Species differences in the pattern of eicosanoids produced by inflamed and non-inflamed tissue
The synthesis of14C labelled arachidonic acid metabolites was measured in colonic tissues obtained from mice, rats, guinea pigs, rabbits, piglets and in colonic biopsies from humans during colonoscopy. The main eicosanoids formed after stimulation with calcium ionophore A23187 were: in humans, 15-hydroxy-eicosatetraenoic acid (15-HETE); in mice, 12-HETE; in rats, 12-HETE, 12-hydroxy-5,8,10-heptadecatrienoic acid (HHT) and 6-keto-prostaglandine F1α (6kPGF1α); in guinea pigs, PGD2; in rabbits, 6kPGF1α, PGE2 and 15-HETE; and in pigs PGE2 and 12-HETE. In inflamed 15-HETE production was increased in man, HHT and 12-HETE production in rats and overall eicosanoid production in mice

Estimating infarct severity from the ECG using a realistic heart model
The early phase of myocardial infarction is accompanied by changes in the ST segment of the ECG. This makes the ST segment the clinical marker for the detection of acute myocardial infarction. The determination of the infarct severity, location and size of the myocardial tissue at risk will support clinical decision making. In this study we used an inverse procedure to estimate the location and size of the infarcted heart region. The method estimates the local transmembrane amplitude based on the ECG amplitude near the J-point of the standard 12 leads signals using a patient specific volume conductor model. For the 5 available patient cases the positions as well as the size of the estimated infarct region were in accordance with results based on MRI
Validation of infarct size and location from the ECG by inverse body surface mapping
This paper describes the incorporation of body surface mapping algorithms to detect the position and size of acute myocardial infarctions using standard 12 lead ECG recording. The results are compared with the results from cardiac MRI scan analysis. In case patient specific volume conductor models are used, the position of the infarction could be accurately determined. When generalized patient volume conductor models were examined, the estimation of the infarct position became significantly less accurate. The calculations of the size of the infarctions need further improvement
Production of inflammatory mediators by human macrophages obtained from ascites
Ascites is a readily available source of human macrophages (Mø), which can be used to study Mø functions in vitro. We characterized the mediators of inflammation produced by human peritoneal Mø (hp-Mø) obtained from patients with portal hypertension and ascites.
The production of the cytokines interleukin-1β (IL-1β), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) was found to be lipopolysaccharide (LPS) concentration dependent (0–10 μg/ml) with a maximal production at 10 μg/ml and also dependent on the time of exposure to the stimulus (0–36 h). IL-1β, IL-6 and TNF-α production after LPS administration reached a plateau at 24 h.
In vitro stimulation for 24 h with LPS does not influence the eicosanoid production from endogenous arachidonate. 13 min of exposure of the cells to the calcium ionophore A23187 gives a significant increase in eicosanoid production from both exogenous and endogenous arachidonate. The main eicosanoids produced are the 5-lipoxgenase products LTB4 and 5-hydroxyeicosatetraenoic acid (HETE). The increase in production of the other eicosanoids is not significant. The eicosanoid production depends on the stimulus concentration. The optimal A23187 concentration is 1 μM.
Oxygen radical production was measured in the Mø by a flowcytometric method. The fluorescence intensity of phorbol 12-myristate 13-acetate stimulated and dihydro-rhodamine 123 loaded. hp-Mø increases significantly after 15 min.
We conclude that LPS stimulation of hp-Mø from liver disease results in similar production of IL-1β, IL-6 and TNF-α, but that the profile of the eicosanoid production of these Mø stimulated with LPS and A23187 differs from Mø of other origin and species
Left ventricular deformation and myocardial fibrosis in pediatric patients with Duchenne muscular dystrophy
Background: Left ventricular (LV) strain and rotation are emerging functional markers for early detection of LV dysfunction and have been associated with the burden of myocardial fibrosis in several disease states. This study examined the association between LV deformation (i.e., LV strain and rotation) and extent and location of LV myocardial fibrosis in pediatric patients with Duchenne muscular dystrophy (DMD). Methods and results: 34 pediatric patients with DMD underwent cardiovascular magnetic resonance (CMR) with late gadolinium enhancement (LGE) to assess LV myocardial fibrosis. Offline CMR feature-tracking analysis was used to assess global and segmental longitudinal and circumferential LV strain, and LV rotation. Patients with fibrosis (n = 18, 52.9%) were older than those without fibrosis (14 ± 3 years (yrs) vs 11 ± 2 yrs., p = 0.01). There was no significant difference in LV ejection fraction (LVEF) between subjects with and without fibrosis (54 ± 6% vs 56 ± 4%, p = 0.18). However, lower endocardial global circumferential strain (GCS), but not LV rotation, was associated with presence of fibrosis (adjusted Odds Ratio 1.25 [95% CI 1.01–1.56], p = 0.04). Both GCS and global longitudinal strain correlated with the extent of fibrosis (r =.52, p = 0.03 and r =.75, p < 0.01, respectively). Importantly, segmental strain did not seem to correspond to location of fibrosis. Conclusion: A lower global, but not segmental, strain is associated with presence and extent of LV myocardial fibrosis in pediatric DMD patients. Therefore, strain parameters might detect structural myocardial alterations, however currently more research is needed to evaluate its value (e.g., prognostic) in clinical practice.</p

To monitor alterations in the pneumococcal population structure, we have developed and utilized Capsular Sequence Typing (CST) in addition to Multiple-Locus Variable number tandem repeat Analysis (MLVA)
Stent placement for renal arterial stenosis: where do we stand? A meta-analysis
PURPOSE: To perform a meta-analysis of renal arterial stent placement in
comparison with renal percutaneous transluminal angioplasty (PTA) in
patients with renal arterial stenosis. MATERIALS AND METHODS: Studies
dealing with renal arterial stent placement (14 articles; 678 patients)
and renal PTA (10 articles; 644 patients) published up to August 1998 were
selected. A random-effects model was used to pool the data. RESULTS: Renal
arterial stent placement proved highly successful, with an initial
adequate performance in 98% and major complications in 11%. The overall
cure rate for hypertension was 20%, whereas hypertension was improved in
49%. Renal function improved in 30% and stabilized in 38% of patients. The
restenosis rate at follow-up of 6-29 months was 17%. Stent placement had a
higher technical success rate and a lower restenosis rate than did renal
PTA (98% vs 77% and 17% vs 26%, respectively; P <.001). The complication
rate was not different between the two treatments. The cure rate for
hypertension was higher and the improvement rate for renal function was
lower after stent placement than after renal PTA (20% vs 10% and 30% vs
38%, respectively; P <.001). CONCLUSION: Renal arterial stent placement is
technically superior and clinically comparable to renal PTA alone
