    Primary Sjogren's syndrome presenting as ptosis and eyelid swelling: A case report

    Key Clinical Message This unique case report of primary Sjogren's syndrome (pSS) shows bilateral ptosis and significant periorbital edema, compromising vision. To avoid misleading diagnosis, antibody tests must be evaluated and interpreted in the context of clinical findings. Abstract Primary Sjögren's syndrome is an idiopathic, autoimmune disorder involving the lacrimal and salivary glands characterized by both localized and systemic manifestations including xerostomia and keratoconjunctivitis sicca. Myasthenia Gravis (MG) is also an autoimmune disorder characterized by the development of auto‐antibodies against nicotinic acetylcholine receptors that causes decreased muscle response to stimulation. It usually presents with ptosis and generalized body weakness. Ophthalmological involvement is common in both disorders but ptosis is very rarely seen in pSS. We report the case of a 27‐year‐old woman presenting to our clinic with the complaint of ptosis and eyelid swelling. She also had a positive anti‐acetylcholine receptor antibody test and her initial presentation mimicked Myasthenia Gravis. Her autoimmune workup revealed a positive titer of Anti Ro SSA antibodies. Myasthenia Gravis was ruled out on electrodiagnostic studies which showed no decremental response, and pSS was confirmed on lip biopsy. Our case highlights that it is important to interpret the antibody test results in the context of clinical findings as we can have spurious results in autoimmune diseases. Autoimmune conditions can have varying presenting complaints hence, clinical judgment should always overrule diagnostic investigations and should thus guide patient management

    Hemophagocytic lymphohistiocytosis (HLH) is a rare disorder. Dysfunction of cytotoxic T and natural killer (NK) cells causes uncontrolled activity of lymphocytes and histiocytes which leads to HLH. Infections, malignancies, and autoimmune disorders are associated with development of HLH. Dengue and Plasmodium vivax are rare causes of HLH. We report the first ever case of a young man who developed fatal HLH that complicated Dengue Hemorrhagic Fever (DHF) and Plasmodium vivax infection

    Antibacterial, Antioxidant and Physicochemical Properties of <i>Pipper nigram</i> Aided Copper Oxide Nanoparticles

    The Pipper nigram (P. nigram) leaf extract was used for the biosynthesis of copper oxide nanoparticles (CuO NPs) and the successful formation of the resultant product was confirmed through several physicochemical techniques. The chemical structure and the elemental composition were analysed through Fourier transform infrared (FTIR) and energy dispersive X-ray (EDX) spectroscopies, respectively. The crystalline structure and crystallite size were investigated through an X-ray diffractometer (XRD) and a monoclinic crystallite with a size of 40.68 nm was reported. Even-distributed particles with an average particle size of 49.75 nm were seen in the scanning electron micrograph (SEM), whereas the thermal stability was checked during the thermogravimetric analysis (TGA). The ultra-violet and visible (UV-Visible) spectroscopy was operated to study the light absorbance phenomena and to determine the band gap energy from the absorption edge, which was found to be 1.47 eV. The CuO NPs were used as antibacterial agents against gram-negative bacteria (GNB) and gram-positive bacteria (GPB), and greater inhibition zones were seen against the former one. The antioxidant test was also carried out against 2,2′-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid) (ABTS) free radicals and the antioxidant potential of CuO NPs was found to be higher than ascorbic acid

    Upregulated type I interferon responses in asymptomatic COVID-19 infection are associated with improved clinical outcome

    Understanding key host protective mechanisms against SARS-CoV-2 infection can help improve treatment modalities for COVID-19. We used a blood transcriptome approach to study biomarkers associated with differing severity of COVID-19, comparing severe and mild Symptomatic disease with Asymptomatic COVID-19 and uninfected Controls. There was suppression of antigen presentation but upregulation of inflammatory and viral mRNA translation associated pathways in Symptomatic as compared with Asymptomatic cases. In severe COVID-19, CD177 a neutrophil marker, was upregulated while interferon stimulated genes (ISGs) were downregulated. Asymptomatic COVID-19 cases displayed upregulation of ISGs and humoral response genes with downregulation of ICAM3 and TLR8. Compared across the COVID-19 disease spectrum, we found type I interferon (IFN) responses to be significantly upregulated (IFNAR2, IRF2BP1, IRF4, MAVS, SAMHD1, TRIM1), or downregulated (SOCS3, IRF2BP2, IRF2BPL) in Asymptomatic as compared with mild and severe COVID-19, with the dysregulation of an increasing number of ISGs associated with progressive disease. These data suggest that initial early responses against SARS-CoV-2 may be effectively controlled by ISGs.
