    An extremely rare case of concurrent BRAF V600E mutation driven hairy cell leukemia and melanoma: case report and review of literature

    BRAF protein is a serine/threonine kinase with 766 amino acids. Approximately 15% of human cancers harbor BRAF mutations as well as other BRAF anomalies (amplifications, fusions). Somatic mutations mainly occur in the catalytic kinase domain (CR3), and the predominant mutation is p.V600E which is the substitution of glutamic acid (E) for valine (V) as result of a mutation at codon 600 of the kinase domain. To our knowledge, the vast majority of the cancers have non-germline BRAF mutations. Here we describe a case of a 60-year-old female with a history of hairy cell leukemia (HCL) who presented with aphasia and forgetfulness. A follow-up Brain CT scan showed three distinct brain lesions which were found to be diagnostic of melanoma (confirmed by immunohistochemistry) with no evidence of a concurrent brain involvement by a B-cell neoplasm. Molecular studies confirmed the same BRAF p.V600E mutation in both malignancies (hairy cell leukemia and melanoma). Thereafter the patient was started on BRAF inhibitor treatment and is now symptom-free after one year of follow up. Having two concurrent malignancies with a shared BRAF mutation is extremely rare and makes this an excellent example of a genomic marker-driven treatment in two histologically and immunophenotypically distinct tumor

    Analysis of Near-infrared (NIR) spectroscopy for chlorophyll prediction in oil palm leaves

    Oil palm nutrient content is investigated with using chlorophyll as a representative factor correlated with NIR spectroscopy spectral absorbance. NIR spectroscopy method of sampling have been tested to overcome time consuming, complex chemical analysis procedure and invasive sampling method in order to identify chlorophyll content in an oil palm tree. Spectral absorbance data from range 900 nm to 1700 nm and chlorophyll data, then tested through five pre-processing methods which is Savitzky-Golay Smoothing (SGS), Multiplicative Scatter Correction (MSC), Single Normal Variation (SNV), First Derivative (1D) and also Second Derivative (2D) using Partial Least Square (PLS) regression prediction model to evaluate the correlation between both data. The overall results show, SGS has the best performance for preprocessing method with the results, the coefficient of determination (R2) values of 0.9998 and root mean square error (RMSE) values of 0.0639. In summary, correlation of NIR spectral absorbance data and chlorophyll can be achieved using a PLS regression model with SGS pre-processing technique. Thus, we can conclude that NIR spectroscopy method can be used to identify chlorophyll content in oil palm with using time saving, simple sampling and non-invasive method

    Analysis of the effect of BER and Q-factor on free space optical communication system using diverse wavelength technique

    This paper presents the analysis of the effect of BER and Q-factor on free space optical communication system using the diverse wavelength technique. The visibility is gradually decreased due to attenuation effects such as weather conditions as the bit error rate increases. Despite this disadvantage, FSO performance could be further improved by using a wide range of wavelengths of the laser beam. The results show good performance for 1550 nm wavelength, where the wavelength can accommodate visibility for 6 km at link range of 1 km, and Q-factor 25 at the system performance BER = 10-124 could improve the system by 67% compared to 850 nm and 785 nm wavelength, which could enhance the system by only 2%. This clearly show that the superior capabilities of 1550 nm wavelength in handling attenuation problems and capability to reduce the power loss compared to 850 nm and 785 nm wavelength

    Simulation realization of 2-D wavelength/time system utilizing MDW code for OCDMA system

    This paper presents a realization of Wavelength/Time (W/T) Two-Dimensional Modified Double Weight (2-D MDW) code for Optical Code Division Multiple Access (OCDMA) system based on Spectral Amplitude Coding (SAC) approach. The MDW code has the capability to suppress Phase-Induce Intensity Noise (PIIN) and minimizing the Multiple Access Interference (MAI) noises. At the permissible BER 10-9, the 2-D MDW (APD) had shown minimum effective received power (Psr) = -71 dBm that can be obtained at the receiver side as compared to 2-D MDW (PIN) only received -61 dBm. The results show that 2-D MDW (APD) has better performance in achieving same BER with longer optical fiber length and with less received power (Psr). Also, the BER from the result shows that MDW code has the capability to suppress PIIN ad MAI

    Performance study on the effect of filter curve in CWDM System for the access network

    This paper presents the study on the effect of filter variation on the coarse wavelength division multiplexing (CWDM) system. The filter curve will affect the performance of the CWDM system due to changes of received power lever and isolation of the signal. The significant impact on the received power level and isolation can be found when the required signal is isolated from unwanted signal by the steep curve of filter. As a result, BER of 1.0x 10-12 was obtained corresponding to receive power level of -24.27 dBm with isolation of 23.22 dB. When the wavelength spacing is reduced to 1nm, the isolation is only 11.30 dB and BER increased to 5.49x10-7 with a received power of -15.39 dBm

    miRNAs in the regulation of mTOR signaling and host immune responses: the case of Leishmania infections

    Micro RNAs (miRNAs), as regulators of gene expression at the post-transcriptional level, can respond to/or interact with cell signaling and affect the pathogenesis of different diseases/infections. The interaction/crosstalk of miRNAs with various cellular signaling networks including mTOR (as a master regulator of signaling relevant to different cellular mechanisms) might lead to the initiation, progression or restriction of certain disease processes. There are numerous studies that have identified the crosstalk between regulatory miRNA expression and the mTOR pathway (or mTOR signaling regulated by miRNAs) in different diseases which has a dual function in pathogenesis. However, the corresponding information in parasitic infections remains scarce. miRNAs have been suggested as specific targets for therapeutic strategies in several disorders such as parasitic infections. Thus, the targeting of miRNAs (as the modulators/regulators of mTOR) by small molecules and RNA-based therapeutics and consequently managing and modulating mTOR signaling and the downstream/related cell signaling/pathways might shed some light on the design of new therapeutic strategies against parasitic diseases, including Leishmaniasis. Accordingly, the present study attempts to highlight the importance of the crosstalk between regulatory miRNAs and mTOR signaling, and to review the relevant insights into parasitic infections by focusing specifically on Leishmania

    Health-related quality of life in infertile couples receiving IVF or ICSI treatment

    <p>Abstract</p> <p>Background</p> <p>Infertile couples might experience psychological distress and suffer from impaired health-related quality of life. This study aimed to examine health-related quality of life in infertile couples receiving either in-vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI) treatment.</p> <p>Methods</p> <p>This was a cross-sectional study of quality of life in infertile couples attending to Vali-e-Asr Reproductive Health Research Center or Royan Institute for either IVF or ICSI treatment in Tehran, Iran. Health-related quality of life was assessed using the Short Form Health Survey (SF-36). Patients' demographic and clinical characteristics were also recorded. Data were analyzed to compare quality of life in infertile women and men and to indicate what variables predict quality of life in infertile couples.</p> <p>Results</p> <p>In all 514 women and 514 men (n = 1028) were studied. There were significant differences between women and men indicating that male patients had a better health-related quality of life. Also health-related quality of life was found to be better in infertility due to male factor. Performing logistic regression analysis it was found that female gender, and lower educational level were significant predictors of poorer physical health-related quality of life. For mental health-related quality of life in addition to female gender and lower educational level, younger age also was found to be a significant predictor of poorer condition. No significant results were observed for infertility duration or causes of infertility either for physical or mental health-related quality of life.</p> <p>Conclusion</p> <p>The findings suggest that infertility duration or causes of infertility do not have significant effects on health-related quality of life in infertile couples. However, infertile couples, especially less educated younger women, are at risk of a sub-optimal health-related quality of life and they should be provided help and support in order to improve their health-related quality of life.</p

    Carbon efficiency evaluation:an analytical framework using fuzzy DEA

    Data Envelopment Analysis (DEA) is a powerful analytical technique for measuring the relative efficiency of alternatives based on their inputs and outputs. The alternatives can be in the form of countries who attempt to enhance their productivity and environmental efficiencies concurrently. However, when desirable outputs such as productivity increases, undesirable outputs increase as well (e.g. carbon emissions), thus making the performance evaluation questionable. In addition, traditional environmental efficiency has been typically measured by crisp input and output (desirable and undesirable). However, the input and output data, such as CO2 emissions, in real-world evaluation problems are often imprecise or ambiguous. This paper proposes a DEA-based framework where the input and output data are characterized by symmetrical and asymmetrical fuzzy numbers. The proposed method allows the environmental evaluation to be assessed at different levels of certainty. The validity of the proposed model has been tested and its usefulness is illustrated using two numerical examples. An application of energy efficiency among 23 European Union (EU) member countries is further presented to show the applicability and efficacy of the proposed approach under asymmetric fuzzy numbers
