Testicular plasmacytoma, an unusal testicular tumor
We report the case of a 78-year-old man with recent diagnosis of Kahler disease and a non-tender swollen left hemiscrotum. Ultrasound findings showed an intratesticular, hypervascular hypoechoic lesion. MRI imaging demonstrated general appearance of a malignant lesion. Specific characteristics however, could not be withheld on MRI. Orchidectomy was performed and plasmacytoma was demonstrated at pathological examination. Although plasmacytoma of the testis is extremely rare, it should be included in the differential diagnosis taking into account the ultrasound and MRI appearances, especially in elderly patients with known Kahler disease
Intestinal angioneurotic edema
Background: This 38-year-old female patient presented at the emergency department with subacute low abdominal pain for 3 days. There was little abdominal tenderness, but no signs of peritonitis. Her blood results showed little inflammation with mild leukocytosis 13.0 (3.5-11.0 x 10*3/µL) and mild CRP elevation 44 (< 5 mg/L). The patient had a CT scan of the abdomen on the same day. The day after, a colonoscopy was performed which was perfectly normal. Because the discrepancy between the findings on CT scan, biochemistry and colonoscopy, the patient underwent an explorative laparoscopy, which showed a normal colon. Two days after the initial CT scan, a control CT scan was performed

Background: A 15-year-old boy complained of pain in the right groin after kicking against a heavy ball. He could still walk, but every time he put weight on his right leg he felt a stabbing pain in his groin area. The pain was very localised, he situated it just above the hip joint
Polyarteritis nodosa of the spleen
Background: A 64-year-old male presented with pain in his right lower leg and progressive swelling of the right ankle, without history of previous trauma. Further anamnesis revealed fever, anorexia and weight loss over the past few weeks. Laboratory results showed increased inflammatory parameters (CRP 203 mg/l). A lower extremity ultrasonographic examination ruled out deep venous thrombosis. A contrast-enhanced CT scan of the abdomen revealed multiple splenic infarctions

Fragile x-associated tremor/ataxia syndrome
Background: A 64-year-old male experiences resting and intentional tremor. The complaints aggravated progressively over the last few years and appeared first at the head and later at the upper limbs. In the end, word finding difficulties, broad-based ataxic gait and erectile dysfunction added to the symptoms. Detailed familial history reveals that 2 nephews of the patient are known to have the fragile X syndrome. Genetic analysis of the FMR1 gene shows the patient carries a pre-mutation expressing 75 repeats
