A Situational Alignment Framework for PACS
This paper reports the outcomes of a study on an integrated situational alignment framework for picture archiving and communication systems (PACS) labeled as PISA. Following the design research cycle, complementary validation methods and pilot cases were used to assess the proposed framework and its operationalized survey. In this paper, the authors outline (a) the process of the frameworkâ development, (b) the validation process with its underlying iterative steps, (c) the outcomes of pilot cases, and (d) improvement opportunities to refine and further validate the PISA framework. Results of this study support empirical application of the framework to hospital enterprises in order to gain insights into their PACS maturity and alignment. We argue that the framework can be applied as a valuable tool for assessments, monitoring and benchmarking purposes and strategic PACS planning
DNA methylation dynamics during intestinal stem cell differentiation reveals enhancers driving gene expression in the villus
Background: DNA methylation is of pivotal importance during development. Previous genome-wide studies identified numerous differentially methylated regions upon differentiation of stem cells, many of them associated with transcriptional start sites. Results: We present the first genome-wide, single-base-resolution view into DNA methylation dynamics during differentiation of a mammalian epithelial stem cell: the mouse small intestinal Lgr5+ stem cell. Very little change was observed at transcriptional start sites and our data suggest that differentiation-related genes are already primed for expression in the stem cell. Genome-wide, only 50 differentially methylated regions were identified. Almost all of these loci represent enhancers driving gene expression in the differentiated part of the small intestine. Finally, we show that binding of the transcription factor Tcf4 correlates with hypo-methylation and demonstrate that Tcf4 is one of the factors contributing to formation of differentially methylated regions. Conclusions: Our results reveal limited DNA methylation dynamics during small intestine stem cell differentiation and an impact of transcription factor binding on shaping the DNA methylation landscape during differentiation of stem cells in vivo

Maturation characteristics and life-history strategies of the Pacific lamprey, Entosphenus tridentatus
Lampreys (Petromyzontiformes) have persisted over millennia and now suffer a recent decline in abundance. Complex life histories may have factored in their persistence; anthropogenic perturbations in their demise. The complexity of life histories of lampreys is not understood, particularly for the anadromous Pacific lamprey, Entosphenus tridentatus Gairdner, 1836. Our goals were to describe the maturation timing and associated characteristics of adult Pacific lamprey, and to test the null hypothesis that different life histories do not exist. Females exhibited early vitellogenesis â early maturation stages; males exhibited spermatogonia â spermatozoa. Cluster analyses revealed an âimmatureâ group and a âmaturingâmatureâ group for each sex. We found statistically significant differences between these groups in the relationships between (i) body mass and total length in males; (ii) Fultonâs condition factor and liver lipids in males; (iii) the gonadosomatic index (GSI) and liver lipids in females; (iv) GSI and total length in females; (v) mean oocyte diameter and liver lipids; and (vi) mean oocyte diameter and GSI. We found no significant difference between the groups in the relationship of muscle lipids and body mass. Our analyses support rejection of the hypothesis of a single life history. We found evidence for an âocean-maturingâ life history that would likely spawn within several weeks of entering fresh water, in addition to the formerly recognized life history of spending 1 year in fresh water prior to spawningâthe âstream-maturingâ life history. Late maturity, semelparity, and high fecundity suggest that Pacific lamprey capitalize on infrequent opportunities for reproduction in highly variable environments.Keywords: Primitive, Petromyzontiformes, Life histor

Incidence of male intersex in adult Pacific lamprey (Entosphenus tridentatus), with a brief discussion of intersex vs. hermaphroditism in lampreys (Petromyzontiformes)
We report the incidence of male intersex in adult Pacific lamprey (Entosphenus tridentatus (Richardson, 1836)) during their pre-spawning migration in fresh water. Although âhermaphroditesâ have been suggested in other adult lampreys, this is the first detailed description and discussion of this phenomenon. A total of 0.5% of our adult Pacific lamprey from Willamette Falls (2 out of 427 adults) were intersex, with oocytes in the testes. This phenomenon was identifiable only by histological examination. The testes of the intersex males were immature, in the beginning stages of meiosis. One intersex male possessed primary growth or perinucleolar stage oocytes loosely interspersed throughout the testes, and the other possessed at least 6 mid-vitellogenic oocytes (0.6 mm, mean long diameter) separate from the testes. Despite technological advances in metabolomics, large parts of the human metabolome are still unexplored. In an untargeted metabolomics screen aiming to identify substrates of the orphan transporter ATP-binding cassette subfamily C member 5 (ABCC5), we identified a class of mammalian metabolites, N-lactoyl-amino acids. Using parallel protein fractionation in conjunction with shotgun proteomics on fractions containing N-lactoyl-Phe-forming activity, we unexpectedly found that a protease, cytosolic nonspecific dipeptidase 2 (CNDP2), catalyzes their formation. N-lactoyl-amino acids are ubiquitous pseudodipeptides of lactic acid and amino acids that are rapidly formed by reverse proteolysis, a process previously considered to be negligible in vivo. The plasma levels of these metabolites strongly correlate with plasma levels of lactate and amino acid, as shown by increased levels after physical exercise and in patients with phenylketonuria who suffer from elevated Phe levels. Our approach to identify unknown metabolites and their biosynthesis has general applicability in the further exploration of the human metabolome
Considerable Variability Among Transplant Nephrologists in Judging Deceased Donor Kidney Offers
Introduction: Transplant clinicians may disagree on whether or not to accept a deceased donor kidney offer. We investigated the interobserver variability between transplant nephrologists regarding organ acceptance and whether the use of a prediction model impacted their decisions.Methods: We developed an observational online survey with 6 real-life cases of deceased donor kidneys offered to a waitlisted recipient. Per case, nephrologists were asked to estimate the risk of adverse outcome and whether they would accept the offer for this patient, or for a patient of their own choice, and how certain they felt. These questions were repeated after revealing the risk of adverse outcome, calculated by a validated prediction model. Results: Sixty Dutch nephrologists completed the survey. The intraclass correlation coefficient of their estimated risk of adverse outcome was poor (0.20, 95% confidence interval [CI] 0.08â0.62). Interobserver agreement of the decision on whether or not to accept the kidney offer was also poor (Fleiss kappa 0.13, 95% CI 0.129â0.130). The acceptance rate before and after providing the outcome of the prediction model was significantly influenced in 2 of 6 cases. Acceptance rates varied considerably among transplant centers. Conclusion: In this study, the estimated risk of adverse outcome and subsequent decision to accept a suboptimal donor kidney varied greatly among transplant nephrologists. The use of a prediction model could influence this decision and may enhance nephrologistsâ certainty about their decision.</p
Why is it difficult to implement e-health initiatives? A qualitative study
<b>Background</b> The use of information and communication technologies in healthcare is seen as essential for high quality and cost-effective healthcare. However, implementation of e-health initiatives has often been problematic, with many failing to demonstrate predicted benefits. This study aimed to explore and understand the experiences of implementers - the senior managers and other staff charged with implementing e-health initiatives and their assessment of factors which promote or inhibit the successful implementation, embedding, and integration of e-health initiatives.<p></p>
<b>Methods</b> We used a case study methodology, using semi-structured interviews with implementers for data collection. Case studies were selected to provide a range of healthcare contexts (primary, secondary, community care), e-health initiatives, and degrees of normalization. The initiatives studied were Picture Archiving and Communication System (PACS) in secondary care, a Community Nurse Information System (CNIS) in community care, and Choose and Book (C&B) across the primary-secondary care interface. Implementers were selected to provide a range of seniority, including chief executive officers, middle managers, and staff with 'on the ground' experience. Interview data were analyzed using a framework derived from Normalization Process Theory (NPT).<p></p>
<b>Results</b> Twenty-three interviews were completed across the three case studies. There were wide differences in experiences of implementation and embedding across these case studies; these differences were well explained by collective action components of NPT. New technology was most likely to 'normalize' where implementers perceived that it had a positive impact on interactions between professionals and patients and between different professional groups, and fit well with the organisational goals and skill sets of existing staff. However, where implementers perceived problems in one or more of these areas, they also perceived a lower level of normalization.<p></p>
<b>Conclusions</b> Implementers had rich understandings of barriers and facilitators to successful implementation of e-health initiatives, and their views should continue to be sought in future research. NPT can be used to explain observed variations in implementation processes, and may be useful in drawing planners' attention to potential problems with a view to addressing them during implementation planning
Implementation of the kidney team at home intervention:Evaluating generalizability, implementation process, and effects
Research has shown that a home-based educational intervention for patients with chronic kidney disease results in better knowledge and communication, and more living donor kidney transplantations (LDKT). Implementation research in the field of renal care is almost nonexistent. The aims of this study were (1) to demonstrate generalizability, (2) evaluate the implementation process, and (3) to assess the relationship of intervention effects on LDKT-activity. Eight hospitals participated in the project. Patients eligible for all kidney replacement therapies (KRT) were invited to participate. Effect outcomes were KRT-knowledge and KRT-communication, and treatment choice. Feasibility, fidelity, and intervention costs were assessed as part of the process evaluation. Three hundred and thirty-two patients completed the intervention. There was a significant increase in KRT-knowledge and KRT-communication among participants. One hundred and twenty-nine out of 332 patients (39%) had LDKT-activity, which was in line with the results of the clinical trials. Protocol adherence, knowledge, and age were correlated with LDKT-activity. This unique implementation study shows that the results in practice are comparable to the previous trials, and show that the intervention can be implemented, while maintaining quality. Results from the project resulted in the uptake of the intervention in standard care. We urge other countries to investigate the uptake of the intervention
Probing photo-ionization: simulations of positive streamers in varying N2:O2 mixtures
Photo-ionization is the accepted mechanism for the propagation of positive
streamers in air though the parameters are not very well known; the efficiency
of this mechanism largely depends on the presence of both nitrogen and oxygen.
But experiments show that streamer propagation is amazingly robust against
changes of the gas composition; even for pure nitrogen with impurity levels
below 1 ppm streamers propagate essentially with the same velocity as in air,
but their minimal diameter is smaller, and they branch more frequently.
Additionally, they move more in a zigzag fashion and sometimes exhibit a
feathery structure. In our simulations, we test the relative importance of
photo-ionization and of the background ionization from pulsed repetitive
discharges, in air as well as in nitrogen with 1 ppm O2 . We also test
reasonable parameter changes of the photo-ionization model. We find that photo-
ionization dominates streamer propagation in air for repetition frequencies of
at least 1 kHz, while in nitrogen with 1 ppm O2 the effect of the repetition
frequency has to be included above 1 Hz. Finally, we explain the feather-like
structures around streamer channels that are observed in experiments in
nitrogen with high purity, but not in air.Comment: 12 figure
Probing photo-ionization: Experiments on positive streamers in pure gasses and mixtures
Positive streamers are thought to propagate by photo-ionization whose
parameters depend on the nitrogen:oxygen ratio. Therefore we study streamers in
nitrogen with 20%, 0.2% and 0.01% oxygen and in pure nitrogen, as well as in
pure oxygen and argon. Our new experimental set-up guarantees contamination of
the pure gases to be well below 1 ppm. Streamers in oxygen are difficult to
measure as they emit considerably less light in the sensitivity range of our
fast ICCD camera than the other gasses. Streamers in pure nitrogen and in all
nitrogen/oxygen mixtures look generally similar, but become somewhat thinner
and branch more with decreasing oxygen content. In pure nitrogen the streamers
can branch so much that they resemble feathers. This feature is even more
pronounced in pure argon, with approximately 10^2 hair tips/cm^3 in the
feathers at 200 mbar; this density could be interpreted as the free electron
density creating avalanches towards the streamer stem. It is remarkable that
the streamer velocity is essentially the same for similar voltage and pressure
in all nitrogen/oxygen mixtures as well as in pure nitrogen, while the oxygen
concentration and therefore the photo-ionization lengths vary by more than five
orders of magnitude. Streamers in argon have essentially the same velocity as
well. The physical similarity of streamers at different pressures is confirmed
