    Factors Limiting Youth Participation in 4-H and Other Youth Development Programs in Underserved Communities

    The purpose of the study described here was to identify factors limiting children\u27s participation in youth development programs in underserved communities and ways to overcome those limitations. Findings are based on focus group interviews conducted with members of two underserved communities in North Carolina. Lack of awareness about the 4-H program, lack of affordability, program scheduling conflicts, lack of transportation, and felt exclusion were identified as the major limitations to children\u27s participation in programs. Engaging with the community to win members\u27 trust, informing the target audience of programming ahead of time, and making programs affordable are potential strategies for improving the situation

    Pelaksanaan tugas dalam konteks pemerintah daerah masih banyak menghadapi permasalahan dalam melaksanakan tugas dan wewenangnya untuk mewujudkan tata kelola pemerintahan yang baik secara menyeluruh. Oleh karena itu, diundangkannya Undang-Undang Pemerintahan Daerah No. 23 Tahun 2014 akan memberikan peluang yang lebih besar untuk melaksanakan prinsip desentralisasi, pemisahan kekuasaan, tugas pembantuan dan otonomi daerah, memungkinkan pemerintah daerah untuk melaksanakan tugas pemerintahan, pembangunan dan pelayanan. masyarakat dengan sebaik-baiknya, tidak lagi terlalu mengandalkan pemerintah pusat (sentralisasi). Di era digital saat ini, inovasi layanan berbasis teknologi merupakan salah satu alat yang digunakan untuk mewujudkan layanan yang akuntabel dan transparan, yang merupakan pilar fundamental dari paradigma good governance. Komponen utama keberhasilan akuntabilitas publik adalah adanya sistem keterbukaan informasi. Transparansi informasi ini merupakan modal penting yang dapat digunakan untuk menilai kinerja pelayanan sektor publik dan menilai akuntabilitas pelaksana sektor publik atas segala keputusan dan tindakannya. Pertanyaan yang diajukan oleh penelitian ini adalah seberapa efektif penerapan prinsip good governance dalam pelayanan publik di era digital (penelitian dilakukan di Kanwil Kota Ternate Utara). Tujuan dari penelitian ini adalah untuk mengetahui seberapa efektif penerapan prinsip good governance dalam pelayanan publik di Kantor Kecamatan Ternate Utara di Kota Ternate.Penelitian ini menggunakan metode deskriptif kualitatif dan pengumpulan data melalui wawancara. Temuan kajian efektivitas penerapan prinsip good governance pada pelayanan publik di era digital (penelitian dilakukan di Kantor Kecamatan Ternate Utara) sangat efektif dalam meningkatkan pelayanan publik. Hal ini terlihat dari partisipasi aktif masyarakat dalam penyelenggaraan pelayanan publik.     Implementation of a novel antimicrobial stewardship strategy for rural facilities utilising telehealth

    A significant portion of healthcare takes place in small hospitals, and many are located in rural and regional areas. Facilities in these regions frequently do not have adequate resources to implement an onsite antimicrobial stewardship programme and there are limited data relating to their implementation and effectiveness. We present an innovative model of providing a specialist telehealth antimicrobial stewardship service utilising a centralised service (Queensland Statewide Antimicrobial Stewardship Program) to a rural Hospital and Health Service. Results of a 2-year post-implementation follow-up showed an improvement in adherence to guidelines [33.7% (95% CI 27.0–40.4%) vs. 54.1% (95% CI 48.7–59.5%)] and appropriateness of antimicrobial prescribing [49.0% (95% CI 42.2–55.9%) vs. 67.5% (95% CI 62.7–72.4%) (P < 0.001). This finding was sustained after adjustment for hospitals, with improvement occurring sequentially across the years for adherence to guidelines [adjusted odds ratio (aOR) = 2.44, 95% CI 1.70–3.51] and appropriateness of prescribing (aOR = 2.48, 95% CI 1.70–3.61). There was a decrease in mean total antibiotic use (DDDs/1000 patient-days) between the years 2016 (52.82, 95% CI 44.09–61.54) and 2018 (39.74, 95% CI 32.76–46.73), however this did not reach statistical significance. Additionally, there was a decrease in mean hospital length of stay (days) from 2016 (3.74, 95% CI 3.08–4.41) to 2018 (2.55, 95% CI 1.98–3.12), although this was not statistically significant. New telehealth-based models of antimicrobial stewardship can be effective in improving prescribing in rural areas. Programmes similar to ours should be considered for rural facilities

    2D DIGE analysis of maternal plasma for potential biomarkers of Down Syndrome

    <p>Abstract</p> <p>Background</p> <p>Prenatal screening for Down Syndrome (DS) would benefit from an increased number of biomarkers to improve sensitivity and specificity. Improving sensitivity and specificity would decrease the need for potentially risky invasive diagnostic procedures.</p> <p>Results</p> <p>We have performed an in depth two-dimensional difference gel electrophoresis (2D DIGE) study to identify potential biomarkers. We have used maternal plasma samples obtained from first and second trimesters from mothers carrying DS affected fetuses compared with mothers carrying normal fetuses. Plasma samples were albumin/IgG depleted and expanded pH ranges of pH 4.5 - 5.5, pH 5.3 - 6.5 and pH 6 - 9 were used for two-dimensional gel electrophoresis (2DE). We found no differentially expressed proteins in the first trimester between the two groups. Significant up-regulation of ceruloplasmin, inter-alpha-trypsin inhibitor heavy chain H4, complement proteins C1s subcomponent, C4-A, C5, and C9 and kininogen 1 were detected in the second trimester in maternal plasma samples where a DS affected fetus was being carried. However, ceruloplasmin could not be confirmed as being consistently up-regulated in DS affected pregnancies by Western blotting.</p> <p>Conclusions</p> <p>Despite the in depth 2DE approach used in this study the results underline the deficiencies of gel-based proteomics for detection of plasma biomarkers. Gel-free approaches may be more productive to increase the number of plasma biomarkers for DS for non-invasive prenatal screening and diagnosis.</p

    Structures of tetrasilylmethane derivatives C(SiXMe2)4 (X = H, F, Cl, Br) in the gas phase and their dynamic structures in solution.

    The structures of the molecules C(SiXMe2)4 (X = H, F, Cl, Br) have been determined by gas electron diffraction (GED). Ab initio calculations revealed nine potential minima for each species, with significant ranges of energies. For the H, F, Cl, and Br derivatives nine, seven, two, and two conformers were modelled, respectively, as they were quantum-chemically predicted to be present in measurable quantities. Variable-temperature 1H and 29Si solution-phase NMR studies and, where applicable, 13C NMR, 1H/29Si NMR shift-correlation, and 1H NMR saturation-transfer experiments are reported for C(SiXMe2)4 (X = H, Cl, Br, and also I). At low temperature in solution two conformers (one C1-symmetric and one C2-symmetric) are observed for each of C(SiXMe2)4 (X = Cl, Br, I), in agreement with the isolated molecule ab initiocalculations carried out as part of this work for X = Cl, Br. C(SiHMe2)4 is present as a single C1-symmetric conformer in solution at the temperatures at which the NMR experiments were performed

    A Uniform Algorithm for All-Speed Shock-Capturing Schemes

    There are many ideas for developing shock-capturing schemes and their extension for all-speed flow. The representatives of them are Roe, HLL and AUSM families. In this paper, a uniform algorithm is proposed, which expresses three families in the same framework. The algorithm has explicit physical meaning, provides a new angel of understanding and comparing the mechanism of schemes, and may play a great role in the further research. As an example of applying the uniform algorithm, the low-Mach number behaviour of the schemes is analyzed. Then, a very clear and simple explanation is given based on the wall boundary, and a concise rule is proposed to judge whether a scheme has satisfied low-Mach number behaviour

This paper describes a principled systems architecture---Encode, Shuffle, Analyze (ESA)---for performing such monitoring with high utility while also protecting user privacy.
