    Multiple Pathways to the Same End: Mechanisms of Myonuclear Apoptosis in Sarcopenia of Aging

    Sarcopenia, the age-related decline in muscle mass and function, represents a significant health issue due to the high prevalence of frailty and disability associated with this condition. Nevertheless, the cellular mechanisms responsible for the loss of muscle mass in old age are still largely unknown. An altered regulation of myocyte apoptosis has recently emerged as a possible contributor to the pathogenesis of sarcopenia. Studies in animal models have shown that the severity of skeletal muscle apoptosis increases over the course of aging and correlates with the degree of muscle mass and strength decline. Several apoptotic pathways are operative in aged muscles, with the mitochondria- and TNF-α-mediated pathways likely being the most relevant to sarcopenia. However, despite the growing number of studies on the subject, a definite mechanistic link between myocyte apoptosis and age-related muscle atrophy has not yet been established. Furthermore, the evidence on the role played by apoptosis in human sarcopenia is still sparse. Clearly, further research is required to better define the involvement of myocyte apoptosis in the pathogenesis of muscle loss at advanced age. This knowledge will likely help in the design of more effective therapeutic strategies to preserve muscle mass into old age, thus fostering independence of the elderly population and reducing the socioeconomic burden associated with sarcopenia

    A novel piggyback strategy for mRNA delivery exploiting adenovirus entry biology

    Molecular therapies exploiting mRNA vectors embody enormous potential, as evidenced by the utility of this technology for the context of the COVID-19 pandemic. Nonetheless, broad implementation of these promising strategies has been restricted by the limited repertoires of delivery vehicles capable of mRNA transport. On this basis, we explored a strategy based on exploiting the well characterized entry biology of adenovirus. To this end, we studied an adenovirus-polylysine (AdpL) that embodied piggyback transport of the mRNA on the capsid exterior of adenovirus. We hypothesized that the efficient steps of Ad binding, receptor-mediated entry, and capsid-mediated endosome escape could provide an effective pathway for transport of mRNA to the cellular cytosol for transgene expression. Our studies confirmed that AdpL could mediate effective gene transfer of mRNA vectors in vitro and in vivo. Facets of this method may offer key utilities to actualize the promise of mRNA-based therapeutics

    Association of Urinary and Plasma Levels of Trimethylamine N-Oxide (TMAO) with Foods

    Introduction: Trimethylamine N-oxide (TMAO) may play a key mediator role in the relationship between the diet, gut microbiota and cardiovascular diseases, particularly in people with kidney failure. The aim of this review is to evaluate which foods have a greater influence on blood or urinary trimethylamine N-oxide (TMAO) levels. Methods: 391 language articles were screened, and 27 were analysed and summarized for this review, using the keywords "TMAO" AND "egg" OR "meat" OR "fish" OR "dairy" OR "vegetables" OR "fruit" OR "food" in December 2020. Results: A strong correlation between TMAO and fish consumption, mainly saltwater fish and shellfish, but not freshwater fish, has been demonstrated. Associations of the consumption of eggs, dairy and meat with TMAO are less clear and may depend on other factors such as microbiota or cooking methods. Plant-based foods do not seem to influence TMAO but have been less investigated. Discussion: Consumption of saltwater fish, dark meat fish and shellfish seems to be associated with an increase in urine or plasma TMAO values. Further studies are needed to understand the relationship between increased risk of cardiovascular disease and plasma levels of TMAO due to fish consumption.     Treatment of thoraco-lumbar curves in adolescent females affected by idiopathic scoliosis with a progressive action short brace (PASB): assessment of results according to the SRS committee on bracing and nonoperative management standardization criteria

    <p>Abstract</p> <p>Background</p> <p>The effectiveness of conservative treatment of scoliosis is controversial. Some studies suggest that brace is effective in stopping curve progression, whilst others did not report such an effect.</p> <p>The purpose of the present study was to effectiveness of Progressive Action Short Brace (PASB) in the correction of thoraco-lumbar curves, in agreement with the Scoliosis Research Society (SRS) Committee on Bracing and Nonoperative Management Standardisation Criteria.</p> <p>Methods</p> <p>Fifty adolescent females (mean age 11.8 ± 0.5 years) with thoraco-lumbar curve and a pre-treatment Risser score ranging from 0 to 2 have been enrolled. The minimum duration of follow-up was 24 months (mean: 55.4 ± 44.5 months). Antero-posterior radiographs were used to estimate the curve magnitude (C<sub>M</sub>) and the torsion of the apical vertebra (T<sub>A</sub>) at 5 time points: beginning of treatment (t<sub>1</sub>), one year after the beginning of treatment (t<sub>2</sub>), intermediate time between t<sub>1 </sub>and t<sub>4 </sub>(t<sub>3</sub>), end of weaning (t<sub>4</sub>), 2-year minimum follow-up from t<sub>4 </sub>(t<sub>5</sub>). Three situations were distinguished: curve correction, curve stabilisation and curve progression.</p> <p>The Kruskal Wallis and Spearman Rank Correlation tests have been used as statistical tests.</p> <p>Results</p> <p>C<sub>M </sub>mean value was 29,30 ± 5,16 SD at t<sub>1 </sub>and 14,67 ± 7,65 SD at t<sub>5</sub>. T<sub>A </sub>was 12.70 ± 6,14 SD at t<sub>1 </sub>and 8,95 ± 5,82 at t<sub>5</sub>. The variation between measures of Cobb and Perdriolle degrees at t<sub>1,2,3,4,5 </sub>and between C<sub>M </sub>t<sub>5</sub>-t<sub>1 </sub>and T<sub>A </sub>t<sub>5</sub>-t<sub>1 </sub>were significantly different.</p> <p>Curve correction was accomplished in 94% of patients, whereas a curve stabilisation was obtained in 6% of patients.</p> <p>Conclusion</p> <p>The PASB, due to its peculiar biomechanical action on vertebral modelling, is highly effective in correcting thoraco-lumbar curves.</p

    Determination of quality of life in adolescents with idiopathic scoliosis subjected to conservative treatment.

    <p>Abstract</p> <p>Introduction and objectives</p> <p>Physical deformities caused by adolescent idiopathic scoliosis (AIS) coupled with conservative treatment of AIS with orthesis unavoidably impacts on patients' quality of life (QoL). The present study aimed at evaluating the QoL in patients affected by AIS treated with brace. The study also sought to determine the ability of different QoL questionnaires to monitor QoL over the course of treatment.</p> <p>Materials and methods</p> <p>Data were collected in 108 consecutive patients (96 females, 16 males) affected by AIS admitted to the outpatient orthopaedic clinic of the Catholic University of the Sacred Heart in Rome (Italy). Patients were subjected to full-time (i.e., 22 hrs per day) conservative treatment with the progressive action short brace (PASB), the Lyon brace or a combination of PASB + Lyon brace. Three instruments were used for QoL determination: the Scoliosis Research Society 22 (SRS-22), Bad Sobernheim Stress Questionnaire (BSSQ) and the Brace Questionnaire (BrQ).</p> <p>Results</p> <p>A significant correlation was detected among the 3 scores (p < 0.001). The BrQ possesses a higher capacity to detect changes in QoL in relation to the patient gender, type of brace, curve severity at baseline and at the completion of treatment, and curve type. Overall, boys displayed a higher QoL than girls. In all 3 questionnaires, higher QoL scores were determined in patients treated with the PASB compared with those using the Lyon brace. QoL scores were significantly correlated with the curve severity. Higher QoL scores were obtained by participants with thoraco-lumbar curves as compared with those with other curves.</p> <p>Conclusions</p> <p>The 3 questionnaires are effective in capturing changes in QoL in AIS patients subjected to conservative treatment. However, the BrQ possesses a higher discriminatory capacity compared with the other questionnaires tested. PASB-based treatment is associated with better QoL than the Lyon bracing.</p

    2011 SOSORT guidelines: Orthopaedic and Rehabilitation treatment of idiopathic scoliosis during growth

    <p>Abstract</p> <p>Background</p> <p>The International Scientific Society on Scoliosis Orthopaedic and Rehabilitation Treatment (SOSORT), that produced its first Guidelines in 2005, felt the need to revise them and increase their scientific quality. The aim is to offer to all professionals and their patients an evidence-based updated review of the actual evidence on conservative treatment of idiopathic scoliosis (CTIS).</p> <p>Methods</p> <p>All types of professionals (specialty physicians, and allied health professionals) engaged in CTIS have been involved together with a methodologist and a patient representative. A review of all the relevant literature and of the existing Guidelines have been performed. Documents, recommendations, and practical approach flow charts have been developed according to a Delphi procedure. A methodological and practical review has been made, and a final Consensus Session was held during the 2011 Barcelona SOSORT Meeting.</p> <p>Results</p> <p>The contents of the document are: methodology; generalities on idiopathic scoliosis; approach to CTIS in different patients, with practical flow-charts; literature review and recommendations on assessment, bracing, physiotherapy, Physiotherapeutic Specific Exercises (PSE) and other CTIS. Sixty-five recommendations have been given, divided in the following topics: Bracing (20 recommendations), PSE to prevent scoliosis progression during growth (8), PSE during brace treatment and surgical therapy (5), Other conservative treatments (3), Respiratory function and exercises (3), Sports activities (6), Assessment (20). No recommendations reached a Strength of Evidence level I; 2 were level II; 7 level III; and 20 level IV; through the Consensus procedure 26 reached level V and 10 level VI. The Strength of Recommendations was Grade A for 13, B for 49 and C for 3; none had grade D.</p> <p>Conclusion</p> <p>These Guidelines have been a big effort of SOSORT to paint the actual situation of CTIS, starting from the evidence, and filling all the gray areas using a scientific method. According to results, it is possible to understand the lack of research in general on CTIS. SOSORT invites researchers to join, and clinicians to develop good research strategies to allow in the future to support or refute these recommendations according to new and stronger evidence.</p

    Self-assembling amphiphilic peptides: A novel approach to create new drug systems and three-dimensional scaffolds for tissue regeneration. A resourceful mine not yet fully explored

    Molecular self-assembly offers unique features for the development of novel supramolecular structures and advanced materials. The inspiration for the creation of these complex structures is often driven by nature, where complex architectures are obtained from simple building block such as amino acids, nucleic acids and lipids. Peptide-based nanostructures constitute a novel route to create well organized self-assembled materials that eventually can be used in a variety of different fields. They can be divided in different classes based on the architecture of the overall molecule. My research is mainly focused on two types of peptide-based structures: peptide amphiphiles, in which an amino acidic sequence is tailed with an hydrophobic molecule, such as palmitic acid, and facially amphiphilic peptides, which is made by proper sequence patterning of hydrophobic and hydrophilic amino acids. In chapter 2 I describe how a peptide amphiphile was designed, synthesized and tested on different types of cancer cell lines, and it proved to be effective in reducing the growth of malignant cells. This peptide works as an inhibitor of the HOX-PBX1 interaction. HOX and PBX I are two transcription factors that are deeply involved in the DNA regulation during embryogenesis and they are found to be highly miss-regulated in several types of cancers. Until now, peptide amphiphiles have been used mostly as drug carriers. This work constitutes one of the first studies in which a peptide amphiphile molecule can work both as a drug and as a drug carrier, simultaneously. 3D-scaffolds for tissue engineering applications are another hot area in which self-assembling peptides are finding several applications. Driven by this motivation, chapter 3, 4 and 5 describe how I designed, synthesized and investigated the properties of a new class of facial peptide amphiphile. These peptides, when dissolved in an aqueous environment and under the right pH and salt conditions, have proven to quickly self-assemble in long and entangled nanofibers that ultimately form stable hydrogels. These hydrogels have potential applications as bio-scaffolds for tissue regeneration, as cells are entrapped in their network. The mechanical properties of these hydrogels can be triggered using different approaches and it is possible to obtain very strong materials, even at very low concentrations of the peptide, such as 1% by weight. In order to be safely used for bio-applications, hydrogel materials must be need to have two main features: biocompatibility and biodegradability. The hydrogels obtained through self-assembling peptides have proven to be both biocompatible and biodegradable, thanks to the introduction of a short amino acid sequence recognized by proteases, which is described in chapter 5. In conclusions, the results collected in this study show how a simple building block, such as a small peptide, can serve as an extremely powerful and flexible tool to design complex self-assembled systems. This is area of research is indeed full of potential that has not yet been completely explored
