Stent fracture after endovascular treatment patient with subclavian vein thrombosis – difficult diagnosis and complication of venous thoracic outlet syndrome
Subclavian vein thrombosis is relatively rare. It constitutes 4 to 10% of all cases of venous thrombosis and is often associated with compression or canniulation of the subclavian vein. We present the case of patient with Paget-Schroetter syndrome treated in our center for right subclavian vein thrombosis as a result of the venous thoracic outlet syndrome (VTOS). In the case described below, VTOS was unrecognized and the stent fracture following endovascular treatment led to a relapse of venous thrombosis. Secondary angioplasty and stenting with subclavian vein decompression was successfully made. The patient was regularly monitored throughout the following year and there was no relapse of clinical symptoms. Conclusions: in case of subclavian vein thrombosis decompression is a very important step in the management of VTOS. It prevents recurrence of thrombosis and potential complications
Risk factors for surgical site infections in rectal cancer patients
Introduction. Rectal cancer surgery is associated with high rates of post-operative complications in which the most common are Surgical Site Infections (SSI).
Materials and methods. Factors responsible for SSI incidence were retrospectively analysed in rectal cancer patients who had undergone surgery at a single centre for oncology between July 2012 and July 2016. The study end-point was patients’ contracting SSI. Statistical analyses were performed by the ‘Statistica 12’ package consisting of the Pearson’s Chi-squared test (χ2), the Kruskal-Wallis test and the Mann-Whitney U-test (with continuity correction). Qualitative variables were analysed using log-linear analysis. The likelihoods of SSI incidence were compared by using odds ratios within 95% confidence limits.
Results. Amongst the 187 patient subjects under observation during their 30 day follow-up, 44 (23.5%) suffered from post-operative complications of which SSI were the most common at 27 (14.4%). In those patients with advanced stage IV cancer, SSI occurred 3-fold more compared to patients with lower stage cancers; respectively 27.3% vs 11.7%, p = 0.021. Multivariate analysis demonstrated that the highest SSI risk was in patients having a low-lying tumour (≤ 5 cm from the anal rim; OR 2.31 (95% CI of 1.15 to 4.62), p = 0.019 and those patients who had undergone Hartmann’s procedure; OR 1.85 (95% CI of 1.04 to 3.31), p = 0.038.
Conclusions. Surgical site infections in rectal cancer patients undergoing surgery occur significantly more in those at advanced stage IV rectal cancer where the tumour is low-lying (0–5 cm from the anal rim) and after having undergone the Hartmann’s procedure
Recurrent carotid artery stenoses
Jednym z poważniejszych powikłań po udrożnieniach tętnic szyjnych są ich nawrotowe zwężenia. Ze względu
na przebieg kliniczny oraz obraz histologiczny miejsca po operowanej tętnicy szyjnej wyróżnia się zwężenia
nawrotowe wczesne i późne.
Mianem restenozy wczesnej określa się zwężenie, które powstaje w ciągu pierwszych 2 lat po zabiegu operacyjnym.
Podłożem nawrotu zwężenia w tym okresie jest intymalna hiperplazja. Zwężenie to charakteryzuje
się niskim ryzykiem powikłań neurologicznych.
Zwężenia późne są to zmiany powstające w późniejszym okresie po zabiegu. W badaniach patomorfologicznych
nie różnią się one od zmian pierwotnych. Również ryzyko powikłań neurologicznych jest podobne jak
w przypadku zwężeń pierwotnych. Konieczność reoperacji zachodzi w razie zwężeń zagrażających okluzją
tętnicy szyjnej wewnętrznej lub w zmianach powodujących objawy neurologiczne. Wyniki odległe powtórnych
zabiegów operacyjnych są podobne do operacji pierwotnych, jednak istnieje większe ryzyko powikłań
śród- i pooperacyjnych. Nadal jednoznacznie nie określono roli zabiegów śródnaczyniowych w leczeniu zwężeń
pierwotnych i wtórnych.Recurrent stenoses belong to the gravest complications after interventions of patency restoration of carotid
arteries. According to the clinical course and the histological picture of the arterial site after intervention,
recurrent stenoses are classified as either early or late.
The term of early restenosis refers to arterial narrowing which occurs within the first two years following the
procedure. In this period, recurrence of stenosis is caused by intimal hyperplasia. This type of stenosis bears
a low risk of neurological complications.
Late stenoses occur in the later period after the initial procedure. They do not differ from the primary lesions
on pathomorphological grounds, and their risk of causing neurological complications is similar to that of the
primary arterial narrowings. Stenoses bearing high risk of complete occlusion of carotid artery or provoking
neurological symptoms necessitate reoperation. Although long-term results of repeated operations are similar
to those of primary interventions, higher risk of intra- and postoperative complications should be taken into
account. The role of endovascular interventions in primary and secondary arterial stenoses remains not
unequivocally determined
Skin microcirculation in the diabetic type 2 patients
Wstęp. Zmiany w obrębie mikrokrążenia leżą u podstaw powikłań spotykanych
u osób z cukrzycą. Celem pracy była ocena mikrokrążenia skórnego u chorych na
cukrzycę typu 2 za pomocą dopplerowskiego przepływomierza laserowego (LDF,laser
doppler flowmeter).
Materiał i metody. Zbadano 60 osób (109 kończyn). Grupę chorych stanowiło
35 osób (65 przebadanych kończyn) w wieku 30–92 lat, ze średnią wieku 62,3 ±
12,6 lat, leczonych z powodu cukrzycy typu 2 (DM, diabetes mellitus) za pomocą
insuliny lub leków doustnych. Grupę kontrolną tworzyło 25 osób zdrowych w wieku
38–76 lat, ze średnią wieku 59,4 ± 10,5 lat (49 przebadanych kończyn). Z badań
wykluczono osoby przyjmujące leki wpływające na mikrokrążenie skórne oraz z
innymi chorobami naczyń kończyn dolnych. Grupę chorych podzielono na podstawie
czasu trwania cukrzycy na 3 grupy (mniej niż 5 lat, 5–10 lat, więcej niż 10
lat). Badania przeprowadzono przy użyciu dopplerowskiego przepływomierza laserowego
Periflux 4001 (Perirmed, Szwecja), umieszczając sondę na grzbietowej stronie
stopy badanej kończyny. Po przeprowadzeniu pomiaru przepływu spoczynkowego (RF,
rest flow), wykonywano kolejno testy prowokacyjne przepływu: test przekrwienia
reaktywnego (RH, reactive hyperemia test), odruchu żylno-tętniczkowego (VAR, veno-arteriolar responde),
test stymulacji termicznej (TSR, thermal stimulation test) w temperaturze 44°C.
Za uzyskane z pomiarów parametry przyznawano określoną liczbę punktów, których
suma stanowiła scoring. Osobnej analizie poddano grupy chorych podzielone na
podstawie czasu trwania cukrzycy. Analizę statystyczną przeprowadzono za pomocą
testu t-Studenta, natomiast zachodzące zależności analizowano, używając testu
korelacji liniowej.
Wyniki. Wielkość perfuzji skóry, mierzona w spoczynku u wszystkich chorych
na cukrzycę, była wyższa (16,9 PU) niż u osób zdrowych (14,9 PU). Nie stwierdzono
znamiennej różnicy w wartościach indeksu mikrokrążenia i RH. Obie grupy różniła
natomiast wartość VAR (średnio 33,6% u chorych na cukrzycę i 54,7% u osób zdrowych)
oraz TSR (średnio 491% u chorych na cukrzycę i 1463% u osób zdrowych). Obliczony
scoring w grupie chorych wyniósł średnio 7,5, a u zdrowych 1,0 (p < 0,001).
Badanie związku pomiędzy czasem trwania cukrzycy a scoringiem wykazało, że istnieje
dodatnia liniowa korelacja między tymi parametrami (r = 0,77).
Wnioski. Przeprowadzone badania wykazały przydatność LDF w ocenie stopnia
mikroangiopatii u chorych we wczesnym okresie choroby. Natomiast perfuzję skóry
u chorych na cukrzycę należałoby oceniać na podstawie kilku parametrów uzyskanych
z testów prowokujących zachowanie się mikrokrążenia.Introduction. Late compilcations of diabetes mellitus are caused by
microangiopathy.The issue of this study was to evaluate skin microcirculation
by means of doppler flowmeter in type 2 diabetic patients.
Material and methods. We examined 60 subjects (109 limbs). The diabetic
group was composed of 35 patients (65 limbs) suffering from type 2 diabetes
mellitus treated by insulin or oral therapy, aged 30 to 92 years. Mean age in
this group was 62.3 ± 12.6 years. Control group was composed of 25 subjects
(49 limbs), aged 38 to 76 years. Mean age was 59.4 ± 10.5 years. Subjects receiving
drugs affecting skin microcirculation or with symptoms of other vascular abnormalities
of lower limbs were excluded. The diabetic group was divided into 3 subgroups
according to duration of diabetes (below 5 years, 5–10 years, above 10 years).
Laser doppler flowmeter Periflux 4001 (Perimed, Sweden) was used in the study.
The probe of LDF was placed on dorsal aspect of foot. Tests affecting microcirculation
were performed after measurement of rest flow (RF) — it means reactive hyperemia
(RH), veno-arteriolar response (VAR) and thermal hyperemia test (TSR) in 44°C.
This parameters were evaluated in points, sum of it formed scoring. Diabetic
group was compared with normals. Previously separated subgroups of diabetics
were analysed additionaly. Statystical analysis was performed with aid of t-Student
and linear correlation tests.
Results. Skin perfusion at rest was higer in diabetic group than in normals
(16.9 vs.14.9 PU). No Significant differences were found in microangiopathy
index and reactive hyperemia test. Diabetics presented significantlydiminished
veno-arteriolar response (33.6% vs. 4.7%) and thermal hyperemia (491 vs. 1463%).
Scoring calculated for diabetics was 7.5 1.0 of normals (p < 0.001). Positive
linear orrelation was found betwween duration of diabetes and mentioned above
parameters (r = 0.77).
Conclusions. Usefulness of laser doppler flowmetry for evaluation of
diabetic microangiopathy in early stage of disease was proved. To improve reliability
of this method test stimulating microcirculation should be used
Hemosyderyna wydalana w moczu jako marker przewlekłych zaburzeń żylnych
Wstęp. Ze względu na dużą powszechność występowania przewlekłe zaburzenia żylne stanowią poważny
problem społeczny krajów rozwiniętych. Istotną rolę w patomechanizmie rozwoju zmian troficznych skóry
w przebiegu niewydolności żylnej odgrywają tkankowe depozyty związków żelaza. Celem badania była ocena
wydalania hemosyderyny w moczu u osób z objawami przewlekłych zaburzeń żylnych.
Materiał i metody. Badano osad pochodzący z moczu, w którym wybarwiano kryształy hemosyderyny za
pomocą roztworu Perla. Przebadano 76 osób, u których niewydolność żylna była w różnych stopniach zaawansowania
klinicznego.
Wyniki. Nie wykazano istnienia związku pomiędzy liczbą wydalanych w moczu kryształów a stopniem
nasilenia patologii układu żylnego. Wnioski. Hemosyderyny nie można traktować jako wyznacznika nasilenia patologii żylnej.Background. Chronic venous insufficiency (CVI) is one of the most significant health problems of developed
countries. Tissue deposits of iron play an important role in the pathogenesis of trophic skin changes. The
purpose of the study was to test the correlation of clinical severity of venous insufficiency with urine excretion
of hemosiderin.
Material and methods. Urine sediments were stained for hemosiderin granules with Perl reagent. The urine
samples of 76 persons representing various categories of venous insufficiency were evaluated.
Results. There was no correlation between clinical category of venous insufficiency and number of hemosiderin
granules noticed in urine.
Conclusions. Thus, hemosiderinuria cannot be a marker of severity of venous disturbances
