Comparative efficacy of melatonin in attenuation of endotoxin/LPS induced hepatotoxicity in BALB/c mice
Background: Sepsis is characterized by overwhelming surge of cytokines and oxidative stress to one of many factors, gram negative bacteria commonly implicated. Despite major expansion and elaboration of sepsis pathophysiology and therapeutic approach; death rate remains very high in septic patients due to multiple organ damage including hepatotoxicity. The present study was aimed to ascertain the adequacy of melatonin (10mg/kg i.p), and its comparability with dexamethasone (3mg/kg i.p), delivered separately and collectively in endotoxin induced hepatotoxicity.Methods: The number of animals in each group was six. Endotoxin/LPS induced hepatotoxicity was reproduced in mice by giving LPS of serotype E. coli intraperitoneally. Preventive role was questioned by giving the experimental agent half an hour prior to LPS injection whereas therapeutic potential of the experimental agent was searched out via post LPS delivering. The extent of liver damage was adjudged via serum alanine aminotransferases (ALT) and aspartate aminotransferase (AST) estimation along with histopathological examination of liver tissue.Results: Melatonin was prosperous in aversion (Group 3) and curation (Group 4) of LPS invoked hepatotoxicity as evident by lessening of augmented ALT (≤0.01) and AST (≤0.01) along with restoration of pathological changes on liver sections (p≤0.05). Combination therapy with dexamethasone in conjunction with melatonin (Group 7) after LPS administration tapered LPS evoked hepatic dysfunction statistically considerably, however the result was comparable to single agent therapy.Conclusions: Melatonin set up promising results in endotoxin induced hepatotoxicity and can be used therapeutic adjuncts to conventional treatment strategies in sepsis induced liver failure. Combination therapies however generated no synergistic results

Newer treatment modalities is the dire need of time. The present study was aimed to ascertain the adequacy of phosphodiesterases inhibitor - pentoxifylline (75mg/kg i.p) in endotoxin/LPS induced hepatotoxicity in BALB/c mice.Methods: The number of animals in each group was six. Endotoxin/lipopolysaccharides induced hepatotoxicity was reproduced in mice by giving lipopolysaccharide of serotype E. coli intraperitoneally. To ascertain the Preventive role, pentoxifylline was administered forehand LPS injection whereas therapeutic potential adjuged via post LPS delivering. The extent of liver damage was evaluated through serum alanine aminotransferases (ALT) and aspartate aminotransferase (AST) estimation along with histopathological examination of liver tissue.Results: Results set forth that serum ALT, AST levels and histological alteration abated considerably (p ≤0.05) both in animals subjected to pentoxifylline pre and post-treatment.Conclusions: Pentoxifylline set up promising results in endotoxin induced hepatotoxicity and can be used therapeutic adjuncts to conventional treatment strategies in sepsis induced liver failure
Wi-Fi Fingerprinting Based Room Level Indoor Localization Framework Using Ensemble Classifiers
Over the past decennium, Wi-Fi fingerprinting based indoor localization has seized substantial attention. Room level indoor localization can enable numerous applications to increase their diversity by incorporating user location. Real-world commercial scale deployments have not been realized because of difficulty in capturing radio propagation models. In case of fingerprinting based approaches, radio propagation model is implicitly integrated in the gathered fingerprints providing more realistic information as compared to empirical propagation models. We propose ensemble classifiers based indoor localization using Wi-Fi fingerprints for room level prediction. The major advantages of the proposed framework are, ease of training, ease to set up framework providing high room-level accuracy with trifling response time making it viable and appropriate for real time applications. It performs well in comparison with recurrently used ANN (Artificial Neural Network) and kNN (k-Nearest Neighbours) based solutions. Experiments performed showed that on our real-world Wi-Fi fingerprint dataset, our proposed approach achieved 89% accuracy whereas neural network and kNN based best found configurations achieved 85 and 82% accuracy respectively
Comparison of hepatoprotective effect of aqueous and ethanolic extracts of Berberis lycium Royale (Sumbloo) in isoniazid induced hepatotoxicity in male mice model
Background: The objective of the study was to compare the hepatoprotective effect of aqueous and ethanolic extracts of stem bark of Berberis lycium Royle in isoniazid (INH) induced hepatotoxicity in mice model.Methods: This randomized controlled in-vivo study conducted in male mice model from 10th April 2014 till 10th May 2014 at National Institute of Health, Islamabad. Group (C) was given normal diet and water. Group (D) was given isoniazid (50 mg/kg BW) to produce hepatotoxicity. Group (LA) and (HA) were given isoniazid (INH) plus low and high dose of aqueous extract of stem bark of Berberis lycium Royle respectively. Group (L.E) and (H.E) were given INH plus low and high dose of ethanolic extract of stem bark of Berberis lycium Royle respectively.Results: Hepatotoxicity produced by INH was shown by raised serum liver function tests (LFT’s), marked hepatocytic ballooning, significant steatosis and inflammation. Mice receiving simultaneous treatment of INH, low and high dose of aqueous extract of Berberis lycium Royle showed decrease serum LFT’s and their liver sections showed improved histological picture but more significant reduction in hepatotoxic effects were observed in animals receiving low and high doses of ethanolic extract.Conclusions: Hepatotoxicity of INH can be more fully reversed by simultaneous use of INH with ethanolic extract as it has better hepatoprotective potential in dose dependent manner as compare to aqueous extract of stem bark of Berberis lycium Royle
Comparing Fetal Weight Estimates in the Third Trimester: Clinical Examination Findings, Ultrasonographic Estimation, and Actual Birth Weight
Objective: To determine the relationship between fetal weight on clinical and ultrasonographic examinations and actual birth weight during the third trimester of pregnancy.
Method: This descriptive cross-sectional study was conducted in Obs/Gynae from January 2023 to June 2023. A total of 1130 women between the ages of 18 and 40, gestational age of > 34 weeks, BMI of 18 to 25 kg/m2, single cephalic fetus and who gave consent were included. The patients with PIH, preeclampsia, eclampsia, anaemia, chronic renal disease, diabetes and conditions such as polyhydramnios, oligohydramnios, and congenital fetal anomalies were excluded from the study. The demographic information was recorded. By using Johnson's formula, patients go through CEFW. The skilled radiologists performed an ultrasound on every female to obtain UEFW. The Shepard formula was used to calculate the FW. Following that, females were monitored till the birth of the fetus. The baby's actual birth weight (ABW) was recorded at birth. Pearson correlation was used to determine the relationship's correlation value between the ABW, CEFW, and UEFW. P values lower than 0.05 were regarded as significant.
Results: The patients' average age was 25.19±4.26 years. According to the gender breakdown of infants, 619 females (54.8%) and 511 males (45.2%) were born. 207 patients (18.3%) were primigravida and 923 patients (81.7%) were multiparous. The connection between CEFW and ABW was high (r=0.890; p 0.001), and UEFW and ABW were correlated with one other likewise (r=0.934; p 0.001). The following were the mean values of the various variables: Gestational age (weeks): 37.51±2.06; height (cm): 163.97±3.39; weight (kg): 76.43±6.65; BMI (kg/m^2): 28.60±2.80; SFH: 36.63±1.46; CEFW (gms) 3061.50±367.66, UEFW (gms) 3014.42±339.71 and ABW (gms) 3059.12±310.32.
Conclusion: According to the findings of our investigation, both clinical estimation and ultrasound estimation produced results that were both practicable and trustworthy. Both displayed a favourable link to the real birth weight
Rates of publication of FCPS dissertations in international and national peer-review journals among residents at AKUH; A cross sectional review of 15 years
Objective: To see the rate of publication of postgraduate residents\u27 dissertation. Methods: The single-centre retrospective cross-sectional study was conducted at the Aga Khan University Hospital, Karachi, and comprised research publications from the residents of the departments of Surgery and Medicine who graduated between 2005 and 2020. The surgical subspecialties included Otolaryngology, Ophthalmology, Dentistry, General Surgery, Orthopaedics, Paediatric Surgery, Urology, Plastic Surgery and Cardiovascular Surgery. Data comprised demographics, current institution, current designation, information on dissertation/paper publication, topic of study, year of completion of dissertation, input from the research department, delay in exam due to incomplete dissertation and whether the paper got published in national or international journal. Data was analysed using SPSS 21. Results: Of the 103 subjects, 70(68%) were males and 33(32%) were females, while 73(70.8%) belonged to surgical specialties and 30(29.2%) were from non-surgical specialties. Of the 22(22.9%) who were able to convert, 12(54.5%) publications were carried by national peer-reviewed journals, while 10(45.4%) were carried by international journals; 9(40.9%) unpaid peer review journals and 13(59.1%) paid journals. Delay in exam due to incomplete dissertation was faced by only 16(16.6%) candidates. Conclusions: The rate of publication for resident dissertation was found to be low
Immune checkpoint inhibitor-associated gastrointestinal and hepatic adverse events and their management
BACKGROUND: Drug-induced colitis is a known complication of therapies that alter the immune balance, damage the intestinal barrier or disturb intestinal microbiota. Immune checkpoint inhibitors (ICI) directed against cancer cells may result in activated T lymphocyte-induced immune-related adverse events (AEs), including immune-related colitis and hepatitis. The aim of this review article is to summarize the incidence of gastrointestinal (GI) and hepatic AEs related to ICI therapy. We have also looked at the pathogenesis of immune-mediated AEs and propose management strategies based on current available evidence. METHODS: A literature search using PubMed and Medline databases was undertaken using relevant search terms pertaining to names of individual drugs, mechanism of action, related AEs and their management. RESULTS: ICI-related GI AEs are common, and colitis appears to be the most common side effect, with some studies reporting incidence as high as 30%. The incidence of both all-grade colitis and hepatitis were highest with combination therapy with anti-CTLA-4/PD-1; severity of colitis was dose-dependent (anti-CTLA-4). Early intervention is associated with better outcomes. CONCLUSION: ICI-related GI and hepatic AEs are common and clinicians need to be aware. Patients with GI AEs benefit from early diagnosis using endoscopy and computed tomography. Early intervention with oral steroids is effective in the majority of patients, and in steroid-refractory colitis infliximab and vedolizumab have been reported to be useful; mycophenolate has been used for steroid-refractory hepatitis
Highly Selective and Reproducible Electrochemical Sensing of Ascorbic Acid Through a Conductive Polymer Coated Electrode
The surface of an Au-disc electrode was modified through electro polymerization of aniline, in the presence of dodecyl benzene sulphonic acid (DBSA) and sulphuric acid (H2SO4) solution. The polymerization conditions were pre-optimized so that micelle formation and solution coagulation could be minimized and surfactant doped polyaniline film could be obtained through a quick, simple and one step polymerization route. The synthesized material was characterized via Fourier transform infrared (FTIR) spectroscopy, thermogravimetric analysis (TGA), scanning electron microscopy (SEM) and cyclic voltammetry (CV). The effective surface area of the Au-disc, calculated through cyclic voltammetry, was immensely increased through a polyaniline (PANI) coating (0.04 and 0.11 cm2 for bare and PANI coated gold respectively). The modified electrode was utilized for ascorbic acid (AA) sensing. The changing pH of electrolyte and scan rate influenced the PANI electrode response towards AA. The modified electrode was highly selective towards AA oxidation and showed a very low limit of detection i.e. 0.0267 μmol·L-1. Moreover, the PANI coating greatly reduced the sensing potential for AA by a value of around 140 mV when compared to that on a bare gold electrode
Plasmodium in the bone marrow: Case series from a hospital in Pakistan, 2007-2015
Background: Malaria is a life-threatening, multisystem disease caused by the plasmodial parasite with a global incidence of approximately 229 million annually. The parasites are known to have unique and crucial interactions with various body tissues during its life cycle, notably the liver, spleen, and recent work has shown the bone marrow to be a reservoir of infection.Methods: This study is a case series of patients in whom examination of bone marrow revealed malarial parasites. A retrospective record review of 35 parasite-positive bone marrow specimens examined at Aga Khan University Hospital (AKUH), Karachi, Pakistan, over the years 2007 to 2015 was conducted. Bone marrow aspirates were collected as per International Council for Standardization in Haematology (ICSH) guidelines.Results: The median age of patients was 22 years (range 1-75), and 60 % (n = 21) were male. 22 patients had evidence of Plasmodium falciparum, 12 had evidence of Plasmodium vivax and 1 patient had a mixed infection. Gametocytes and trophozoites were the most common stages identified on both peripheral blood and bone marrow examinations. Indications for bone marrow examination included fever of unknown origin and the workup of cytopenias and malignancies.Conclusions: The incidental finding of Plasmodium in samples of bone marrow suggests the reticuloendothelial system may be regularly harbour these parasites, be the infection acute or chronic in character
