    Obstetrician-Gynecologists’ Knowledge of Health Disparities and Barriers among American Indian/Alaska Native Women in Washington State

    Background: Health disparities between American Indian/Alaska Native (AI/AN) women and other races/ethnicities have long been noted. Obstetricians-Gynecologists (Ob-Gyn) play a significant role in well-woman care and are often the first and most frequent point of medical contact for women, particularly among minority and low-income women. Objective: This study aimed to assess Ob-Gyns’ knowledge, beliefs, and practices related to health disparities among AI/AN women. Method: A self-administered questionnaire, consisting of questions about knowledge, beliefs, and practices of health disparities among AI/AN women, was mailed to 722 members of The American College of Obstetricians and Gynecologists (ACOG) practicing in the state of Washington in September 2013-February 2014. Results: The majority of respondents were knowledgeable about numerous health care disparities among non-pregnant AI/AN women, while slightly fewer were aware of disparities among pregnant AI/AN patients. Ob-Gyns reported low confidence in their training and knowledge of AI/AN culture and health disparities, but high confidence in their ability to treat AI/AN patients. Participants reported dissatisfaction with their AI/AN patients’ breastfeeding rates. Conclusion: Ob-Gyn knowledge of health disparities among AI/AN women is adequate. In spite of this, barriers to quality care are still present and increased identification and implementation of effective resources is needed

    Processing of affective faces varying in valence and intensity in shy adults: an event-related fMRI study

    Recent behavioral and electrocortical studies have found that shy and socially anxious adults are hypersensitive to the processing of negative and ambiguous facial emotions. We attempted to extend these findings by examining the neural correlates of affective face processing in shy adults using an event-related fMRI design. We presented pairs of faces that varied in affective valence and intensity. The faces were morphed to alter the degree of intensity of the emotional expressive faces. Twenty-four (12 shy and 12 non-shy) young adult participants then made same/different judgments to these faces while in an MR scanner. We found that shy adults exhibited greater neural activation across a distinct range of brain regions to pairs of faces expressing negative emotions, moderate levels of emotional intensity, and emotional faces that were incongruent with one another. In contrast, non-shy individuals exhibited greater neural activation across a distinct range of brain regions to pairs of faces expressing positive emotions, low levels of emotional intensity, and emotional faces that were congruent with one another. Findings suggest that there are differences in neural responses between shy and non-shy adults when viewing affective faces that vary in valence, intensity, and discrepancy

    Sub-wavelength terahertz beam profiling of a THz source via an all-optical knife-edge technique

    Terahertz technologies recently emerged as outstanding candidates for a variety of applications in such sectors as security, biomedical, pharmaceutical, aero spatial, etc. Imaging the terahertz field, however, still remains a challenge, particularly when sub-wavelength resolutions are involved. Here we demonstrate an all-optical technique for the terahertz near-field imaging directly at the source plane. A thin layer (<100 nm-thickness) of photo carriers is induced on the surface of the terahertz generation crystal, which acts as an all-optical, virtual blade for terahertz near-field imaging via a knife-edge technique. Remarkably, and in spite of the fact that the proposed approach does not require any mechanical probe, such as tips or apertures, we are able to demonstrate the imaging of a terahertz source with deeply sub-wavelength features (<30 μm) directly in its emission plane

    Need to know: the need for cognitive closure impacts the clinical practice of obstetrician/gynecologists

    Need for cognitive closure (NFCC) has been shown to be a consistent and measurable trait. It has effects on decision making and has been associated with more rapid decision making, higher reliance on heuristics or biases for decision making, reduced tolerance for ambiguity, and reduced interest in searching for alternatives. In medical practice, these tendencies may lead to lower quality of decision making. This study measured NFCC in 312 obstetrician/gynecologists using a survey-style approach. Physicians were administered a short NFCC scale and asked questions about their clinical practice. Obstetrician/gynecologists with high NFCC were found to be less likely to address a number of clinical questions during well-woman exams, and were more likely to consult a greater number of sources when prescribing new medications. NFCC of physicians may have an important impact on practice. It is possible that increased training during residency or medical school could counteract the detrimental effects of NFCC, and steps can be taken through increased use of electronic reminder systems could orient physicians to the appropriate questions to ask patients.https://doi.org/10.1186/s12911-014-0122-

    Testosterone, cortisol, and serotonin as key regulators of social aggression: A review and theoretical perspective

    In human and non-human animals the steroid hormones cortisol and testosterone are involved in social aggression and recent studies suggest that these steroids might jointly regulate this behavior. It has been hypothesized that the imbalance between cortisol and testosterone levels is predictive for aggressive psychopathology, with high testosterone to cortisol ratio predisposing to a socially aggressive behavioral style. In this review, we focus on the effects of cortisol and testosterone on human social aggression, as well as on how they might modulate the aggression circuitry of the human brain. Recently, serotonin is hypothesized to differentiate between impulsive and instrumental aggression, and we will briefly review evidence on this hypothesis. The aim of this article is to provide a theoretical framework for the role of steroids and serotonin in impulsive social aggression in humans

    Deficient maternal care resulting from immunological stress during pregnancy is associated with a sex-dependent enhancement of conditioned fear in the offspring

    Activation of maternal stress response systems during pregnancy has been associated with altered postpartum maternal care and subsequent abnormalities in the offspring’s brain and behavioral development. It remains unknown, however, whether similar effects may be induced by exposure to immunological stress during pregnancy. The present study was designed to address this issue in a mouse model of prenatal immune activation by the viral mimic polyriboinosinic–polyribocytidilic acid (PolyI:C). Pregnant mice were exposed to PolyI:C-induced immune challenge or sham treatment, and offspring born to PolyI:C- and sham-treated dams were simultaneously cross-fostered to surrogate rearing mothers, which had either experienced inflammatory or vehicle treatment during pregnancy. We evaluated the effects of the maternal immunological manipulation on postpartum maternal behavior, and we assessed the prenatal and postnatal maternal influences on anxiety- and fear-related behavior in the offspring at the peri-adolescent and adult stage of development. We found that PolyI:C treatment during pregnancy led to changes in postpartum maternal behavior in the form of reduced pup licking/grooming and increased nest building activity. Furthermore, the adoption of neonates by surrogate rearing mothers, which had experienced PolyI:C-induced immunological stress during pregnancy, led to enhanced conditioned fear in the peri-adolescent and adult offspring, an effect that was exclusively seen in female but not male subjects. Unconditioned (innate) anxiety-related behavior as assessed in the elevated plus maze and open field explorations tests were not affected by the prenatal and postnatal manipulations. Our results thus highlight that being raised by gestationally immune-challenged surrogate mothers increases the vulnerability for specific forms of fear-related behavioral pathology in later life, and that this association may be mediated by deficits in postpartum maternal care. This may have important implications for the identification and characterization of early-life risk factors involved in the developmental etiology of fear-related neuropsychiatric disorders
