    Implementation of web based biotelemetry applications on WiMAX networks

In this paper, it is designed and conducted a web based biotelemetry application works on WiMAX. First; Patient Communication Node (PCN), Client Communication Node (CCN) and Administrative Communication Node (ACN) systems are designed. Then these systems are implemented on WiMAX networks.     A novel solution approach and protocol design for bio-telemetry applications

Medical networks are communication networks that serve for the purpose of monitoring and protecting human health. These networks are designed to use biotelemetric ways to transmit the vital data to health observers such as doctors, nurses, first-aid teams, hospitals, and health agencies. These networks are also used in collective damages that may occur in situations such as flood, earthquake, war and terror and for treatments and follow-up of patients and to organize health teams more effective and efficiently. Implementations using this model presented here provides a reference design. In addition MCP (Medical Communication Protocol) and MMP (Medical Management Protocol) are designed to reveal how communications between modules designed.     Mobil telefonların yaydığı elektromanyetik dalgaların E.coli’nin gelişimine etkisi

İstanbul,2011Bu çalışmada mobil telefonların yaydığı elektromanyetik dalgaların bakteri gelişimine etkisini tespit etmek amacıyla deneysel çalışma yapılmıştır. Deney kapsamında E. coli bakteri hücreleri kullanılmıştır. Deneyde kullanılan bakteri kültürü, logaritmik faza ulaşınca steril koşullarda iki gruba ayrılmıştır. Birinci grup deney düzeneğine yerleştirilmiş ve mobil telefonun yaydığı elektromanyetik dalgaya maruz bırakılmıştır. Ikinci grup ise kontrol grubu olarak ayrılarak elektromanyetik dalga uygulanmayan aynı özelliklerde başka bir fiziksel ortama konulmuştur. Her iki ortamda ortam sıcaklığı 37 oC de sabit tutularak mobil telefonların oluşturulduğu ısıl etki minimize edilmiştir. Daha sonra deney grubu bakteri hücreleri mobil telefonların yaydığı elektromanyetik dalgaya maruz bırakılarak, kontrol grubu ise elektromanyetik dalgaya maruz bırakmaksızın 10 saat süresince inkübe edilmiştir. Elektromanyetik dalgaya maruz kalan ve kontrol grubu E. coli hücrelerinden her saat başı örnekler alınmıştır. Alınan örneklerin optik yoğunlukları ölçülerek, mobil telefonların yaydığı elektromanyetik dalgaların bakteri gelişimine olan etkileri tespit edilmiştir

    Son yıllarda kablosuz haberleşme sistemlerindeki hızlı gelişim birlikte, çoklu sinyal yollarının tasarımı, kapsaması ve kontrol edilebilir frekans bandları önemli konular haline gelmiştir. Bu işlemlerin esnek veya eş zamanlı yönetimi daha çok gündeme gelmeye başlamış ve çok sayıda uygulamayı destekleme ve çok sayıda abone ve makineye yüksek hızlı erişim sağlama gibi ihtiyaçlar çok giriş-çok çıkışlı (MIMO) sistemleri kablosuz iletişim sistemlerinin en önemli unsuru haline gelmiştir. Birden fazla teknolojiyi içermesine rağmen, MIMO temelde aynı iletişim kanalı üzerinden aynı anda birden fazla veri sinyalinin iletilmesini ve alınmasını sağlayan bir kablosuz ağ yapısı olarak değerlendirilmektedir. MIMO sistemlerin diğer sistemlere göre en büyük avantajı, daha fazla spektrum gerektirmeden kablosuz bir bağlantı kapasitesini çoğaltabilmesidir. Verici ve alıcı anten bakımından daha fazla donanıma (çoklu bağlantı) sahip bir sistemde, sinyal yolları da o kadar fazla olmakta ve bu durum veri hızı ve bağlantı güvenilirliği açısından performansı artırmaktadır. Bu durum daha hassas optimize olanağı sunmaktadır. Çok giriş ve çok çıkışlı sistemlerin birden fazla kullanıcıya ve birden fazla cihaza eşzamanlı olarak en üst performansla hizmet verme yeteneği 5G?deki en önemli avantajdır ve bu alandaki ihtiyaçları karşılamak için mükemmel bir teknolojidir. Yüksek veri çıkışı ve çok düşük gecikme süresi ihtiyacı düşünüldüğünde bu amaçlara hizmet edebilecek yeni tasarımlar haberleşme sistemlerinin performansı üzerinde büyük etki oluşturacaktır. Proje kapsamında, son zamanlarda ilgi odağı olan MIMO sistemlerde kullanılmak üzere, bu sistemlerin alt bileşenleri olan SISO, SIMO ve MISO yapılarının daha kompakt ve daha esnek tasarım imkânı sunacak şekilde çok modlu çok bandlı yeni filtre ve çoğullayıcı tasarımlarının gerçekleştirilmesi ve bu yapıların teorik modeli için önemli konulardan olan kuplaj matrisi sentez tekniğini modifiye etmek üzere gerekli işlem adımlarının tanımlanması, geliştirilmesi ve basitleştirilmesi hedeflenmektedir. Yeni devre bileşenleri tasarımları yanı sıra nihai aşamada elektromanyetik ve mikrodalga problemlerin bilgisayar destekli çözümlerinde kullanılmak üzere kullanıcılar için daha pratik daha hızlı karar verme yetkinliği sağlayabilecek nitelikte teorik desteklerin literatüre kazandırılması planlanmaktadır

    Clinical Characteristics and Outcomes of COVID-19 in Turkish Patients with Hematological Malignancies

    Objective: Patients with solid malignancies are more vulnerable to severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection than the healthy population. The outcome of SARS-CoV-2 infection in highly immunosuppressed populations, such as in patients with hematological malignancies, is a point of interest. We aimed to analyze the symptoms, complications, intensive care unit admissions, and mortality rates of patients with hematological malignancies infected with SARS-CoV-2 in Turkey. Materials and Methods: In this multicenter study, we included 340 adult and pediatric patients diagnosed with SARS-CoV-2 from March to November 2020. Diagnosis and status of primary disease, treatment schedules for hematological malignancies, time from last treatment, life expectancy related to the hematological disease, and comorbidities were recorded, together with data regarding symptoms, treatment, and outcome of SARS-CoV-2 infection. Results: Forty four patients were asymptomatic at diagnosis of SARSCoV-2 infection. Among symptomatic patients, fever, cough, and dyspnea were observed in 62.6%, 48.8%, and 41.8%, respectively. Sixtynine (20%) patients had mild SARS-CoV-2 disease, whereas moderate, severe, and critical disease was reported in 101 (29%), 71 (20%), and 55 (16%) patients, respectively. Of the entire cohort, 251 (73.8%) patients were hospitalized for SARS-CoV-2. Mortality related to SARS-CoV-2 infection was 26.5% in the entire cohort; this comprised 4.4% of those patients with mild disease, 12.4% of those with moderate disease, and 83% of those with severe or critical disease. Active hematological disease, lower life expectancy related to primary hematological disease, neutropenia at diagnosis of SARS-CoV-2, ICU admission, and first-line therapy used for coronavirus disease-2019 treatment were found to be related to higher mortality rates. Treatments with hydroxychloroquine alone or in combination with azithromycin were associated with a higher rate of mortality in comparison to favipiravir use.     Stoma-free Survival After Rectal Cancer Resection With Anastomotic Leakage: Development and Validation of a Prediction Model in a Large International Cohort.

    Objective:To develop and validate a prediction model (STOMA score) for 1-year stoma-free survival in patients with rectal cancer (RC) with anastomotic leakage (AL).Background:AL after RC resection often results in a permanent stoma.Methods:This international retrospective cohort study (TENTACLE-Rectum) encompassed 216 participating centres and included patients who developed AL after RC surgery between 2014 and 2018. Clinically relevant predictors for 1-year stoma-free survival were included in uni and multivariable logistic regression models. The STOMA score was developed and internally validated in a cohort of patients operated between 2014 and 2017, with subsequent temporal validation in a 2018 cohort. The discriminative power and calibration of the models' performance were evaluated.Results:This study included 2499 patients with AL, 1954 in the development cohort and 545 in the validation cohort. Baseline characteristics were comparable. One-year stoma-free survival was 45.0% in the development cohort and 43.7% in the validation cohort. The following predictors were included in the STOMA score: sex, age, American Society of Anestesiologist classification, body mass index, clinical M-disease, neoadjuvant therapy, abdominal and transanal approach, primary defunctioning stoma, multivisceral resection, clinical setting in which AL was diagnosed, postoperative day of AL diagnosis, abdominal contamination, anastomotic defect circumference, bowel wall ischemia, anastomotic fistula, retraction, and reactivation leakage. The STOMA score showed good discrimination and calibration (c-index: 0.71, 95% CI: 0.66-0.76).Conclusions:The STOMA score consists of 18 clinically relevant factors and estimates the individual risk for 1-year stoma-free survival in patients with AL after RC surgery, which may improve patient counseling and give guidance when analyzing the efficacy of different treatment strategies in future studies
