    Brief Report: Out of Sight Out of Mind - Preventable Childhood Kidney Disease in the Far North

    APSGN is an immune-mediated kidney disease that occurs after a Streptococcus pyogenes skin or throat infection in children and contributes to chronic kidney disease later in life. It is a disease of poverty and regrettably common in Aboriginal and Torres Strait Islander children. There have been seven documented APSGN outbreaks across Far North Queensland in the last nine years. Despite this disease being notifiable in both Western Australia and the Northern Territory, Queensland is yet to acknowledge the importance of early notification in the management of APSGN. Notification-driven publication of APSGN incidence should help raise its profile and stimulate better public health policy

    Strongholds of Ostrea edulis populations in estuaries in Essex, SE England and their association with traditional oyster aquaculture: evidence to support a MPA designation

    The flat oyster Ostrea edulis has declined significantly in European waters since the 1850s as a result of anthropogenic activity. Ostrea edulis was designated a UK Biodiversity Action Plan Species and Habitat in 1995, and as a Feature of Conservation Importance (FOCI) within the UK Marine & Coastal Access Act 2009. To promote the recovery of oyster beds, a greater understanding of its abundance and distribution is required. Distribution of O. edulis across the proposed Blackwater, Crouch, Roach and Colne MCZ in Essex was determined between 2008 and 2012. Ostrea edulis were present in four estuary zones; with highest sample abundance in the Blackwater and Ray Sand zones. Size structure of populations varied, with the Ray Sand and Colne zones showing a significant lack of individuals with shell height <39 mm. Ostrea edulis occurred in highest number on shell substratum, followed by silty sediments. There were no significant associations between O. edulis abundance or size structure with water column Chl a, suspended solids, oxygen, nitrate or ammonium concentrations, temperature or pH. Highest abundance and most equitable population shell-size distribution for O. edulis were located within, or adjacent to, actively managed aquaculture zones. This suggests that traditional seabed management contributed to the maintenance or recovery of the species of conservation concern. Demonstration that the Essex estuaries were a stronghold for Ostrea edulis in the southern North sea area led to the designation of the Blackwater, Crouch, Roach and Colne estuaries Marine Conservation Zone in 2013

    Id1 Promotes Tumor Cell Migration in Nonsmall Cell Lung Cancers

    Id1, which belongs to the Id family of helix-loop-helix transcription factors has been most associated with tumor progression and metastatsis; however, its significance in lung cancers has not been extensively explored. Here we seek to evaluate the expression of Id1 in a pilot study of nonsmall-cell lung cancers (NSCLCs) and determine its diagnostic and functional significance in these tumors. Paired normal and malignant lung tissues as well as a panel of NSCLC primary tumors and cell lines were evaluated for Id1 expression using Western blotting and quantitative RT-PCR. Functional assays were performed to evaluate the role of Id1 in tumor cell growth, migration and progression. We find Id1 expression is upregulated in squamous cell carcinoma when compared to adenocarcinoma of the lung and that expression of Id1 versus the normal control is variable in NSCLCs. We also note that Id1 expression in NSCLC cells is largely growth factor dependant and constitutive expression of Id1 in NSCLC cells significantly increases tumor cell migration without affecting cell proliferation. We conclude that Id1, as a mediator of tumor cell migration, may be an indicator of aggressive potential in nonsmall-cell lung cancers

    Modulation of ecosystem services by animal personalities

    Conservationists rarely consider the roles individuals, with their own unique behavior, physiology, and genome, play in shaping ecosystem processes and consequently ecosystem services, but this is changing. An ongoing surge in research on animal personalities (that is, behavioral differences among individuals that are consistent over time and across contexts) is exposing the ecological roles of individuals to scientific scrutiny. Here, we present four broad examples of ecosystem services that are likely to be shaped by personalities: (1) pollination and seed dispersal, (2) regulation of pest species, (3) ecotourism, and (4) maintenance of soil quality. Although researchers have suggested diverse links between animal personality and ecosystem function, very few have examined this association. We outline a four-step process for quantifying and validating these linkages, leading to application for conservation practitioners, and conclude by recommending that accounting for behavioral variation should be incorporated into the management of ecosystem services

    Divided we fall: clinicians and academic psychiatrists need to stand together

These problems affect academic psychiatrists in the universities, and clinicians in publicly funded mental health services. Academic units are isolated and endangered, finding it difficult to recruit. They could benefit from closer relationships with public mental health services, in terms of recruitment, shared teaching and clinical research. However, mental health services are preoccupied with their own problems, particularly in relation to acute clinical demand. How can we stand together to improve academic units and clinical psychiatry? Conclusions: Clinicians and academic psychiatrists can stand together on important matters, but it takes initiatives from local leaders to overcome the structural barriers between health services and the universities. An example is given of united action by clinicians and academic psychiatrists to address a crisis within a state mental health system. First, psychiatrists undertook independent health services research that compared the state system with those in other Australian and international jurisdictions. The comparative data was used to generate solutions, which were presented at every level from ministerial offices through to service managers. Finally, psychiatrists took up joint academic and clinical leadership roles in the university and the mental health system. This united research-led approach turned around the crisis in the state mental health syste

    Anorectal Disorders

    This report defines criteria and reviews the epidemiology, pathophysiology, and management of the following common anorectal disorders: fecal incontinence (FI), functional anorectal pain, and functional defecation disorders. FI is defined as the recurrent uncontrolled passage of fecal material for at least 3 months. The clinical features of FI are useful for guiding diagnostic testing and therapy. Anorectal manometry and imaging are useful for evaluating anal and pelvic floor structure and function. Education, antidiarrheals, and biofeedback therapy are the mainstay of management; surgery may be useful in refractory cases. Functional anorectal pain syndromes are defined by clinical features and categorized into 3 subtypes. In proctalgia fugax, the pain is typically fleeting and lasts for seconds to minutes. In levator ani syndrome and unspecified anorectal pain, the pain lasts more than 30 minutes, but in levator ani syndrome there is puborectalis tenderness. Functional defecation disorders are defined by ≥2 symptoms of chronic constipation or irritable bowel syndrome with constipation, and with ≥2 features of impaired evacuation, that is, abnormal evacuation pattern on manometry, abnormal balloon expulsion test, or impaired rectal evacuation by imaging. It includes 2 subtypes: dyssynergic defecation and inadequate defecatory propulsion. Pelvic floor biofeedback therapy is effective for treating levator ani syndrome and defecatory disorders

    Development and Application of STORMTOOLS Design Load (SDL) Maps

    Under the STORMTOOLS initiative, maps of the impact of sea level rise (SLR) (0 to 12 ft), nuisance flooding (1–10 yr), 25, 50, and 100 yr storms, and hindcasts of the four top ranked tropical storms have been developed for the coastal waters of Rhode Island (RI). Estimates of the design elevations, expressed in terms of the Base Flood Elevation (BFE) and thus incorporating surge and associated wave conditions, have also been developed, including the effects of SLR to facilitate structural design. Finally, Coastal Environmental Risk Index (CERI) maps have been developed to estimate the risk to individual structures and infrastructure. CERI employs the BFE maps in concert with damage curves for residential and commercial structures to make estimates of damage to individual structures. All maps are available via an ArcGIS Hub. The objective of this senior design capstone project was to develop STORMTOOLS Design Load maps (SDL) with a goal of estimating the hydrostatic, hydrodynamic, wave, and debris loading, based on ASCE/SEI 7–16 Minimum Design Standards methods, on residential structures in the RI coastal floodplain. The resulting maps display the unitized loads and thus can be scaled for any structure of interest. The goal of the maps is to provide environmental loads that support the design of structures, and reduce the time and cost required in performing the design and the permitting process, while also improving the accuracy and consistency of the designs. SDL maps were generated for all loads, including the effects of SLR for a test case: the Watch Hill/Misquamicut Beach, Westerly, along the southern RI coast. The Autodesk Professional Robot Structural Analysis software, along with SDL loading, was used to evaluate the designs for selected on-grade and pile-elevated residential structures. Damage curves were generated for each and shown to be consistent with the US Army Corps of Engineers empirical damage curves currently used in CERI
