Rehabilitation in social security of persons with disabilities
The article shows the role of comprehensive rehabilitation in restoring good health, ability and in social and professional activation of the growing population of the disabled. The author discusses changes that have occurred in the approach to disability over time. The paper stresses the importance of comprehensive rehabilitation including medical, social and professional rehabilitation due to the fact that rehabilitation results depend on the work of a team of professionals from different fields working under the direction of a doctor specialised in rehabilitation medicine. It also presents acts of international and domestic law pursuant to which Poland should provide the disabled with equal and quick access to comprehensive rehabilitation, thus enabling them to participate in all aspects of life as fully as possible
Rehabilitation and employment of persons with disabilities in Poland
Wstęp. Obecna koncepcja niepełnosprawności zakłada, że
stopień niepełnosprawności jest wypadkową stanu zdrowia
fiycznego i psychicznego oraz warunków środowiskowych,
w których żyje osoba niepełnosprawna. Dlatego również na
społeczeństwie spoczywa odpowiedzialność za taką modyfkację środowiska, aby poprawie uległy warunki uczestnictwa
i aktywności osób niepełnosprawnych w życiu społecznym
i zawodowym.
Cel. Celem pracy jest przedstawienie problematyki rehabilitacji osób niepełnosprawnych i kwestii związanych z ich
zatrudnieniem.
Materiał i metoda. Oparto się na danych opublikowanych
przez Zakład Ubezpieczeń Społecznych (ZUS), Kasę Rolniczego Ubezpieczenia Społecznego (KRUS) i Państwowy Fundusz Rehabilitacji Osób Niepełnosprawnych (PFRON), biuro
Pełnomocnika Rządu ds. Osób Niepełnosprawnych oraz
na wynikach Narodowego Spisu Powszechnego Ludności
i Mieszkań przeprowadzonego w Polsce w 2011 roku. Przeanalizowano ponadto akty prawa krajowego i międzynarodowego z ostatnich kilkunastu lat.
Wyniki: W 2013 roku w Polsce rentę z tytułu niezdolności
do pracy pobierało 1 075 000 osób., zaś rentę z tytułu niezdolności do pracy w gospodarstwie rolnym otrzymywały 216 373 osoby. Według Badania Aktywności Ekonomicznej
Ludności Polski (BAEL) w 2013 roku aktywnych zawodowo
było 27,3% osób niepełnosprawnych w wieku produkcyjnym.
Istotne znaczenie ma zapewnienie osobom niepełnosprawnym zdobycia lub utrzymania zatrudnienia, a więc możliwie
pełnej integracji społecznej. Wydane na ten cel pieniądze są
inwestycją, która się zwraca, a nawet daje korzyści społeczne
i ekonomiczne.
Podsumowanie, Konieczne jest holistyczne podejście i koordynacja całego procesu leczenia, rehabilitacji i zatrudnienia
osób niepełnosprawnych uwzględniające współodpowiedzialność pracodawcy, połączenie usług medycznych, spo-
łecznych i zawodowych, odpowiednie doradztwo, zastosowanie modelu biopsychospołecznego (ICF) w ocenie efektów
rehabilitacji osób niepełnosprawnych.Introduction. According to the current concept of disability
the degree of impairment is a product of physical and
psychological health as well as environmental conditions in
which a disabled person lives. Therefore, it is also the society
that is responsible for modifying the environment so as to
improve the conditions of participation and activity of the
disabled persons in social and professional life.
Objective. The objective of this work is to present the
problem of rehabilitation of persons with disabilities and
issues related to their employment.
Materials and method: The work is based on data published
by ZUS (the Polish Social Insurance Institution), KRUS (the
Agricultural Social Insurance Fund), PFRON (the State Fund for
Rehabilitation of Disables Persons), Offi of the Government
Plenipotentiary for Disabled People and on the results of
the 2011 Polish Census of Population and Housing. Also acts
of Polish and international law from the last dozen of years
have been analysed.
Results. In 2013 in Poland, 1 075 000 people received the
inability to work benefi and 216 373 persons received the
benefi related to inability to work on a farm. According to
the Polish Research on Economic Activity of the Population
(BAEL), 27.3% of disabled persons in the working age led an active professional life in 2013. It is crucial to ensure that
the disabled persons access or maintain employment and
thus to provide for their largest possible social inclusion. The
expenses on this are an investment that pays back and even
generates social and economic profis.
Conclusion. It is necessary to adopt a holistic approach and to
coordinate the entire process of treatment, rehabilitation and
employment of persons with disabilities, taking into account
employer’s co-responsibility, combination of medical,
social and vocational services, appropriate counselling and
application of a biopsychosocial model (ICF) in evaluating the
effcts of rehabilitation of the disabled persons
Assessment of health, functioning and disability of a population aged 60–70 in south-eastern Poland using the WHO Disability Assessment Schedule (WHODAS 2.0)
Introduction
There is a growing number of older people in Poland. This phenomenon results in the need to assess their problems related with functioning in everyday life. This is the first study conducted in Polish society which evaluates the prevalence of disability and limitations in functioning by means of WHODAS 2.0 questionnaire.
Objective
Evaluation of the health, functioning and disability of people aged 60–70 years living in south-eastern Poland.
Material and Methods
The researched material was a randomly- selected sample of 1,000 inhabitants of south-eastern Poland. The study was conducted by use of direct interviews applying the WHODAS 2.0. For the purpose of statistical analysis, measures of descriptive statistics and non-parametric tests of significance were used.
Results
Limitations in functioning were reported by 67.00% of participants aged 60–70 years, including 46.20% with a mild disability, 14.50% – moderate, 6.30% – significant and extremely large disability. The highest level of disability occurred in areas related to participation in social life (mean = 20.77), performing activities of daily living (mean = 17.42) and mobility (mean = 17.23). A significantly higher level of disability (p <0.0001)was observed among unmarried people, the elderly and those with a greater number of chronic diseases. Higher level of physical activity was associated with lower disability level in the studied population (p<0.009).
Conclusions
Regarding the studied population, it was found that many health problems become worse over the years. The state of health that deteriorates with age causes limitations in daily functioning, which lead to disability, activity limitations and participation in everyday life. The progressive ageing of the Polish population will cause an increasing demand for medical care and on the social services
Zastosowanie analizy log – liniowej do wyboru czynników determinującychniepełnosprawność wśród starszych mieszkańców Polski południowo-wschodniej
Introduction: Many variables in research in the area of
medical and health sciences are qualitative in nature.
A common statistical tool used to analyze them is the χ2
test. However, it does not allow us to assess the relationship
between a number of variables and distinguish the factors
determining the investigated phenomenon. A more accurate
tool is the log-linear analysis, which enable the researcher
to evaluate the dependences and interactions between the
studied variables.
Purpose: Description of the use of log-linear analysis on the
example of the cross-sectional study on disability of the elderly.
Material and methods: The assessment of disability and the
choice of the factors that determine it was carried out on the results of a survey of 800 randomly selected people aged 71-80
years from the area of south-eastern Poland. The research
tool was a WHODAS 2.0 questionnaire and a respondent’s
particulars. The log-linear model was used for the analysis.
In order to evaluate the fitting of the model, the Pearson’s χ2
and the χ2 maximum likelihood statistics, R2 and A coefficients
were used .
Results: Education, adjustments of a house / flat, physical
exercises have a significant impact on the prevalence of
disability in the study group .
Conclusions: The log-linear analysis allows us to determine
the effect not only of individual variables on the formation of
an independent variable, but also their interactions and the
determination the odds occurrence of a dependent variable
according to different qualitative categories of dependent
variables. The information obtained in this way is a valuable
clue to take practical action to decrease or increase the
severity of the studied phenomenon.Wstęp: Wiele zmiennych w badaniach z obszaru nauk
medycznych oraz nauk o zdrowiu ma charakter jakościowy.
Powszechnym narzędziem statystycznym stosowanym do ich
analizy jest test χ2. Nie pozwala on jednak na dokonanie oceny
powiązań między większą liczbą zmiennych oraz wyodrębnienie
czynników determinujących badane zjawisko. Dokładniejszym
narzędziem jest analiza log-liniowa, pozwalająca na
ocenę zależności i interakcji między badanymi zmiennymi.
Cel: Przedstawienie zastosowania analizy log-liniowej na
przykładzie przekrojowego badania niepełnosprawności
osób starszych.
Materiał i metody: Ocenę niepełnosprawności oraz wybór
czynników, które ją determinują przeprowadzono na wyni-wyodrębkach
badania 800 losowo dobranych osób w wieku 71-80 lat
z terenu Polski południowo-wschodniej. Narzędzie badawcze
stanowił kwestionariusz WHODAS 2.0 oraz kwestionariusz
metrykalny. Do analizy wykorzystano model log-liniowy. W
celu oceny dopasowania modelu wykorzystano statystyki
χ2 Pearsona oraz χ2 największej wiarygodności, współczynniki
R2 oraz A.
Wyniki: Istotny wpływ na występowanie niepełnosprawności
w badanej grupie miały: wykształcenie, dostosowanie domu/
mieszkania, wykonywanie ćwiczeń fizycznych.
Wnioski: Analiza log-liniowa pozwala na ustalenie wpływu
nie tylko pojedynczych zmiennych na kształtowanie się
zmiennej niezależnej, lecz także ich interakcji oraz określenie
szansy wystąpienia zmiennej objaśnianej w zależności od
poszczególnych kategorii jakościowych zmiennych objaśniających.
Uzyskiwane w ten sposób informacje są cenną wskazówką
do podjęcia praktycznych działań w celu zmniejszenia
lub zwiększenia stopnia nasilenia badanego zjawiska
Analysis of health, functioning and disability of rural inhabitants aged 60–80 living in south-eastern Poland – a cross sectional study
Introduction
Since the beginning of the 20th century, the aging of the European population has been observed, and the potential long-term social and economic problems have been flagged.
Objective
The aim of the study was assessment of the level of disability and multidimensional analysis of determining factors among the elderly living in rural areas.
Material and Methods
The study was conducted in a population of 973 people aged 60–80 living in south-eastern Poland. The research tool used in the study was the WHO Disability Assessment Schedule – WHODAS 2.0.
Results
As many as 28.26% of the study group had at least a moderate level of disability; severe disability was observed in 11.51 % of the participants, and extreme disability was experienced by 0.21%. The highest average level of disability in the study group was found in performing activities of daily living (mean = 28.94; SD = 30.04), participation in everyday life (mean = 28.40; SD = 23.29) and mobility (mean = 26.04; SD = 27.57). Statistically significant relationships were observed between age, gender, education level, physical activity and number of chronic diseases.
Conclusions
An increase in the level of education and popularization of physical activity may be helpful in reducing the prevalence of disability among the elderly inhabitants of rural areas. Increasing access to health care, aimed at rapid diagnosis of chronic diseases, treatment and rehabilitation, seems to be a reasonable action preventing the disability in rural areas
The Use of Static Posturography Cut-Off Scores to Identify the Risk of Falling in Older Adults
Background: Falling is the most common accident that occurs in daily living and the second leading cause of unintentional injury death worldwide. The complexity of the risk factors associated with falling makes older people at risk of falling difficult to identify. The aim of the study was to identify the cut-off scores of standing posturography measures that can be used to predict the risk of falling in older adults. Methods: This observational study involved 267 elderly people aged 65 to 85 years (73.99 SD 7.51) living in south-eastern Poland. The subjects were divided into two groups: a group with a high risk of falling and a group with a low risk of falling, based on their timed up-and-go test. Postural stability was assessed during eyes-open and eyes-closed trials using the two-plate stability platform CQ Stab 2P. Results: The best accuracy, sensitivity, and specificity were observed for the sway path, anterior-posterior sway path, and medial-lateral sway path with open and closed eyes. The clinical cut-off score to predict the risk of falling was 350.63 for the sway path with open eyes, 272.64 for the anterior-posterior sway path, and 159.63 for the medial-lateral sway path. The clinical cut-off score for sway path with closed eyes was 436.11. Conclusions: Static posturography screenings in clinical practice may also be useful for detecting typical balance changes in older adults
Ocena mobilności, równowagi ciała oraz ryzyka złamania u osób starszych
ABSTRACT
Introduction. In recent years, we have seen significant
changes in the highly dynamic process of ageing amongst the
population of Europe, leading to increasing life expectancy.
Prolonging the life of the elderly, in which full independence
and functional performance are maintained, is a challenge
facing modern medicine. It is necessary to take actions to
improve health and quality of life during old age.
Purpose. The aim of this research is to assess the differences
in mobility level and static/dynamic balance, as well as the
risk of bone fractures between people aged 50 and 65 years
and the elderly aged between 66 and 80 years. An analysis of
the suitability of selected clinical tests is conducted to assess
the above mentioned parameters.
Material and methods. The study included a group of 200
randomly selected people living in Rzeszów, a city in southeastern
Poland, who were aged between 50 and 80 years.
All the results of the research, which ultimately involved 138 people – 103 women and 35 men – have been analysed. To
assess the mobility and static/dynamic balance, the following
clinical tests were used: Timed Up and Go, Functional Reach,
Tandem Stance, Tandem Walk and Tandem 180° Pivot. For the
assessment of fracture risk, the Fracture Risk Assessment Tool
(FRAX) calculator was used, while the data were collected on
the basis of direct interviews.
Results. It was found that, with age, the frequency of dynamic
balance, mobility disorders and the risk of bone fractures
significantly increase. In a group of people aged 66 to 80
years, a decrease in the efficiency level of mobility and
balance was observed in 30% of respondents on average,
depending on the kind of functional test used (20-60%), in
relation to people whose age did not exceed 65 years.
Conclusions. It is necessary to take preventive actions,
especially amongst a group of people aged over 65 years, in
order to improve functional efficiency and balance, thereby
reducing the risk of fractures.Wprowadzenie. W ostatnich latach obserwujemy w Europie
postępujący z dużą dynamiką proces starzenia się ludności.
Wobec stałego wzrostu średniej długości życia zadaniem
współczesnej medycyny staje się wydłużenie czasu życia
w pełnej niezależności, a co za tym idzie sprawności funkcjonalnej
osób starszych. Koniecznością jest podjęcie działań na
rzecz poprawy stanu zdrowia i jakości życia w okresie starości.
Cel pracy. Celem pracy jest ocena różnic w poziomie mobilności,
równowagi statycznej i dynamicznej oraz ryzyka złamania
kości pomiędzy osobami w wieku 50–65 lat a osobami
starszymi w wieku 66–80 lat, jak również analiza przydatności
wybranych testów klinicznych do oceny wyżej wymienionych
parametrów.
Materiał i metoda. Badaniem objęto grupę 200 losowo
wybranych osób zamieszkujących teren miasta Rzeszowa
(południowo-wschodni region Polski), w wieku 50–80 lat.
Ostatecznie analizie poddano kompletne wyniki badań 138 osób, w tym 103 kobiety i 35 mężczyzn. Do oceny mobilności,
równowagi statycznej i dynamicznej wykorzystano
testy kliniczne, w tym m.in.: Timed Up & Go Test, Functional
Reach Test, Tandem Stance Test, Tandem Walk Test oraz Tandem
Pivot 180°. Do oceny ryzyka złamania posłużono się
kalkulatorem FRAX, a dane zebrano na podstawie wywiadu
bezpośredniego.
Wyniki. Stwierdzono, że wraz z wiekiem istotnie wzrasta częstość
zaburzeń równowagi dynamicznej, mobilności oraz
ryzyka złamania kości. W grupie osób w wieku 66–80 lat zaobserwowano
obniżanie się poziomu sprawności w zakresie
mobilności i równowagi u średnio 30% badanych, zależnie
od rodzaju przeprowadzanego testu funkcjonalnego (20–
60%), w stosunku do osób, których wiek nie przekraczał 65 lat.
Wnioski. Niezbędne jest podjęcie działań profilaktycznych,
zwłaszcza w grupie osób powyżej 65 r.ż., mających na celu
poprawę sprawności funkcjonalnej i równowagi, a tym
samym obniżających ryzyko wystąpienia złamani
Aktywność fizyczna w czasie wolnym dzieci 11-letnich z powiatu rzeszowskiego
Introduction: Physical activity is an integral part of
human life. Younger school age is a period in which child’s
development is conditioned by their activity. Physical
stimulation during this period is important from a medical
and psychological perspective. The aim of the study was to
evaluate the influence of sociodemographic factors on the
level of physical activity and ways of spending leisure time
by 11-year-old children from the district of Rzeszow.
Material and methods: The study group consisted of 313
primary school children from the district of Rzeszow, 75
pupils from the city and 238 from rural areas, 164 girls and
149 boys aged 11. The study was conducted by means of a
diagnostic survey in a form of a questionnaire prepared by
the researchers.
Results: In the study group, no significant statistical
differences between a place of residence and a number of
days devoted to physical activity by children were found. The most common form of activity in their leisure time were
classes at the swimming pool and table tennis. The most
popular passive form of leisure was watching TV. Statistically
significant differences between gender and the amount of
time spent at the computer were found. Boys spent more
time at the computer than girls.
Conclusions: There was no relationship between social
environment and the level of physical activity and the way of
spending leisure time in the tested children. Boys spent more
time on passive forms of recreation. Statistically significant
correlation between gender and time spent at the computer
was found.Wstęp: Aktywność fizyczna jest nieodłącznym elementem
życia człowieka. Młodszy wiek szkolny to okres, w którym rozwój
dziecka uwarunkowany jest jego aktywnością. Stymulacja
fizyczna w tym okresie ma duże znaczenie z medycznego
oraz psychologicznego punktu widzenia. Celem pracy była
ocena wpływu czynników socjodemograficznych na poziom
aktywności fizycznej oraz sposobu spędzania wolnego czasu
przez 11-letnie dzieci z powiatu rzeszowskiego.
Materiał i metody: Grupę badaną stanowiło 313 dzieci
szkół podstawowych na terenie powiatu rzeszowskiego, 75
uczniów z miasta i 238 z terenów wiejskich, 164 dziewczęta
i 149 chłopców w wieku 11 lat. Badania przeprowadzono za
pomocą sondażu diagnostycznego z zastosowaniem kwestionariusza
ankiety własnego autorstwa.
Wyniki: W badanej grupie nie wykazano istotnych statystycznych

dni poświęcanych przez dzieci na aktywność fizyczną. Najczęstszą formę aktywności w czasie wolnym stanowiły zajęcia
na basenie oraz tenis stołowy. Do najczęstszych biernych
form spędzania czasu należało oglądanie telewizji. Wykazano
istotne statystycznie różnice pomiędzy płcią a ilością spędzanego
czasu przed komputerem. Chłopcy spędzali więcej
czasu przed komputerem niż dziewczęta.
Wnioski: Nie wykazano zależności między środowiskiem
społecznym a poziomem aktywności fizycznej oraz sposobem
spędzania wolnego czasu przez badane dzieci. Chłopcy
więcej czasu poświęcili na bierne formy wypoczynku. Wykazano
istotną statystycznie zależność między płcią a czasem
spędzanym przed komputerem
Evaluation of the usefulness of selected scales and methods used in the assessment of balance and physical finess in seniors – a pilot study
Wstęp i cel pracy: Wraz z wiekiem znacząco wzrasta ryzyko
upadków. Celem pracy była analiza przydatności wybranych
skal i metod stosowanych w ocenie równowagi i sprawności
fiycznej seniorów.
Materiał i metoda: Badaniem pilotażowym objęto grupę 25
osób w wieku 60–77 lat. Do oceny sprawności funkcjonalnej
wykorzystano specjalistyczny test dla osób starszych Fullerton Functional Fitness Test. Do oceny równowagi seniorów
wykorzystano testy kliniczne: Up&Go Test, Functional Reach
Test, Tandem Stance Test, Tandem Walk Test oraz Tandem
Pivot 180° oraz pomiary na platformie stabilometrycznej
Cosmogamma.
Wyniki: W wyniku przeprowadzonych badań wykazano
występowanie relacji pomiędzy wynikami testów klinicznych a pomiarem sprawności funkcjonalnej. Wykazano też
zależność pomiędzy wynikami testów klinicznych a wynikami pomiaru długości ścieżki środka pola podparcia na
platformie stabilometrycznej w staniu na jednej kończynie
dolnej z oczami otwartymi. Pomimo że w literaturze istnieją
doniesienia dotyczące badań nad oceną wydolności systemu kontroli równowagi seniorów, brak jest ujednoliconych procedur wskazujących, które testy i metody ilościowe
oceny równowagi powinny być analizowane, aby zapewnić
wiarygodność badań. Wykazano, że na podstawie wyników
jednego ze wskazanych w pracy testów można przewidywać
z dość dużą pewnością wyniki dla innego.
Wnioski: Zastosowane w pracy testy: Up&Go, Tandem Walk,
Tandem Pivot 180° i Functional Reach są wartościowymi
narzędziami do jakościowej oceny równowagi w populacji
osób powyżej 60 roku życia. Stabilometryczna ocena równowagi seniorów pozostaje w zależności z wynikami testów
klinicznych. Konieczne są dalsze badania obejmujące większą
grupę osób w celu potwierdzenia wiarygodności uzyskanych
wyników.Introduction and aim: The risk of falls increases signifiantly
with age. The aim of the study was to analyze the usefulness
of selected scales and methods used in assessing the balance
and physical finess in seniors.
Material and methods: The study included a pilot group of 25
people aged 60-77 years of age. Fullerton Functional Fitness
Test also called the Senior Fitness Test was used to assess the
functional capacity. The following clinical tests were used to
assess balance in seniors: “Up&Go Test”, Functional Reach Test,
Tandem Stance Test, Tandem Tandem Walk, Tandem Pivot
180° and measurements on Cosmogamma stabilometric
platform.
Results: The studies demonstrated the relationship between
the results of clinical tests and the measurement of functional
capacity, i.e. the relationship between the results of clinical
tests and the results of measurement of path length of centre
of pressure on stabilometric platform in standing on one leg
with eyes open. Although there are reports in the literature
concerning the research on evaluation of usefulness of the
control system used to assess balance in seniors, there are no
uniform procedures indicating which tests and quantitative methods for assessing the balance should be analyzed to
ensure the credibility of the research. It was demonstrated
that the results of one of the tests concerned in this paper can
be a fairly reliable prediction of the results for another one.
Conclusions: The tests: “Up & Go”, Tandem Walk, Tandem
Pivot 180° and Functional Reach, which were used in this
study, are valuable tools for qualitative assessment of the
balance in the population of people over 60 years of age.
Stabilometric assessment of the balance in seniors correlates
with the results of clinical tests. Further studies are necessary
involving a larger group of people in order to confim the
reliability of the results
Determinants of Physical Activity in Older Adults in South-Eastern Poland
The aim of our study is to assess factors determining the uptake of physical activity (PA) by older people living in south-eastern Poland. This is a cross-sectional study. The study included 858 older people aged 75 and over living in south-eastern Poland. PA was assessed by asking about the time spent on any at least moderate PA per week and about doing planned strengthening exercises to improve muscle strength and muscular endurance. Functional status, disability and quality of life in older people were also assessed. Logistic regression models were used to identify the factors related to PA. In the study group, only 25.64% performed a minimum of 150 min of moderate-intensity exercise, while strengthening exercises were performed by 22.49%. The most important factors influencing the uptake of PA were age, number of chronic diseases, place of residence, education, social activity, housing conditions, quality of life and health status. In summary, the study population represents a low level of PA uptake, with the majority not meeting the World Health Organization recommendations for PA uptake by older people. Our findings suggest individualized efforts to promote public health and increase PA among older people over 75 years of age
