The evaluation of physicalefficiency with patients in cardiological rehabilitation on an outpatient basis performed with an objective and subjective manner
W leczeniu zawałów serca kluczową rolę odgrywają zabiegi
przezskórnej plastyki tętnic wieńcowych (PCI). Niezbędnym
uzupełnieniem tych interwencji pozostaje leczenie farmakologiczne
oraz rehabilitacja kardiologiczna. Aby zaplanować
i bezpiecznie przeprowadzić rehabilitację kardiologiczną
niezbędna jest ocena wydolności fizycznej chorych z zawałem
mięśnia sercowego.
Cel: Porównanie obiektywnej i subiektywnej metody oceny
wydolności fizycznej u osób przechodzących ambulatoryjną
rehabilitację kardiologiczną z powodu zawału mięśnia sercowego.
Metodyka: W badaniu wzięło udział 40 pacjentów, którzy
przeszli zabieg angioplastyki naczyń wieńcowych z powodu
zawału mięśnia sercowego i uczestniczyli w ambulatoryjnej
rehabilitacji kardiologicznej. Program rehabilitacji zawierał 24
sześćdziesięciominutowe sesje z wykorzystaniem treningu
ergometrycznego. Do oceny poprawy wydolności fizycznej
wykorzystano pomiar obciążenia dobrze tolerowanego (11-
12 pkt w skali Borga) w trakcie treningu oraz wyniki próby wysiłkowej wykonanej przed rehabilitacją i po jej zakończeniu .
Wyniki: U pacjentów przechodzących ambulatoryjną rehabilitację
kardiologiczną z powodu zawału mięśnia sercowego
wzrasta wydolność fizyczna w trakcie rehabilitacji. Wskazują
na to wyniki próby wysiłkowej (wzrost obciążenia submaksymalnego),
jak i oceny tolerancji wysiłku (wzrost obciążenia
wywołującego małe zmęczenie, 11–12 pkt w skali Borga). Występują
jednak pewne różnice w ocenie wydolności fizycznej.
Stopień poprawy wydolności fizycznej obserwowany przy
pomocy próby wysiłkowej nie koreluje z wielkością poprawy
wydolności fizycznej wykazanej za pomocą metody subiektywnej
(skala Borga).
Wnioski: Obiektywna i subiektywna metoda pomiaru wydolności
fizycznej, niezależnie od siebie, wskazują na skuteczność
ambulatoryjnej rehabilitacji kardiologicznej u pacjentów po
przebytym zawale mięśnia sercowego. Wykorzystanie skali subiektywnej
w ocenie wydolności fizycznej chorych z zawałem
mięśnia sercowego wymaga jednak pewnego okresu adaptacji
pacjentów do zwiększonych obciążeń wysiłkiem fizycznymIn the treatment of heart infarctions, a key role is played
by the PCI – percutaneous coronary intervention. These
interventions need to be assisted with pharmacological
treatment and cardiological rehabilitation. In order to plan
and carry out such a rehabilitation in a safe manner, the
physical efficiency of the patients with myocardial infarction
need to be performed.
Purpose: The comparison of the objective and subjective
methods of evaluation of physical efficiency with patients
undergoing cardiological rehabilitation within the treatment
of the myocardial infarction.
Methods: The study comprised 40 subjects who had
undergone the procedure of percutaneous coronary
angioplasty within the treatment of myocardial infarction
and who took part in cardiological rehabilitation. The
rehabilitation programme consisted of 24 sixty-minute
sessions with the use of ergometric training. The physical
efficiency was evaluated with the use of the measurement
of well-tolerated load (the score of 11–12 on the Borg scale) during the training and the results of the effort stress test
performed before the rehabilitation and after its completion.
Results: Physical efficiency increases during the rehabilitation of
the patients undergoing outpatient cardiological rehabilitation
after myocardial infarction. This is illustrated by the results of an
effort stress test (an increase of the sub-maximal load) and the
evaluation of the tolerance of the effort (an increase of the effort
inducing light exertion, 11–12 points on the Borg scale). There
are, however, some differences in the evaluation of physical
efficiency. The degree of the improvement of physical efficiency
as observed with an effort stress test does not correlate with
the size of the improvement of the physical efficiency shown
with the use of the subjective method (Borg scale).
Conclusions: The objective and subjective scale of the
measurement of physical efficiency, independently from
each other, point to the efficacy of an outpatient cardiological
rehabilitation of the patient with a history of myocardial
infarction. The use of the subjective scale in the evaluation of
physical efficiency of the patients with a history of myocardial
infarction requires some period of patient adaptation to an
increased load with physical effort
The Importance of the Double Product in the Six-Minute Walk Test to Predict Myocardial Function
Introduction. The Six-Minute Walk Test (6MWT) is a widely used test to measure the physical performance of patients to assess the effectiveness of treatment, to qualify for rehabilitation, and to evaluate its effects.. Aim. This paper focuses on the assessment of the growth of a double product (DP) during the 6MWT and its diagnostic value in the assessment of patients with heart failure. Material and Methods. The paper has retrospective character. We analyzed medical records of 412 patients hospitalized for cardiac reasons, in whom a 6MWT was performed. The patients were divided into two groups: one with diagnosed heart failure and a control group. Results. The patients with diagnosed heart failure, compared to the control group, were characterized by a shorter walking distance and greater DP increase at equal walking intervals. After distinguishing the group with the preserved and decreased left ventricle ejection fraction, the value of the DP increase was still higher compared to the control group. The mean DP increase corresponding to one meter of walk was the only one that correlated negatively with the left ventricular ejection fraction. Conclusion. The assessment of the increase of the DP during the march test seems to be a better parameter reflecting the efficiency of the myocardium from the distance of the march
Platelet-Based Liquid Biopsies through the Lens of Machine Learning
Liquid biopsies offer minimally invasive diagnosis and monitoring of cancer disease. This biosource is often analyzed using sequencing, which generates highly complex data that can be used using machine learning tools. Nevertheless, validating the clinical applications of such methods is challenging. It requires: (a) using data from many patients; (b) verifying potential bias concerning sample collection; and (c) adding interpretability to the model. In this work, we have used RNA sequencing data of tumor-educated platelets (TEPs) and performed a binary classification (cancer vs. no-cancer). First, we compiled a large-scale dataset with more than a thousand donors. Further, we used different convolutional neural networks (CNNs) and boosting methods to evaluate the classifier performance. We have obtained an impressive result of 0.96 area under the curve. We then identified different clusters of splice variants using expert knowledge from the Kyoto Encyclopedia of Genes and Genomes (KEGG). Employing boosting algorithms, we identified the features with the highest predictive power. Finally, we tested the robustness of the models using test data from novel hospitals. Notably, we did not observe any decrease in model performance. Our work proves the great potential of using TEP data for cancer patient classification and opens the avenue for profound cancer diagnostics
