number and the value of gifts from 2014 to 2019, and of remunerations and contractual agreements from 2017 to 2019.Results KOLs represented 0.24% of the total number of physicians in France. The total value of gifts declared in the French database for all physicians amounted to euro818M (US$936M, 741M) pound. At least one gift was declared for 83% of KOLs. KOLs' gifts represented 0.68% of the total number of gifts to physicians and 1.5% of the total value of gifts, with a mean of euro3700 per capita per year. The total value of contractual agreements declared for all physicians amounted to euro125M. Contractual agreements involving the KOLs represented 0.72% of the number of contractual agreements with physicians and 2.5% of the value of the agreements, with a mean of euro1900 per capita per year. A total of euro156M in remunerations were declared for all physicians. KOL remunerations represented 2.3% of the number of physician remunerations and 4.4% of the total value of the remunerations paid to physicians, with a mean of euro4100 per capita per year. Almost all professional medical associations (99%) had at least one KOL in their board with a financial tie to the industry, but the amount varied widely among the associations.Conclusion Financial relationships between KOLs and the industry in France are extensive. KOLs have much more financial ties than non-KOL practitioners
