    Causes and circumstances of maternal death: A secondary analysis of the community-level Interventions for pre-eclampsia (CLIP) trials cohort

    Background: Incomplete vital registration systems mean that causes of death during pregnancy and childbirth are poorly understood in low-income and middle-income countries. To inform global efforts to reduce maternal mortality, we compared physician review and computerised analysis of verbal autopsies (interpreting verbal autopsies [InterVA] software), to understand their agreement on maternal cause of death and circumstances of mortality categories (COMCATs) in the Community-Level Interventions for Pre-eclampsia (CLIP) cluster randomised trials.Methods: The CLIP trials took place in India, Pakistan, and Mozambique, enrolling pregnant women aged 12-49 years between Nov 1, 2014, and Feb 28, 2017. 69 330 pregnant women were enrolled in 44 clusters (36 008 in the 22 intervention clusters and 33 322 in the 22 control clusters). In this secondary analysis of maternal deaths in CLIP, we included women who died in any of the 22 intervention clusters or 22 control clusters. Trained staff administered the WHO 2012 verbal autopsy after maternal deaths. Two physicians (and a third for consensus, if needed) reviewed trial surveillance data and verbal autopsies, and, in intervention clusters, community health worker-led visit data. They determined cause of death according to the WHO International Classification of Diseases-Maternal Mortality (ICD-MM). Verbal autopsies were also analysed by InterVA computer models (versions 4 and 5) to generate cause of death. COMCAT analysis was provided by InterVA-5 and, in India, by physician review of Maternal Newborn Health Registry data. Causes of death and COMCATs assigned by physician review, Inter-VA-4, and InterVA-5 were compared, with agreement assessed with Cohen\u27s κ coefficient.Findings: Of 61 988 pregnancies with successful follow-up in the CLIP trials, 143 maternal deaths were reported (16 deaths in India, 105 in Pakistan, and 22 in Mozambique). The maternal death rate was 231 (95% CI 193-268) per 100 000 identified pregnancies. Most deaths were attributed to direct maternal causes (rather than indirect or undetermined causes as per ICD-MM classification), with fair to good agreement between physician review and InterVA-4 (κ=0·56 [95% CI 0·43-0·66]) or InterVA-5 (κ=0·44 [0·30-0·57]), and InterVA-4 and InterVA-5 (κ=0·72 [0·60-0·84]). The top three causes of death were the same by physician review, InterVA-4, and InterVA-5 (ICD-MM categories obstetric haemorrhage, non-obstetric complications, and hypertensive disorders); however, attribution of individual patient deaths to obstetric haemorrhage varied more between methods (physician review, 38 [27%] deaths; InterVA-4, 69 [48%] deaths; and InterVA-5, 82 [57%] deaths), than did attribution to non-obstetric causes (physician review, 39 [27%] deaths; InterVA-4, 37 [26%] deaths; and InterVA-5, 28 [20%] deaths) or hypertensive disorders (physician review, 23 [16%] deaths; InterVA-4, 25 [17%] deaths; and InterVA-5, 24 [17%] deaths). Agreement for all nine ICD-MM categories was fair for physician review versus InterVA-4 (κ=0·48 [0·38-0·58]), poor for physician review versus InterVA-5 (κ=0·36 [0·27-0·46]), and good for InterVA-4 versus InterVA-5 (κ=0·69 [0·59-0·79]). The most commonly assigned COMCATs by InterVA-5 were emergencies (68 [48%] of 143 deaths) and health systems (62 [43%] deaths), and by physician review (India only) were health systems (seven [44%] of 16 deaths) and inevitability (five [31%] deaths); agreement between InterVA-5 and physician review (India data only) was poor (κ=0·04 [0·00-0·15]).Interpretation: Our findings indicate that InterVA-5 is less accurate than InterVA-4 at ascertaining causes and circumstances of maternal death, when compared with physician review. Our results suggest a need to improve the next iteration of InterVA, and for researchers and clinicians to preferentially use InterVA-4 when recording maternal deaths.Funding: University of British Columbia (grantee of the Bill & Melinda Gates Foundation)

    Prediction of Pouch of Douglas obliteration : Point-of-care ultrasound vs. pelvic examination

    Study Objective: To evaluate point-of-care pre-operative transvaginal ultrasound (TVUS) sliding sign, in comparison to palpation of a nodule on digital pelvic examination, for the prediction of pouch of Douglas (POD) obliteration. Design: Analysis of data from a prospective data registry (Canadian Task Force Classification II-2). Setting: Tertiary referral center. Patients: Women with suspected endometriosis who had pre-operative pelvic examination and point-of-care TVUS, followed by laparoscopic surgery between August 2015 and December 2016. Intervention: Women were pre-operatively assessed for prediction of POD obliteration with pelvic examination for a nodule and point-of-care TVUS uterine-cervix sliding sign. Measurements and Main Results: The study included 269 women, of which 15.2% (41/269) had POD obliteration at the time of surgery. A pre-operative negative sliding sign had a sensitivity of 73.2% (95% CI: 57.1-85.8%) and specificity of 93.9% (95% CI: 89.9-96.6%) in prediction of POD obliteration, compared to pre-operative palpation of a nodule on pelvic examination which had a sensitivity of 24.4% (95% CI: 12.4-40.3%) and specificity of 93.4% (95% CI: 89.4-96.3%). The difference in sensitivity was statistically significant (McNemar test, p < .001). A negative sliding sign was also associated with longer operating times and more difficult surgery including need for ureterolysis. Conclusion: Point-of-care TVUS sliding sign demonstrated significantly improved sensitivity compared to palpation of a nodule on pelvic examination for prediction of POD obliteration.     A quantitative analysis of sleep quality in women with endometriosis

    Background: Endometriosis is a complex condition that can negatively affect a woman’s quality of life, including her sleep. This study aims to assess the multifactorial variables associated with poorer quality of sleep in women with endometriosis. Methods: Data from the Endometriosis Pelvic Pain Interdisciplinary Cohort (EPPIC) data registry were analyzed for women who underwent surgery at the BC Women’s Center for Pelvic Pain and Endometriosis with histopathological confirmation of endometriosis (June 2015 to June 2017). The primary outcome was quality of sleep pre-operatively, from the Chronic Pain Sleep Inventory. Bivariate analysis and multivariable linear regression were done to determine any significant associations between pre-operative patient variables and overall quality of sleep, based on p-value of 0.05. Results: Two hundred and seventy-five women met the study criteria. Poorer overall quality of sleep was independently associated with poorer functional quality-of-life (EHP- 30)(b = -.18, p = .0026), more depressive symptoms (PHQ-9) (b = -1.62, p < .001), and painful bladder syndrome (b = -5.82, p = .035). This indicates that a 1 point increase in the EHP-30 (worsening quality-of-life), a 1 point increase in the PHQ-9 (worsening depression) and the presence of painful bladder syndrome, increased the primary outcome (i.e. towards poorer quality of sleep) by 0.18, 1.62, and 5.82 points. Conclusion: Poorer quality of sleep in women with endometriosis is associated with poorer quality-of-life, more depressive symptoms, and bladder pain.     Chronic pelvic pain in an interdisciplinary setting : 1 year prospective cohort

    Background: Chronic pelvic pain affects ~15% of women, and presents a challenging problem for gynecologists due to its complex etiology involving multiple comorbidities. Thus an interdisciplinary approach has been proposed for chronic pelvic pain, where these multifactorial comorbidities can be addressed by different interventions at a single integrated center. Moreover, while cross-sectional studies can provide some insight into the association between these comorbidities and chronic pelvic pain severity, prospective longitudinal cohorts can identify comorbidities that are associated with changes in chronic pelvic pain severity over time. Objective: To describe trends and factors associated with chronic pelvic pain severity over a 1 year prospective cohort at an interdisciplinary center, with a focus on the role of comorbidities and controlling for baseline pain, demographic factors, and treatment effects. Methods: Prospective 1 year cohort study at an interdisciplinary tertiary referral center for pelvic pain and endometriosis, which provides minimally invasive surgery, medical management, pain education, physiotherapy, and psychological therapies. Exclusion criteria included menopause or age>50. Sample size was 296 (57% response rate at 1 year; 296/525). Primary outcome was CPP severity at 1 year on a 11-point numeric rating scale (0-10), which was categorized for ordinal regression (none-mild 0-3, moderate 4-6, severe 7-10). Secondary outcomes included functional quality-of-life and health utilization. Baseline comorbidities were endometriosis, irritable bowel syndrome, painful bladder syndrome, abdominal wall pain, pelvic floor myalgia, and validated questionnaires for depression, anxiety, and catastrophizing. Multivariable ordinal regression was used to identify baseline comorbidities associated with the primary outcome at 1 year. Results: Chronic pelvic pain severity decreased by a median 2 points from baseline to 1 year (6/10 to 4/10, p<0.001). There was also an improvement in functional quality-of-life (42% to 29% on the pain subscale of the Endometriosis Health Profile-30, p<0.001), and a reduction in subjects requiring a physician visit (73% to 36%, p<0.001) or emergency visit (24% to 11%, p<0.001) in the last 3 months. On multivariable ordinal regression for the primary outcome, chronic pelvic pain severity at 1 year was independently associated with a higher score on the Pain Catastrophizing Scale at baseline (OR=1.10, 95% CI=1.00-1.21, p=0.04), controlling for baseline pain, treatment effects (surgery), age and referral status. Conclusion: Improvements in chronic pelvic pain severity, quality-of-life, and health care utilization were observed in a 1 year cohort in an interdisciplinary setting. Higher pain catastrophizing at baseline was associated with greater chronic pelvic pain severity at 1 year.     Prospective cohort of deep dyspareunia in an interdisciplinary setting

    Introduction: Deep dyspareunia is a common symptom in women, including in half of women with endometriosis, but little is known about its response to treatment and predictors of persistent deep dyspareunia over time. Aim: To follow up deep dyspareunia severity over a 1-year prospective cohort at an interdisciplinary center, and to identify baseline predictors of more persistent deep dyspareunia at 1 year. Methods: Prospective 1-year cohort study at a tertiary referral center for pelvic pain and endometriosis, where a range of interdisciplinary treatments are provided at a single center (surgical, hormonal, physical, and psychological therapies). Exclusion criteria were menopause, age >50 years, and never previously sexually active. Primary outcome (deep dyspareunia severity) and secondary outcome (sexual quality of life) were followed up over 1 year. Ordinal logistic regression was performed, controlling for baseline severity of deep dyspareunia, to identify baseline predictors of deep dyspareunia severity at 1 year. Main outcome measure: Primary outcome was severity of deep dyspareunia on an 11-point numeric rating scale (0-10), categorized into absent-mild (0-3), moderate (4-6), and severe (7-10); secondary outcome was sexual quality of life measured by the Endometriosis Health Profile-30. Results: 1-year follow-up was obtained for 278 subjects (56% response rate at 1 year; 278/497). Severity of deep dyspareunia improved over the 1 year (McNemar test, P < .0001): the proportion of patients in the severe category decreased from 55.0% to 30.4%, the moderate category remained similar from 17.7% to 25.0%, and the absent-mild category increased from 27.3% to 44.6%. Sexual quality of life also improved (56% to 43% on the sex subscale of the Endometriosis Health Profile-30) (Welch t test, P < .001). On ordinal regression, severity of deep dyspareunia at 1 year was independently associated with younger age (OR = 0.94, 95% CI = 0.91-0.97, P = .008), and with a higher baseline depression score on the Patient Health Questionnaire-9 (OR = 1.07, 95% CI = 1.03-1.11, P = .01). Clinical implications: Clinicians should consider employing an interdisciplinary approach for deep dyspareunia, and screening for and treating depression symptoms in these women. Strength & limitations: Strengths of the study include its prospective nature, and assessment of deep dyspareunia specifically (as opposed to superficial dyspareunia). Limitations include non-randomized design, and the patients lost to follow-up over the 1 year.
