Some Result on fixed point Theorem in Hilbert Space
This paper puts up a result regarding the generalization of the Banach contraction principle in the Hilbert space, It consist of four rational square terms in the inequality. Further the corollary of Koparde and Wag mode was obtained by considering vanishing values to some constant towards the end of this result. A rare case report of primary renal squamous cell carcinoma
Primary renal squamous cell carcinoma is a very rare entity. Only few cases are reported in the world literature. Most of the patients present at a late stage resulting in poor prognosis. We report a 44 year old female who presented with a history of lump in right flank accompanied with a dull intermittent pain for 2 months duration which intensified in the last 15 days. Antecedent risk factors like renal calculi, analgesic use were absent. Computed Tomography (CT) examination revealed a mass arising from the superior pole of kidney with extension to the posterior segment of liver. Histopathology following nephrectomy showed histological features of well differentiated squamous cell carcinoma
Active Semantic Electronic Medical Record
The healthcare industry is rapidly advancing towards the widespread use of electronic medical records systems to manage the increasingly large amount of patient data and reduce medical errors. In addition to patient data there is a large amount of data describing procedures, treatments, diagnoses, drugs, insurance plans, coverage, formularies and the relationships between these data sets. While practices have benefited from the use of EMRs, infusing these essential programs with rich domain knowledge and rules can greatly enhance their performance and ability to support clinical decisions. Active Semantic Electronic Medical Record (ASEMR) application discussed here uses Semantic Web technologies to reduce medical errors, improve physician efficiency with accurate completion of patient charts, improve patient safety and satisfaction in medical practice, and improve billing due to more accurate coding. This results in practice efficiency and growth by enabling physicians to see more patients with improved care. ASEMR has been deployed and in daily use for managing all patient records at the Athens Heart Center since December 2005. This showcases an application of Semantic Web in health care, especially small clinics
ASSESSMENT OF ADVERSE DRUG REACTIONS OF ANTIDEPRESSANT DRUGS USED IN PSYCHIATRY DEPARTMENT OF A TERTIARY CARE HOSPITAL
Objective: The aim of the study was to analyze adverse drug reactions (ADRs) reported in patients prescribed antidepressants at tertiary care hospital.
Methods: A prospective and observational study was conducted during January 2020–July 2021 at Department of Pharmacology in collaboration with the Department of Psychiatry, GSVM Medical College, and Kanpur. All patients diagnosed with depression and receiving pharmacotherapy were included in the study. ADRs were monitored using the standard form of the Central Drugs Standard Control Organization and causality was determined using the Naranjo algorithm. Data were evaluated for patient’s demography, risk factors for ADRs, and pattern of ADR.
Results: A total of 293 ADRs were recorded from 110 patients. The most common ADRs observed were anxiety (47.42%) and insomnia (19.22%). Men (58.18%) were most commonly affected than women (41.82%). The most common causal drugs among antidepressants were Escitalopram (27.27%) and Venlafaxine (21.81%), respectively. The most common system involved was central nervous system (53.24%) followed by gastrointestinal system (41.63%). The majority of ADRs (97.95%) were possible according to the Naranjo’s scale.
Conclusion: Anxiety, insomnia, and dizziness were the common ADRs which were associated with the use of antidepressants. This study offers a representative profile of the ADRs which can be expected in the psychiatry outpatients
Microfilaria in malignant pleural effusion: an unusual incidental finding or causative association?
Lymphatic filariasis is common in tropical countries and is endemic in India. Filaria has a wide spectrum of presentation. Filarial lung involvement is usually in the form of tropical pulmonary eosinophilia with pulmonary infiltrates and peripheral eosinophilia. Filariasis presenting with pleural effusion is an unusual presentation. Malignancy in association with filarial pleural effusion is extremely rare and its role in tumorigenesis is controversial. In this context, we hereby report a case of 60 year old male, chronic smoker, who presented with left sided chest pain, cough, breathlessness, generalized weakness and swelling over left infrascapular region. Pleural fluid cytology repeated thrice due to degenerative changes, finally revealed malignant cells along with microfilaria. FNAC from left infrascapular swelling showed cytological features suggestive of metastatic deposits of Adenocarcinoma
Appearance of isolated thrombocytopenia in a patient of acute subdural hematoma
Thrombocytopenia in critically ill patients probably reflects the severity of the underlying illness and these patients have a higher mortality due to the severity of overall clinical status. In present article we report a case of acute subdural hematoma who developed progressive thrombocytopenia resulting in fatal outcome. A 75 year gentleman presented with history of sudden onset headache and lapsed into altered sensorium. There was history of trivial fall before he developed headache. The patient was deeply comatose with pupils bilateral 2 mm and reacting to light. Computerized tomography (CT) scan brain showed large right fronto-temporo-parietal acute subdural hematoma with mass effect and midline shift. Post-operatively he was kept on elective ventilation. On following days the patient continues to develop thrombocytopenia (Day-3 Platelet count 75,000, day-4 60,000, day-5 25,000). Thrombocytopenia is a common but a potentially life-threatening condition problem and has been considered to play a role in worsening the prognosis of critically ill patients in intensive care unit. Few studies statistically examine the strength of the association between risk factors and outcomes related to thrombocytopenia, additional research is recommended to identify putative molecular determinants
Massive infarction and cavernous sinus thrombosis: An uncommon complication of tooth extraction
Cavernous sinus thrombosis (CAST) is a rare and potentially fatal complication following tooth extraction. In present case of a 55 year old male known case of diabetes mellitus underwent tooth extraction. After 5 days, he noticed swelling around the cheek, high grade fever, and frontal headaches. Diagnosis of alveolar abscess after inspection was made, for which incision and drainage was done. Next morning, he noticed that the cheek swelling progressed to left eye and there was CT brain was performed which showed hyperdense areas in cavernous sinus on left side and left sylvian fissure with hemorrhagic venous infarct in left temporal and frontal lobes. More caudal sections revealed mucosal thickness in left maxillary and ethmoidal sinuses, edema over cheek, preseptal orbital swelling, retro-orbital fat stranding and axial proptosis. A diagnosis of rhino-orbital infection from dental source with cavernous sinus extension causing left temporo-frontal hemorrhagic venous infarction was made Emergency surgery for decompression was performed but the patient did not responded to the treatment and succumbed to the infection. This case is a reminder that in patients with uncontrolled diabetes, undergoing dental procedures should be carefully dealt with appropriate antibiotic cover. Early signs like unilateral facial edema, orbital chemosis, edema, and proptosis should raise high index suspicion of cavernous sinus thrombosis
Tolerance indices based evaluation of wheat (Triticum aestivum) genotypes under terminal heat stress conditions
Present study was carried out during winter (rabi) seasons of 2021–22 and 2022–23 at Crop Research Centre, Sardar Vallabhbhai Patel University of Agriculture and Technology, Meerut, Uttar Pradesh to evaluate the terminal heat tolerance ability of 30 wheat (Triticum aestivum L.) genotypes using 8 heat stress tolerance indices. The results indicate that 12 genotypes showed low yield reduction as compared to average yield reduction. The stress susceptibility index (SSI) was observed as robust indicator for identifying heat-tolerant genotypes. Stress tolerance index (TOL) was observed crucial which reflects genotype's ability to maintain grain yield under terminal heat stress. Analysis of correlation revealed positive relationships between grain yield under normal and stress conditions, suggesting that best-performing genotypes under normal conditions also perform well under terminal heat stress conditions. Principal component analysis and biplot analysis further confirmed the importance of studied tolerance indices for identification of heat tolerance genotypes. PC1 was associated with yield potential and heat tolerance, while PC2 was associated with stress susceptibility. Based on analysis, genotypes, viz. DBW14, DBW90, HD2864, HD2932, HD2985, HS375, HUW234, RAJ4083, WH1021, WH1124, DBW71 and PBW757 exhibited higher terminal heat stress tolerance. Cluster analysis revealed two major clusters, cluster I contained 12 heat tolerant genotype while cluster II was consisting 18 heat susceptible genotypes.
