Outcomes of endovascular treatment of thoracic aorta pathologies: 10-year single-center results
PURPOSEThoracic endovascular aortic repair (TEVAR) is a safe and effective treatment method for a variety of thoracic aortic pathologies. We aimed to investigate the mortality and complication outcomes and associated factors of TEVAR treatment in Turkey.METHODSIn this single-centered retrospective study, patients with thoracic aorta pathologies treated with TEVAR at Gazi University School of Medicine, Department of Radiology, between January 2009 and January 2020 were included. Perioperative, early, and late mortality, complications, and technical success were the outcomes.RESULTSThe sample comprised 58 patients with 68 TEVAR interventions. Eleven (16.2%) patients were female, the mean age was 60.1 ± 13.4 years. Emergent TEVAR was required in 20.7% of the patients. The main indications of TEVAR were intact descending aorta aneurysms in 37.9% of the sample, 31.0% Stanford type-B dissection, and 12.1% traumatic transections. The technical success rate of primary and secondary interventions was 98.3% and 100%, respectively. The mortality rate in the first 30 days was 8.6%. Seventeen (29.3%) cases had at least 1 complication related to TEVAR treatment. The most common complication was type-1A endoleak (10.3%). Having acute symptoms, stroke, and acute renal failure were significantly associated with mortality (P=.020, .049, and .009, respectively).CONCLUSIONThis study reported the outcomes of TEVAR treatment from a tertiary medical center in Turkey over a decade. Patients presenting with acute symptoms and who developed stroke and acute renal failure after the procedure should be carefully followed up as these factors were found to be associated with mortality
Hastanelerde halkla ilişkiler uygulamalarinin sağlik personelinin iş doyumuna etkisi (Tarsus Devlet Hastanesi ve Mersin Özel Forum Yaşam Hastanesi)
Sağlık kuruluşlarının her basamağı ve özellikle de hastaneler, verdiği hizmetlerin toplum için önemi ve çalışma şartlarının zorluğu ile halkla ilişkiler faaliyetlerine çok fazla ihtiyaç duyulan sektörlerin başında gelmektedir. Halkla ilişkiler faaliyetleri, hastanelerin hedef kitlesi olan hasta ve yakınları için önemli olduğu kadar kurum çalışanları için de önem arz etmektedir. Çünkü başarılı bir halkla ilişkiler politikası ile hedef kitleyi memnun ederek, kurumun amaçlarına ulaşmak kolaylaşacağı gibi hedef kitleyi memnun etmesi beklenilen sağlık çalışanlarının iş motivasyonunu da arttıracaktır. Bu çalışmanın amacı sağlık kuruluşlarında yapılan halkla ilişkiler çalışmalarının hem çalışan açısından hem de hasta ve yakınları açısından işlev ve önemini vurgulamak, yapılan çalışmaların daha profesyonel düzenli çalışmalar haline getirmesine dikkat çekmektir. Araştırmanın temel bulguları özetle; halkla ilişkiler çalışmalarına kamu hastanelerine nazaran özel hastanede daha fazla önem verilmektedir. Bunun yansıması olarak da; özel hastanede çalışan sağlık personelinin iş doyumu devlet hastanesinde çalışan sağlık personeline göre daha yüksektir. Ayrıca hedef kitle ve sağlık çalışanlarına uygulanan halkla ilişkiler çalışmalarında denge kurulması da iş doyumunun sağlanması açısından son derece önemlidir.Each step of the health institutions especially hospitals with the importance of servicing for society and difficulties of working conditions, come first of the sectors that need a lot of public relations activities. Public relations activities are as important for the patients and patients' relatives who are the target group of the hospitals as the employee in the institution. Because succesful public relations policy will facilitate reaching the institution goals by satisfying the target group. And it will also increase the motivations of health employee who are anticipated to satisfy the target group. The purposes of this study are emphasising the function and importance of public relationsactivities carried out by health institution for patients and patients' relatives and calling attention to be more professional at public relations activities. To sum up; the fundamental results of the study are aligned. First private hospitals consider more importance for public relations activities compared to public hospitals. Second the job satisfaction of the employee of private hospitals is more than public hospitals. Third balancing the public relations activities between target group and health employee is so important to provide job satisfaction
Cloud Manufacturing Architecture Based on Public Blockchain Technology
With Industry 4.0, IT infrastructure has started to be used more effectively in the manufacturing sector. Cyber physical systems, IoT, cloud manufacturing, big data are some of the technologies that make up the concept of Industry 4.0. These technologies have solved many problems in the manufacturing sector. One of these technologies, cloud manufacturing technology, has emerged with the idea of pay as you go. This technology has enabled manufacturing resources to be leased and shared on a global scale. However, it has problems arising from its central structure and the need for a reliable 3rd party. Reliability, security, continuity, scalability, data lock-in, single point failure, data manipulation are some of the main problems. Blockchain (BC) is a decentralized and distributed technology. The data stored on the BC network cannot be altered in any way. With these features, we believe that BC-supported cloud manufacturing systems can overcome the aforementioned problems and eliminates the need for a reliable 3rd party. Based on this belief, in this study the agreements and communication between the resource provider and the customer, which is one of the basic functions of cloud manufacturing platforms, are realized with a decentralized application using BC-based smart contracts (SCs). The designed application is called the decentralized cloud manufacturing application (DCMApp). DCMApp does not operate on a fully public BC network, it has a hybrid structure and uses the Ethereum network as a public BC network. These features make DCMApp different from other BC-based cloud manufacturing applications. DCMApp’s hybrid structure has enabled more transparent, economic and safe manufacturing agreements. It is also possible to store agreements on the BC network at a low cost without installing any server infrastructure. The use of Ethereum network makes it almost impossible to manipulate agreements

Deleterious effects of local corticosteroid injections on the Achilles tendon
Purpose: The purpose of this study is to examine the pathologic changes in the Achilles tendon and its paratenon after intratendinous corticosteroid injections at different intervals and to reveal the effects of this drug on healthy tendon, and also to search the effects of these injections comparing to that of compression with a clamp on the Achilles tendons of the rats. Material and Method: Fifty-two Achilles tendons of twenty-six male Vistar rats were included in the study to search the effects of intratendinous Betamethasone injections comparing that of compression with a clamp. Betamethasone injections were applied to the left tendons in different intervals, while the right tendons served for compression with Mosquito clamps in varied durations. At the end of 30 days, all tendons were excised and examined histopathologically according to a semiquantitative scoring system. Results: Histopathologic evaluation showed some degree of degeneration in both groups. Statistical analysis showed no significant difference among two groups, but in macroscopic evaluation, the tendons in Betamethasone group demonstrated enlargement and strong adhesiveness to the subcutaneous tissue. Discussion: It is concluded that intratendinous Betamethasone injections are as harmful as compression of it with a clamp and it can be used as a degeneration performing model in further studies. Enlargement of the tendon mass and strong adhesiveness to the subcutaneous tissue can be due to injection of the Betamethasone partly outside of tendon
