    Tratamiento de la hipertensión arterial en la menopausia

    Introducción: la hipertensión arterial en la menopausia está influencia por el descenso de estrógenos que induce disfunción endotelial y aumento del índice de masa corporal. Objetivos: describir los mecanismos fisiopatológicos de la hipertensión arterial en la menopausia y los antihipertensivos recomendados en la actualidad. Metodología: se realizó una revisión bibliográfica en las bases de datos PubMed y SciELO utilizando las palabras claves: hipertensión arterial y menopausia. Resultados: no existen diferencias en la respuesta a los diferentes antihipertensivos y ambos sexos se benefician en la reducción de la morbimortalidad cardiovascular con la disminución de la presión arterial. La efectividad preventiva de complicaciones cardiovasculares con los antihipertensivos no difiere según el sexo y por lo tanto la elección del medicamento no puede basarse en este criterio en mujeres menopáusicas. Conclusión: la deficiencia de estrógeno induce disfunción endotelial y aumento del índice de masa corporal siendo las principales causas de hipertensión en la menopausia. Todos los antihipertensivos están recomendados en la menopausia

    Time of arrival through interacting environments: Tunneling processes

    We discuss the propagation of wave packets through interacting environments. Such environments generally modify the dispersion relation or shape of the wave function. To study such effects in detail, we define the distribution function P_{X}(T), which describes the arrival time T of a packet at a detector located at point X. We calculate P_{X}(T) for wave packets traveling through a tunneling barrier and find that our results actually explain recent experiments. We compare our results with Nelson's stochastic interpretation of quantum mechanics and resolve a paradox previously apparent in Nelson's viewpoint about the tunneling time.Comment: Latex 19 pages, 11 eps figures, title modified, comments and references added, final versio

    Percepción de imagen corporal como factor condicionante del índice de masa corporal en médicos residentes del Hospital de Clínicas

    Uno de los factores condicionantes del peso corporal es la percepción que la persona tiene de su imagen corporal. El objetivo fue conocer la percepción de imagen corporal según el índice de masa corporal (IMC) en 107 Médicos Residentes del Hospital de Clínicas.Se utilizó un diseño observacional descriptivo de corte transverso. Se exploraron las variables edad, sexo, circunferencia abdominal, peso, talla, IMC, percepción del peso corporal. La circunferencia abdominal fue de riesgo elevado en 37% de los varones y en 16% de las mujeres, de riesgo muy elevado en 13% de varones y en 29% de las mujeres (p=0,009). En cuanto al IMC la media fue de 25,7±4,6; estando en peso normal el 45% y en sobrepeso el 43%. Se encontró más mujeres con peso normal (63% vs 28%). Los varones presentan más sobrepeso (61% vs 28%) (p=0,0009). En cuanto a la percepción del peso corporal se perciben con obesidad 23%; con sobrepeso 47% y con peso normal 28%. Cruzando los datos con los IMC verdaderos, tenemos que el 43% no tiene la percepción real de acuerdo a su IMC. La concordancia en cuanto a IMC real y la percepción es del 63%. Casi la mitad de los residentes estudiados no tiene la percepción real de acuerdo a su IMC. La percepción de obesidad es mayor en mujeres que en hombres acorde a su IMC real. Se requiere de más investigaciones en referencia al tema aplicándolo a toda la población del hospital para crear medidas educativas de oportuna intervención en la salud del personal sanitario

    Applying the Maternal Near Miss Approach for the Evaluation of Quality of Obstetric Care: A Worked Example from a Multicenter Surveillance Study

    Objective. To assess quality of care of women with severe maternal morbidity and to identify associated factors. Method. This is a national multicenter cross-sectional study performing surveillance for severe maternal morbidity, using the World Health Organization criteria. the expected number of maternal deaths was calculated with the maternal severity index (MSI) based on the severity of complication, and the standardized mortality ratio (SMR) for each center was estimated. Analyses on the adequacy of care were performed. Results. 17 hospitals were classified as providing adequate and 10 as nonadequate care. Besides almost twofold increase in maternal mortality ratio, the main factors associated with nonadequate performance were geographic difficulty in accessing health services (P < 0.001), delays related to quality of medical care (P = 0.012), absence of blood derivatives (P = 0.013), difficulties of communication between health services (P = 0.004), and any delay during the whole process (P = 0.039). Conclusions.     Depicting the tree of life in museums: guiding principles from psychological research

    The Tree of Life is revolutionizing our understanding of life on Earth, and, accordingly, evolutionary trees are increasingly important parts of exhibits on biodiversity and evolution. The authors argue that in using these trees to effectively communicate evolutionary principles, museums need to take into account research results from cognitive, developmental, and educational psychology while maintaining a focus on visitor engagement and enjoyment. Six guiding principles for depicting evolutionary trees in museum exhibits distilled from this research literature were used to evaluate five current or recent museum trees. One of the trees was then redesigned in light of the research while preserving the exhibit’s original learning goals. By attending both to traditional factors that influence museum exhibit design and to psychological research on how people understand diagrams in general and Tree of Life graphics in particular, museums can play a key role in fostering 21st century scientific literacy

    The Fourteenth Data Release of the Sloan Digital Sky Survey: First Spectroscopic Data from the extended Baryon Oscillation Spectroscopic Survey and from the second phase of the Apache Point Observatory Galactic Evolution Experiment

    The fourth generation of the Sloan Digital Sky Survey (SDSS-IV) has been in operation since July 2014. This paper describes the second data release from this phase, and the fourteenth from SDSS overall (making this, Data Release Fourteen or DR14). This release makes public data taken by SDSS-IV in its first two years of operation (July 2014-2016). Like all previous SDSS releases, DR14 is cumulative, including the most recent reductions and calibrations of all data taken by SDSS since the first phase began operations in 2000. New in DR14 is the first public release of data from the extended Baryon Oscillation Spectroscopic Survey (eBOSS); the first data from the second phase of the Apache Point Observatory (APO) Galactic Evolution Experiment (APOGEE-2), including stellar parameter estimates from an innovative data driven machine learning algorithm known as "The Cannon"; and almost twice as many data cubes from the Mapping Nearby Galaxies at APO (MaNGA) survey as were in the previous release (N = 2812 in total). This paper describes the location and format of the publicly available data from SDSS-IV surveys. We provide references to the important technical papers describing how these data have been taken (both targeting and observation details) and processed for scientific use. The SDSS website (www.sdss.org) has been updated for this release, and provides links to data downloads, as well as tutorials and examples of data use. SDSS-IV is planning to continue to collect astronomical data until 2020, and will be followed by SDSS-V.Comment: SDSS-IV collaboration alphabetical author data release paper.     Skin color and severe maternal outcomes: evidence from the brazilian network for surveillance of severe maternal morbidity

    Taking into account the probable role that race/skin color may have for determining outcomes in maternal health, the objective of this study was to assess whether maternal race/skin color is a predictor of severe maternal morbidity. This is a secondary analysis of the Brazilian Network for Surveillance of Severe Maternal Morbidity, a national multicenter cross-sectional study of 27 Brazilian referral maternity hospitals. A prospective surveillance was performed to identify cases of maternal death (MD), maternal near miss (MNM) events, and potentially life-threatening conditions (PLTC), according to standard WHO definition and criteria. Among 9,555 women with severe maternal morbidity, data on race/skin color was available for 7,139 women, who were further divided into two groups: 4,108 nonwhite women (2,253 black and 1,855 from other races/skin color) and 3,031 white women. Indicators of severe maternal morbidity according to WHO definition are shown by skin color group. Adjusted Prevalence Ratios (PRadj - 95%CI) for Severe Maternal Outcome (SMO=MNM+MD) were estimated according to sociodemographic/obstetric characteristics, pregnancy outcomes, and perinatal results considering race. Results. Among 7,139 women with severe maternal morbidity evaluated, 90.5% were classified as PLTC, 8.5% as MNM, and 1.6% as MD. There was a significantly higher prevalence of MNM and MD among white women. MNMR (maternal near miss ratio) was 9.37 per thousand live births (LB). SMOR (severe maternal outcome ratio) was 11.08 per 1000 LB, and MMR (maternal mortality ratio) was 170.4 per 100,000 LB. Maternal mortality to maternal near miss ratio was 1 to 5.2, irrespective of maternal skin color. Hypertension, the main cause of maternal complications, affected mostly nonwhite women. Hemorrhage, the second more common cause of maternal complication, predominated among white women. Nonwhite skin color was associated with a reduced risk of SMO in multivariate analysis. Nonwhite skin color was associated with a lower risk for severe maternal outcomes.     Search for W W/W Z resonance production in ℓνqq final states in pp collisions at √s=13 TeV with the ATLAS detector

    A search is conducted for new resonances decaying into a W W or W Z boson pair, where one W boson decays leptonically and the other W or Z boson decays hadronically. It is based on proton-proton collision data with an integrated luminosity of 36.1 fb −1 collected with the ATLAS detector at the Large Hadron Collider at a centre-of-mass energy of s=13 TeV in 2015 and 2016. The search is sensitive to diboson resonance production via vector-boson fusion as well as quark-antiquark annihilation and gluon-gluon fusion mechanisms. No significant excess of events is observed with respect to the Standard Model backgrounds. Several benchmark models are used to interpret the results. Limits on the production cross section are set for a new narrow scalar resonance, a new heavy vector-boson and a spin-2 Kaluza-Klein graviton.[Figure not available: see fulltext.]
