    Biliary Adenofibroma of the Liver: Report of a Case and Review of the Literature

    We herein report the clinicopathologic features of a rare case of biliary adenofibroma (BAF) of the liver in a 79-year-old man. Grossly, tumour presented as a well-circumscribed, 5.5-cm mass with a solid and microcystic appearance. Histological examination was typical of biliary adenofibroma, showing a proliferation of variable-sized tubulocystic structures embedded in a moderately cellular fibrous stroma. Immunohistochemistry, revealing immunoreactivity of the epithelial component to cytokeratins 7 and 19, was consistent with a bile duct origin. Notably, the stromal cells had a myofibroblastic profile, showing a diffuse and strong expression of vimentin and α-smooth muscle actin. Differential diagnosis with Von Meyenburg complex, biliary adenoma, biliary cistadenoma, congenital biliary cystsy, and hepatic benign cystic mesothelioma is provided. The occasionally reported expression of p53 in biliary adenofibroma has suggested that this tumour could represent a premalignant lesion. The absence of both cytological atypia and p53 immunoreactivity in our case confirms that BAF is a benign tumour with an indolent clinical behaviour. However, a careful histological examination of BAF is mandatory because malignant transformation of the epithelial component has been documented in two cases

    La nécropole de Serra Cicora (Nardò – Lecce, Pouilles, Italie)

    Serra Cicora (Nardò - Lecce) is a neolithic Necropolis dated to the 5th millennium BC, with numerous funeral monuments, among which the burial T7 emerges for its monumentality. It is a drystone structure, whose upper part has fallen down but which likely rose with a truncated cone shape, similarly to the present rural architecture in Salento. The burial contained five individuals, of which the last laid down was a 21-24 years old female, without grave goods. The necropolis is located on the flat top of a hill facing the ionic coast of Salento, in a dominant position on the surrounding landscape. The entire area is surrounded by a wall which made the complex visible from a long distance with a remarkable impact on the landscape. At the center of the area a ritual hut has to be evidenced, around which the burials were located.Serra Cicora (Nardò - Lecce) est une nécropole néolithique (Ve millénaire av. C’est une construction à sec dont la partie supérieure s'est écroulée mais qui devait s'élever en tronc de cône, une forme connue dans l'architecture rurale actuelle du Salento. Sans mobilier, elle contenait cinq sépultures dont la dernière déposée était une femme de 21-24 ans. La nécropole est située sur un plateau surplombant la côte salentine Ionienne, en position dominante par rapport au territoire environnant. En partie ceint d'un mur, ce plateau est visible de loin et l’impact sur le paysage est considérable. En son centre, signalons également une cabane rituelle autour de laquelle étaient disposées les tombes

    Antiangiogenic agents after first line and sorafenib plus chemoembolization: A systematic review

    Transarterial chemoembolization (TACE) is the standard treatment for intermediate stage, although the combination of TACE with sorafenib may theoretically benefit HCC patients in intermediate stage. Owing to the significant antiangiogenic effect of sorafenib and the limitation of TACE, it is rational to combine them. Though the strategy of combining TACE and sorafenib has been increasingly used in patients with unresectable HCC but the current evidence is controversial and its clinical role has not been determined yet. In first-line therapy, patients receiving sorafenib had increased overall survival and progression free survival. Therefore several antiangiogenic agents have entered clinical studies on HCC, many with negative results.     APache Is an AP2-Interacting Protein Involved in Synaptic Vesicle Trafficking and Neuronal Development

    Synaptic transmission is critically dependent on synaptic vesicle (SV) recycling. Although the precise mechanisms of SV retrieval are still debated, it is widely accepted that a fundamental role is played by clathrin-mediated endocytosis, a form of endocytosis that capitalizes on the clathrin/adaptor protein complex 2 (AP2) coat and several accessory factors. Here, we show that the previously uncharacterized protein KIAA1107, predicted by bioinformatics analysis to be involved in the SV cycle, is an AP2-interacting clathrin-endocytosis protein (APache). We found that APache is highly enriched in the CNS and is associated with clathrin-coated vesicles via interaction with AP2. APache-silenced neurons exhibit a severe impairment of maturation at early developmental stages, reduced SV density, enlarged endosome-like structures, and defects in synaptic transmission, consistent with an impaired clathrin/AP2-mediated SV recycling. Our data implicate APache as an actor in the complex regulation of SV trafficking, neuronal development, and synaptic plasticity

    Seismic Reassessment of the Leaning Tower of Pisa:Monitoring, Site Response and SSI

    The Tower of Pisa survived several strong earthquakes undamaged over the last 650 years, despite its leaning and limited strength and ductility. No credible explanation for its remarkable seismic performance exists to date. A reassessment of this unique case history in light of new seismological, geological, structural, and geotechnical information is reported, aiming to address this question. The following topics are discussed: (1) dynamic structural identification based on recorded earthquake data; (2) geophysical site characterization using a two-dimensional array; (3) seismic hazard and site response analysis considering horizontal and vertical motions; and (4) soil-structure interaction (SSI) analysis calibrated using lab and field data. A substantial shift in natural period, from about 0.35 s to over 1 s (a threefold increase, the largest known for a building of that height) caused by SSI, a wave parameter (1∕σ) of about 0.3, and a minor effect of vertical ground motion are identified and may explain the lack of earthquake damage on the Tower. Recommendations for future research, including the need to establish a seismic bedrock deeper than 500 m, are provided

    What is the role of the placebo effect for pain relief in neurorehabilitation? Clinical implications from the Italian consensus conference on pain in neurorehabilitation

    Background: It is increasingly acknowledged that the outcomes of medical treatments are influenced by the context of the clinical encounter through the mechanisms of the placebo effect. The phenomenon of placebo analgesia might be exploited to maximize the efficacy of neurorehabilitation treatments. Since its intensity varies across neurological disorders, the Italian Consensus Conference on Pain in Neurorehabilitation (ICCP) summarized the studies on this field to provide guidance on its use. Methods: A review of the existing reviews and meta-analyses was performed to assess the magnitude of the placebo effect in disorders that may undergo neurorehabilitation treatment. The search was performed on Pubmed using placebo, pain, and the names of neurological disorders as keywords. Methodological quality was assessed using a pre-existing checklist. Data about the magnitude of the placebo effect were extracted from the included reviews and were commented in a narrative form. Results: 11 articles were included in this review. Placebo treatments showed weak effects in central neuropathic pain (pain reduction from 0.44 to 0.66 on a 0-10 scale) and moderate effects in postherpetic neuralgia (1.16), in diabetic peripheral neuropathy (1.45), and in pain associated to HIV (1.82). Moderate effects were also found on pain due to fibromyalgia and migraine; only weak short-term effects were found in complex regional pain syndrome. Confounding variables might have influenced these results. Clinical implications: These estimates should be interpreted with caution, but underscore that the placebo effect can be exploited in neurorehabilitation programs. It is not necessary to conceal its use from the patient. Knowledge of placebo mechanisms can be used to shape the doctor-patient relationship, to reduce the use of analgesic drugs and to train the patient to become an active agent of the therapy

    How future surgery will benefit from SARS-COV-2-related measures: a SPIGC survey conveying the perspective of Italian surgeons

    COVID-19 negatively affected surgical activity, but the potential benefits resulting from adopted measures remain unclear. The aim of this study was to evaluate the change in surgical activity and potential benefit from COVID-19 measures in perspective of Italian surgeons on behalf of SPIGC. A nationwide online survey on surgical practice before, during, and after COVID-19 pandemic was conducted in March-April 2022 (NCT:05323851). Effects of COVID-19 hospital-related measures on surgical patients' management and personal professional development across surgical specialties were explored. Data on demographics, pre-operative/peri-operative/post-operative management, and professional development were collected. Outcomes were matched with the corresponding volume. Four hundred and seventy-three respondents were included in final analysis across 14 surgical specialties. Since SARS-CoV-2 pandemic, application of telematic consultations (4.1% vs. 21.6%; p < 0.0001) and diagnostic evaluations (16.4% vs. 42.2%; p < 0.0001) increased. Elective surgical activities significantly reduced and surgeons opted more frequently for conservative management with a possible indication for elective (26.3% vs. 35.7%; p < 0.0001) or urgent (20.4% vs. 38.5%; p < 0.0001) surgery. All new COVID-related measures are perceived to be maintained in the future. Surgeons' personal education online increased from 12.6% (pre-COVID) to 86.6% (post-COVID; p < 0.0001). Online educational activities are considered a beneficial effect from COVID pandemic (56.4%). COVID-19 had a great impact on surgical specialties, with significant reduction of operation volume. However, some forced changes turned out to be benefits. Isolation measures pushed the use of telemedicine and telemetric devices for outpatient practice and favored communication for educational purposes and surgeon-patient/family communication. From the Italian surgeons' perspective, COVID-related measures will continue to influence future surgical clinical practice
