    On Streaming and Communication Complexity of the Set Cover Problem

    We develop the first streaming algorithm and the first two-party communication protocol that uses a constant number of passes/rounds and sublinear space/communication for logarithmic approximation to the classic Set Cover problem. Specifically, for n elements and m sets, our algorithm/protocol achieves a space bound of O(m ·n [superscript δ] log[superscript 2] n logm) using O(4[superscript 1/δ]) passes/rounds while achieving an approximation factor of O(4[superscript 1/δ]logn) in polynomial time (for δ = Ω(1/logn)). If we allow the algorithm/protocol to spend exponential time per pass/round, we achieve an approximation factor of O(4[superscript 1/δ]). Our approach uses randomization, which we show is necessary: no deterministic constant approximation is possible (even given exponential time) using o(m n) space. These results are some of the first on streaming algorithms and efficient two-party communication protocols for approximation algorithms.     Note on alternating directed cycles

    The problem of the existence of an alternating simple dicycle in a 2-arc-coloured digraph is considered. This is a generalization of the alternating cycle problem in 2-edge-coloured graphs and the even dicycle problem (both are polynomial time solvable). We prove that the alternating dicycle problem is NP -complete. Let f(n)(g(n), resp.) be the minimum integer such that if every monochromatic indegree and outdegree in a strongly connected 2-arc-coloured digraph (any 2-arc-coloured digraph, resp.)     Pulsed Feedback Defers Cellular Differentiation

    Environmental signals induce diverse cellular differentiation programs. In certain systems, cells defer differentiation for extended time periods after the signal appears, proliferating through multiple rounds of cell division before committing to a new fate. How can cells set a deferral time much longer than the cell cycle? Here we study Bacillus subtilis cells that respond to sudden nutrient limitation with multiple rounds of growth and division before differentiating into spores. A well-characterized genetic circuit controls the concentration and phosphorylation of the master regulator Spo0A, which rises to a critical concentration to initiate sporulation. However, it remains unclear how this circuit enables cells to defer sporulation for multiple cell cycles. Using quantitative time-lapse fluorescence microscopy of Spo0A dynamics in individual cells, we observed pulses of Spo0A phosphorylation at a characteristic cell cycle phase. Pulse amplitudes grew systematically and cell-autonomously over multiple cell cycles leading up to sporulation. This pulse growth required a key positive feedback loop involving the sporulation kinases, without which the deferral of sporulation became ultrasensitive to kinase expression. Thus, deferral is controlled by a pulsed positive feedback loop in which kinase expression is activated by pulses of Spo0A phosphorylation. This pulsed positive feedback architecture provides a more robust mechanism for setting deferral times than constitutive kinase expression. Finally, using mathematical modeling, we show how pulsing and time delays together enable “polyphasic” positive feedback, in which different parts of a feedback loop are active at different times. Polyphasic feedback can enable more accurate tuning of long deferral times. Together, these results suggest that Bacillus subtilis uses a pulsed positive feedback loop to implement a “timer” that operates over timescales much longer than a cell cycle

    Selection for Translation Efficiency on Synonymous Polymorphisms in Recent Human Evolution

    Synonymous mutations are considered to be “silent” as they do not affect protein sequence. However, different silent codons have different translation efficiency (TE), which raises the question to what extent such mutations are really neutral. We perform the first genome-wide study of natural selection operating on TE in recent human evolution, surveying 13,798 synonymous single nucleotide polymorphisms (SNPs) in 1,198 unrelated individuals from 11 populations. We find evidence for both negative and positive selection on TE, as measured based on differentiation in allele frequencies between populations. Notably, the likelihood of an SNP to be targeted by positive or negative selection is correlated with the magnitude of its effect on the TE of the corresponding protein. Furthermore, negative selection acting against changes in TE is more marked in highly expressed genes, highly interacting proteins, complex members, and regulatory genes. It is also more common in functional regions and in the initial segments of highly expressed genes. Positive selection targeting sites with a large effect on TE is stronger in lowly interacting proteins and in regulatory genes. Similarly, essential genes are enriched for negative TE selection while underrepresented for positive TE selection. Taken together, these results point to the significant role of TE as a selective force operating in humans and hence underscore the importance of considering silent SNPs in interpreting associations with complex human diseases. Testifying to this potential, we describe two synonymous SNPs that may have clinical implications in phenylketonuria and in Best's macular dystrophy due to TE differences between alleles

    Pre-Participation ECG Screening in Military Recruits

    FUNDAMENTO: Os achados e investigações adicionais necessários com base na triagem pré-participação com eletrocardiograma (ECG) entre os recrutas militares estão mal definidos na literatura. OBJETIVOS: Este estudo foi elaborado para avaliar a taxa de achados anormais na triagem pré-participação com ECG em adultos jovens e as avaliações adicionais necessárias com base nestes resultados. MÉTODOS: Um estudo de coorte retrospectivo foi realizado no centro aero-médico da Força Aérea de Israel (IAF), para candidatos das unidades das academias de voo e de tropa s de elite. Os candidatos das unidades das Academias de voo e de elite passam por uma triagem pré-participação com ECG antes do alistamento nas Forças de Defesa de Israel (IDF). Desde 2010, todos os ECGs são realizados no centro aero-médico da IAF. Todos os ECGs realizados desde janeiro de 2010 foram analisados por um de três cardiologistas e todos aqueles nos quais resultados significativos foram identificados foram encaminhados para uma avaliação mais detalhada, a pedido do cardiologista. As causas de encaminhamento para avaliação posterior, as avaliações realizadas e os resultados dessas avaliações são notificados para a população de estudo. RESULTADOS: 1.455 ECGs foram realizados nos anos 2010-2011. Desses, 1.388 (95,39%) foram interpretadas como normais. 67 indivíduos foram encaminhados para uma avaliação mais detalhada com base nos achados do ECG. Os achados mais comuns levando a uma avaliação mais detalhada foram alterações da onda T (16 casos, 23,88%), padrão de pré-excitação (14, 20,89%) e critérios de voltagem para hipertrofia ventricular esquerda (11; 16,41%). Apenas 7 indivíduos (10,44%) tinham resultados anormais que foram considerados clinicamente significativos no final da avaliação médica. CONCLUSÕES: A taxa de achados significantes levando à desqualificação para a atividade militar é extremamente baixa e o encaminhamento para investigações adicionais baseado nos achados do ECG de 12 derivações deve ser criterioso

    Current use of D-dimer for the exclusion of venous thrombosis in hospitalized patients

    Abstract D-dimer assay’s utility for excluding venous thromboembolism (VTE) in hospitalized patients is debatable. We aimed to assess the current use of D-dimer as a diagnostic tool for excluding VTE in hospitalized patients and examine a mandatory age-adjusted D-dimer (AADD) threshold for diagnostic imaging. Retrospective cohort study between 2014 to 2019 that included patients from medical and surgical wards with a positive AADD result drawn during their hospitalization. The outcomes were determining a D-dimer threshold requiring further evaluation and assessing the prognostic value of D-dimer in predicting clinically relevant VTE in hospitalized patients. The cohort included 354 patients, 56% of them underwent definitive diagnostic imaging, and 7.6% were diagnosed with VTE after a positive AADD within 90 days of follow-up. Mortality rates were higher in patients diagnosed with VTE (33.3% vs. 15.9%, p = 0.03). Patients with pneumonia and other infectious etiologies were less likely to be further evaluated by definitive imaging (p = 0.001). Patients with a respiratory complaint (p = 0.02), chest pain (p  X2 the AADD were at increased risk of VTE [OR 3.87 (1.45–10.27)]. At 90 days of follow-up, no excess mortality was observed for patients without diagnostic evaluation following elevated AADD. D-dimer may be used in hospitalized patients to exclude VTE using the traditional AADD thresholds, with a high negative predictive value. D-dimer levels > X2 the AADD usually mandates further diagnostic imaging, while lower levels, probably do not require additional workup, with a sensitivity of almost 80% and no excess mortality
