High-flow nasal cannula on diaphragm
Background : Diaphragm dysfunction is a serious problem. However, a few management techniques exist for diaphragm dysfunction. Methods : Adult patients treated with high-flow nasal cannula (HFNC) in the intensive care unit were included in this study. The diaphragm function was evaluated using ultrasound measurement of thickening fraction before and after HFNC liberation. Normal diaphragm contraction was defined as thickening fraction ≥ 15% without HFNC, whereas decreased or paradoxical diaphragm contractions were 0%–15% or < 0%, respectively. Results : Forty patients were enrolled, and 16 (40%) had normal diaphragm contraction, whereas 19 (48%) or 5 (13%) had decreased or paradoxical diaphragm contractions, respectively. Thickening fraction increased after HFNC liberation (27.0% ± 25.7% vs. 38.8% ± 34.5%, p = 0.03 in HFNC vs. no HFNC) in patients without diaphragm dysfunction. In patients with decreased diaphragm contraction, thickening fraction did not change with or without HFNC (8.9% ± 11.7% vs. 6.7% ± 5.2%, p = 0.35), whereas paradoxical contraction decreased with HFNC (1.0% ± 10.2% vs. –10.3% ± 2.7%, p = 0.04) in patients with paradoxical diaphragm contraction. Conclusions : The work of breathing decreased with HFNC in patients without diaphragm dysfunction, but did not decrease in patients with decreased diaphragm contraction. Confectionery intake and serum phosphorus
Hyperphosphatemia is associated with an increased risk of developing cardiovascular disease. Recently, it has been shown that high serum phosphorus levels are associated with increased cardiovascular events in healthy subjects, but the dietary factors determining serum phosphorus level have not been fully investigated. The study investigated the influence of habitual dietary factors on serum phosphorus levels in healthy young participants. This cross-sectional study conducted fasting blood sampling in 109 healthy young people and used a brief-type self-administered diet history questionnaire to evaluate the subject's habitual meals. Since the bioavailability of dietary phosphorus depends on the food sources, habitual phosphorus intakes from different food groups and intake frequency (times/month) of processed foods were calculated. The mean serum phosphorus level was 3.9±0.5 mg/dl ; 10.1% of the subjects had serum phosphorus levels that exceeded the reference levels of ≤4.5 mg/dl. Total phosphorus intake and phosphorus intake from animal-based food did not differ between serum phosphorus quartiles. Higher intake of confectionery was associated with increased serum phosphorus levels. This study showed that frequent consumption of confectionery was associated with elevated serum phosphorus levels. Additional studies are needed to determine whether this is a causal relationship
Detection of Polarimetric Variations Associated with the Shortest Time-Scale Variability in S5 0716714
We present the result of near-infrared and optical observations of the BL Lac
object S5 0716714 carried out by the KANATA telescope. S5 0716714 has
both a long term high-amplitude variability and a short-term variability within
a night. The shortest variability (microvariability) time-scale is important
for understanding the geometry of jets and magnetic field, because it provides
a possible minimum size of variation sources. Here, we report the detection of
15-min variability in S5 0716714, which is one of the shortest time-scales
in optical and near-infrared variations observed in blazars. The detected
microvariation had an amplitude of mag in band and a blue
color of . Furthermore, we successfully detected
an unprecedented, short time-scale polarimetric variation which correlated with
the brightness change. We revealed that the microvariation had a specific
polarization component. The polarization degree of the variation component was
higher than that of the total flux. These results suggest that the
microvariability originated from a small and local region where the magnetic

Dwarf Novae in the Shortest Orbital Period Regime: I. A New Short Period Dwarf Nova, OT J055717+683226
We report the observation of a new dwarf nova, OT J055717+683226, during its
first-ever recorded superoutburst in December 2006. Our observation shows that
this object is an SU UMa-type dwarf nova having a very short superhump period
of 76.67+/- 0.03 min (0.05324+/-0.00002 d). The next superoutburst was observed
in March 2008. The recurrence time of superoutbursts (supercycle) is, hence,
estimated to be ~480 d. The supercycle is much shorter than those of WZ
Sge-type dwarf novae having supercycles of >~ 10 yr, which are a major
population of dwarf novae in the shortest orbital period regime (<~85 min).
Using a hierarchical cluster analysis, we identified seven groups of dwarf
novae in the shortest orbital period regime. We identified a small group of
objects that have short supercycles, small outburst amplitudes, and large
superhump period excesses, compared with those of WZ Sge stars. OT

Optical and Near-Infrared Photometry of Nova V2362 Cyg : Rebrightening Event and Dust Formation
We present optical and near-infrared (NIR) photometry of a classical nova,
V2362 Cyg (= Nova Cygni 2006). V2362 Cyg experienced a peculiar rebrightening
with a long duration from 100 to 240 d after the maximum of the nova. Our
multicolor observation indicates an emergence of a pseudophotosphere with an
effective temperature of 9000 K at the rebrightening maximum. After the
rebrightening maximum, the object showed a slow fading homogeneously in all of
the used bands for one week. This implies that the fading just after the
rebrightening maximum ( less or equal 1 week ) was caused by a slowly shrinking
pseudophotosphere. Then, the NIR flux drastically increased, while the optical
flux steeply declined. The optical and NIR flux was consistent with blackbody
radiation with a temperature of 1500 K during this NIR rising phase. These
facts are likely to be explained by dust formation in the nova ejecta. Assuming
an optically thin case, we estimate the dust mass of 10^(-8) -- 10^(-10)
M_solar, which is less than those in typical dust-forming novae. These results
support the senario that a second, long-lasting outflow, which caused the
rebrightening, interacted with a fraction of the initial outflow and formed

Copy Number Variations Due to Large Genomic Deletion in X-Linked Chronic Granulomatous Disease
Mutations in genes for any of the six subunits of NADPH oxidase cause chronic granulomatous disease (CGD), but almost 2/3 of CGD cases are caused by mutations in the X-linked CYBB gene, also known as NAD (P) H oxidase 2. Approximately 260 patients with CGD have been reported in Japan, of whom 92 were shown to have mutations of the CYBB gene and 16 to have chromosomal deletions. However, there has been very little detailed analysis of the range of the deletion or close understanding of the disease based on this. We therefore analyzed genomic rearrangements in X-linked CGD using array comparative genomic hybridization analysis, revealing the extent and the types of the deletion genes. The subjects were five Japanese X-linked CGD patients estimated to have large base deletions of 1 kb or more in the CYBB gene (four male patients, one female patient) and the mothers of four of those patients. The five Japanese patients were found to range from a patient exhibiting deletions only of the CYBB gene to a female patient exhibiting an extensive DNA deletion and the DMD and CGD phenotype manifested. Of the other three patients, two exhibited CYBB, XK, and DYNLT3 gene deletions. The remaining patient exhibited both a deletion encompassing DNA subsequent to the CYBB region following intron 2 and the DYNLT3 gene and a complex copy number variation involving the insertion of an inverted duplication of a region from the centromere side of DYNLT3 into the deleted region
The 2006 November outburst of EG Aquarii: the SU UMa nature revealed
We report time-resolved CCD photometry of the cataclysmic variable EG Aquarii
during the 2006 November outburst During the outburst, superhumps were
unambiguously detected with a mean period of 0.078828(6) days, firstly
classifying the object as an SU UMa-type dwarf nova. It also turned out that
the outburst contained a precursor. At the end of the precursor, immature
profiles of humps were observed. By a phase analysis of these humps, we
interpreted the features as superhumps. This is the second example that the
superhumps were shown during a precursor. Near the maximum stage of the
outburst, we discovered an abrupt shift of the superhump period by
0.002 days. After the supermaximum, the superhump period decreased at the rate
of =, which is typical for SU UMa-type dwarf
novae. Although the outburst light curve was characteristic of SU UMa-type
dwarf novae, long-term monitoring of the variable shows no outbursts over the
past decade. We note on the basic properties of long period and inactive SU
